02:28I always had compliments on my looks.
02:31I was on top of the world.
02:33Job, school, just everything is just going perfect.
02:39I was driving down the freeway at nine at night.
02:44And my car hit a pothole.
02:46I couldn't turn or brake.
02:48It hit the divider at 75 plus miles per hour.
02:52And my car flipped about three or four times.
02:57My car was a legitimate pancake.
03:00I'm just lucky to be alive.
03:03The accident caused me to have severe lacerations all over my body and specifically my head.
03:09My recovery was seven months and it was extremely painful physically and mentally.
03:16Once the head guys came off, my scars were very prominent.
03:21When I re-entered the world, I noticed that the way that people treated me was different.
03:27No matter how I dressed, no matter how I behaved, no matter what I was doing,
03:32it was almost like a scared feeling, almost as if I was going to hurt them.
03:38They say I look intimidating, like, do you sell drugs or like, you in gangs?
03:43And they legitimately treated me as if I was a criminal just because I'm black with scars on my face.
03:50My whole life, I've been trying to battle and fight that stigma that comes with being African-American.
04:00That is what changed me.
04:02I became withdrawn and less sociable.
04:18The scars make me feel like a legitimate monster.
04:25I've always believed that my path was to do something for people and promote love.
04:30I'm not trying to change who I am.
04:33I just want what I naturally am to show.
04:35Getting rid of the scars that mean the world to me is unlocking the potential that I feel I already

05:01I'm Nicholas Bravo.
05:02Graduated with my degree in psychology and I'm just an entrepreneur.

05:07So what kind of businesses are you building?
05:09So right now, I'm focused on something
05:11in the transportation industry.
05:13So tell us a little bit about your story.
05:14Why are you here?
05:15Yeah, so I got into a really, really bad car accident.
05:19Almost lost my life.
05:21After the accident, I was left with scars,
05:24a lot of them on my forehead here,
05:27the corner right here in between my eyebrows,
05:31then above my eyelid, my left eyelid here.
05:34I was lucky that it healed a certain way,
05:37but I literally could see the difference after the accident.
05:42People just didn't want to be around me
05:43almost like I had leprosy or something.
05:45You could tell they instantly had questions,
05:48and dealing with that every single time
05:52you step out of the house for seven years
05:54is very, very traumatizing.
05:56It changes you on many levels.
05:58My whole life, I've always been confident,
06:00but after, like, two years,
06:02I began to become insecure about the scars.
06:07Jobs that I applied for, dating, of course,
06:11that was different.
06:12I've had people tell me, like,
06:14I just look gangster, thuggish, dangerous,
06:18all these different types of things.
06:20But that's just not who you are.
06:21No. Not at all.
06:22So it's just really distracting to what I'm trying to portray
06:26and what I'm trying to do with my future,
06:28especially at a start-up company.
06:30I just look less trustworthy.
06:33Unfortunately, we live in a world where the reality is
06:36what you said.
06:37Facial scars are associated with being hard,
06:39and we just, we need to take that away.
06:42You couldn't be further from that?
06:43Yeah.
06:44Well, Jamie and I are gonna put our heads together,
06:47and really, we wanna help you,
06:49because you're so inspiring,
06:51and honestly, there's no reason for these scars
06:53to get in the way of your life and your success
06:56and your confidence for, like, one more second.
06:59So we're gonna get some scans done.

07:01and see what's really gonna be the best treatment for you.

