00:01After that little scare with Anna, I just got this knot in my chest every now and then.
00:07I have decided to step away from teaching because I already found the best possible replacement.
00:12Me?
00:13You must have some kind of protege who's ready to take the reins.
00:16What happened?
00:17I'm thinking about retiring.
00:19I haven't had a panic attack since medical school.
00:23I'm in the candle at both ends.
00:24I started to volunteer at a suicide hotline, being the guy on the other end of the line.
00:29I don't know, it kind of helps.
01:15Greater Chicago Lifeline. This is Dan. What's going on tonight?
01:24Hello?
01:27Are you there? Did I lose you?
01:33I'm here.
01:35Oh, there you are. Tell me what's happening.
01:40I want to die.
01:44Thank you for your honesty.
01:46Uh, I also appreciate you saying that out loud because that's not easy to do.
01:54That's all I can think about.
01:56My brain just keeps, I don't know, running in circles and it always comes back to that.
02:03I can't eat. I can't sleep.
02:06Not sleeping absolutely sucks.
02:10Do you mind if I ask your name or at least, you know, what you'd like me to call you?
02:18It's Gio.
02:19Gio.
02:21Okay Gio, thank you.
02:23It was an accident.
02:25It was my fault.
02:27What was, what was your fault?
02:32I killed my girlfriend.
02:36Okay.
02:38Think it might help to, um, tell me what happened?
02:43Why? Why? How, how would that help?
02:46Well, you know, you said that it was your fault and that it was an accident, which is interesting because,
02:51you know, I mean, accidents are, well, I mean, they're accidents, right? I mean, they're not on purpose.
02:59We went to a holiday party.
03:01By the end of the night, she was pretty hammered, but I only had a few beers, so I drove
03:07us home.
03:09She started feeling sick, so I was driving pretty fast. I didn't want her to puke in the car, you
03:15know what I mean?
03:15Oh, I sure do. That's the worst.
03:18Yeah.
03:21We were coming up on a big intersection when the light turned yellow, so I gunned it.
03:30And out of nowhere, a woman stepped into the crosswalk early.
03:36I slammed on my brakes. I tried to see around her, but the car spun out of control and slammed
03:45into a traffic pole on the passenger side.
03:53When I looked over at her, her eyes were open, but she was gone.
04:03Man, I'm just so sorry, Gio. That sounds, that sounds awful. I mean, awful.
04:08Oh, it's like a tape loop in my brain.
04:11Oh, it hurts so much, and it never, ever stops. I need to make it stop.
04:16Okay. You know, there is a big difference, though, right, between being involved in an accident and intentionally causing someone's
04:25death.
04:26Right? And I want you to trust me on this. Like, grief and guilt are really, really good at blurring
04:32the difference.
04:33It was my job to get her hope safe.
04:36You want to know what the cruelest thing is about accidents that happen really quickly?
04:41It's that after the fact, we slow them way down, and then we start to play them over and over
04:48again in our head,
04:49which creates the illusion that we might have done something differently, right?
04:53That we could have somehow created a different outcome, when the truth is that that was just never, ever going
05:00to happen.
05:05Look, you're clearly a nice guy, but none of this stuff is going to bring her back.
05:10No, it's not. And I can't even imagine how difficult it must be to accept that.
05:17Gio, can I ask you, do you have anything with you that you might use to hurt yourself tonight?
05:24A gun.
05:27Got it. And, uh, where is the gun right now?
05:33My hand.
05:35Okay. Look, I'm going to ask you to do something for me, all right?
05:38And that is, I just want you to put it somewhere, just for now, put it somewhere where you can't
05:43see it.
05:48You still there, buddy?
05:56Sometimes it can actually help to talk about people that we've lost, right?
06:02You think that might help to talk about her a bit? I mean, there is no wrong answer to this
06:06question.
06:06I'm, I'm just asking.
06:11Her name was Darby.
06:16She's gone.
06:18Forever.
06:20You know what? It's, it's a mistake.
06:24What is?
06:27Gio, look.
06:28It's so important that you feel this stuff.
06:30Lucky.
06:31It is.
06:32You can't possibly understand.
06:34It's torture.
06:35Buddy, listen.
06:36I can't do it anymore.
06:37No, no.
06:38No, no.
06:39I'm sorry.
06:40Gio, listen.
06:41Loss like this brings a kind of pain that can't be solved or explained away.
06:45You just got to talk about it.
06:46You just got to feel it, right?
06:48But just because you're feeling it doesn't mean you have to act on it.
06:51Feeling guilty doesn't mean you don't deserve...
06:55Gio.
07:09My brain just keeps...
07:12I don't know.
