04:23help stop the bleeding hello is in the bathroom yeah I can't come out the hospital with it

04:53lean over sink that while you do this 77 year old Robert recently had surgery to treat a bleed on
05:00his brain tonight after bleeding heavily from his nose his son called 999 what I've had it happen for
05:09it's usually stopped within you know 10 minutes 10 minutes but it's been half an hour yeah how much
05:16blood would you say that you've estimated a lot yeah pressure let me just pop that on there any
05:24headache no any blurred vision no no just the nose bleed Robert was in quite the mess when we first
05:31arrived on scene my major concern was the amount of blood loss and to make sure that his airway was
05:36clear well we'll see what we do we've got like this little clip that we can pop on your nose
05:41that can
05:41try and stop yeah the bleeding all right right this is supposed to clamp can you feel that pressure
05:50oh yeah it's gone it's not very tight though no let me try a bit higher up never get trying

05:57and trying in school oh there we go right keep that there for a bit I think it's doing its
06:03job it's
06:04stopped bleeding now it's stopped dripping down there we go fixture can you breathe through your mouth okay
06:11yeah yeah you're not feeling anything oh no it's still dripping I was quite worried at this point
06:17because we had no other option to stop the nosebleed apart from him applying pressure himself and I was
06:22really worried about the amount of blood loss what we'll do we're just going to contact in our team
06:28we've got a drug that's a blood clotting drug yeah the fact that it's still going after half an hour
06:33and
06:33yeah it is losing quite a bit I'm thinking that that might be a go ahead if I'm honest just
06:38to see if
06:39that stops the bleeding all right as Robert has recently undergone brain surgery Amy needs to speak
06:47with a senior clinician before she can give blood clotting drugs I mean I'm just waiting for a response
06:54mate no worries it's just my reasoning is the fact that it's been continuous for over half an hour now
06:59and it's still yeah going quite yeah I'm going to take this off because it really isn't doing nothing
07:07is it if you try and pinch a little bit more up here as well if you can that normally

07:14how's your walking like is that all okay yeah before I went to the hospital I could hardly walk and

07:20dripping up and falling over but since I had the operation and come out of hospital I've been fine

10:13he when he fell here yeah okay yeah he's helping me retired cabbie ron fell over while trying to
10:20switch on his electric blanket daughter karen rushed over when she heard her dad was hurt
10:27wrong we need to just try and straighten you up a little bit this might be a bit
10:31very uncomfortable i'm just really sorry is it just in your hip
10:40a voice over here back here here yeah our main concern was figuring out where this pain was
10:49why he was in pain and getting this under control we did suspect that he may have broke his hip
10:58i'm not gonna i'm not gonna touch you i'm not touching you
11:16he's just had a massive clot about this big just come out from his mouth that's gone up his nose

