Spain train crash: At least 68 people have been killed after two high-speed trains collided near Toledo, Spain, and investigators are now examining a safety system failure and the decisions made in the control room.

The crash happened on Wednesday morning, October 28, 2026, between Urda and Los Yebenes in Toledo province, about 140 kilometres south of Madrid. A high-speed train run by Spain's state operator Renfe struck the rear of a stationary train run by the private company Iryo. Both trains were travelling from Madrid towards Seville. The Renfe train was carrying 397 passengers and the Iryo train 147 passengers. Spain has begun three days of national mourning.

Spain's transport minister, Oscar Puente, said a computer malfunction disrupted a safety system along a 63-kilometre stretch of railway, causing trains in the area to stop. He said a central control centre then instructed the Renfe train to resume movement without speed restrictions, despite a stop signal remaining active, describing that decision as inexplicable. According to reports in Spanish media, audio recordings of communications between drivers and the control centre at Madrid's Atocha station suggest both trains were told to resume at normal speed. Officials have stressed that the full sequence of events and responsibility are still to be established. This is the second deadly high-speed rail collision in Spain in less than a year, after a crash at Adamuz in January killed more than 40 people.