07:13Running in circles.
07:14It always comes back to that.
07:16I can't eat.
07:17I can't sleep.
07:28Bells.
07:30Bells.
07:32Bells.
07:33Bells.
07:34Bells.
07:35Bells.
07:37Bells.
07:38Bells.
07:38Bells.

09:14i have found
09:14so come on dad this doesn't have anything to do with me this is about you whenever you start fixating
09:20on my life it means something's going on with you it's called projecting yeah i'm familiar with the
09:25concept how about deflection you ever heard of that one okay how about this you go to work i'll wash
09:32my clothes and we'll pretend this conversation never took place okay
09:39so could your supervisor trace the call using his phone number only emergency services is allowed
09:44to do that and get this once we make that move right once we escalate it that's it no follow
09:51-up
09:51allowed whatsoever it's a legal thing right we are not allowed to find out what happened in one way
09:56or the other damn that's brutal tell me about it yeah but look you did everything you can and you
10:01don't know what happened and maybe that's a good thing you also know what you signed up for right
10:06of course i do so come on daniel you can't save everybody
10:13why not
10:18oh hey do you have a moment i do talk later maybe a little cocktail after work
10:25yes please all right i have a patient that just came in travis abbott says he fell and hit his
10:31head but he's been knocked around pretty bad uh there was blood and perineal lacerations near his
10:37rectum and his tox screen came back positive for ghb oh okay you're thinking sexual assault yeah but
10:43he won't go there lead the way i don't know what the big deal is all right i i had
10:50too much to drink
10:50i tripped and smashed my head in the bathroom i just came here to make sure i didn't have a
10:54concussion
10:54and dr ripley said i don't so can i please go home now here's the thing we do need to
10:59inform you
11:00that we are legally obligated to reach out to law enforcement and provide you access to counseling
11:05when we're confronted with evidence i'm just raped
11:13you know um my dad he wasn't around long but uh he did teach me to handle pain with silence
11:22because that's what real men do right someone kicks your ass you suck it up someone makes you cry
11:28you suck it up but that crap eats you up inside trust me it's not worth it you gotta learn
11:39to
11:40ask for help
11:47oh that was super moving but none of it applies to me
11:54got it i really just have one more question screw you
12:00hey go fuss over somebody else who do y'all think you are my mama
12:14you creeping on your daughter's facebook page dr charles
12:17i'm not creeping on anything it's my it's my daughter
12:20and it's a public platform i last checked how about you mind your own business
12:25sorry
12:28but just for the record she moved to instagram a few years back
12:32what what do you mean how do you know that
12:35well because i follow her and you don't
12:41well if you must know i think she might have a new boyfriend and i'm just worried he's a little
12:45sketchy
12:46anna has a new boyfriend
12:49what she's got a new new boyfriend how many boyfriends does she have
12:52dr charles you available for a consult
12:56yeah okay
12:5932 year old sage ellison
13:01she claims she's in searing abdominal pain
13:03and is demanding i perform exploratory surgery and she
13:07put on a real show too
13:09i ran every test under the sun and i didn't find anything
13:12so what you're saying is you think we might have a
13:14big time hypochondriac in there
13:16definitely
13:18thank you
13:19thank you
13:21so could you give me just a rough idea of when you first became aware of of the pain
13:26about two years ago
13:28okay
13:29and always pretty much been concentrated in your digestive system
13:34yeah it always starts off as minor but inevitably it always gets a lot worse
13:39i keep losing weight because eating is torture i mean
13:44it sucks
13:45sounds like it
13:46anything
13:48i don't know
13:49stressful going on in your life at the time
13:53there it is
13:54what
13:56skepticism
13:57you're a psychiatrist
13:58so you think i'm either drug seeking or it's all in my head
14:02the only thing i was thinking is that you didn't answer my question
14:07are you okay
14:08sage
14:11do you really care
14:13actually i do
14:15no
14:17nothing emotionally stressful happened in my life
14:20that triggered me
14:23but i get it
14:25i'm a woman
14:27i'm used to men discounting my physical pain
14:31all right
14:32i hear that
14:34but what i was about to say was
14:36people tend to start making moral judgments
14:39you know when we can't
14:41identify traditional sources of pain
14:43and as far as i'm concerned you know like pain
14:45it's pain
14:47do you know how many stories i've heard of doctors discounting a woman's pain only to later discover that she
14:52had cancer
14:54and that by then it was terminal
14:56because too much time had been wasted
14:58are you worried that this could be cancer
15:00that's my point
15:01i don't know
15:04but my stomach feels like it's being ripped apart by a chainsaw
15:07and nobody seems to care
