10:51John's ECG shows that his heart is functioning normally
11:01so it's still unclear what caused him to lose control of his vehicle
11:05John will be taken by Land Ambulance to Barnsley Hospital so he's easier to monitor
11:37so we've already got IV access
11:43for the air ambulance teams training is an integral part of the week
11:55charge back on the chest
12:03they practice medical procedures on mannequins to help prepare them for real life situations
12:09and today at the top cliff base paramedics Wayne and Sam are setting up for a training exercise
12:16three two one
12:18or attempting to
12:20what we're gonna do with a one arm and one leg dummy
12:27but now Dave on the air desk has a real job for them
12:3099 send a job through to the car if you've got it
12:33it's an arrest in Ripon
12:35the helicopter is undergoing essential maintenance today
12:39and so the crew will be taking the bases rapid response car to the job
12:43both bases have a car permanently on site as a backup for when the helicopter can't fly
12:49these cars can often reach a patient faster than a land ambulance
12:54when the helicopter's offline we've got the rapid response circle
12:57so we can respond to jobs in a smaller area
13:00but still bring the same capability that we would if we were on an aircraft
13:04their patient has gone into cardiac arrest while shopping in Ripon
13:0810 miles from base
13:10go ahead
13:12yeah just an update
13:14there will be a first responder on scene in the shop
13:17a patient has had three shocks
13:20three shocks so far
13:22Roger thank you
13:23community first responders are trained local volunteers
13:27who can be dispatched by the ambulance service
13:30to attend emergencies like cardiac arrest
13:32we knew we were going to a cardiac arrest
13:34their heart stopped beating and stopped breathing for themselves
13:37so that is the worst you can possibly get
13:40this patient had had three shocks and CPR was ongoing
13:43so we knew this was someone that minutes mattered
13:46the first responder is still giving CPR with the patient's wife watching on
14:03we'll just get everything sorted out mate
14:05and then we'll just step back and get a hand over
14:07is that alright
14:08remember to push hard
14:10do you want Steve to take over CPR mate
14:13yeah I'll take over mate
14:14ok alright
14:16you get that stuff cut off and I'll
14:18then you can take over it
14:20seventy-five year old Richard Sykes was at the till when he collapsed
14:24ready
14:25there are the electrodes
14:27ok let's take those pens off
14:29you have one minute
14:31you are just moving your AED away mate
14:33you've had three shocks is that right
14:34Steve can you get the bag open for us
14:36thank you
14:37shop staff start at CPR within seconds of Richard's collapse
14:41and the first responder has been using a high street automatic defibrillator
14:46to try and shock his heart back into rhythm
14:49I agree
14:50when you go into cardiac arrest the clock's ticking
14:53for every one minute of time that goes by
14:56it's a ten percent less chance of survival
14:59so early bystander CPR
15:01early defibrillation and early advanced care is really really important
15:05you are helping our Sam with the airway mate
15:08can I delegate access to you mate
15:10Sam is keeping Richard's airway clear by tilting his head back
15:14he's using a bag and mask to give him oxygen
15:19so we stamped ROSC now mate
15:21air desk minute nine eight ROSC at this time thank you
15:24ROSC is a return of spontaneous circulation
15:27so in a cardiac arrest a patient isn't breathing for themselves
15:31and the heart isn't beating for itself
15:33so as soon as a patient's heart starts beating
15:35we would call that ROSC
15:37Richard's heart is beating again
15:39but he's still not breathing for himself
15:41so what do we know?
15:43well we know that he is a seventeen five year old gentleman
15:46who's had a background of AF and previous stents
15:48and he's just collapsed while shopping at the minute
15:50ok so we've got a small pulse
15:52you're doing the access there
15:54Richard had been doing the weekly shock with his wife Carol
15:58he just collapsed at the till
16:00and what's his past medical so he's got AF
16:03he's got two stents
16:04ok had a cardiac arrest before
16:07Richard?

16:16Richard was still really unstable and really really poorly
16:19the survival rate for an out of hospital cardiac arrest in the UK
16:22is around eight percent
16:23and that's across any age of any patient in the UK
16:26so for Richard who had a previous significant cardiac history
16:30and was of the older generation
16:33his outlook we were really really concerned about
16:36have we got anything on GCS while is it?
16:38three
16:39no he was blinking in his eyes
16:40but I don't think it's anything meaningful wise
16:47see how he's breathing for himself at the minute
16:49not sure this is going to work fantastic though
16:54I think there's probably going to be some form of reflex with this
16:58with Richard's conscious level low
17:00one of the crew's biggest priorities
17:02is to keep his airway open
17:05so Sam is going to attempt to intubate him
17:08which involves inserting a tube into the top of his lungs
17:15tube please
17:16can I have the tube?
17:23okay we'll come out with that
17:24come out with the tube?
17:25yeah that's fine
17:26with Richard we had to do a manoeuvre
17:28just to be able to open his airway
17:30by passing a tube through his vocal cords into his lungs
17:33so intubating him
17:34normally with unconscious patients they'd tolerate this
17:37but because Richard was increasing his level of consciousness
17:40he wasn't able to tolerate that intervention
17:42I think we'll just stick on with this you know
17:44and just see how he does for now
17:45he's slowly regaining consciousness I think a little bit from my own eyes
17:51it's vital the team continue to manage Richard's airway
17:53so that oxygen can reach his lungs and bloodstream
17:58right let's prioritise packaging him up then shall we
18:00once we get him on we'll reassess and do another ECG
18:03see if it's settled down then we'll just make some decisions on where we're going okay
18:06eye gel size 4 in case he re-arrests
18:10we've got an output back so his heart's beating
18:14right which is good and he's breathing for himself to a certain extent
18:18so we're still having to support his breathing a little bit
18:21but it's coming round better than we'd expect really so that's good
18:26he's still really really poorly obviously
18:28so his heart had stopped for a period of time and he wasn't breathing for himself
18:31as you know he can't really hear us at the end
18:33he's not able to respond to anything we're doing
18:35so he's still really really in an unconscious level
18:37so we're going to take him to Harrogate Hospital
18:40and they're going to do some scans from there
18:43but in terms of everything of how it's looking with us at the minute
18:46well you've got him bad
18:47yeah he's had every possible chance
18:49yeah I see them being there
18:52yeah

