01:38coffee orders i already know your coffee order paramedics aaron and laura's afternoon shift
01:54is just getting started if you know my coffee order what is it i'm pretty sure from memory
02:01it's an almond flat white or an almond latte no no that's changed since last year you're definitely
02:15an almond latte half shot with a dash of caramel a little bit of char spice on the side
02:21that sounds delicious and not as full as normal double cupped as well wow it's hot
02:27but the light-hearted mood quickly shifts
02:33when a disturbing job comes through aaron we are going to a 36 year old female
02:42diagnosed with surgery cervical cancer bleeding on the floor in a big puddle of blood
02:49she's so young 36 yeah that is young really young i'm extremely worried for this 36 year old not
02:59just because she could potentially be bleeding out but also she's been diagnosed with such an awful
03:04cancer and at such a young age i think i like how much blood has she lost like it's a non-compressible
03:11bleed so it's really hard to put pressure on and stop it i'm really worried about how much she's
03:18bleeding and why is her diagnosis getting worse because at the end of the day blood loss kills
03:25people and if it doesn't stop your heart doesn't pump your brain doesn't get the blood so she's got to
03:30be terrified
03:40as the ambulance tell me exactly what's happened um yeah my partner ate like an extremely hot chili
03:47and had like a crazy reaction to it he's like crying and living in pain across town a dare
03:55devil's fiancee is on the line at the south australian emergency operation center is he responding
04:01normally no well we're coming lights and sirens we're going to be there as quickly as we come
04:06intensive care paramedic lisa is immediately dispatched so we're going to a young gentleman
04:17who's had a chili incident by the sounds of it can't say i've ever been to something like this before
04:22he's named a scorpion so it sounds pretty deadly he's experiencing burning significant pain to his
04:30abdomen pretty dire consequences a really hot chili it can cause ulcerations as it goes down and that's
04:38excruciating so a bit of an update the scorpion type chili is the one of the hottest chilies in the world
04:47our communication center told me that scorpion chili is 400 times the heat of a jalapeno
04:53which is it sounds deadly i'm lisa mackenzie you obviously haven't had any no no i'm a sweet chili
05:08kind of girl mackenzie's fiancee james consumed the homegrown chili at a friend's house james yeah yeah
05:16hey mate i'm lisa and i'm greeted with someone that's naked oh he's just groaning and writhing what
05:23happened i ate a chili you ate a chili yeah okay oh nothing in your tummy the fact that he's in
05:30excruciating pain suggests that he's he's actually done some serious damage and how much of it did
05:36you consume the whole thing you ate a whole chili yeah oh my gosh did you know what might happen
05:42yeah i didn't think this i mean like yeah all right do you have any allergies no that's all right
05:49okay when you see someone in excruciating pain like that you start to worry what's actually
05:54happening on the inside it's like stabbing or like you just stabbing pain just burn all through the
06:00bottom yeah yeah
06:03aaron and laura are on their way to a mum of two who is fighting cervical cancer
06:19reckon that one just there the 35 year old's partner david called triple zero when she started
06:26bleeding heavily who are we here for i'm my partner coming coming yeah coming the house is lit up like
06:35a christmas tree there's toy cars out the front there's funny signs there's everything going on at
06:40once you've got the two young lads playing your ipod just trying to keep them out of the kitchen
06:45fair enough sit down yeah come in man hey hey hi hello hi tammy my name's laura i've got my partner
06:55aaron what's going on um i had stage three salutes cancer okay and i cough really hard okay and i bled out
07:04straight away i've been bleeding since and i i said to my partner leisurely and able it because i felt well
07:08faint you've just felt it continually bleeding yeah okay there's blood from the kitchen floor to
07:15the bench to the chair it's more confronting than i expected i made myself lay on the ground
07:21okay it was safer for me too because you felt quite faint and are you undergoing like any treatment or
07:26anything for it my treatment plan is just about to begin so you've just been diagnosed recently um
07:31it was new year's eve we realized that i had stayed so it's always cancer okay it's spread now to my
07:36lymph node how did you get diagnosed i thought i had a miscarriage and the doctors tested me and
