01:35consultant dr. Laura Buxton where am I now you're at Barnsley Hospital so get ready to share a shift no
01:43brilliant women everybody with the team at Barnsley casualty
01:48there's a couple of y'all patients and going to ward I'm just wondering are they ready to move
02:05sister Georgia is starting her shift and she's arriving to a backlog of patients causing congestion in the
02:14department every day on shift you don't like have problems and you have got to move staff around and
02:22help areas that are struggling will you have a look at some of these just see if they're ready to move and
02:27I can get ready to move on I've just asked Emma to sort cubicle four I do quite like doing that because
02:35obviously we need to make sure that his patients are safe we've got 56 patients in and with a two-hour
02:42doctor wait we've got currently 10 waiting for AMU and so we're just going to try and get these
02:50patients up to ward as fast as we can to move the patients around Georgia needs the hospital porters but
02:57there's a long wait before they're free we've got all these beds coming up but porters are on an hour
03:04and eight minutes we want to get them out because we're getting ambulances we've got to win we've got
03:09two on route and three expects so we're going to get five ambulances in and we need space they've not
03:15answered it's a party got a little bit busy at minute and we're just having issues with porters getting
03:24patients up to ward we just need some parties to come and take these patients up an ambulance is on
03:38its way to casualty four-year-old Skyler is being rushed into pediatric care with recurring seizures
03:49mum Katie and dad Jack are alongside Skyler who's been looked after by advanced clinical practitioner
04:02Gemma what's the normal seizures like mom usually it's just her eyes and her head all drift off to one
04:08side it might have lent you in there but that's usually it but last two is when she started with
04:13the jerks okay when she's got to us I've been asked by our nurse to actually come and have a
04:20little look at her because she seems quite what we call flat so not really responding how she should
04:25normally normally normally normally the right side from what I can remember yeah it's usually the
04:35right side that's the jerks or she takes off too that's what I've just said to me
04:48seizures is something Skyler has suffered with previously but the length of this episode is a
04:54serious concern for the team she needed to be in an environment we're a bit more bit more safe and
05:01secure and we could have a bit more help around so that's why we transferred it to resus
05:06having a epileptic seizure there's a lot of risks associated it one of the main ones is that you can't
05:23actually breathe properly by yourself and so you can maintain that oxygen supply to your brain to you
05:31all your rest of your organs and that's risky in itself there's a risk that she could stop breathing
05:42and that's why we need to get some immediate access so we need to get a cannula and get
05:47sort of look what the next medication is
05:50especially in with seizures if you're not ventilating or breathing properly you're not getting that oxygen
06:09to supply your brain and that's where you're risking things like brain damage so yes Skyler's got to the
06:21point now where we need to intubate and take over that breathing for us so intubation is protect what
06:27we call protecting that airway essentially we are breathing for her we're being her lungs we're making
06:34sure that that oxygen is supplying all her organs and making sure we're not depriving her of oxygen
06:39she's being intubated as we speak she's definitely jerking a lot more than last time and struggling to
06:50breathe a bit more every time she's having a seizure it could be damaging her brain more it could
07:00be doing more damage internally so while she's asleep and intubated she's probably in the safest place
07:05possible so hopefully my carbs her entire body so the seizures will stop I think that's basically how
07:13they described it to us it's now an anxious wait to see if the intubation results Skyler's seizures
07:25she's killed for ready to move yeah yeah sister Georgia is juggling patients and
07:55beds while she waits for portals to become available to move people currently about three
08:01four five six we've got six patients awaiting AMU that beds are ready but we just haven't got the
08:07portraits to take them up and obviously A&E don't stop so we've got a constant flow of patients coming
08:14in and sometimes we've just got no room to like say anyone anywhere really whilst Georgia waits for the
08:22reporters to begin transferring her patients she's presented with a new task so this COVID and flu
08:31swabbing thing there's a CBU one major says no but CBU two saying that we have ITU are wanting COVID and
08:40flu swabs now but we've not got any machines set up yet so we're just trying to work out whether or not
