The disc's attempts to repair the tear may stimulate nerves to grow into the tear. Irritation of these nerves by inflammation (chemicals) or mechanical stress (stretching / compression) can cause additional and on-going pain. Plain CT and MRI pictures will often not detect these tears. A provocative discogram can be used to show that the disc is the source of the pain by injecting dye and fluid into the disc to assess whether the increased disc pressure stimulates your pain. The dye may enter the tear, and be seen on a subsequent CT scan. If all conservative treatments fail, then surgery may be an option.
