03:21he's burnt his pants he's leaked in his feet and his stomach a devastating accident at a family
03:28beach house has left a 74 year old grandfather with shocking burns we're just running water on him
03:35at the moment just keep him under that water okay yep i'm organizing help for you now
03:40all righty mim we have a burns case oh yeah let's have a look as the closest ambulance to the remote
03:53coastal location mim and ben raced to the time critical case we have a 74 year old male spilled boiling water
04:02on hands feet and abdomen they are estimating it's more than 18 percent of the body area burn though
04:08so that's like level one trauma yeah already my alarm bells are ringing because burn jobs can be
04:14really really complicated first of all how much damage to the tissue is there are we talking loss
04:21of toes loss of limb it's going to be a difficult one to manage potentially as well as that skin keeps
04:28the heat in the body so a burn injury can lead to someone becoming hypothermic which is a life
04:34threatening condition well i wonder if he's maybe been cooking it's like a pot of pasta or whatever
04:39he's dropped it on him maybe yeah oh poor thing it's so painful there's a chance he'll need skin grafts yeah

05:45back in the city it's also a big night for intensive care paramedic lisa my very first shift back i'm
06:04feeling a bit nervous but i'm excited to get back out there see my colleagues and practice the job that
06:11absolutely love for the past year lisa has swapped paramedic duties for a new role as a first-time mum
06:19i've absolutely loved having the last 15 months at home with my baby boy sully it's been amazing to
06:25see him grow and to see his curiosity for life and i'm definitely going to miss him

06:45i've always been obsessed with babies and children but being gay it's hard to know what that journey
06:53would look like and i knew that i had gynecological problems with endometriosis as well so it was going

07:11yeah but finally lisa decided to give ivf one last try

08:07at the emergency operation centre a worried mum is on the line after finding her baby convulsing in her cot
08:16she's laying my husband's arm is she breathing she's breathing but heavily and it's actually pulsating

08:30on lisa's first day back from maternity leave a paediatric emergency is a challenging first-up case
08:42it's incredibly scary for the family is to see this type of thing
08:46seizures in young children have a wide range of causes from a high fever or infection to head trauma and epilepsy
08:55oh we're just about to approach
08:57any seizure in a baby can be life-threatening
09:00the shaking the loss of consciousness it means that they are not really protecting their own airways

09:52i've just finished cooking with crabs
09:53yeah and i've got the boiling water that's left in the bucket

10:14malcolm's trembling like a leaf i can see that the skin is already peeling off
10:20so malcolm needs to see a surgeon immediately
10:24i think he's fallen and this arm has come over the top of the pot

10:32malcolm's close-knit family was enjoying time together at their beach house when the accident happened
10:38have we done any calling guys cold water 25 minutes 25 minutes thank you just with the hose
10:44his quick-thinking daughter-in-law sarah jane immediately grabbed the hose to cool her father-in-law down
10:50i reckon if we go nine percent there then i reckon if we go four and a half we've got another one there that's another five percent

11:03yeah we've got potential circumferentials
11:06the severity of malcolm's burns means that he's at the risk of developing what we call compartment syndrome
11:13this is where the skin becomes really tight
11:16the blood is essentially cut off from the end of the limb and it will eventually die
11:21the worst case scenario is that he's going to lose his toes his feet and potentially his hands
11:27i'll get the bed if we just get him in the back and just get the heater on because he's he's cold
11:32as well as the risk to his limbs mim and ben are increasingly worried malcolm's life could be in imminent danger from a rapid drop in body temperature
11:43hyperthermia is a huge risk in geriatric burns his body doesn't have the ability to regulate his internal temperature as well as someone that's a bit younger
11:50so we need to get his temperature under control and we need to get moving now
11:55we'll get you in our ambulance and warm you up all right
12:02you're about the same age as my little one
12:10intensive care paramedic lisa is on an urgent call out for one-year-old aliana
12:16after mum vicky found her having a seizure
12:21okay what's happened mom
12:23um actually she went to her
12:25yeah and um everything was normal
12:28the next minute she was crying we went to her
12:30yeah and i felt she was really hot
12:32tried taking everything off and then i couldn't tell that she was just not
12:35not quite responding
12:37yeah
12:38oh you've done everything right okay
12:41i can see that eliana's seizure has stopped
12:44do you want to hold her and get her a cuddle
12:47but she's looking really floppy which is incredibly concerning
12:51so i need to take some observations to work out how serious this is
12:55all right we'll check her temperature