08:46we're doing it or not really the swabs for COVID and flu will add further time pressure to sister Georgia's
08:54it will cause some delays in A&E because it takes 15 minutes to do the swab so if we could get it done as fast as we can then it obviously stops the delays really
09:07while sister Georgia deals with delays caused by additional swabbing in resus
09:22advanced clinical practitioner Gemma is monitoring four-year-old Skyler
09:26so she started having a seizure we couldn't stop her from seizing so we had to intubate her and that's
09:41the way that you try and stop people seizing we attempted to extubate her first time so I attempted
09:48to wake her up but what happened was she started again to have a to have a seizure so it wasn't safe to
09:55wake her up because she would being able to maintain her own airway if after a second time we can't wake
10:06Skyler up then she would need to be transferred out to another specialist area which would be
10:12pediatric intensive care with her vital signs heading in the right direction anesthetic consultant Rakesh
10:20Basra helps prepare a second attempt to wake Skyler up so anesthetic team it's their bread and butter
10:30essentially that is their speciality when she's been sedated they're the ones that know how to manage
10:36that airway they're more familiar with the type of things we need for the airway and also the medications
10:41that they need to give all right sweetheart Skyler you're just waking up mum is here all right darling
10:56so we'd not even got this far and she'd started fitting last time with her wake up but she's had quite a
11:07lot of medication to make her sleepy so we're not expecting her to sit up and open her eyes the
11:14important thing we want to to know is whether she can maintain her own own airway so can she breathe
11:21by herself the last time we tried to wake her up she started fitting straight away Skyler
11:28Skyler must now be closely monitored to ensure no further seizures occur
11:34another ambulance has rushed 85 year old Maureen to casualty she has a slow heartbeat
11:53Maureen has dementia and a son Matthew has accompanied her to the hospital
12:11I got a phone call today from the care home just to state that my mum had low oxygen and she wasn't
12:19very well they ran for an ambulance and then the ambulance had brought her in because her stats are
12:29not very well I don't know until you're better
12:34my mum will be 86 on the 12th of October so she's a really strong for her age 85 and she's just a just witnessed
12:52my dad passing four days ago he were 91 so that's gonna that's gonna put a big strain on her as well
13:00what what happened to your dad is looking after you why did he die he wasn't very well and he was a bit
13:12tired when did he die a few days ago you were there just a few days ago yeah you were there you held his
13:21hand and you were holding his hand and saying I love you George George yeah yeah my house that's
13:26your beautiful husband my my gorgeous dad my hero
13:30okay when did he die Thursday hey Thursday Thursday yeah I know I know my dad really did well looking
13:45after a few carers coming in but my dad did the main bulk of it all and being at 91 I think it's just
13:52took its toll a bit and so he's now at peace and resting and he'll be waiting for my mom
13:57coming soon I think what why did he die it wasn't very well he got an infection it couldn't get right
14:07right it couldn't get right now sorry sorry
14:21she's very posh very posh well we'll get on well then dr. Buxton will be in charge of Maureen's care
14:29today and he's here to take a blood sample right I'm gonna see if I can have another go at popping
14:34that needle in the back of your hand okay okay I'll be as gentle as I can all right right you're
14:40ready Maureen keep nice and still here we go oh I don't know you can look mom you're very brave
14:49I think for now Matthew and Maureen the priorities are that your mum's pain settle down that discomfort
14:58she was having this morning settled yep she looks a better colour yep and her blood pressure is
15:03holding steady yep so whatever the reason for her heart having gone so very slow this morning
15:08got a little bit of time to work that out yep and a bit of time to come up with the right plan for how to
15:13sort it yep you sort of realize that time is very precious I'm all for making me dad proud I'm all
15:21for making me mum proud I'm just just feel so lost at the moment your heartbeat is very slow today and I
15:31can't let you go home while it's this slow okay where am I now you're at Barnsley Hospital in A&E
15:43okay so we'll take good care of you until we've worked out what's going on and what needs to happen
15:50next Maureen's heartbeat remains slow and she will need to be closely monitored in the coming hours
15:56but there's just quite a lot of DTA but they've not ticked ready to move so we're just going to work
16:17through all ready to move in the hub sister Georgia has patients ready to go to wards but the hospital's porters
