There is a constant perception that anybody can get a large behind base on his or her expectations. People search the Internet and claim that have had 2000 cc of fat injected into each buttock. There are surgical considerations as well as anatomical consideration that completely contradict these claims by either patients or surgeons. For example, the first thing that you need realize is that how big your buttocks is going to get has nothing to do completely on the amount of fat that you inject. In other words if you inject a 1000 cc, your buttock will be much bigger than somebody that he has 800 cc. When it comes to buttock projection many factors like the tightness of the skin, how much projection you have before surgery, the anatomy of the muscles among others ultimately will determine how much projection you are going to get, just because the perception of higher number of cc translate into a bigger buttock that does not necessarily translate into a bigger behind when it comes to surgical implications of buttock augmentation. In addition, when you inject fat, the fat is alive, it is not a static implant like a buttock implant or a breast implant. When we inject fat in the buttocks it needs to be done in such a way that you inject the maximum amount of fat and avoid complications and improve fat survival. When we inject fat in the buttock, the fat needs to have a recipient bed with rich blood supply to survive if not the fat will be reabsorbed and in some cases it can turn into fat necrosis or dead fat. In other words, the more fat that you inject in any particular area the less chance is that fat is going to survive because the buttock has a specific space. Think about it like a cup of water. You put enough water until the water spills out, same thing is with the buttock. It is not that you can inject fat indefinitely without having surgical consequences like increased complications of seroma, fat necrosis, and increased rate of fat reabsorption. For example, in addition when you inject fat in the buttock there is an increase in the pressure around the soft tissue, the skin expands to reach any given projection. As the skin span outwards there is more pressure that causes the fat to spill out through the incisions that we use to inject the fat, so 1 of the endpoints of buttock augmentation if the surgeon notice the fat is coming through the incision after significant amount of fat have been injected then this is a tell-tale sign that enough fat has been injected and the buttock itself has reached its capacity. You need to understand that is not that you can choose any given number when it comes to buttock augmentation, same way with breast augmentation. The bigger the breast the more complications you are going to have due to soft tissue interaction that is out of the control of the surgeon. Be careful of what you wish for, just because your friend got any given amount of fat that does not mean that you are going to get the same amount. It does not mean that the more fat you get the bigger your butt is going to get. Complications will occur secondary to injecting more fat that the buttock can handle, that does not mean that a 1200 cc is not possible, but it depends on each patient individually. Somebody that is really small is not going to be able to handle this amount of fat. Somebody who is completely flat with a very small bottom the skin is not going to stretch to the level that you want. Setting expectation and understanding the surgical implications as well as the reasons of having an increase in buttock projection with surgery is important for you to decide what size of the buttock you can't realistically accomplish and at the same time decreasing the complications rate.
