00:00You've been, your chest was on.
00:08With Teresa's condition becoming worse by the minute,
00:11the team needs to work out what is causing her to feel so unwell.
00:17Paramedics are bringing in a 65-year-old female,
00:20who they suspect is having a stroke.
00:22Nurse Becky takes the handover.
00:30What we do now is, this morning at Harpesh,
00:33she got and had a sudden onset of a central chest pain,
00:36saying she couldn't breathe, couldn't get a seat,
00:39and it was a little wobbly, and it says,
00:41no, it's a very side repair, it's got a bit room,
00:43but that's normal for her belt, wasn't it?
00:46She's been unable to recall that she's been to Slimming World at all,
00:49which is a shame, because she's had a good week.
00:55Teresa and husband David are taken to cubicle nine,
00:59where physician associate Wayne has noticed her sudden deterioration.
01:04Your chest is on.
01:14So what happened today?
01:16So it's Monday, we're on this morning,
01:18and I've got a phone call saying come down because she's not well.
01:35It's got a good morning that's sorted.
01:39Because of Teresa's chest pain,
01:41Wayne does an urgent ECG to check her heart.
01:44With Teresa's condition becoming worse by the minute,
02:11the team needs to work out what is causing her to feel so unwell.
02:16In charge of her care is consultant Dr Laura Buxton.
02:21So, it says in my notes that you've been confused,
02:26more confused than normal.
02:29Does that fit with how you feel?
02:33Can you remember back to when you got up this morning and how you felt?
02:36Can you see my face normally?
02:47Yeah.
02:48It's a bit blurred, but yes.

02:51Dr Buxton quickly assesses Teresa
02:53for any signs that she could be having a stroke.
02:56What I'm going to do is get you to stretch both hands out in front of you
03:00and keep them up.
03:04Shut your eyes.

03:07And I'm going to push on them and you're going to keep them up, OK?

03:13Can you lift this leg up?
03:15Up strong.

03:17And this one.

03:19You've got weakness in your right arm and in your right leg
03:25and I just think that there's a possibility it might be a stroke.
03:30So, I'm going to get a brain scan, see where that takes us
03:33and I'll get one of the stroke nurses to come and have a look at you.
03:35OK.
03:37It's just a little bit of an unusual presentation
03:39for somebody who might have had a stroke.
03:41She does present with a mixed picture of confusion.
03:47She's got quite a complicated past medical history.
03:50So, it's not a typical case of a straightforward stroke.
03:54If Teresa is having a stroke,
03:56she'll need treatment straight away to limit any damage to her brain.
04:02I've looked at your CT scan.
04:03I can't see anything untowards.
04:06So, I just need to get a clear picture, basically,
04:11of what's been going off
04:12and how long this has been going off, right?
04:16I remember being at Slimming World, but that's it.
04:21When you're saying you can't remember...
04:24I just can't remember.
04:25Yeah.
04:25She's got memory problems to start with.
04:28I had a cardiac arrest.
04:30In 2012.
04:31And since then, has she been...
04:33She was starved with oxygen.
04:35Right, so...
04:36and memory problems.
04:37And the tireless she gets,
04:39the more confused she gets.

04:42So, you've been having these...
04:43So, these memory problems have been long-spanning since 2012.

04:55It's normally quite good, what I remember,

05:18There's lots of different things, I think, that's going off,
05:21and she probably will need an MRI.
05:23That's my concern, that she's having fluctuating episodes...
05:25Yeah.
05:25..where she has complete lucidity in between.
05:30Yeah.
05:31And for her to have a process that was causing memory loss,
05:35confusion, that was transient,
05:37that got better with a rest at night,
05:40seems unusual.
05:43I think it's very unlikely that it's stroke-related.

06:04So, we've got some concerns about your memory and about your confusion,
06:09and what we want to do is admit you for a bit of time
06:13so that we can work to get to the bottom
06:16of what all these symptoms are being caused by.
06:20OK.
06:20Don't keep me for long.

06:23We'll do our very best.
06:24We don't think this has been a big stroke.
06:26And it's something maybe going on in the brain
06:29that's causing you to have these acute episodes of confusion.
06:32I could sleep on a washing line.
06:36But, yeah, I just need some sleep.
06:39I'd sooner be going home and going to bed,
06:41so I could sleep properly.
06:43It could be a couple of days, actually.
06:45But, er...
06:47I know there's something that are going wrong
06:50and that needs sorting out.
06:53It's just one of them things.
06:54But there'll be a lot of people in here,
06:56a lot worse off than me, so...
06:58Teresa stayed overnight
07:00when it was discovered her symptoms
07:02were caused by her pre-existing heart issues.
07:06She's now home, and her treatment is ongoing.