11:20come out his right bear with me sorry we've just had a clot probably about three centimeters
11:24by three centimeters round just come out of his mouth he said it was up his nose it was up
11:29his
11:29nose then he felt like he's gone into the throat and spat it out and it's about that big let's
11:34just get
11:34going then to the west of birmingham paramedics mark in amia with robert having recently undergone
11:43brain surgery they're concerned his severe nosebleed won't stop there we go right then you're okay
11:52don't feel dizzy at all no let's get going then just nice and steady down the stairs okay
11:59robert needed to go to hospital as he had had recent brain surgery nosebleeds can be a symptom if
12:05something's gone wrong with the brain surgery so we needed to get him there fast just put some straps
12:11on you and again just checking no headache no dizziness
12:21in the future just found 999 don't worry about anything we'd rather come to something like this
12:26a lot of people say oh it's only a nosebleed but there's different circumstances with yours you know
12:30what i mean you have arrived at your destination it has slowed down don't pick it at russell's hall
12:45hospital in dudley doctors will investigate why robert's nose won't stop bleeding they'll also want
12:52to see if it's related to his recent brain surgery
13:04oh it came down my leg it came down your leg yeah it's like right to the south of birmingham
13:11paramedic daisy and student paramedic morgan are with ron who's in agony after a fall
13:18day's i'm not sure if we're going to get this stretcher in do you know what i mean do you
13:21want
13:21to try or do because i just don't think we're going to get him on character i don't think we
13:26are
13:26i think putting him in even more pain just to get him out i think we need to try other
13:30ways first
13:33yeah i'm going to give him treatment here first yeah i don't want to move him
13:38ron has very clearly hurt himself we think he might have broken his hip so he needs to go to
13:42hospital
13:44now we need to figure out how we're going to get him there ron what can you stay really really
13:50still
13:51for me really still whilst we do this heart tracing yeah before attempting to move ron morgan runs an
13:59ecg to check the state of his heart really still he's always dead oh ron let's not say things like
14:08that what about your daughter no you're not she loves you she gave up a lot of time for me
14:17well you
14:18gave up a lot of time when she were a baby when ron said that he didn't want to be
14:24alive anymore
14:25it was quite upsetting and it was clearly because he was in so much pain so we just wanted to
14:31make
14:31him more comfortable try and get him feeling back to himself right i'm going to give you some paracetamol
14:38through that vein okay oh just by the leg was it shooting up and down your leg oh ron
14:55daisy and morgan give ron liquid paracetamol intravenously but it's having little effect
15:02right it's given something stronger you know i don't know i don't think we're going to move him
15:08without getting him something stronger if i'm honest i agree ron had very clearly hurt himself
15:13we do believe that he had broken his hip so in order to move him we knew we were going
15:18to need to
15:18give him some strong pain relief just to try and keep him as comfortable as we can during the process
15:24is it all right if we give you some morphine carry on please please we're just going to grab it
15:31and
15:31we're going to grab our stretcher okay yes thank you don't move whilst we're gone all right stay still
15:42we ain't got a prayer mate no moving on
15:51there's not really any other way to get him out other than on the chair
15:55it was quite difficult to figure out a way to extricate ron we couldn't fit the stretcher through
16:02the door we couldn't use a spinal board the only other option that we had was a chair which was
16:10quite uncomfortable but it was just a short-term solution until we got him sorted i'm gonna give you
16:17some morphine i'm sorry i'm feeling so sorry oh why are you sorry this is our job do you want
16:24to try
16:24some gas and air ron actually yeah right so if you pop this in your mouth if you suck it
16:32in you got it
16:38ron ron if you suck that in it should ease the pain but you've got to keep sucking on it

16:47breathe as normal with it daisy and morgan have now given ron all the pain relief available to them
16:54right ron we're gonna move you okay keep sucking on that whilst we move you because this might be
17:00quite painful i'm gonna move your legs okay i'm gonna sit you up ready

19:37roughly to travel that's what we sat navs for that's why you are no good at geography because
19:42you realize on your sat nav it's a country that potentially had vikings uh sweet no norway yeah
19:54believe it or not norway has the longest coastline you know i'm going to look at that on the map

