04:5677-year-old Robert recently had surgery to treat a bleed on his brain.
05:01Tonight, after bleeding heavily from his nose, his son called 999.
05:07What I've had it happen before, it's usually stopped within, you know, ten minutes or so.
05:13Ten minutes, but it's been half an hour.
05:14Yeah, it's good.
05:16How much blood would you say that you've estimated?

06:30The fact that it's still going after half an hour and yet it is losing quite a bit.

09:14We've got 92-year-old male, presses care line, leg injury, fall injuries unknown.

09:23We could have a neck of fema fracture.

09:30I think when you get called to a 92-year-old that's fallen, we are thinking,
09:35is he going to have broken something?

09:39Generally, it's something that happens, isn't it, when they're older?

09:43And they're a bit more frail.

11:16he's just had a massive clot about this big just come out from his mouth that's gone up his nose
11:20and then come out his right bear with me sorry we've just had a clot probably about three
11:24centimeter voice three centimeters round just come out of his mouth he said it was up his
11:28nose and it's about that big let's just get going then to the west of Birmingham paramedics Mark and
11:40Amy are with Robert having recently undergone brain surgery they're concerned his severe nosebleed
11:46won't stop there we go right then you okay don't feel dizzy at all let's get going then just nice
11:56and steady down the stairs okay Robert needed to go to hospital as he had had recent brain surgery
12:03nosebleeds can be a symptom if something's gone wrong with the brain surgery so we needed to get
12:08him there fast it's put some straps on you and again just checking no headache no dizziness
12:21in the future just found 999 don't worry about anything we'd rather come to something like this
12:25a lot of people say oh it's only a nosebleed but there's different circumstances with yours you know
12:35what I mean you have arrived at your destination it has slowed down don't pick it at Russell's Hall
12:45Hospital in Dudley doctors will investigate why Robert's nose won't stop bleeding they'll also want
12:52to see if it's related to his recent brain surgery to the south of Birmingham paramedic Daisy and student
13:12paramedic Morgan are with Ron who's in agony after a fall days I'm not sure if we're gonna get this
13:19stretch your head do you know what I mean do you want to try or do because I just don't
13:23think we're
13:24gonna get him on carry chair I don't think we are I think putting him in even more pain just
13:28to get
13:28him out I think we need to try other ways first yeah I'm gonna game treatment here first yeah I

13:36want to move him Ron has very clearly hurt himself we think he might have broken his hip so he
13:42needs
13:42to go to hospital now we need to figure out how we're gonna get him there Ron what can you

13:49really really still for me really still whilst we do this heart tracing yeah before attempting to
13:57move Ron Morgan runs an ECG to check the state of his heart really still he's always dead oh Ron

14:07not say things like that what about your daughter no you're not she loves you she give up a lot

14:16time for me well you gave up a lot of time when she were a baby when Ron said that
14:23he didn't want
14:24to be alive anymore it was quite upsetting and it was clearly because he was in so much pain so

14:30just wanted to make him more comfortable try and get him feeling back to himself right I'm gonna
14:36give you some paracetamol through that vein okay was it shooting up and down your leg oh Ron
14:54Daisy and Morgan give Ron liquid paracetamol intravenously but it's having little effect
15:02why it's good from something stronger you know I don't think we're gonna move him without getting
15:08him so much stronger if I'm honest I agree Ron had very clearly hurt himself we do believe that he
15:14had
15:14broken his hip so in order to move him we knew we were gonna need to give him some strong
15:19pain relief
15:19just to try and keep him as comfortable as we can during the process is it all right if we
15:25give
15:25you some morphine carry on please we're just gonna grab it and we're gonna grab our stretcher okay
15:32yes thank you don't move whilst we're gone all right stay still we ain't got a prayer mate
15:46no moving on there's not really any other way to get him out other than on the chair it was
15:56quite
15:56difficult to figure out a way to extricate Ron we couldn't fit the stretcher through the door we
16:03couldn't use a spinal board the only other option that we had was a chair which was quite uncomfortable
16:11but it was just a short-term solution until we got him sorted I'm gonna give you some morphine I'm
16:18sorry I'm feeling so sorry why are you sorry this is our job do you want to try some gas
16:25and air Ron
16:27yes right so if you pop this in your mouth if you suck it in you got it
16:38Ron Ron if you suck that in it should ease the pain but you've got to keep sucking on it

16:47breathe as normal with it Daisy and Morgan have now given Ron all the pain relief available to them
16:54right Ron we're gonna move you okay keep sucking on that whilst we move you because this might be

17:44broken his hip he's not alone 70,000 people break their neck of femur every year there you go