15:10so please
15:12find someone who does
15:13or even better just send someone down here to prep me for my surgery
15:22oh it's like a table in my brain
15:24oh that's so much
15:26it never ever stops
15:28i need to make it stop
15:37oh come on
15:41what's the problem
15:43now we just schooled me on wordle
15:45again
15:46what's the word
15:48lunar
15:50i completely blanked on the sixth and final guess
15:53and she gets it in two
15:55oh
15:56you okay
15:57seem a little sad
15:58don't
15:59don't
15:59don't even
16:00with me right now
16:01two guesses dr howard
16:03i'm a lifelong astronomy buff so the word came easy
16:07one thing this small town girl doesn't love about the big city is
16:10you can't see the stars
16:12i miss them
16:14wow that is sad
16:16sure is
16:17i gotta go
16:19oh also
16:22i rule
16:26it's a good one
16:29going on there
16:32absolutely nothing
16:34really
16:36sure we've had our moments
16:37okay but we're strictly friends
16:40got it
16:41she has a boyfriend
16:44really
16:45look
16:46there's been a lot of bad timing between us
16:48but
16:49i have a feeling that this new boyfriend is a short timer
16:52next time i won't miss my chance
16:56next time huh
16:59you are judging me aren't you
17:00next times aren't guaranteed
17:02you know
17:03gather ye rosebuds while ye may
17:11yeah
17:12yeah
17:13hey daniel i'm sorry for summoning you but i just got a weird call from the provost of porter university
17:23school of medicine asking for a reference
17:26oh my god
17:27okay look this is howie all right who decided to retire now that's a terrible idea in my opinion but
17:35in his mind i'm the only person in the entire universe who can replace him as a dean of psychiatry
17:40okay i got it as long as it's all in his head and not in yours oh trust me ain't
17:45going nowhere and just for the record i am flattered by your concern
17:49oh and i ran into dr lennox she said you're consulting on a patient who's demanding a surgery she may
17:56not need
17:57yeah sage ellison
17:59poor thing
18:00she's a tough one
18:01but really suffering just not necessarily in the way she might have us believe
18:06okay oh and uh daniel
18:10i'm glad you're sticking around
18:13me too
18:22travis
18:24travis wait
18:24nope
18:27i'm out
18:33well travis just checked himself out against medical advice
18:37sorry i couldn't help you move that needle pal
18:39yeah me too
18:42but i couldn't help noticing that you almost did
18:46so look
18:48none of my business
18:50but i couldn't help wondering
18:53are you in therapy
18:56good catch
18:57buddy
18:58it was always such a hard no for you
19:00i mean what
19:01what got you there
19:03i'm just curious
19:03i don't know maybe uh
19:05hannah and sadie dumping me for the same reason
19:08oh man those personal hygiene issues they finally caught up with you
19:12that and um my savior complex
19:15oh come on isn't that basically just i don't know being a doctor
19:21my three exes all said to me in so many words
19:24just do what you're gonna do danny
19:26because that's what you're best at
19:54ouch
19:54dr charles can i talk to you in private
19:58jake driscoll theater major
20:00he's a prick
20:02he's not ugly
20:03turned 22 on december 19th
20:05any way to tell how long they've been dating
20:06oh i have no clue dr charles there's no hint of anna here no photos as a matter of fact
20:13he still has his dating profile set to single
20:15she was horrified when i called him her boyfriend
20:19oh
20:20well maybe they're not exclusive
20:22maybe this is like
20:24more of a casual thing
20:25he does give me wild child sagittarius vibes and miguel is also
20:29that's great that's great just show me the blog
20:30okay
20:33oh he's been in and out of rehab three times
20:36hmm well he hasn't posted in like a month and a half
20:38i hope he's okay what do you mean do you think he relapsed
20:46alara i am very well aware of how busy you are and so i just wanted to uh
20:51thank you again for taking the time to come in and talk to me about your sister i really
20:54appreciate it just understand i don't want to see sage and she can't know i was here you have my
21:00word
21:03so sounds to me like maybe you guys aren't really in touch right now
21:07not for the last five years you mind if i ask why
21:14let me guess why i'm here
21:17sage is an unbearable pain and she wants emergency surgery to fix it
21:22but you can't seem to find anything wrong
21:25okay so this has happened before
21:30sage is my kid sister we grew up in the foster system musical families some nice
21:40some not so nice but when they weren't nice i was always there to protect her
21:46but when i aged out of the foster system one kid cracked her skull with a bat and she had
21:51emergency
21:52surgery the doctors and nurses knew her situation so they showered her with attention to make up for it
21:59she's been chasing that high ever since
22:03did it ever occur to you the possibility that sage's pain
22:07might be real to her
22:10you're giving her too much credit
22:11the mind can be extraordinarily adept at creating physical pain alora it can she's not in any pain
