For our RSP members, what is hoarding and what are the common signs and symptoms of hoarding disorder?

Like what you pointed out, many of our countrymen have symptoms

where they collect things that they don't need and there's a clutter.

So we call this hoarding disorder because we can see that these symptoms

affect our functionality and our capability to be able to live our lives properly.

So for example, hoarding disorder, there are three things that we have to establish.

So one, you have an urge to collect and keep things that you don't need to use.

So it's common to see paper bags that are too many

because we think that we will use plastic bags,

especially bottles and take-out containers.

So we see that it's very, very common.

And then we also see that because of this collecting, it's too much.

So there's also a clutter.

So we can see that it's chaotic inside the house because it's taking up so much space

and yet it's hard for our patients to let go.

And then finally, we also see that their habits affect their function

and their well-being and lifestyle.

Doc, I feel like we're all guilty here because we have moments like this in life.

Doc, what are the possible causes and triggers of hoarding disorder?

Well, you know what?

Before, they considered hoarding disorder as part of obsessive-compulsive disorder

but they saw that it's not the case.

There are explanations that there is a decrease in function in a particular part of the brain.

But overall, we also see that it's not actually that well understood.

But we also consider that there's a genetic component to it.

It's also noticeable that what we see is more of an advance of age.

So possibly, epigenetics could also be a culprit then.

Because if you notice that these particular generations are products of the world war.

So for example, resources are very scarce and your adaptation is to hoard, to keep things.

Because we're stockpiling for the possibility that things may be scarce again

and we also need it eventually.

So it becomes a habit for us.

And then it's carried over from generation to generation.

Because that's what our parents and grandparents are teaching us.

But there are multifactorial components.

But overall, when we're talking about hoarding disorder,

the statistics are high.

They estimate about 3 to 5 percent of the general population

with a lifetime prevalence of 14 percent.

So it's actually quite a common court condition.

But what's concerning is that sometimes these habits are so severe

that it can also cause health problems or hazards.

Like for example, if there's a lot of clutter inside the house

and we're wearing an earthquake belt.

For example, if it's hot and we don't have enough water.

It's also a fire hazard.

It can catch fire and it can also be dangerous.

And then it can also be a falling hazard.

For example, if there's a lot of stuff, it can fall

and it's hard to navigate your way inside the house.

And then there's also a health risk because it attracts pests.

And then for some extreme cases, we've been collecting things that are

very infectious already.

I do recall watching a particular TV series called Hoarders.

And there was an episode where this person was collecting already used diapers

and it piled up in the bathtub.

So it could be extreme measures.

And then it can cause disease and death.

Especially hoarding.

It's not limited to the use alone.

It can also be animals, pets, strays that are rescued.

But unfortunately, they are neglected.

So yeah, it's a very concerning condition actually.

My mom likes to put things next to her.

I like to put less and less so it's always clean.

But here's the thing.

Some of our fellow countrymen are just sentimental.

So what's the difference between being simply sentimental

or really fond of collecting and disorder of hoarding?

Well, like what you pointed out when it comes to hoarding.

What we hoard are useless items.

And then it accumulates to significant numbers already.

So how much paper bags do we actually need through the years?

So for example, it's a whole room full of paper bags already that you don't use.

Maybe you have to reconsider.

But for example, we're talking about a collection.

So for example, people collect sneakers, figurines.

That would be a different case.

It's probably displayed properly, organized, and with intent.

And like you pointed out, there is probably some value to it.

But for hoarding, what we usually see is we tend to collect things of insignificant value.

Like you pointed out, pieces of mail that are 10 years old already.

Newspapers that are stockpiling already.

Now what good would it be for?

And I also mentioned that it has to establish the three criteria

that there is a collection of useless things and the inability to let it go.

And then there will be clutter in the house.

So the emphasis is on the word clutter.

And then the third is it also causes dysfunction in our capacity.

It impedes our daily function, our daily lives.

Well, unfortunately for hoarding disorder,

when we talk about hoarding disorder,

I'm talking about the actual psychiatric diagnosis.

Because hoarding can be used as a verb.

Like okay, I'm hoarding or stockpiling canned goods just in case something happens

and I'm not able to refill our groceries.

Although like I pointed out, hoarding has insignificant value.

But talking about the psychiatric disorder itself,

unfortunately hoarding disorder is treatment resistant.

So studies show that although the prognosis is very poor,

but the recommendation is to consider psychotherapy.

So a particular type of psychotherapy that is recommended is cognitive behavioral therapy.

And then another, in terms of medication,

we can also consider antidepressants usually from the family of SSRI,

Selective Serotonin Reuptake Inhibitors.

Again, our expectations are very limited.

Because probably one factor that could make this condition quite difficult to manage

is usually we diagnose it late.

So usually it's advance of age and then we see,

wow, there are so many and the house is so tight and the house is already in a danger zone.

And then sometimes it could be extremes already,

that our patients are already being evicted

because when the landlord saw it, it's already dangerous to be inside the house

and it's very unsanitary already.

So unfortunately, these are our challenges when managing hoarding disorder.