16:25aren't free to move them causing a backlog
16:28Reena you know your patients that have got DTAs will you just click ready to move when they're ready to move on
16:34tele-tracking please have you put it on
16:36but there's one patient she can't move on who's been found in possession of a large quantity of drugs there
16:42were a bag of cocaine on side and somebody reported it to police to come and collect from a patient that
16:48we've got in currently they've come and picked it up and I believe they're still in their in with patient
16:54now
16:58so I am up to me I don't know what's in there well apparently that's what's been ended up from night
17:05staff that there's cocaine in there I don't know how much I don't know what the outcome will be but
17:17it's been reported anyway whilst Georgia helps the police with their inquiries
17:23in minor injuries 65-year-old Sean has taken a tumble down his cellar steps
17:37we're doing big shock today I've gone down for
17:43what carriers down there there's some down there as I've got near bottom about three front bottom
17:50the big concrete steps as well I've slipped and my foot's gone under it's what's gone underneath me
17:56I'm Claire emergency nurse practitioner Claire is on hand to assess the damage
18:04right my love tell me what's happened I've slipped me foot's gone underneath me so underneath it yeah yes
18:15it's gone underneath there what were you on your feet what do you have on your feet these silly slippers
18:21are all right did they stay on uh that one did that one came off okay and you've injured your toenail as well yeah
18:31lots of patients come in from having injuries based on poor footwear uh slippers is one of the bane of the life of the
18:39practitioner because yeah they're sleepy and they slip on and off and they're not supportive so people can
18:45go and do something really innocuous and then fall and and obviously injure themselves can you stand on
18:52it after you've done it to be honest I've not tried okay I don't feel like trying but I will if you
18:57won't know it's all right I just wanted to know if you had any pain at back no any pain around there no
19:05okay so is it sorry yeah yeah yeah yeah yeah right then let's go to x-ray thank you

20:15well did i know yeah you were there holding his hand where where at the care home and then he
20:24died oh he went to sleep he went to sleep yeah you you were there sweetie you've probably forgot
20:34don't be upset there's nothing there's nothing to be upset about is there
20:39i don't know he's not listen he's not hurting he's watching over you
20:46did he really yeah it's a strange disease because she's really really good at remembering old times
20:54from from long long ago it's just a short-term memory you've got to keep repeating herself and she does
21:00retain some like she knows my dad's passed away but then she forgets sometimes and where is it it's it's
21:06it's it's it's having the the the death over and over and over and over again every single day

21:18i'm frightened though there's nothing to be frightened of you've got all these doctors and nurses i'm looking
21:24after you i won't leave you and you've got vicky just georgia and all grandkids coming over
21:29you've got those other people at care home looking after you and my dad more importantly my dad's
21:36looking after you they've been married and they're all school married so for better for
21:43worse death goes apart and unfortunately i mean my dad's my dad died and he's he's now apart but
21:48hopefully looking looking after her with maureen's heart rate remaining lower than doctors would like
21:55dr buxton has ordered a chest x-ray you just got a quick picture of your chest to do today
22:01pardon her she's going to take an x-ray i promised my dad i would look after my mom
22:06and she wouldn't suffer so that's what i'm doing now
22:12everything is just a flood of emotion it just washes over you and just takes you away and then
22:16you're all right and then you feel a bit numb and a bit angry and then
22:20sadness again it's i've never experienced it to be fair
22:26maureen and matthew must now wait for the results of the chest x-ray
22:30to find out what can be done to improve her heart rate
22:46having begun to clear a backlog of patients with the help of the hospital porters sister georgie is
22:55now helping someone who's having a mental health crisis yeah we're in transport to kendra
23:02the patient was found in possession of a large amount of drugs and the police have been called
23:07the patient is currently being taken care of by healthcare assistant paul
23:18bill me a guy who's been doing well at minute um quite poorly blessed him so i've been sat with him
23:24while waiting for special out for his specialty team to come and see him so he's now gone to
23:32another hospital so we can get the care he needs uh it's took a bit of resources from staff but we've
23:37managed to get the right help he needs i've just uh took him to ambulance and said goodbye to him
