00:00 So what are we talking about this month?
00:02 In fact we're talking about prostate cancer and also the consequences of various treatment
00:08 and this week we're going to talk about radical prostatectomy and the consequence of erectile
00:13 dysfunction.
00:20 We all know that prostate is closely linked to a nerve called neurovascular bundle and
00:26 that neurovascular bundle is very important for the nerve and also for the vessels for
00:31 the supply of the penis for the purpose of erection.
00:35 So therefore an operation such as radical prostatectomy for the purpose of prostate
00:40 cancer can result in the destruction of this nerve and therefore men who previously was
00:48 having no erection problem after the operation might have the erection problem ranging between
00:54 30 to 80% depending on the integrity of the vessels.
00:59 So how do we prevent this from happening?
01:02 One of the way is called penile rehabilitation.
01:06 What does it mean?
01:07 It's actually meaning that before the operation the patient receives a medication to enhance
01:12 the nerve supply and also the blood supply to the penis and during the operation the
01:19 surgeon will do something called neurovascular bundle preservation and as a result of that
01:26 the chances of erectile dysfunction is very low.
01:29 However sadly some men presented with the disease quite advanced and the actual compromise
01:35 of neurovascular bundle is completely unavoidable so some men after the operation ended up with
01:42 the dreaded erectile dysfunction.
01:45 How do you deal with that?
01:47 Sometimes medicine such as the blue pills, BDE5 inhibitors will work.
01:51 However if it doesn't work, injection such as injection directly into the penis in order
01:57 to enhance circulation may be a solution.
02:01 So therefore now you know there's always a solution for the complication of erectile
02:06 dysfunction.
02:07 There is no reason to avoid this dreaded problems of prostate cancer and eradications with radical
02:15 surgery.