20:08ambulance service is the patient breathing yes it's me i've got something wrong with the heart
20:14valve and i'm supposed to have it operated on what symptoms do you have my love it's painful and my
20:22heart's racing and can you feel the pain right now yes i think i'm having a heart attack
20:43that's what we can do
20:48well look at the call came in she's woken up with chest pain that can be quite concerning
20:54she's never a good sign we're only nine minutes away
20:58a bit tight it's scary isn't it some of the rural communities and how isolated and vulnerable
21:06they they actually are this was potentially quite a serious case just given her age given the location
21:12of where it is and the amount of time it would take to get there yeah the location of a
21:17property was up
21:17a really long narrow road which just made it extra hard to get to her
21:26so what's been going on then well i've got a heart valve problem and you know waiting to have it
21:33done
21:34but oh tonight it really got me i woke up and i got this real pain and my heart was
21:43racing
21:44it when racehorse breeder rosalie was woken in the night with pains in her chest
21:50she rang 999 husband peter is with her what time did she wake up with the chest pain though
21:59about three o'clock do you have an irregular heartbeat by any chance
22:06can see they've they put it in she's got aortic valve stenosis rosalie was experiencing some chest
22:13pain which could be caused by her pre-existing heart condition aortic valve stenosis is the
22:19narrowing of a valve within a heart which affects the blood flow into the arteries it effectively means
22:26the heart has to work a little bit harder so you've got palpitations in your chest at the minute
22:29sweetheart no no no did did you have okay the longer i spent with rosalie the more concerned i got
22:37she was telling me about needing the heart operation how she's now got chest pains and palpitations
22:42so the longer i was there the more concerned i was becoming for her nice and still a second
22:49adam carries out an ecg to check the electrical activity of rosalie's heart
22:54yep we'll be going you're going to hospital sweetheart is it all right or is it you've
23:00got a heart rate at the minute it's about 160. yeah it should be ideally between sort of 60 and
23:06100.
23:07it's very fast basically i knew something wrong anyway i've been getting short of breath
23:12have you i'm not surprised with a heart rate like that so it's saying stemmy
23:19yep so we're gonna have to ring yeah we'll give him a buzz once we completed a heart tracing on
23:24rosalie
23:25it showed us that not only was her heart beating pretty fast at over 160 beats per minute but it
23:32was also showing us that she was actively having what we call a stemmy which is a heart attack
23:45as the west midlands sleeps we're on board with eight paramedic crews working the night shift until
23:516 30 in the morning we are going to be taken at a hospital and we are probably going to
24:00be going
24:01quite quickly on the outskirts of stoke paramedics colleen and adam with rosalie who they suspect is
24:09having a heart attack i'm just going to give you a spray on your tongue if i can yeah this
24:14will hopefully
24:14take some of the chest pain away adam gives rosalie a spray under her tongue this opens up her blood
24:21vessels reducing the strain on her heart we're going to look and see if we can give her aspirin
24:27then we're going to look at giving her a needle in her arm as well i'm just going to pop
24:30in your
24:31hall and make a phone call okay okay colleen calls the hospital to let them know they'll be arriving
24:37soon with a patient who's having a heart attack so i did the right thing calling oh yeah i don't
24:43like
24:43to think i'm wasting anybody's time nope you have definitely done the right thing by corn they said
24:49go to a and e initially but give resource a quarter to go there which a and e stoke rosalie
24:55has to go
24:55to hospital because she's actively having a heart attack she needs further treatment that we cannot
25:00provide her it's imperative that we get her there pretty quickly to get the treatment she needs as
25:04fast as she can we're going in on blue lights we're going to be going quite quickly she is unwell
25:09so it's up to you what you choose to do but if you do you're doing your job anyway that's
25:14all
25:14they'll look after me i know that we'll get onto the ambulance and we'll get you to stoke as quick
25:18as we can okay okay no no absolutely not no all right absolutely not well your heart rate's already
25:26going to 160 try and get you up and about you'll be on the floor i find with the older
25:31generation they
25:32don't want to make a fuss they just want to get on with things but when something like this is
25:36happening
25:36you really should make a fuss about it
25:43you're not walking anywhere not with your heart rate like that
25:47well you know your job indeed how are you feeling not too bad so like we said we will be
25:55going off
25:55going in with the blues and twos okay okay so we need to get you there sooner rather than later
26:07breaking bad news is never fun but unfortunately we need to do it in a way that the patient
26:12understands so they have all the information they need hello resource it's adam on the paramedics can
26:19i put a medical standby in please okay and we'll see you in about 20 minutes we're going to be
26:25waiting
26:25for you so we'll go straight in straight to see someone hopefully i feel a bit of a fraud going

26:50at royal stoke university hospital the cardiac team are ready and waiting to treat rosalie
27:02rosalie was lovely wasn't she yeah don't think she wanted to bother us she absolutely needed
27:09to though no didn't she like as soon as i seen that ecg yeah i was like hmm that's not
27:13right not
27:14right at all it would not seem particularly bothered about it she was really chilled yeah
27:21that's what i mean they weren't more chilled than i'd be bless her they don't make them like her
27:25anymore do they oh definitely absolute trooper
27:40so is he able to respond to you at all and speak to you is she awake at the moment
27:46every minute of every day the call assessors of west midlands ambulance service are working hard to
27:52save lives there is always something that is completely out of the box and you're not expecting
28:01it and i think that's what makes this job so interesting ambulance service is the patient breathing

28:14is she breathing yes yes okay are they conscious are they awake she's panting she's ready breathing
28:22okay i just need to know if they're conscious are they responding to you
28:26is she responding to it no i don't think she is okay is is her breathing noisy or abnormal
28:34it's like gargling heavy it's gargling okay is she responding to you is she making any effort to move
28:46okay okay caller listen to me it's really important that you follow my instructions okay i need you to
28:52have a look at her breathing is she breathing normally is she breathing normally no not really
29:00as soon as i hear the words that they're not conscious and not breathing it's almost like a wall
29:06a wall that comes down around me and i'm completely locked in on that call nothing else around me
29:12matters okay we've got some help arranged i need you to listen to my instructions okay are you within
29:17easy reach of her yeah somebody's on the roof uh she's on a roof she's on a what sorry she's
29:26on a roof ledge
29:32yeah when i heard the patient was on the roof i think i was genuinely really confused but it just
29:38changed things in the moment then it meant that i needed to then think about how we were going to
29:42get
29:43the caller to the caller to the patient safely to do cpr so when you say she's on a roof
29:48ledge what do
29:49you mean it's like a ledge it's like a ledge is it is it a ledge on top of the
29:55house is it like a balcony
29:57can you see the ambulance caller is the other paramedics there one on your left door on your left