07:44went no there was no pregnancy there right and then they wanted to figure out they started all the
07:48scans yeah cervical cancer is such an awful disease and such an aggressive form of cancer
07:55tammy's in her 30s she's got two young kids it's hard for anyone to hear that diagnosis especially with
08:00someone who's got so much of their life yet to live you've never had to have like a blood transfusion or
08:06i've had one blood transfusion already because of this yes okay all righty what we'll do we're just
08:10shaking your blood pressure if it's good i'll get you to sit on the chair it's quite difficult when
08:14someone's bleeding internally to be able to compress that bleed it's not like if someone had a bleed
08:19from their arm and you can kind of put pressure on that so it is really just about getting there
08:23somewhere where they can give them red blood cells or operate if need be cool your blood pressure's good
08:28so we'll just slowly work towards setting up and there is a bit of blood on the floor so don't
08:34sleep yes we're on the ground yeah great mate thank you despite tammy's radial pulse being there
08:40it doesn't mean she's stable you're doing well jesus tammy's numbers can change in the blink of an
08:52eye nice slow breaths nice and slow good work yeah are you dizzy or yeah okay that's it well done
09:03you're all right we'll look after you you can suddenly go from being good to crashing and
09:07she can drop at any stage i don't know how he's going to get me out of here
09:11i ate a chili and how much of it did you consume
09:2329 year old thrill seeker james decided earlier today to eat a scorpion chili
09:29one of the world's hottest and is now paying the price you ate the whole thing did you cut it up in
09:35pieces or you took one bite and then a second bite all right intensive care paramedic lisa is
09:41worried her patient is burning up inside then what happened after that you swallowed it like obviously
09:47yeah yeah you can send it it was like hot were you salivating and stuff like that like yeah yeah um
09:53so that's past that bit yeah yeah that was like hours ago and then yeah i went to my nap
09:59i woke up to like insane pain pain in the lower abdomen yeah all right buddy this chili is insane
10:06in its hotness 400 times the heat of a jalapeno we're gonna get you sorted okay i just need to take
10:13some quick observations but i think you need some pain relief okay the lining of the tummy is quite
10:18sensitive and you can cause a kind of a chemical burn or a ulcer so it was called a scorpion scorpion peach
10:27chili oh a red one or like yellow like it looked like a peach actually he just forgot i never sleep
10:35oh god mate he just like yeah popped it and went home your heart rate's on the lower side and your
10:42blood pressure's borderline as well so no wonder you're feeling absolute shit so any intense tummy
10:47pain can kind of drop your blood pressure and heart rate james is in absolute agony he's quite sweaty
10:53quite pale he's just trying to find that comfortable position which is not happening for him so my
10:58immediate thoughts are i need to get some pain relief into him as soon as possible now i've got
11:03this pen throw so um you put your lips around it suck it in out through your nose because you're in so
11:10much pain we have to take you up to hospital okay yeah the ambulance is here so we do want to kind of get
11:15you onto a bed a backup crew has just arrived to help get james to hospital the bed is by your front
11:22door are you feeling strong enough to get onto the bed yeah let's just do a sprint yeah awesome let me
11:27just yeah and recommend eating one of these mate i would on a unlike it's stuck i think i'm gonna pass james
11:37will eat any single chili that he can get his hands on he thrives taking risks you know being on death's
11:43store set that's him he loves it let's do it again what oh my gosh are you serious you've like
11:52yeah you've called an ambulance you're naked um you're in excruciating pain and yet you do it again
11:59so no regrets zero zero regrets we'll take you up to a hospital okay
12:04pale lips don't you and shows feel a bit more clammy now paramedics aaron and laura are treating
12:1735 year old tammy who tragically has stage three cervical cancer roll her onto her back do a slide
12:26sheet okay yeah only three months since the mum of two's heartbreaking diagnosis she's already
12:32suffering debilitating cervical bleeding a patient's appearance can tell us a million words and tammy is
12:40as white as a ghost good job that's a result of her blood loss great work so we'll give you a little
12:46bit of fluid okay your blood pressure is still good but i know you feel awful and you are losing some
