05:00his brain tonight after bleeding heavily from his nose his son called 999 what I've had it happen for
05:09it's usually stopped within you know 10 minutes 10 minutes but it's been half an hour yeah how much
05:16blood would you say that you've estimated a lot yeah pressure let me just pop that on there any
05:24headache no any blurred vision no no just the nose bleed Robert was in quite the mess when we first
05:31arrived on scene my major concern was the amount of blood loss and to make sure that his airway was
05:36clear well we'll see what we do we've got like this little clip that we can pop on your nose
05:41that can
05:41try and stop yeah the bleeding all right right this is supposed to clamp can you feel that pressure
05:50oh yeah it's gone it's not very tight though no let me try a bit higher up never get trying

06:04stopped bleeding now it's stopped dripping down there we go fixture can you breathe through your mouth okay

06:17because we had no other option to stop the nosebleed apart from him applying pressure himself and I was
06:22really worried about the amount of blood loss what we'll do we're just going to contact in our team
06:28we've got a drug that's a blood clotting drug yeah the fact that it's still going after half an hour
06:33and
06:33yeah it is losing quite a bit I'm thinking that that might be a go ahead if I'm honest just
06:38to see if
06:39that stops the bleeding all right as Robert has recently undergone brain surgery Amy needs to speak
06:47with a senior clinician before she can give blood clotting drugs I mean I'm just waiting for a response
06:54mate no worries it's just my reasoning is the fact that it's been continuous for over half an hour now
06:59and it's still yeah going quite yeah I'm going to take this off because it really isn't doing nothing

11:22We've just had a clot probably about three centimetres by three centimetres round
11:26just come out of his mouth.
11:28He said it was up his nose.
11:29It was up his nose and then he felt like it's gone into the throat and spat it out.
11:32And it's about that big.

14:32try and get him feeling back to himself.

14:38through that vein, OK?

15:13We do believe that he had broken his hip,
15:15so in order to move him,
15:17we knew we were going to need to give him some strong pain relief
15:19just to try and keep him as comfortable as we can
15:22during the process.

16:11but it was just a short-term solution
16:13until we got him sorted.

20:48all the time the call came in she's woken up with chest pain that can be quite concerning
20:54she's never a good sign we're only nine minutes away a bit tight it's scary isn't it some of the
21:02rural communities and how isolated and vulnerable they actually are this was potentially quite a
21:10serious case just given her age given the location of where it is and the amount of time it would
21:15take to get there yeah the location of a property was up a really long narrow road which just made
21:21it extra hard to get to her so what's been going on then well i've got a heart valve problem
21:31and you
21:32know waiting to have it done but oh tonight it really got me i woke up and i got this
21:39real
21:41pain and my heart was racing
21:46when racehorse breeder rosalie was woken in the night with pains in her chest
21:50she rang 999 husband peter is with her
21:55what time did she wake up with the chest pain though
21:59about three o'clock do you have an irregular heartbeat by any chance
22:05i can see they've they put it in she's got aortic valve stenosis rosalie was experiencing some chest
22:13pain which could be caused by her pre-existing heart condition aortic valve stenosis is the
22:19narrowing of a valve within a heart which affects the blood flow into the arteries it effectively means
22:26the heart has to work a little bit harder so you've got palpitations in your chest at the minute
22:29sweetheart no no no did did you have okay the longer i spent with rosalie the more concerned i got
22:37she was telling me about needing the heart operation how she's now got chest pains and palpitations
22:42so the longer i was there the more concerned i was becoming for her nice and still a second
22:49adam carries out an ecg to check the electrical activity of rosalie's heart
22:54yeah we'll be going you're going to hospital sweetheart is it all right or is it you've got
23:00a heart rate at the minute it's about 160 yeah it should be ideally between sort of 60 and 100
23:06it's very fast basically i knew something wrong anyway i've been getting short of breath
23:12have you i'm not surprised with a heart rate like that so it's saying stemmy
23:19yep so we're going to have to ring yeah we'll give him a buzz once we completed a heart tracing
23:24on
23:24rosalie it showed us that not only was her heart beating pretty fast at over 160 beats per minute
23:31but it was also showing us that she was actively having what we call a stemmy which is a heart
23:36attack
23:45as the west midlands sleeps we're on board with eight paramedic crews working the night shift until
23:516 30 in the morning we are going to be taken at a hospital and we are probably going to
24:00be going
24:01quite quickly on the outskirts of stoke paramedics colleen and adam with rosalie who they suspect is
24:09having a heart attack i'm just going to give you a spray on your tongue if i can yeah this
24:14will hopefully
24:14take some of the chest pain away adam gives rosalie a spray under her tongue this opens up her blood
24:21vessels reducing the strain on her heart we're going to look and see if we can give her aspirin
24:27and we're going to look at giving her a needle in her arm as well i'm just going to pop
24:30in your hall
24:31and make a phone call okay colleen calls the hospital to let them know they'll be arriving soon with a
24:38patient who's having a heart attack so i did the right thing calling oh yeah i don't like to think
24:44i'm
24:44wasting anybody's time nope you have definitely done the right thing by conch they said go to
24:49a and e initially but give resource a quarter to go there which a and e stoke rosalie has to
24:55go to
24:55hospital because she's actively having a heart attack she needs further treatment that we cannot
25:00provide her it's imperative that we get her there pretty quickly to get the treatment she needs as
25:04fast as she can we're going in on blue lights we're going to be going quite quickly she is unwell
25:09so it's up to you what you choose to do but if you do you're doing your job anyway that's
25:14all
25:14they'll look after me i know that we'll get onto the ambulance and we'll get you to stoke as quick
25:18as we can okay okay no no absolutely not no all right absolutely not well your heart rate's already
25:26going to 160 try and get you up and about you'll be on the floor i find with the older
25:31generation they
25:32don't want to make a fuss they just want to get on with things but when something like this is
25:36happening
25:36you really should make a fuss about it