30:12yeah is that the crew with you yeah okay i'll leave you with them
30:19even though we get calls quite often for people
30:23in unconventional places it's not often that you get somebody that's on a roof
30:27unconscious not breathing needing cpr and trying to figure out how we're going to get to that patient
30:36minutes later control has informed the woman was trying to take her own life
30:42we've got some updates on that case so she's alive they've now got her off the roof fire
30:50were there as well i assume that's to help get her down off the roof yeah because they weren't
30:55quite sure how they were going to get her down to be fair i just couldn't couldn't get my head
30:59around it no it just didn't make sense as to why she would be on the roof she's on the
31:03back of the
31:03ambulance so hopefully she'll be okay
31:05are you sweet rather than savory then if i have a tub of ice cream
31:27there's no like scooping it out and putting it in a bowl i just eat all of it do you
31:32know what
31:33really annoys me though about say if you've got ice cream and you get it out the freezer and you've
31:37got to wait for it to defrost a bit and your spoon just bends because it's the ice you put
31:42in the
31:42microwave it for about 10 15 20 seconds depending there has been a few mishaps on a couple of occasions
31:49where it has been too soft so then do i eat it or do i put it back in the
31:57freezer to freeze up a little
31:59bit more but then am i going to be starting back at the same point where i'm gonna have to
32:04put it in
32:04the microwave this is too much it's a science it's it's a science i do like ice cream when it's
32:17all
32:36right okay so that could be a factor a lot of people struggle with that don't they yeah yeah
32:46diverticulitis is a condition in the bowel which can cause bleeding and discomfort and sometimes
32:51a lot of pain we knew this lady had been bleeding and so it was quite important that we found
32:56out

33:08hello what's been going on sweetheart i've been having bleeds i've got diverticulitis okay i'm used
33:14to having small bleeds i had to do a runner to the bathroom i had a massive bleed you can
33:24go look in
33:24the room right okay 74 year old maddie has lived with chronic inflammation of the bowel for 30 years
33:33but tonight after experiencing heavy bleeding she called for help
33:39and are you getting any pain yeah is the pain different to normal diverticulitis pain yeah it's
33:46a crossy yeah let's do your blood pressure ducky right so would you say the amount of blood that
33:54you've lost and like tonight and that isn't is fine it's getting worse oh yeah with the amount of blood
34:03maddie was losing i was concerned as things can get quite serious quite fast oh right okay so it's
34:10definitely red fresh yeah
34:28oh goodness tired bob i am tired yeah i definitely prefer the day shifts what do you prefer day shifts
34:34a hundred percent it's always better when you're working with a friend so when me and sean like
34:38together it's not really like working together because we're quite good friends anyway yeah yeah
34:43definitely if you have a laugh with someone time goes so fast yeah it does and you bring loads of
34:48snacks so that definitely helps that's very true that's very true can't wait for a cup of tea in bed

35:31when a patient's heart is highly unstable but is still beating it's called peri-arrest it's the stage
35:38before a full cardiac arrest onwards and upwards let's go see

35:53so i mean sunday morning hello it'd be a good guess to say that he might have been out

36:09he's breathing oh yeah yeah i can detect my injuries he's laying in his own throw up so
36:14hiya mate can you wake up for us can you wake up definitely had a good night
36:22we'll get the stretcher and get him on the ambulance yeah
36:27a doorman called 999 after finding the man lying unconscious on the pavement
36:33when we arrived on scene we came across a man covered in vomit we both took a good guess that
36:38he'd been on a night out the night before and he was just taking a sleep on the side of
36:42the road
36:43right mate it's time to get on this stretcher let's set you up you've definitely had a good night
36:54all right shall we get your legs round oh you're able to pull him up from that way yes mate
37:00oh he's
37:01wet himself as well yeah hello all right good for you just get you to send it we've got to
37:09get
37:09you on this stretcher don't worry about me it's not there again oh no we are worried about you with
37:17the ambulance you're in the street you've been sick do i want man okay when he sat up and came
37:25round a bit it was very difficult to talk to him he didn't want to speak to us and we
37:30very quickly