07:43Here's the plan for Nicholas.
07:45I wanna start by softening his brow line
07:47by giving him cortisone injections.
07:50Cortisone is often used to reduce inflammation in the body.
07:53It can also be injected locally
07:55to break down scar tissue and hypertrophic scars,
07:58like the ones above Nicholas's brow.
08:01And then I'll be using a series of lasers
08:03to resurface and soften that area.
08:05Once I finish balancing out the structure of the brow,
08:09then we're gonna work on restoring the actual eyebrows
08:11with some cosmetic tattooing.
08:14So would you do a little bit of filler under his eyes?
08:17I definitely would.
08:18I just wanna talk to him and feel it out
08:20because you can see he has more lipoatrophy on this side.
08:23So I could add a little bit more filler.
08:26I could add a little bit of filler here and here.
08:28He just didn't mention any of that.
08:30Certain things, especially men, they just don't know it exists.
08:33So if you tell them,
08:34listen, I'm just gonna throw this out there.
08:36I'm giving you all of the possibilities.
08:38So I think anything that we can do
08:40to really make him look refreshed,
08:43powerful, confident, all of those, healthy.

08:47Next, we'll be restoring the volume with some filler
08:50and then improving the overall skin texture
08:52with a technique called microdosing.
08:54But this is the guy that I really think
08:57just throws the look of the face off
08:59because he has these great eyes and this just distracts.

09:37The thing that stands out to me the most
09:39is this scar right here and this one on your nose.
09:42So after we take some cortisone
09:45and kind of lower this where the trajectory is off,
09:48it's protruding too much.
09:49Then it's this discoloration, this pigmentation
09:52that you have here and here that I want to remove.
09:55So I'm just going to put a little bit of cortisone in there.
09:58We'll probably do some microneedling.
09:59The other thing is where they approximated this wound,
10:02where they sewed you up.
10:03You've lost some of your eyebrow there.
10:05I have a girl in my office that can do cosmetic tattooing.
10:09What it's going to do is kind of just trick the eye
10:12into completing this brow.
10:14So you have a little bit of loss of fat here.
10:17So since I'm going to be in there and doing stuff,
10:19if you want to, I can do some filler.
10:24Have you ever heard, you've heard of filler?
10:26Yeah, I've heard of it.
10:27People think that filler is just used to like make big lips
10:32and like crazy women walking around the streets.
10:34But really think of filler more as like putting things back
10:37where they used to be rather than cosmetic.
10:41It's just going to complete the look.
10:43Because your eyes underneath it just looks a little tired
10:47even when you're not.
10:48And I don't think you're...
10:49Are you wearing any sunscreen?
10:50I was always told that black people don't need sunscreen.
10:53Is this...

10:54Yeah.
10:54Everyone needs sunscreen.

10:59I mean, you're less likely to get skin cancer,
11:02but absolutely you need to protect your skin.
11:05Just so you know, the reason why your scars healed so dark
11:08is because you got sun on them.
11:10Whenever your skin's pink, it's susceptible to the sun.
11:13We call pink scars immature.
11:14As long as your scar is immature, you get sun on it,
11:18it's going to heal darker than your normal skin.
11:20Our goal is to get you taking more photos of yourself
11:24because we see a beautiful man.
11:28We don't want these scars to represent you anymore.
11:35I love my job.
11:36It's so powerful when I can help to wipe away trauma
11:40and scars and build people back up again.

11:49The first thing I'm going to do is
11:51I'm probably going to have to do some cortisone injections.
11:54Then we're going to do some dermal fillers
11:56just to restore some volume.
11:58Then we'll do a little bit of infrared
12:00just to kind of heal the area.
12:02All right, here it goes.

12:06For Nicholas, I'm going to use steroid injections,
12:08not the kind that makes you bob,
12:10the kind that's used to break down scar tissue.
12:15Oh, man, oh, man.
12:16You've got a lot of scar tissue here.
12:18When I really start poking around,
12:20it's going to take more than one time,
12:21but it's going to be better.
12:23So you have some fat loss from where this stuff has happened.
12:28It does make the right side of the face
12:30a little bit more hollow.
12:31And I'm just going to kind of restore a few things.
12:35Because of the accident,
12:36Nicholas has lost a lot of volume on one side of his face.
12:41So I'm going to use filler to restore that volume.
12:47I'm really good at filler.
12:48I mean, I'm like the best in the world at filler.
12:52Two more tiny pokes close to the eye.
12:54Basically, I got into this field because I wanted to get my lips done,
12:58and I was a young woman with no money.
13:00I worked for a group of derm and plastic surgeons,
13:03and I just, I loved it.
13:05And I had a knack for it, and it just kind of, I never left it.
13:12All right, so you're going to sense a bright light.
13:14I just went to town on Nicholas's face,
13:16so I'm going to use LED light therapy.
13:18I can't tell you how cool this machine is.
13:21It was originally created by NASA,
13:23developed for plant growth experiments on space shuttle missions.
13:27It has since been proven to help inflammation in the body,
13:30reduce pain, and produce collagen,
13:31which is really good to calm down the skin
13:34and help the healing process.