22:18she's just a liar well
22:21the attention-seeking behavior you're describing is at the core of a very serious condition called
22:26factitious disorder or as it's more commonly known you know munchausen syndrome i don't care what it's
22:32called i just know i can't deal with it anymore i blamed myself for a long time maybe if i
22:39had taken
22:39her with me when i turned 18 she'd be normal
22:45sage kept me on the hook with that guilt well she kind of weaponized it didn't she
22:49you're damn right i believed her lies enabled her defended her to doctors who knew better
22:57her until i realized i had to cut her out of my life if i was ever going to actually
23:03have one
23:08it was my fault
23:11i killed my girlfriend
23:15hi i'm dr charles dr frost asked me to stop by lee holloway but i'm not the patient oh he
23:22is
23:35you know cats don't talk dr charles excellent point what is this uh this kitty's name slater
23:46but i'd call him adelaide for your own safety adelaide it is uh lee brought him in with chronic
23:53vomiting he's been eating cat food uh-huh and um how long has adelaide been doing that for three days
24:01since our cat died oh well i'm so sorry thank you
24:07adelaide was my partner in crime for 18 years i adopted her my freshman year at depaul pre-husband
24:14pre-motherhood she got me through many breakups but it's a pretty extreme way of trying to make me
24:21feel better isn't it lee would you mind if i just had a moment alone with adelaide would that be
24:26all right of course just follow me this way thank you adelaide why don't you just um
24:36want me to jump up on the bed there's a lot of fleas on the floor
24:44wow so look i'm gonna ask you a few questions okay and how about if the answer is yes you
24:54meowed and if
24:56the answer is no you just don't say anything sound good okay maybe the reason that you turned into a
25:06cat
25:06was because your your mom was so sad and you wanted to make her feel better
25:14okay well look i think it's so wonderful how hard you're working to make your mom feel better but i
25:19want to let you in on a little secret moms actually left their kids more than anything anything in the
25:26whole wide world even their favorite pet so what i think is that who mom actually misses the most
25:36just later
25:44it was my fault what was your fault mommy had jersey chocolates on the top shelf of the pantry
25:52i'm not supposed to know because she doesn't want me eating too much sugar
25:58hmm
26:00so i snuck in the pantry after she went to bed one night and ate with you but i forgot
26:07to put the
26:08chocolates back on the shelf i left them on the floor oh i see and adelaide found him
26:18mommy found her dead in the morning she'll never forgive me yes she will i promise you that but
26:29first we have to tell her what happened i know that that sounds scary but i'm gonna help you okay
26:35and
26:36after we do trust me everybody's gonna feel so much better especially you
26:49what kind of doctor are you why do you ask i thought i was gonna have to be a cat
26:55forever
26:55oh well i'm really glad that i could help should we talk to mom
27:16any luck any leads and shields from don't worry dad i kept your name often since his life was at
27:23imminent risk i was able to use accident circumstances to trap the phone fantastic
27:29and and was purchased at a 7-eleven pay as you go no database can ever hit the number so
27:35i got up
27:36quick on a trap and trace ping order but it was turned off right after your hotline oh phone did
27:43ping got an area downtown but it's so dense with high rises and the ping has a 1 000 meter
27:49range of
27:49error so it's like kind of a needle in a haystack thing i did my best i tried to track
27:55every geo every
27:56Giovanni nothing so what are you thinking kind of a dead end i wish i had more for you look
28:03buddy i knew
28:04this was a long shot i just really appreciate you looking into it for me really i do okay at
28:10least i can
28:11do doc so you're calling me a liar no you just told me my pain isn't real it's not what
28:17i said at all
28:17what i said was just because there could be a psychological element at play here doesn't mean
28:23that this is any less real to you okay yeah great mother jeresa but but our nervous systems can learn
28:30to express emotional pain in the form of physical symptoms there it is again what did i tell you it's
28:38all in my head no what i'm saying what i'm saying is that your body and your mind could be
28:43colluding
28:43in ways you're not even aware of because it's not voluntary stop hiding behind all your fancy
28:48double talk and send me someone who will actually listen sage trust me i'm listening you're so full
28:53of it i mean do you actually know how full of it you are i understand the power of attention
28:59trust me
29:00medical attention included for people who feel lonely and abandoned oh i'm lonely and abandoned am i
29:11well you're a human being okay well if i need medical attention to feel whole do you know what that
29:20makes you i don't know what does that make me the freak who needs sick people like me to feel