19:36know which direction roughly to travel that's what we sat navs for that's why you are no good at geography
19:42because you realize on your sat nav it's a country that potentially had Vikings there
19:50Sweden no Norway yeah but or not Norway has the longest coastline you know I'm gonna look at that on

20:15and I'm supposed to have it operated on what symptoms do you have my love it's painful my heart racing

20:48all that time the call came in she's woken up with chest pain that can be quite concerning
20:54she's never a good sign we're only nine minutes away hmm bit tight it's scary into some of the rural
21:03communities and how isolated and vulnerable they actually are this was potentially quite a serious
21:10case just given her age given the location of where it is and the amount of time it would take
21:15to get
21:15there yeah the location of a property was up a really long narrow road which just made it extra hard
21:21to get to her
21:26so what's been going on then
21:29well I've got a heart valve problem and you know waiting to have it done but
21:35oh tonight it really got me I woke up and I got this real
21:41pain and my heart was racing
21:46when racehorse breeder Rosalie was woken in the night with pains in her chest she rang nine nine nine
21:52husband Peter is with her
21:54what time did she wake up with the chest pain though
21:57how little time do you want to go for chest pain at three o'clock at three o'clock do
22:03you have an irregular heartbeat by any chance
22:05not normal I don't think
22:06you can see they've they put it in she's got aortic valve stenosis
22:11Rosalie was experiencing some chest pain which could be caused by her pre-existing heart condition
22:17aortic valve stenosis is the narrowing of a valve within a heart which affects the blood flow into the arteries
22:24it effectively means the heart has to work a little bit harder
22:27so you've got palpitations in your chest at the minute sweetheart

22:34the longer I spent with Rosalie the more concerned I got
22:37she was telling me about needing a heart operation
22:39how she's now got chest pains and palpitations
22:42so the longer I was there the more concerned I was becoming for her
22:45nice and still a second
22:49Adam carries out an ECG to check the electrical activity of Rosalie's heart
22:54yep we'll be going
22:55you're going to hospital sweetheart

22:59you've got a heart rate at the minute it's about 160

23:03it should be ideally between sort of 60 and 100

23:08I knew something was wrong anyway

23:16so it's saying STEMI
23:19yep so we're going to have to ring
23:20yeah we'll give him a buzz
23:22once we completed a heart tracing on Rosalie
23:25it showed us that not only was her heart beating pretty fast
23:29at over 160 beats per minute
23:31but it was also showing us that she was actively having what we call a STEMI
23:35which is a heart attack
23:45as the West Midlands sleeps we're on board with eight paramedic crews working the night shift until 6.30 in
23:52the morning
23:57we are going to be taking her to hospital and we are probably going to be going quite quickly
24:03on the outskirts of Stoke paramedics Colleen and Adam are with Rosalie who they suspect is having a heart attack
24:10I'm just going to give you a spray under your tongue if I can
24:13yeah
24:13this will hopefully take some of the chest pain away
24:16Adam gives Rosalie a spray under her tongue
24:19this opens up her blood vessels reducing the strain on her heart
24:24we're going to look and see if we can give her aspirin
24:27then we're going to look at giving her a needle in her arm as well
24:29I'm just going to pop in your hall and make a phone call okay

24:34Colleen calls the hospital to let them know they'll be arriving soon with a patient who's having a heart attack
24:40so I did the right thing calling you

24:42I don't like to think I'm wasting anybody's time

24:48they said go to A&E initially but give resource a quarter to go there

24:54Rosalie has to go to hospital because she's actively having a heart attack
24:58she needs further treatment that we cannot provide her
25:00it's imperative that we get her there pretty quickly to get the treatment she needs
25:04as fast as she can
25:05we're going in on blue lights we're going to be going quite quickly
25:08she is unwell
25:09so it's up to you what you choose to do
25:11but if you do
25:14they'll look after me I know that
25:16we'll get you on to the ambulance and we'll get you to Stoke as quick as we can okay

29:40it meant that I needed to then think about how we were going to get the caller

30:37control has informed the woman was trying to take her own life
30:42we've got some updates on that case
30:45so
30:46she's alive
30:48they've now got her off the roof by her
30:51we're there as well
30:52I assume that's to help get her down off the roof
30:54yeah
30:55because they weren't quite sure how they were going to get her down to be fair
30:57I just couldn't get my head around it

31:00it just didn't make sense as to why she would be on the roof
31:02and she's on the back of the ambulance so hopefully she'll be okay

45:11had rectal bleeding spent three days in hospital she may require surgery to repair her intestines we
45:19went straight through to see a consultant who was brilliant so hopefully he's going to have some answers