13:38But you see, like, mine is this.
13:40The swelling's still there, and this is swollen.
13:42So I'm going to be sending Nicholas home with some skin care.
13:45The thing about a lot of my male clients
13:47is they're typically oblivious to the need for skin care.

13:57Face healed up, like, after three days.
13:59I didn't notice a change from too many people,
14:02but what I did notice was a change with my,
14:05surprisingly, with my confidence.

14:33Being a mom of five kids is chaotic.
14:37It's craziness.

14:42But then there's the great times
14:43when everybody is wonderful and loving,
14:46but mostly it's like a hurricane set on fire.
14:50Me and my husband struggled with infertility
14:53for three years.
14:54We went and seeked fertility help,
14:57and we started the IUI process,
14:59and we were lucky to get pregnant the first try
15:02with our first son, Jax.
15:04Then when he turned one, I was getting that itch,
15:07so we went back to the fertility clinic,
15:10and I got pregnant with four babies.
15:15I was so happy.
15:19My husband, after he got used to the idea,
15:22he was super excited to be the quad dad.
15:26He's a proud dad.
15:27I'm a proud mom.
15:30The first pregnancy, I gained a good amount of weight.
15:34I call it happy weight
15:35because I was so happy I was pregnant.
15:36But with the quadruplets,
15:39there were so many babies pushing on
15:41so many different things inside of me.
15:43It was like my whole entire body was pregnant,
15:47and I knew that there is no coming back from this.
15:55And then after giving birth to them,
15:57the stress of taking care of four babies,
16:00plus managing my two-year-old at the time,
16:04who just got diagnosed with autism.
16:06So that was a big hit for me.
16:10It was like, okay, now how am I gonna help him?
16:13Well, I'm trying to help these four other babies.
16:19I have these five kids that need every part of me.
16:23How dare I think about myself?
16:26But I know that I need to really take a little bit of time
16:32for myself because that, in the end,
16:35is gonna make me a better mom.
16:36I'm ready.
16:37I'm so ready to bring back the old me
16:40and have self-confidence again.
16:50So I saw a patient once
16:52use this peel on herself at home,
16:54like a very strong one
16:55that you should, like, have no business doing at home.
16:57And she gave herself second-degree burns
16:59was in the burn unit.
17:00And the first question she asked me
17:02is when can I do a peel again?

17:39and took a toll on my body.
17:40My sister has quads
17:41and also had a one-year-old
17:43when she had quads.

17:45Yeah, I had a two-year-old when I ate one.
17:46There you go.
17:47So it's almost like having quintuplets.
17:49Yeah, it really is.
17:49So tell us kind of what brings you here.
17:51So I had quadruplets.
17:53Yes.
17:54And it does something to your body that you can't, like, imagine.
17:58Like, you just saw parts sticking out everywhere.
18:01My skin's been stretched to the max.
18:03They went in places they weren't supposed to be going.
18:06It still feels, in some things that I wear, like I still look pregnant.
18:10When we're in the bed, it's like me, my stomach kicking over,
18:14and then my husband.
18:15So, like, don't touch me right here.
18:16Don't touch me when I'm over here.
18:17Mm-hmm.
18:17Above the shoulders, you're good.
18:20Like, it has changed our relationship so much.
18:23I don't like me now.
18:24Like, I still am not comfortable in this body.
18:26Even looking in the mirror, I'm like, that's me.
18:28Like, that's really me.
18:30I just think it's so beautiful that you've come to this place
18:32that you're going to take care of yourself for a little while.

