00:03suture suspension gives you pure lift.
00:05So it's ideal for someone who wants a lift and reshaping.
00:11Hi, I'm Rachel and I'm here getting
00:13a suture suspension lift today.
00:15I'm a mom of two kids and I work full time.
00:18I spend most of my days on Zoom calls staring at my face.
00:21And I think over the past year I've learned
00:23that I'm more resilient than I thought I was.
00:26I'm more positive, I have a better sense of humor.
00:28But when I was looking at myself on those Zoom calls,
00:30I didn't see that person anymore.
00:32I'm really excited to look like myself again
00:34and to feel like myself again.
00:36Hi, I'm Dr. Lara Devkin and I'm a board certified
00:39plastic surgeon in New York City
00:40and I specialize in facial optimization.
00:43Facial optimization is the combination of surgical
00:46and non-surgical procedures that I use
00:48to optimize facial beauty while preserving facial identity.
00:51Tell me a little bit about what you're hoping
00:53to get out of this.
00:54What I'm noticing in my face is that my laugh lines
00:56have become very deep.
00:58This is the Mailar fat powder, the apple of the cheek.
01:00And when you're 25, it's right here.
01:02And over time, it comes down a little bit.
01:04So you're getting a little bit of mid-face flattening.
01:07We'll do a couple of little tweaks here and there.
01:09I think it's going to be awesome.
01:12Suture suspension is a modern technique that allows us
01:15to lift the deep planes of the face using a tiny little needle
01:19access site and dissolvable surgical stitches.
01:21I have had Botox in the past and I have had a little bit
01:26of filler in my cheeks and both times I love the effect.
01:29My only fear with having this procedure is that I'm going
01:31to love it so much, I'm going to want to do it more.
01:37We'll use injected numbing medicine to make sure
01:40that the patient feels comfortable.
01:42And that's really the only part of the procedure
01:44that hurts a little bit.
01:45It feels like a little bee sting for about 10 seconds.
01:52Once the injected numbing is in place,
01:54I'll then turn my attention toward the procedure itself.
01:58The first step is to make a tiny access hole and a pilot hole
02:03using a little needle prick that's hidden right
02:06in the sideburn area.
02:10Next, I'll use sterile cannulae, which are basically
02:13stainless steel, surgical, tiny little tubes,
02:16like thinner than a coffee straw.
02:18And those will be inserted into the tissue in the deep plane.
02:22So the deep plane of the face is the deep muscular layer
02:25that I actually will lift in a surgical facelift.
02:28The amazing thing about the modern generation
02:30of suture suspensions is that I'm able to access the deep plane
02:33of the face through a tiny little needle hole.
02:46So once I insert the cannulae, those will deliver barbed suture suspensions.
02:52And the sutures that are being placed are made out of a dissolvable material
02:56that's a complex sugar molecule.
02:58It stays in the tissues for about a year until it fully dissolves.
03:02And in that time, it also causes the body to stimulate collagen production.
03:10Once the sutures are placed, I double-check the vectors.
03:14So they can be placed in many different orientations depending on the patient
03:18and their goals in anatomy.
03:21I'll suspend the sutures.
03:23I'll tighten them.
03:30I'll trim them.
03:32And then I'll re-drape the patient's soft tissues
03:35with respect to the deep plane of their face.
03:57You might hear a little bit of clicking because some of the suture suspension
04:03gets transmitted through bony hearing conduction into the ear canal.
04:25And then finally, the patient will sit up.
04:28I'll take a look at everything in the mirror and we'll do little adjustments
04:31to build structure with injectables in addition to the lift that we created
04:36with the procedure so looking really beautiful you want to take a look yes
04:51oh my god oh my god i was absolutely blown away when i first saw myself it was like looking
04:59in a
05:00mirror from 10 years ago i look like myself again i love how uplifted my face looks my eyes are
05:06brighter i just i feel so great i'm so glad you're happy you look so beautiful and refreshed so what
05:13we did was give you a lift of the mid face and definition of the jawline to pick up the
05:19jowl
05:19and we have a highlight on the cheekbone a low light in the cheek sulcus shadow and then right
05:24here is an almost textbook gonial angle or angle of the mandible that's looking very beautiful
05:29i expected the treatment to be painful you know the surgery does involve barbs going into your face
05:35which sounds really painful and the biggest surprise for me was that it really wasn't
05:39it looks amazing i don't think anyone would know that i had had something done they would just say
05:44i look great yeah and that's the idea after a suture suspension i suggest no exercise for two to three
05:50days and avoiding alcohol aspirin advil and blood thinners most of my patients will come in for a
05:57follow-up visit around two to three weeks to make sure that everything's looking perfect
06:00dr debgan is incredible i love the way she would make a small change to my face step back and
06:08assess
06:08the overall symmetry and effect of what she was doing it made me feel like she was really paying
06:13attention to the overall effect and i think it really shows i think what i do is all about empowerment
06:18i think beauty can be very misunderstood but real beauty is about feeling good about yourself and
06:24feeling like you want to present yourself to the world and you want to participate in the world
06:29so what i do is really not about how lifted your cheekbone is or how sculpted your lip is it's
06:35about
06:35how you feel and how confident you are in making your decisions i would absolutely do this again in fact
06:42one of the questions i asked dr debgan was how many times can i do this procedure and it turns
06:47out
06:47there really is no limit but the results surpass my expectations i feel younger and brighter and i can
06:54actually feel the difference in my face i no longer feel that sagging and i feel more like myself
