When oral medication isn't fast enough and an IV is too invasive, the IM method (Intramuscular Injection) is the clinical gold standard. It’s not just about "poking" a muscle; it’s about utilizing the high vascularity of deep muscle tissue to ensure a steady, systemic release of medication.

In this clinical walkthrough, we explore the precise biomechanics of the IM method. We move past the basics to look at "Vascular Absorption Rates." You’ll learn why a 90-degree angle is non-negotiable for bypassing the subcutaneous fat layer and reaching the target muscle belly. We dive into the "Z-Track Technique"—a method where the skin is displaced before injection to "lock" the medication in the muscle and prevent it from tracking back into the sensitive dermal layers, reducing pain and tissue staining.

✅ Site Selection: Mastering the Deltoid, Vastus Lateralis, and the Ventrogluteal "Safe Zone."

✅ The 90-Degree Entry: Why perpendicular is the only way to ensure intramuscular delivery.

✅ The Z-Track Technique: The professional secret to preventing "leakage" and irritation.

✅ Aspiration Debate: What the current CDC guidelines say about "pulling back" on the plunger.

✅ Needle Gauge Science: Choosing the right length based on the patient's "Adipose-to-Muscle" ratio.

00:00This video will show you how to administer an intramuscular injection.
00:05Wash hands and introduce yourself to your patient.
00:08Identify your patient and explain the rationale for the performance of the procedure.
00:13Allow your patient to ask any questions and verify the medication with your patient.
00:25After hands are washed, make sure you apply gloves.
00:38Select and prepare the injection site with an alcohol prep pad.
00:42Allow the alcohol to air dry.
00:53Prepare the syringe and hold the syringe securely to insert the needle at a 90 degree angle.
01:07With one hand, hold the barrel of the syringe while pulling back on the plunger slightly with the other hand
01:13to make sure a blood vessel has not been penetrated.
01:15This is known as aspirating.
01:18If no blood appears in the syringe, proceed by slowly pushing down on the plunger to expel the medication into
01:24the muscle.
01:30Remove the needle quickly by the same angle of injection, apply the safety, and discard into the sharps.
01:38Wipe the site gently and make sure your patient is okay.
01:43Apply a bandage as needed.
01:46Check on your patient.
01:49Remove your gloves.
01:51Wash hands.
01:56Assist your patient down and complete documentation in their chart.
02:01Text your patient.
02:02Text your patient up on the other hand to repeat.
02:03Twist your patient.
02:03Distant your patient.
02:04Simply use your patient.