18:35If I'm not happy, really, our family's not going to be happy.

18:41You really do have to put your oxygen mask on first.
18:44You have to take care of you because you can't pour from an empty cup.

18:47And you carry yourself differently, your kids are going to feed off of that.

19:12and the results that she wants, she's going to be a better surgical candidate.
19:16Laser and diet and exercise are not going to do the trick for her.
19:20I think there's definitely things that can be done non-surgically,
19:23but it'll take months and months and months and months.
19:25And there's also the misconception that it'd be cheaper.
19:28It's not.
19:29Because sometimes it's so many more treatments.

19:31So she just needs a little bit of love from you.
19:35I always look at a patient and I say, what's their goal?
19:38And can I get there without surgery?
19:40And if the answer is no, I have no qualms about jumping to a surgical procedure.

19:48I'm going to perform a tummy tuck, liposuction her upper and lower back,
19:52and do a breast lift to give her a nice shape.

20:54So, yeah, there are definitely some things I could do non-surgically,
20:57but there is no exercise for the skin.
20:59So, in this case, I'm going to defer to Dr. Nazarian.
21:04Looking at your shape, you have hips.
21:06I don't want to make your breasts smaller,
21:08because then you'll look like a pear.
21:09We don't want that.
21:10We want you to look like an hourglass.

21:12So, we're going to give you a breast lift and a tummy tuck.

21:18Is that what you, like, sort of found out in your research?
21:20Yes.
21:20Just by lifting your breasts up,
21:22it's not going to fill out that top part.
21:24So, to fight that, I'm actually going to take some fat
21:28from your upper and lower back,
21:30give you a really nice silhouette,
21:31a nice waist,
21:32and use some of that fat in the top part of the breast.
21:35Okay.
21:35And just by taking this fat out,
21:37your butt's going to look like,
21:39ooh, I got a lift.
21:40It's a dream.
21:41You're going to look so good.
21:42I can see it in my head.

21:46So, there is another option that I do want to discuss with you.
21:49So, if you hold that mirror up,
21:50there is something that is done minimally invasively,
21:52and it actually can be done awake,
21:54or it can be done along with other surgeries.
21:57This is your buccal fat,
21:58and it's a completely different type of fat in the face.
22:01Now, when you were a kid and you were lighter,
22:03did you always kind of have a fuller cheek?
22:05Yeah, I always had.
22:05Yeah, because it's a different type of fat.
22:07So, through an incision on the inside of the cheek,
22:10so there's no external scarring at all,
22:11I can actually pull this little fat out.
22:14So, suck in your cheeks a little bit.
22:15Yeah.
22:16Now your jaw makes sense with your face.
22:18Mm-hmm.
22:19The shadowing underneath your lips is less.
22:21Mm-hmm.
22:22And your face went into that sweetheart position
22:24without volume.
22:25Mm-hmm.
22:25So, if you're open to that,
22:27I think that that would really just make you look lifted.
22:29And who doesn't want lift, right?

22:31And the buccal fat would be, like, more permanent.
22:33It would be more permanent.
22:35My only concern on that is that as we age,
22:37your face is going to become exponentially leaner.

22:40So, I worry about you having that fat as you age.

22:44And that is a concern.
22:45The one question that I always ask is,
22:47when you were younger, did you still have that cheek?
22:49And you said yes.
22:50So, that just means if you lose weight,
22:51you're not going to lose the cheek.

22:52And the second thing is,
22:53who did you get the cheek from?
22:55Uh, my mom.

22:56Is your mom gaunt now?
22:57Or does she still have a full cheek?
22:59She still has it.
23:00Yeah.
23:00So, I'm not worried about you getting gaunt as you age.
23:03So, I really do recommend that.