23:44and he's now been in a merry way to uh kendra hospital so he he's had a bit of a breakdown so
23:51we still treat everybody the same and hopefully it'll be better in a few days
24:09in recess four-year-old skylar is being woken up by nurse liv having been intubated to prevent seizures
24:16all right darling where's mummy skylar hey mummy's here
24:31it's much better than the last time we tried to wake her up
24:33so everything she's doing by herself now so it's just waiting for her to wake up a little bit and
24:54then we can monitor her further and hopefully get her to the ward i know you're gonna rub this off your
25:00nose i know thankfully this has worked she's stopped seizing she's actually starting to breathe
25:07on her own so now it's just a case that we need to keep her a close eye on her i'm sure you don't
25:12need to check in she's had quite a lot of medication that's gonna make her make her sleepy
25:18so i wouldn't expect her to she might be out fast asleep at rest at night the main thing is as long as
25:25she's breathing on her own and maintaining her airway and not having a seizure
25:30you're waking up hi yeah look at you waking up after a groggy start skylar is now coming round
25:42and it looks like her seizures have stopped we're definitely waking up a lot more now aren't we
25:47i'm not telling is that annoying shall we take it out soon do you want to take it out yeah are we
25:57messing what are you setting up where's mummy you see where she is kids tend to bounce back a lot more
26:08quickly than adults but they also can deteriorate quite quickly as well which is quite why you need to keep
26:15an eye on them and that's the the risk we have with children skylar spent a night being closely
26:23monitored on the children's ward the following day she went home with mum and dad
26:31so when do you do this today yeah yeah this morning in minor injuries 65 year old sean is awaiting the
26:50results of an x-ray after a fall down his cellar steps left him with a suspected broken toe like i was
26:57saying we're doing i don't don't sell around we're going to do it as a playroom for kids
27:02yeah wallpaper came yesterday is that for seller no oh that's for back bedroom for grandkids
27:13we're doing it to you know for them when they for them when they come yeah wallpaper and painting
27:21carpet but for now sean's diy is going to have to wait as emergency nurse practitioner claire has the
27:29results of his x-ray right let's have a look at the damage then shall we
27:37a little break in the big toe just there on the joint and looking at your big toe you've got bruising
27:44underneath which is in combination with that where it's broken emergency nurse practitioner claire
27:53has spotted something else that could be a problem for sean there's a joint here on your midfoot
28:00your lis frank joint which you need to be sure about that it's not a injury to that area this is
28:07something that we need to refer to orthopedics and then they will further assess the patient take a further
28:13detailed history and then most of these patients get a ct scan to see more in depth whether that
28:20actually i've got that injury when you go over like this you stress that joint there and that can
28:27cause fractures across that midfoot that lis frank joint which is quite significant for your mobility
28:32of your foot and your arch of your foot so we need to be sure about that
28:36they might do some weight bearing x-rays and then they'll potentially plan for him to have a scan
28:47so he'll probably go home in a black walker boot and come back for a scan another day i'm not too
28:52bothered about it you have a broken toe that's fine i expected it it's it's that extra bit on on top
28:59that she was saying it it affects your walking and all that hopefully it's just soft tissue and it's
29:07nothing nothing that's going to affect me right i've spoken to orthopedics and they've said that
29:15they'll see you on that area called estec okay so i'll take you down there they're waiting for you okay
29:21lovely thank you definitely no decorating for the foreseeable and no more sleepy slippers on the cell
29:30estate a further scan revealed no additional damage leaving sean free to carry on sorting out his cellar
29:38having spent her day organizing patient movements sister georgia is catching up with nurse olivia
29:54who's also having difficulty with patients on the move cubicle 16 yeah basically he's got some
30:01med school he's supposed to win this award i've done them got to his cubicle and he's not there
30:06have we tried ringing him yeah i've rang him it's gone straight to voicemail but he so he's if he's
30:11gone he's gone with a cannula ring so when somebody goes missing it's a bit of a nightmare really because
30:17you've got to make sure that the patient's safe ring them um hopefully they answer the phone and they
30:22say actually yeah i'm at home i'm safe i'm fine but if not sometimes we have to send um police or
30:29ambulance out to do a welfare check yeah we've been after those ring ambulance and tell him that he's
