In the first instance, the Philippine Heart Association officially announced the Heart Failure Awareness Week.
00:09
Its purpose is to raise awareness about heart failure and the effect of early detection and its management.
00:18
And aside from that, Audrey, we will now be joined by the immediate past president of the Philippine Heart Association, Dr. Ronald E. Cuyco.
00:28
Good morning, Doc, and welcome to RSP.
00:31
Good morning, Doc.
00:32
Good morning. Thank you for inviting us. Of course, thank you, Noel and Audrey.
00:37
And of course, to our viewers of this program.
00:42
And of course, we will talk about important heart diseases.
00:47
Well, this is one of the reasons why Filipinos are dying.
00:54
But Doc, for our viewers to understand, as a beginner, what is heart failure?
01:01
So, this heart failure is actually a condition of the heart, a heart disease,
01:07
where the ability of the heart to supply blood to different parts of the body, such as the brain, kidney,
01:17
is limited or compromised because of this problem, such as a heart attack.
01:23
So, there are different reasons for this.
01:25
In other words, this heart failure is at the end of different types of heart diseases.
01:30
For example, there is a blood clot in the heart.
01:32
This is what we call ischemia.
01:34
If there is an attack in the heart, this causes a weak heart attack.
01:42
Another reason for this problem is the valve.
01:45
In the Philippines, this is what we call as rheumatic heart disease.
01:49
And another one is the muscle itself.
01:52
The muscle of the heart sometimes weakens suddenly because of an infection.
01:57
This is common during COVID.
01:59
This is common when a patient dies because the muscle weakens and there is an infection.
02:05
We call it myocarditis.
02:07
Because of this disease, what are the symptoms of heart failure
02:12
and how can you differentiate the symptoms of heart failure compared to other heart diseases?
02:19
So, it is very important that we mention the symptoms one by one.
02:24
What does a patient feel when he has a heart failure?
02:28
First of all, of course, it is easy to get tired.
02:31
If you used to walk a long distance or you can climb three flights of stairs,
02:38
now, you can only do it up to the first floor.
02:40
This means that something is happening.
02:42
The oxygen in your whole body is running out.
02:46
The other thing is towards the late stage of heart failure,
02:50
you can see that the heart is getting bigger.
02:54
And sometimes, it is hard for them to sleep.
02:56
It is different, right?
02:58
It means that if it grows because of heart failure, it is because there is water.
03:03
So, the heart can't pump and it can't circulate well.
03:08
So, other patients have water in their lungs.
03:14
So, when they lie down, they can't lie down flat.
03:17
So, they would prefer to stand up.
03:19
And the other thing that is dangerous is that they can only sit when they sleep.
03:23
So, most of the time, they can't sleep well.
03:26
Their food intake is low.
03:27
Their body is lacking in nutrients because their intake is low.
03:34
Others have chest pain.
03:37
So, it is hard to differentiate the symptoms of heart failure from other types of heart disease.
03:43
Because it is part of the continuum of heart disease.
03:47
So, if there is a heart-related disease or heart valve disease,
03:51
towards the end, they will present as heart failure.
03:55
So, it is actually the same.
03:57
So, the message that we want to convey to the countrymen is that there should be early detection.
04:05
If you feel breathless and it is new to you,
04:08
so, if you see your heart beating or palpitations,
04:12
consult immediately.
04:14
Don't be consistent.
04:15
Because sometimes, it is because of fatigue or any other reason.
04:21
But if you consistently feel in your heart that it is tightening,
04:26
that is what you should bring to the doctor.
04:28
Is that right?
04:29
That is right, Audrey.
04:30
We also need to observe ourselves.
04:32
But if you have risk factors like diabetes, high blood pressure,
04:38
or severe pancreatitis,
04:40
then you should consult.
04:42
But if you are in a situation where there is stress,
04:47
like if you wake up early in the morning,
04:50
sometimes, there is a little bit of fatigue.
04:53
But if it goes back,
04:55
if there is joint pain in the chest,
04:57
if you feel breathless,
04:58
I think you should consult a doctor.
05:00
I think it is also related to the family history.
05:03
Yes.
05:04
It is like a genetic predisposition.
05:09
When it comes to treatment,
05:11
how is the treatment to control the symptoms of heart failure?
05:16
Most patients are fluid overloaded.
05:20
There is water in the body.
05:22
As we said, there is water in the lungs because the pressure is increasing in the heart.
05:26
The lungs are connected to the heart.
05:28
Sometimes, there is a fever.
05:30
So, what we give is to make the heart beat faster.
05:33
And then, their heart rate increases faster.
05:35
So, we control it.
05:37
These medicines can relieve it.
05:39
But of course, we have to emphasize,
05:41
what is the cause of heart failure?
05:45
So, you have to treat it.
05:47
For example, if there is a bullet in the heart,
05:49
fix the bullet.
05:50
If there is a problem with the valve,
05:52
it is limited.
05:53
Fix the valve.
05:54
It is a limited operation.
05:55
It is connected.
05:56
Yes, it is connected.
05:57
So, overall,
05:59
what we want to emphasize is that
06:01
we should not reach that situation.
06:03
We should address the risk factors of this disease,
06:07
such as high blood pressure, diabetes, paninigerilio,
06:11
all of them, so that we do not reach that situation.
06:13
Prevention is better than cure.
06:15
That's right, Noel.
06:16
Well, Doc, since we are celebrating
06:18
the Heart Failure Awareness Week,
06:20
are there programs and activities
06:22
being prepared by the Philippine Heart Association?
06:24
Of course, this is February.
06:26
This is Heart Month.
06:28
So, in line with the celebration of Heart Month,
06:30
one of our focuses
06:32
at the Philippine Heart Association
06:34
is heart failure.
06:37
The prevalence rate is increasing.
06:39
In the Philippines, it is about 1.6% years ago.
06:42
Even in our neighboring countries,
06:44
the prevalence rate is really low.
06:46
So, one of the things we gave to the Philippine Heart Association
06:48
is that we re-launched
06:50
and gave life to
06:52
what we call the National Heart Failure Network.
06:55
This is brought together
06:57
by different medical societies
06:59
in partnership with,
07:01
of course, the government,
07:03
the private sector.
07:05
This is brought together with the primary intention
07:07
to spread,
07:09
to explain to people
07:11
what heart failure is
07:13
and what are the medicines
07:15
and treatment options for it.
07:17
First and foremost,
07:19
how to avoid it.
07:21
I would also like to announce, Noel,
07:23
that apart from the Heart Month celebration of the PHA,
07:25
usually, we do this
07:27
National Heart Month celebration
07:29
and we are going to hold that
07:31
in Bicol, Saligas, next week.
07:33
Again, we emphasize
07:35
the Heart Failure.
07:37
Again, we are joined by
07:39
Colonel Quico. Thank you very much
07:41
for sharing with us information about heart failure.
07:43
Again, thank you very much, Doctor.
07:47
Thank you very much, Doc.