23:05Or it's something that we can always do before or after with injectables as well.

23:10I'm leaning more towards removing the buccal fat.

24:51So, I decided to do liposuction.
24:53And also, I needed the fat for fat transfer into her breasts.
24:56So, it's gonna make her butt look a lot more projected.

25:02Growing up in Iran, my mom used to always say,
25:05Oh, I wish I could take the fat from here and put it up here.
25:07And we'd laugh and be like, ha-ha, whatever.
25:09But that's exactly what we're doing now.
25:11So, you can really see the fat coming out now.
25:12You see the yellow?
25:15Sometimes, it's just putting a little fat in.
25:17It gives it a little bit of a lift.
25:19When I look at a patient, I used to get super excited and be like,
25:23I could do this and this and this and this and this.
25:25Because there's hundreds of things we can do on anybody.
25:27I'm almost done with the rest.
25:30But it's really about what's affecting that person's life.
25:33Because if it doesn't bother them, you're not really improving their quality of life.
25:45I will need electrocautery for this portion of the case.
25:50Buckle fat is actually quite different from the fat on your body.
25:53So, you can actually see how much fat was in her cheeks.
25:56It's like immediate results.
25:57The fat on our body is like tiny little globs versus the buckle fat.

26:04Moms are such incredible people.
26:06We affect so many people's energy, that confidence.

27:20is why I want to use a laser that's really good for darker skin tones.
27:25The laser is so effective because it delivers energy in a picosecond, shattering the dark pigment
27:30without a lot of heat, which means faster results without the downtime.
27:35If 10 is the worst pain you ever had, what would you put that as pain-wise?

27:43I'm now using laser resurfacing to break up the pigmentation for his facial scarring.
27:48The reason I love this machine is because it hits the pigment, fragments it, and then

29:56and it's going to really help make that more smooth.

30:02It went from 25 to 45, like, within the kids, and I feel like it chills on my face.
30:07So, like, all these things that we're doing, I think it's going to bring back the time that I feel

31:33It's crazy what I see in the mirror right now.
31:36This is definitely not a mom of five.
31:41Once I became a mom, the Michelle before that was gone.
31:44And now this brought her back.
31:47I feel my age.
31:49I feel 30.
31:50I feel just alive again.
31:52Before, I had so much weight in the front of me from my boobs and from my stomach.
31:57And now I have that muscle back.
32:00I can hold myself up without feeling that weight pulling me forward.
32:03My boobs are way perkier.
32:05They pulled them back up where they belonged.
32:07My waist is there again.
32:10And then all my facial features are poppy.
32:14The biggest surprise out of this whole process, I think, is my mental state.
32:19I didn't think I was going to have as much confidence that I have now.
32:23I always thought that no matter what, I was going to have some kind of insecurity.
32:26But I love myself again.
32:31I think every mom needs to take time for yourself to do the things that you need to do for
32:37you.
32:37If you're not happy, your kids aren't happy.
32:39Your husband's not happy.
32:41I don't have boundaries with my husband anymore.
32:44There's nothing that's off limits.
32:45It's almost like we started dating again in a weird way.
32:51And then as a mom, I'm not worried about, oh, am I going to be able to chase my kids?
32:55Now it's being present in the moment with my children and my husband.

33:04There was a light at the end of the tunnel, and she opened so many doors for me in so
33:07many different ways.

33:37For so long, I was fixated on the scars because I thought that defined me.
33:42I thought getting the scars removed from my face would allow people to be open to seeing who I am
33:49and not dealing with profiling and judgment and mistreatment.
34:00Now, I realize it's not about that. It's just about how I feel about myself, me living in my truth,
34:06bringing out my light as best as I can.
34:21I look in the mirror and I like what I see. I love what I see. I don't see a
34:28monster. I just see someone that truly wants to love everybody and just uplift everybody, all of humanity.