30:35got yeah with a cannula ring right with a patient missing in cubicle 16 there's a new arrival in cubicle
30:4519 85 year old eric has a badly swollen leg
30:52last night i said to my wife i don't feel so well and anyhow i managed to get upstairs
30:58about half past 12 but i couldn't get published and on bed and i couldn't lift up and all my legs
31:07fell late up healthcare assistant paul arrives to take a blood sample from eric so what's brought you in
31:17here today my leg i couldn't get i couldn't get him in and out of bed because i've had cellulitis
31:27and lymphedema oh i'm sorry to hear that but i've had sepsis no you've had sepsis before yeah so i'm
31:34gonna pop a needle in your arm and text somebody is that okay with you yeah whilst eric isn't hitting a
31:40high note today he was previously a singer i hear you used to be a next singer hi for a long time
31:48how long ago i did it for 32 years oh that's a long time did you enjoy it oh brilliant so where did you
31:56sing then did you sing all of it well oh oh i've been in tenerife benedon portugal or sunday wow
32:06but i've done all clubs in barnesley doncastle wigan sundland wow
32:13eric must now wait for his blood test results to discover whether his leg can be treated with
32:18antibiotics sister georgia and nurse olivia are trying to locate a missing patient
32:38obviously just check it waiting room because he could have just wondered and not known where to
32:42yeah i'll show you if it's a waiting room and then if not i'll read that if not yeah is that all right
32:50darren we've got a patient that's left with a cannula in but he's got no fixed aboard
32:56so i can't do anything right i'll just document it then and i'll document about
33:00because you can't tell the police to go yeah and we tried we tried um phoning him so
33:12that's what's going on in cubicle 16 68 year old janet has been struggling with persistent vomiting for
33:20four days sister georgia is looking after her
33:29and so you've been taking oral and sickness but you can't keep it down
33:34so this lady attended two days ago we have to have pain and vomiting she was diagnosed with a
33:51foot poisoning or stomach bug and she was given anti-sickness to take home but it's not stopped
33:56her um sickness at all um but the concern that this lady had was that she's on chemo medication
34:04for her cancer and she's not able to keep them down either just can't keep anything down but that's
34:10bad because you can't keep your medication now which i need to take that's what concerns as much as
34:17anything anything husband brian is keen to ensure janet gets the best possible care well we've been married
34:24four to seven years we've been together about 50 years
34:31i got married day after my 21st birthday we met when we were 18 you were 18 now 24 24 24
34:42we're a holiday romance

34:48so holiday romances do work sometimes if you work at them
34:55nurse ivana brings medication for janet right i've got your anti-sickness and some paracetamol
35:03so i've got some on danza how much is your anti-sickness okay and then some paracetamol

35:20hopefully stop her being sick which is what you want isn't it a minute last year
35:27are you managing to keep your water down no
35:30no no no no janet must now hope that the anti-sickness strip enables her to take her vital cancer

35:54sister georgia has been juggling beds patients and porters throughout his shift today
36:00we've got um quite a few ambulances that are going to be coming in as well so we need to free up some
36:05space so just escalated it to site to try and get us um some more support with porters and then
36:10hopefully we'll be able to get some patients out at department and clear a bit of space
36:15the availability of porters is improving just at the right time for georgia
36:21i'm sure a big stretch that sent me around here oh that was fast thank you somebody to go yeah
36:27oh right yeah we'll get them up yeah thank you mandy are you able to go up to ward with cube eight
36:36please thank you i think it's really important um keeping a flow in the department and it's really
36:44difficult sometimes i ain't got anybody to send with you at minute that's the only issue
36:50if we're down on porters um it can make your shift really difficult because you've got patients that
36:55are needing transferring to wards or for cts or x-rays and if there's a delay in that it impacts
37:02everything massively can you go away um look please resource from transfer from resource
37:16in cubicle 19 85 year old eric has an inflamed leg and he's waiting for the results of a blood test
37:25keeping him company his granddaughter lauren the good people hospitals when they were in before
37:35they were brilliant with me everybody eric wants to go home as soon as possible
37:40to see his beloved pet birds i've been doing all my life since i was a little boy
37:45uh pigeons canary's and ground i like talk up from right advanced clinical practitioner charlene has