12:50volume my worst concern is that she continues to bleed and then she loses her blood pressure her
12:56heart rate goes up and she could potentially go unconscious and i don't want to say that
13:02just you live the rest of your life laying on the floor just bury me here that's fine i'm not digging
13:07the whole load you go to a job and you picture a patient but you don't always picture the husband
13:13and the two young kids it's hard to imagine what they're going through you know like slide you out on
13:19this thank you i'm sorry no you're not a pain at all don't be sorry while aaron and laura work on
13:25getting tammy out of the house back up paramedic craig distract six-year-old dante and five-year-old
13:32rider with a tour of the ambulance what number ambulance is yours yes good boy it must be so scary for
13:45young kids to see their mum like this good to go yeah so to distract them we'll try and make the
13:51situation a little bit more positive by showing them around the ambulance i think that's really
13:54important and definitely a big part of our job wow okay it's cool pretty cool hey what's that
14:03that's the bed that's where mum's gonna go yeah mum's gonna go on that why because she needs to lay
14:09down and just have a bit of a chillax for a while mum's everything to them oh they love her to the max
14:14they don't listen to me they listen to mum all right nailed it all right i'm feeling pressure to get
14:32tammy on the way to hospital she's lost a lot of blood she's feeling quite weak and unable to move
14:39it's really hard we've got worried family watching over us can have your arm again but we're racing
14:46against the clock
14:57i'm pretty sure the chili has won this one intensive care paramedic lisa is taking 29 year old james to
15:04hospital after the thrill seeker ate one of the world's most lethal chillies on an empty stomach
15:11well he's going through the low intestine oh my god the chili lover was out with his fiancee mackenzie
15:17and a group of friends i don't think james quite comprehends exactly what's gonna happen on
15:23the other end so yeah i'm a bit worried that it's gonna gonna cause some burning on the way out
15:29with pain medication kicking in all right i'll meet you there thanks patenzie james is feeling a lot
15:35brighter what were you hoping to kind of achieve by consuming the chili was it just like i can do this
15:42no yeah i ate it not to contain this beast yeah i ate it because i was like oh i want to see what
15:47the flavors of this chili was yeah and what flavors did you get it was like nice hint of citrusy almost
15:54like peach my mate offered me the chili oh really wow he owes you yeah because he ate the chili yeah
16:02now look at me i'm going to hospital james is definitely one of the most unique people i've met i don't
16:09think this is his first time doing something silly where did you meet mackenzie work we used to work
16:15together i had no interest in her at all don't say that in the mouth with the penthrine now in james's
16:23system it's having the truth serum effect but she pursued me she harassed me yeah so we're hanging out
16:31a bit and then she was ticking boxes that i wanted someone for you know wife and then going beyond that
16:36like boxes that i didn't know and me ticked yeah and apparently it was the same thing for her yeah
16:40very quickly that we knew that you know we were able to be together so we went into a relationship
16:44and then bada bing bada boom yeah james is such a character you need someone that's pretty easy
16:49going and tolerant of certain decisions and behaviors and that's mackenzie
16:56i do hope that james hasn't done some lasting damage but i don't think this is the last time i'll see
17:01him are you worried about it coming out it's gonna burn your ring bro it's probably not had his last
17:07battle with the hot chili is this the first um time you've ended up in hospital it was two yeah
17:19paramedics laura and aaron are getting ready to take tammy to hospital

17:38tammy she's she's my life she changed my life around
17:44she's took me from being a run-em-up to being a loving father

18:04i'm sorry would you just call him dad dad and he called each other mom i was so confused inside
18:08when he was calling you mom and i was like do you know why the kids run around going this is not adding
18:12up tammy tammy tell me and i go oh my gosh so we call each other mom and dad
18:23tammy means the world to me i think she's gonna be awry she's gonna be alive the next day
18:31on the way to hospital we can't take our eyes off tammy we have to watch her heart rate her blood
18:36pressure and the blood loss in case she crashes do you feel lightheaded at the moment yeah tammy's
