00:09hemorrhage at a Birmingham hospital. Craig Green had a scan at the Queen Elizabeth Hospital
00:14months before his death, which showed a brain aneurysm, but neither he nor his GP were informed
00:20at the time. The hospital trust said there was communication failures by staff involved in his
00:26care and during the wider referral process. An inquest concluded the death of Mr Green was
00:31from natural causes, but the family say he had a right to know. And the trust says changes are
00:38now being made across services to prevent a repeat.