18:57a bit shaken up because he's
18:59I've literally spoke to him a minute before he collapsed
19:01and his last words to me were
19:03I'm just going to sit down here and wait for me why
19:05it's a sudden collapse
19:06and then suddenly out the corner of my eye I saw him keel over
19:09yeah
19:10and we got him into the recovery position
19:12yeah you've done a good job mate
19:13alright well done
19:14thanks to Malcolm's swift actions to get Richard's CPR
19:18he's been given the best chance of survival
19:20but he still needs to reach hospital
19:22this chap's had a cardiac arrest at the tilt
19:24with immediate bystander CPR
19:26as we got here he'd actually got a pulse back
19:28the community first responders done a fantastic job
19:30and given him a number of shocks about six shocks
19:33he had a weak pulse but it was output regardless
19:36and he's continued to improve quite rapidly
19:38although he's not regaining consciousness at the minute
19:40so we're going to keep an eye on him and go to Harrogate Hospital
19:43Sam and Wayne will monitor Richard in the ambulance
19:55he could go back into cardiac arrest at any moment
19:58it's a 25 minute drive on blue lights to Harrogate
20:08where he'll be taken straight to Rhesus
20:10Richard's wife Carol is already at the hospital anxiously awaiting news

20:28yeah you know he's been no different to when we got here
20:30his numbers are getting better and he's starting to make some movement
20:32so he's gone the right way
20:34he's obviously still quite unwell

20:38I think CPR it can't be underestimated really can it
20:42you know it's so important that everyone knows how to do it
20:44and from the manager of the store where we were
20:46and the CFR who was just around the corner
20:48that immediate bystander CPR and immediate access to shocks
20:51that's really what saves lives
20:53we just come and do some other stuff
20:55but you know those are the really important bits
20:57that's going to give Richard the best chance
20:59so he's had every fighting chance possible
21:02so fingers crossed he makes a good recovery
21:21while you were out I made a nice cherry pie
21:24on the air desk at the Nostal Base in Wakefield
21:27Gemma's got an important job for paramedic Sam
21:30I'm busy on the desk now

23:23and what injuries they actually have
23:24so you're always thinking the worst especially with motorcycle incidents often they're either fatal or quite significant injuries
23:30so although you may hear that they've only just got either a broken leg or something like that
23:35it's always going through your head that actually this patient might have more injuries that we need to find

24:3432 year old Greg Mason was only minutes into a motorbike ride with his friend when the crash happened
24:40although we could see Greg had a broken leg we needed to make sure that there weren't any other injuries that we were missing
24:45motorcycle incidents can cause a lot of damage so being able to rule out any other potentially life threatening injuries is really important
24:51what we'll do we'll get a little needling and we'll give you some painkillers okay

26:45additional to what a road ambulance would carry
26:47one of those is ketamine
26:49in this circumstance we used it as more of a sedative analgesic effect
26:54and it works really really well it works really quickly
26:57and it allows us to be able to get Greg to such a place that we can straighten his leg
27:01and cause him essentially no pain

28:43the team are using a Kendrick traction splint
28:45a device that realigns broken bones by applying a pulling force
28:49so with a broken bone especially a broken femur you get two bits of bone which rub against each other
28:56that causes a lot of pain but can also cause damage to internal structures like blood vessels or nerves
29:01so a Kendrick splint allows us to be able to put that around the leg and essentially pull the limb straight to where it would have normally been
29:08okay everyone ready

29:21Dan has straightened Greg's leg which will help with the pain in the short term

30:48although Greg had a broken femur and was in a lot of pain
30:51he was still really really worried about the bike
30:53and I think that just goes to er
30:55goes to show how much it means to him

31:12it's just an isolated femur
31:14closed so he's going to go to Barnsley with the crew

31:17we are clear on seeing returning to the aircraft
31:19Greg's been really lucky
31:20thankfully the bone didn't come through the skin
31:22it was just a closed femur fracture
31:23and there was nothing else that we could find of any significance
31:25he now seems to be quite pain free in a jolly mood
31:27and he's going to go off to Barnsley with the crew
31:29it was a character anyway
31:30and then you give him ketamine it just made his character even more
31:32but er
31:33yeah

33:02and then fly her to the right hospital the first time
33:04so a major trauma centre
33:05it also gives us an ability to look from the air as well

33:08when you're trying to find someone in a rural environment

36:16if we unnecessarily move it we could turn it into an open fracture
36:19where the bone ends stick out
36:20that could cause more bleeding
36:22you can get damage to the nerves and the circulation of veins and the arteries
36:25that supply a blood flow below the fracture
36:28if we start moving it around so that it's important that we do splint it
36:32to stop the bones from moving and it provides pain relief as well