18:43been bleeding for two hours now which is quite concerning how many times since you've been
18:48diagnosed have you been to hospital for it twice twice yeah with a non-compressible bleed or a bleed
18:54that we can't put pressure on she could potentially bleed out in front of me are they wanting you to
18:59have surgery at any point you can't on cradle it you don't have to live with it is that what they've
19:04said to you yeah all right take this one off thank you take this thank you hi it's all right we're
19:16taking him for you didn't do anything laura did this all you drove me in hospital they'll do a blood test
19:23to figure out what her hemoglobin levels are and they'll also figure out if they can stop the bleeding
19:28and what to potentially do for next time yeah well hopefully we'll just be in here they'll get
19:32you sorted they'll send you home and you can start your treatment tammy's gonna need some pretty
19:37aggressive treatment if she's gonna fight against this cancer and raise her two boys and be with her
19:42husband but it's gonna be a long road ahead thank you if you ever need anything you can always call us
19:47say ambulance tell me exactly what's happened my husband tom uh he has emphysema struggling to
20:04breathe his pulse has gotten up to 108 and he's shaking all over it back at the emergency operation
20:12center a distressed wife is on the line as her husband's condition rapidly deteriorates is he
20:19changing color his face is ready to be true all right let's get some help out there for him
20:28solo responder bill is on the road and dispatched as the closest intensive care paramedic
20:34so we're on our way to a 75 year old male he's got a medical history of copd or angiocema which is
20:43basically a destruction of your lung tissue so driving to this job i'm thinking a couple of big
20:49things being 75 and having trouble breathing it's not a good sign the fact that he's got a history of
20:55copd makes that even more concerning the copd or angiocema if you can imagine
21:03sticking a straw in your mouth and only breathing from that straw and not being able to suck any
21:07more air into your lungs you feel panicked you feel anxious you feel like you're drowning
21:13so i can only imagine how awful that will feel if you can't breathe you die so the fact that this
21:19guy is struggling to breathe we need to get to him as quick as we can as bill pulls up another crew
21:27has arrived only minutes earlier hello how are you going the paramedics have already given the patient
21:35a life-saving oxygen mask called a nebulizer how can i help but his condition is still not stable
21:42suffocation is a really good word to describe how a patient might potentially feel with emphysema
21:47you can't actually get oxygen across from your airways into your bloodstream
21:52so your respiratory rate's going to go up your ability to oxygenate your body is going to go
21:57down and it's just going to get worse and worse and worse we're founded with a rest rate of 40.
22:02okay we count someone's respiration rate for how many breaths that they take in a minute
22:07so someone sitting on the couch watching telly would be sitting with a respiratory rate of anywhere
22:11between 12 to potentially 16. so the 40 is extremely fast respirator is very high
22:18the chronic respiratory distress it'd be like doing a marathon every single day okay you're
22:24working very hard aren't you yeah yeah it's horrible and it's scary and it can make these
22:29people feel like today's going to be their last and then you're quite shaky okay yeah
22:34good mates carlos and nee have just started their night shift i remember going to a job just at that
22:48curry joint just there yeah we needed another hand just to help us move this guy because he's in like
22:53the corner of this curry shop and i was like can can someone help me and they all literally just stared
22:57at me like i was speaking chinese or something and they were just eating their butter chicken i was
23:02like can someone please help me just ate slower just ate slower but with dinner still hours away
23:09their first job comes in
23:14so carlos we are going to a 53 year old male complaining of chest pain there's nausea acid
23:20reflux can't swallow anything and smelling faint plastic symptoms or having a heart attack
23:26absolutely chest pain is something that we don't muck around with whatever the cause we just need to get
23:31to this guy urgently oh he's got history of diabetes i think it says yeah it's a high risk factors
23:37already high risk he's already ticking some of the boxes yeah yeah and feeling nauseous and faint as
23:43well even though right now it sounds like heart attack the first thing that they teach us in