13:00she's got so much hair

13:07i'm shocked 40 degrees is extremely high extremely dangerous
13:13it's no wonder she's had the seizure
13:15my concern is that she'll have another one
13:17she probably has an underlying illness
13:19it's a matter of trying to work out what that might be
13:22all right so we'll monitor her heart now
13:25with her temperature skyrocketing
13:27lisa's worried eliana's body is fighting a potentially life-threatening infection
13:32mama's here i'm just gonna put this one here
13:37if her heart rate is elevated and with her temperature so high
13:42these are signs that she could be in a state of sepsis
13:46which is when nasty bacteria has got into the bloodstream
13:50poor thing so yeah her heart rate's up
13:55eliana's heart rate comes back at 200. this is double what it should be
14:00we want to get panadol into her
14:04the next thing that could drop is her blood pressure
14:08or her level of conscious state
14:10that's incredibly dangerous
14:12now that that paracetamol's gone in
14:14it should take effect and start lowering that temperature
14:17lisa has called for urgent ambulance backup
14:20to transport eliana to hospital
14:22fortunately it's only minutes away
14:25you've got a lovely family around

14:39vicky is a rock she's being incredibly strong
14:43but i can see some really concerned eyes
14:45i really feel for this family and the concern that they've got for eliana right now

15:00if she has a really nasty infection that has got into her bloodstream organs can't cope and they start to shut down

15:33do we want to get some plastic bags on his feet and then he can take a stand because otherwise they're going to get dirty and infected
15:38but before the 74 year old grandfather goes anywhere
15:42mims come up with a practical solution to protect his raw skin from infection

15:53burn injuries have lots of serious complications
15:56if they develop infections they could end up with sepsis
16:00he's going to need a good clean
16:02so plastic bags for his feet is not textbook

21:13is it all partial thickness or is there some full thickness

21:17we've got potential for a circumferential foot burn
21:19around the fingers on the left hand

21:23a circumferential burn is time critical
21:25because if you burn your limb
21:27all the way around that can stop your blood supply
21:29due to the swelling
21:31which can cause tissue death
21:33so malcolm could lose a limb
21:35that one looks like the worst i reckon
21:37his skin's coming up all over it
21:39all the way up to the nail

24:16at the emergency operations centre a distraught teenager is on the line
24:21he's just witnessed his mate's disastrous attempt to scale a two metre high mental school fence
24:27the gate to the back usually they unlock it but it was locked
24:31so he tried jumping over and the spikes on the top of the gate cut straight to his arm
24:35just confirming where is the wound on his arm
24:38his entire bottom of his arm is cut open
24:41it is bad
24:42okay i'm going to get you some help just stay on the line please
24:45just try and be quick it looks really bad
24:51alright Sam we are off to a college for a 17 year old
24:56well there's not much information but it says lower arm is ripped open and they can see the muscle
25:02oh wow yeah
25:04this patient is at a real risk of going into hypovolemic shock
25:09meaning he's lost so much blood that he can no longer supply enough blood to all of his major organs
25:14which will mean they will start to shut down
25:18clear left
25:20as well as extreme blood loss the boys are worried about the long-term damage to the teenager's arm
25:26they would have got arteries veins nerves
25:28the other thing is this could be a huge infection risk as well
25:32he's only 17 and to potentially have to lose your arm at that age that's
25:37that's catastrophic
25:39i'm really worried that he could one lose function in his arm
25:42or two potentially lose the arm altogether
25:45which would completely change the course of his life
25:51is this guy for real?

26:08i just picked up my leg hurt so i can't get up i can't stand up
26:13a distressed woman is calling in to the emergency operations centre
26:18after an agonising fall in her front yard

26:29try to stay as still as you can until we can get that help out there okay
26:32intensive care paramedic bill is now on his way lights and sirens to the 72 year old
26:42they're concerned this patient might have a fractured femur
26:45the femur is that big long bone that makes up the majority of your leg
26:50and if you do break that that's extremely painful
26:53and it can actually lose a lot of your blood volume into that leg
26:57if it is broken sometimes the bone inside the leg can actually sever an artery

27:03poor lady
27:04she then will have a huge lack of blood supply and oxygenation to that leg
27:08and worst case scenario she could lose her leg