25:47well you know your job indeed how are you feeling not too bad so like we said we will be
25:55going off

27:08she absolutely needed to though no didn't like as soon as i seen that ecg yeah all right that's not

27:21chilled yeah that's what i mean way more than i'd be bless her don't make them like her anymore do

27:44the moment every minute of every day the call assessors of west midlands ambulance service are
27:51working hard to save lives there is always something that is completely out of the box and
28:00you're not expecting it and i think that's what makes this job so interesting ambulance service is

28:14yeah is she breathing yes yes okay are they conscious are they awake she's panting she's ready

28:27to it no i don't think she is okay is is her breathing noisy or abnormal it's like gargling heavy

28:46okay right okay okay caller listen to me it's really important that you follow my instructions

28:56is she breathing normally no not really as soon as i hear the words that they're not conscious and not
29:03breathing it's almost like a wall comes down around me and i'm completely locked in on that call
29:10nothing else around me matters okay we've got some help arranged i need you to listen to my
29:16instructions okay are you within easy reach of her yeah somebody's on the roof uh she's on a roof
29:24she's on a what sorry not like a lay a roof ledge she's on a roof ledge yeah when i
29:33heard the patient
29:33was on the roof i think i was genuinely really confused but it just changed things in the moment
29:39then it meant that i needed to then think about how we were going to get the caller to the
29:44patient safely
29:45to do cpr so when you say she's on the roof ledge what do you mean is it is it
29:54a ledge on top of the
29:55house is it like a balcony out there yeah like like a roof can you see the ambulance caller are
30:04the
30:04paramedics there one on your left door on your left hello caller yeah is that the crew with you
30:14yeah okay i'll leave you with them even though we get calls quite often for people in unconventional
30:24places it's not often that you get somebody that's on a roof unconscious not breathing needing cpr
30:32and trying to figure out how they're going to get to that patient minutes later control has informed
30:39the woman was trying to take her own life we've got some updates on that case so she's alive
30:48they've now got her off the roof fire were there as well i assume that's to help get her down
30:54yeah because they weren't quite sure how they were going to get her down to be fair
30:57i just couldn't couldn't get my head around it no it just didn't make sense as to why she would
31:01be
31:02on the roof and she's on the back of the ambulance so hopefully she'll be okay
31:22are you sweet rather than savory then if i have a tub of ice cream
31:27there's no like scooping it out and putting it in a bowl i just eat all of it do you

31:33annoys me though about say if you've got ice cream and you get it out the freezer and you've
31:37got to wait for it to defrost a bit and your spoon just bends because it's the ice you put
31:42it in
31:42the microwave wave it for about 10 15 20 seconds depending there has been a few mishaps on a couple
31:49of occasions where it has been too soft so then do i eat it or do i put it back
31:57in the freezer to
31:58freeze up a little bit more but then am i going to be starting back at the same point where
32:03i'm going
32:03to have to put it in the microwave this is too much it's a science it's it's a science i
32:08do like ice
32:09cream when it's all running though i'll purposely mix it until it goes i used to do that when i
32:12was
32:12little kid i don't want it sloppy now i want it just nice

32:25category two 70s lady with something very unfortunate she has rectal bleeding
32:35she's got diverticulitis as well right okay so that could be a factor a lot of people struggle
32:43with that don't they yeah yeah diverticulitis is a condition in the bowel which can cause bleeding
32:49and discomfort and sometimes a lot of pain we knew this lady had been bleeding and so it's
32:55quite important that we found out what was going on hello what's been going on sweetheart i've been
33:10having bleeds i've got diverticulitis okay i'm used to having yeah small bleeds i had to do a runner to

44:38a plate in his hip had come loose he spent four hours in surgery having it screwed back in place
44:47rosalie who was having a heart attack was admitted to the cardiology ward
44:52here she received treatment for blocked coronary arteries
44:57robert whose nose was bleeding heavily spent six hours in hospital the bleeding was stopped
45:04and he was prescribed a nasal cream to treat any possible infection
45:09and maddie who had rectal bleeding spent three days in hospital she may require surgery to repair her
45:17intestines we went straight through to see a consultant who was brilliant so hopefully