23:48paramedicine is to never assume there are 101 things that can cause chest pain but obviously the biggest one
23:55is a heart attack if it is a heart attack things can change within a split second
24:04so jobs like these really do get my heart pumping hello it's the ambulance
24:11have we had to listen to his shit because you are struggling very hard with your breathing
24:22intensive care paramedic bell is trying to help 75 year old tom get desperately needed oxygen into his
24:30system you are very shaky aren't you he is flushed agitated oxygen saturation's low he was already on a
24:37nebulizer he is just really really trying and fighting to stay alive and stay breathing i'm
24:43gonna have a listen to your breath mate no talking just breathing
24:48the chronic emphysema sufferer was at home with his wife christine when he started gasping for breath
24:55you sound very quiet on that left side sorry mate just sit me forward so i have a listen to tom's chest
25:00which is vitally important for someone who's not breathing properly okay so yeah just very quiet in that
25:06bottom left but not wheezy which is odd usually you'd expect to hear wheezes or crackles or some
25:12sort of fluid or infection build up but in tom's we can't hear anything at all i think i've got
25:1933 percent that's what i've got left of eyes okay right so your lung capacity is shocking yeah someone
25:25who's got emphysema is very similar to a patient having asthma he hasn't got much lung functional
25:30capacity left which means that he might not be getting any airflow to those lower areas of his
25:35lungs so he's going to be starved of oxygen i think we can start on cpap so he can take away that
25:40work of breathing i really want to try tom on something called cpap because i'm really concerned
25:44about his oxygen this is a big mask makes a lot of noise it basically forces air in so it might be a
25:50little bit difficult to breathe against initially so it sounds very loud so you might feel quite
25:57claustrophobic you might feel quite anxious but try to work through it and it will get easier i swear
26:03if it doesn't get easier you just let me know all right all right you're okay i'm going to pop this
26:09over here you're okay can be quite claustrophobic doing very very well it feels very strange but
26:19eventually it does help and he will feel better but i just hope that it works because tom is not
26:23looking good good good man no that's okay no you're all right you want me to take that one off
26:32too much too claustrophobic okay that's all right we'll chuck on your nasal sex again that's all right
26:37we gave it a crack it didn't work tom's getting really tired he's got a heart rate through the roof
26:42his blood pressure is working hard i'm out of my treatment options all i want to do now is get tom to
26:47hospital we're going to try to get you onto this green chair tom met his wife of 47 years after he
26:53returned from the vietnam war do you want some help christine has watched him struggle with emphysema for
26:59over five years but tonight was unlike any other he's struggling to breathe and not really able to talk
27:08just nice slow breaths he is my rock and my soul mate he tells me he was put on this earth to look
27:17after me and we were meant to be together forever i just couldn't imagine life without tom
27:27i'm worried that he's going to die you just focus on your breathing all right and you just try to stay
27:38nice and relaxed hello paramedics knee and carlos have been called to 54 year old sean who sounds like
27:53he could be having a heart attack hello it's the ambulance hi how you doing it was his fiancee who
28:00panicked and called for an ambulance when he started suffering chest pains hey sean hey how you doing
28:08mate my name's knee i've got calls um what's happened today mate i see sean wrapped up in his blankets he's
28:17looking quite pale sweaty clammy as well so we're thinking worst case scenario it is a heart attack
28:23i've just noticed my chest has worked really tight okay carlos is now setting up the ecg to take a
28:29picture of sean's heart before you're having a chat to us i'm just going to check you over touch a couple
28:34of things onto you and when did the pain in your chest start probably about um one o'clock today one
28:40o'clock today they just stayed in bed all day today i've done absolutely nothing yeah right i gave you pop your
28:44arms by your side we'll get a tracing of your heart rhythm what does pain feel like sean i did like
28:49you said nine good nine out of ten yeah yeah this does sound like classic heart attack symptoms he's
28:55got the pain in his chest he's feeling short of breath he's in the high risk categories and nothing
