Foyle Hospice prescribing model to be rolled out throughout the North

00:00The family maybe person would see the patient in their own home they didn't

00:05actually have the prescription pad there and didn't actually there wasn't a

00:09process for them to actually use their prescribing qualification and that

00:15patient obviously could be an OMA and the doctor who's on at the time could

00:21could be an Altona Galvin and then there's travel distance as you can

00:26imagine between decision to change that prescription and that prescription being

00:31written and what we've done we took a look at that we've worked along with the

00:36Foil Hospice our primary care partners Andrea Linton from new models of

00:42prescribing and SPPG has worked along with myself and we've chaired a steering group

00:46over the last year where we've put a process in place and it wasn't easy

00:52and because once you start crossing boundaries between trusts and and

00:57different care settings there was you know that communication process because

01:02one of the things you don't want to do is where you have somebody prescribing a

01:07drug and there's a potential for duplication with a GP and prescribing

01:12that drug as as well or not knowing that that drug has actually been prescribed so

01:17there was a bit of to and froing to get the communication processes aligned there

01:22we managed that and we had a celebration event in the Foil Hospice last week where

01:30we presented the findings and it has shown us that we have significantly reduced the

01:37time between decision to prescribe and the patient receiving the medication in

01:42their own home small-scale study but we're looking now through it you know what

01:49our next steps our next meeting will be how do we scale and spread that across

01:54Northern Ireland to have a bigger impact through celebrating local innovation

01:58showcasing good practice the program supported the scale and spread of

02:01initiatives such as just-in-case boxes pioneered in the Western Trust and now

02:05adopted across Southern Trust and being progressed in Belfast and Southeastern
