An internal investigation has revealed negligence in the runaway train incident that occurred on the Tenom-Pangi railway line on March 3.

The Sabah State Railway Department Board of Enquiry, which convened on March 9, found the train driver failed to conduct a proper inspection before beginning the journey from Tenom.

00:27said the driver also failed to close the isolating valve at the Tenom-End driving cab and did
00:33not carry out a dynamic test to ensure the braking system was functioning properly.
00:38He said the negligence caused the braking system of Railbus RB8801 to malfunction, resulting
00:45in a collision with motorcycle trolleys at kilometre 134-4.
00:49In the incident, at least a dozen passengers, mostly primary school teachers, escaped unhurt
00:55after villagers quickly placed a motorcycle trolley on the tracks to stop the train, whose brakes
01:00had apparently failed along the hilly Tenom to Pangi railway line.
01:04After the train came to a stop at kilometre 133-8, the driver inspected the system and corrected
01:11the position of the isolating valve.
01:14A subsequent test confirmed the braking system was functioning properly, allowing the train
01:20to return to Tenom station without requiring rescue or towing.
01:24Railway services resumed normal operations a day later using the same vehicle and have
01:30since been operating without issues.
01:33Ewan said disciplinary action will be taken against the driver following the findings of
01:38the Internal Investigation Committee.