37:55reviewed the results of eric's blood tests and has concerns so his inflammatory markers are raised so
38:00that would indicate that he has got some infection going up and it's like the cellulitis
38:04that's right she returns with bad news for eric
38:14we need to admit yeah for a couple of days
38:17it just is what it is you know we can't risk you getting poorly with sepsis but i'll stay with you
38:23a lot of time you know i'm all sorted i know it's not what you wanted to hear but we need to get you
38:28better whilst eric's keen to get back to his feathered friends advanced clinical practitioner
38:34charlene knows that keeping him in overnight is for the best sometimes that's a problem that we
38:39are trying to get people to stay when they want to go home but he's not well enough to be at home
38:43he needs to be in hospital he'll be settled on a bed soon thank you all right you take care
38:47see you later thank you bye-bye further tests confirmed eric was suffering from cellulitis
38:56and lymphedema he was discharged from hospital four days later
39:05back in cubicle 16 68 year old janet has ongoing sickness which is affecting her ability to take
39:13cancer medication dr hannah everett has now taken over her care obviously with the fact that you're
39:19on quite a few medications that we need you to be able to take and the length of time that it's been
39:24going on for and i think we might need to bring you in to hospital to give you some fluids through
39:30the vein and then also give you some antiseptics through the vein as well dr everett needs to perform
39:36a blood test on janet not looking like you've got brilliant veins from her no i'm sure it is
39:45you had it on the feet and everything yeah not good news for me that is it no no
39:59we're in luck tells you people's a date
40:01any questions or anything else no no no dr everett needs to discuss with a consultant
40:12whether janet should stay in overnight for observation
40:19but she said yeah she's not been able to keep her anti-emetics down so she's just continuing to
40:22vomit yeah what's your plan i've had a chat with the consultant and he agrees with me that we need to
40:28bring you in for some fluids and some for anti-sickness through the veins at least hopefully
40:33i should be able to get some meds down with her yeah janet's feeling better already and thinking
40:39about what brian can make her when she's ready to leave hospital yeah i'd probably have a honey
40:46honey glazed gammon steak what equals
40:53no i think she'll be fine a bit of fluids a bit of anti-sickness um she should start
40:57feeling a bit better yeah janet was taken to the ward and was discharged four days later
41:16that nice back in recess maureen has been waiting with her some matthew for the results of a chest x-ray
41:23so what i'm seeing on here is that her heart this is the outline of her heart
41:27it's much bigger than it should be and that her heart muscle doesn't pump as good as well as it
41:33should it's a bit like in your boiler the your boiler in your house is like your heart and the
41:39radiators are like your lungs i suppose and if the heart is not pumping very well then the pressure
41:45in the radiators backs up so you need the heart to be pumping the blood effectively around the body and
41:51also clearing fluid out of the lungs i'm reasonably happy things are settling down okay wow were i in
42:00trouble a little bit this morning why your heart was going a bit too slow was it yeah
42:07all right you look better want a little drink you look a lot better
42:21so you won't be going home today they might keep you in a couple of days
42:25what what they picked on me because you're beautiful
42:36i know i know everybody thinks that all right are we ready to go where we're going to ward all right
42:44you're going upstairs now to be looked after again thank you everybody why did you pick me
42:50i'm going to carry on where my dad left off uh my dad my dad did it with every ounce of his body
42:56and his strength and and and finally that got the better of him so i'm going to take over and hopefully
43:01make me dad proud and my mum proud as well maureen's heart was monitored on the cardiology ward for four
43:08days she left the hospital on reduced medication and is now stable
43:20after a non-stop day sister georgia's shift is coming to a close
43:24so we are still busy we've got patients awaiting ward um porterings got better we haven't got as many
43:36waiting to get up now um so hopefully we can just keep on top of it ready for nights coming on
43:42it's been a hectic day but there's time to relax before it all starts again i feel like some days
43:50you do have to kind of switch off because if you took things home that you see in the emergency
43:54department you you sometimes were really upset you see like challenging behaviors and things like that
44:02i am quite a chill person anyway to repair but um yeah you can't take it home with you so i'll be back
44:08tomorrow to do it all again hold down