29:01that you've done relieves the pain in your chest no the results of the ecg are coming through
29:11and fortunately there's some good news for sean all these vital signs are looking pretty good at
29:17the moment so we can rule out a heart attack is that painful yeah yeah it's just there it's right
29:24there went for experience last night and a bit of chicken yep straight away got caught in my throat
29:30yeah and then um it just did not want to go right it's been like that ever since last night right
29:36feel stuck there right about here yeah okay like a blocking pain or like i've been like this been
29:41like this ever since ever since all last night so yesterday had some chicken and that got stuck in
29:46your throat had a beer to try and wash it down yeah can you swallow at the moment no never so i'll take
29:52a drink get away it just comes back up again right it can be quite dangerous to have something stuck down
29:59your throat because if that restricts your airway essentially you can't breathe can't get it down
30:16easy i'll come sit in the back with you tom yeah beautiful intensive care paramedic bell
30:21is treating 75 year old tom whose lung disease has taken a nasty turn tonight hey how you going
30:28now better i think from the first time i looked at you you actually do look better after a get
30:34together with mates tom started struggling to breathe prompting his worried wife of 47 years
30:41to call an ambulance yeah i'm ready to go whenever you are yeah cool i saw some interesting photographs
30:49while i was in your house yes did you serve yes you did where'd you serve we didn't have a bit
30:54shit sorry excuse my language far out how old were you uh 21 21 far out that would have been life
31:02changing i'm sure yes national service they pulled your date of birth out of a barrel that was it
31:08why you went so you had no choice it was just oh you could nick off and you could run away or you
31:13could hide yeah but you went and served two years far out while tom was going to war in his early
31:2020s i was going to university and having pub nights so the contrast is unbelievable it was like 53
31:26years ago does it still haunt you ah yeah every now not all the time yeah although some nights have
31:34bad nights and yeah i can't be in a room with the door shut yeah fair enough ptsd and psychological
31:42trauma is something that's not spoken about much it's definitely getting better but for all of the
31:48emergency services people out there we are all prone to this potential in the future and i just
31:53hope that we've all got the family and the friends and the networks and the support to be able to get
31:56us all through i can only imagine what you've seen and what you've experienced and what you've been
32:01through yeah there's been some times yeah i bet that's heavy stuff i thank you for your service
32:06oh my service has got nothing on yours so there's nothing nothing you need to thank me for
32:17that's all right if you're if you're struggling to breathe you just breathe
32:22tom's still breathing extremely fast and really shallow so at any moment i know tom could go back
32:27downhill so we've just rocked up at the hospital chronic obstructive airways disease is one of those
32:36awful diseases that will riddle you since your diagnosis and the fact that tom can't even hold
32:41a conversation without getting breathless is is awful to watch and it must be so frustrating to have to
32:47deal with might be a little bit jittery because it's cold but otherwise you're all good
32:51both fight for your country when you're young and to fight for every breath that you take is extremely
32:58brave
33:09can't get it down
33:12paramedics knee and carlos are about to take 54 year old sean to hospital stick out your tongue for me mate
33:18with food dangerously lodged in his throat you can get a little drip in your arm and give you
33:23something for the nausea he and partner blessy were out to dinner when a piece of chicken got stuck
33:30and he hasn't been able to clear it you hate needles you got a full arm of a tattoo you got a sleeve
33:36i guess it's a different it's different pain isn't it i've lost count how many tattoos i have and i
33:42hate needles as well so i'm on sean's side this is a bit of a sharp scratch okay mate
33:49you're up to walk yeah righty give us that
33:54i am worried about sean's condition but then i look to the top of his bed frame and there's this
34:00massive portrait of him and his dog it's got the dog's nipples and everything very detailed which is
34:06a very nice portrait but just very bizarre we're just admiring your self-portrait of yourself and
34:13your dog it's right in it you even got the tattoos all yeah in there as well yeah what's the dog's name
