00:00Hello there, how time flies. We're coming to the end of July and we're also coming to the end on
00:06how to improve or reverse your problem with male infertility. Today we're going to talk about
00:13something called azuspermia. Azuspermia just means that you're ejaculating normally,
00:19but it's not quite normal because there's no sperms.
00:22So azuspermia predominantly is due to obstruction. However, obviously if somebody has got
00:35non-obstructive azuspermia, i.e. there is no spermatogenesis, then it is a lost call
00:43because it's difficult to retrieve the actual sperm itself. Obstructive azuspermia technically
00:49is either congenital or acquired. Acquired one is, for example, vasectomy is one of them.
00:57The other one is infection such as chlamydia and gonorrhea resulting in obstruction. Since the
01:05scarring of the vas is quite extensive, reconnection is impossible. And on the other hand, a condition
01:13such as congenital bilateral aplasia or vas deferens, the actual disconnection is also quite variable,
01:22and reconnection is also impossible. So under that circumstance, how do you overcome such obstacle?
01:32The answer is very easy. We use the helping hands of science through ART or ART. ART stands for
01:43artificial reproductive technology. In simple terms, it's called test tube babies. In order to make
01:50test tube babies, obviously you need the eggs and the sperm. If the sperm is not coming out, you need to go and
01:56find them. There are three ways to find them. One is called PESA. It's percutaneous epididymal sperm
02:07aspiration. And through that, we will find whether there's a healthy sperm and actually put into the eggs
02:15to fertilize it. This is not always possible because sometimes the sperms are very difficult to extract
02:23through the skin. So the next thing we do is open up the scrotum and do microscope MESA. MESA
02:33stands for microscopic epididymal sperm aspiration. And through that, we'll find plenty of sperms along
02:41the maturation site of epididymis. But again, it might not be possible and we opt for the final one,
02:48which is TESE. T-E-S-E stands for testicular exploration sperm extraction. By opening up the
02:58testicle, all the manufacturing areas from there tease out every single one. Hopefully, we find one
03:04good sperm, create that test tube baby and hence overcome the obstructions and overcome the problem of
03:13infertility. For the next month of August, we're going to talk about aging process. I'm sure a lot of you
03:21like me past the age of 50 are contemplating what are the problems of aging. The first thing we're going
03:28to talk about is what exactly happens to your sperm counts when you're aging. So stay tuned for next
03:34week's putting Dr G on the spot.
