00:00The new digital hospital passports are now available at University Hospitals of Morecambe

00:05NHS Trusts. You can complete the passports online, on your smart device or send us a paper copy if

00:13you'd prefer. You can let us know about your preferences, how you like to be looked after

00:18and all the things we can do to help make any hospital visits less scary and more comfortable

00:23for you.

00:30Once you've filled in the passport online, you can save a copy for yourself. Meanwhile,

00:39a digital copy will be automatically uploaded to your healthcare record and shared with the

00:44community healthcare teams and other healthcare providers so that everyone can help give you

00:48the treatment you deserve and save you repeating the same information wherever you go.
