04:49Because when you're having a seizure, you're not getting enough oxygen to your brain.

14:32It doesn't necessarily mean that she's having a stroke, but it could be a sign towards something neurological happening.

14:45I don't know if you're realising that you're doing it.
14:47But I think all of that together warrants a trip up to hospital.
14:52And now it's just about getting you out because you're not very steady on your feet, are you?
14:57We thought maybe it could be that she's having a bleed potentially that is developing as we're spending more time

21:05any fluid down not eaten for a few days
21:13they've been allocated a 83 year old female reporting dizziness and she's currently bed bound
21:21with an elderly patient with these kind of symptoms weakness diarrhea vomiting and not eating you're
21:26thinking about their recovery time and how they aren't as able to bounce back from an illness
21:31as there's somebody who's younger would be able to

27:48she lost any blood from anywhere all her knees are all knocked about and scuffed

29:57support and they need a safety net and they need a blanket of of help the neighbor she's run to

35:24can't swallow food it's got gradually got worse over the last couple of days
35:30right we've got a category two um 90 year old lady chest pain back pain lip swelling difficulty swallowing
35:38and pain so let's have a look
35:44this lady it stated she was 90 and she was in a lot of pain so i think instantly we
35:49started to think
35:49what kind of pain is she in where is it coming from and how much is it affecting her with
35:54obviously her age

36:1290 year old nova has been suffering with pain in her back and mouth for the past four years
36:19but tonight when the pain became unbearable daughter sue called 999
36:25we'll get all these checks sorted and then we'll make a plan of what we're going to do all right
36:29sweet heart every four minutes this is happening and it just comes in like it then it'll go but she
36:39can't lean on her back pain in the back triggers the pain in face she can't eat a meal when
36:47it goes in
36:47the mouth she's getting a pain at the back of the tongue okay it was quite perplexing as to what
36:54was
36:55going on with the pain in the face the pain in the back how it was related so we were
36:59quite confused as
37:00to what was going on when you get this pain sweetheart i know this i know it's really difficult to
37:06describe
37:07pain but what sort of pain is it is it burning stinging it's coming quickly are you ready yeah
37:14it sounds very it's coming it's coming more frequently yeah okay blood pressure's high at the
37:20minute could be because of the pain that would make sense i'm just going to do a little pinprick
37:27on your thumb sweet top sharp scratch okay can i pop these stickers across your chest to do an ecg
37:35is that all right because your heart rate's jumping around a little bit yeah anything you want
37:41nick carries out an ecg this will provide a trace of nova's heart and might give a clue as to
37:48what's
37:48causing her pain have you still got your tonsils no where's that accent from i thought it was
37:56somewhere down there yeah you don't sound like a stokey duck in your mouth yeah are you okay too
38:07but you can tell your lips swollen oh up there looks a bit swollen up there yeah okay okay and

38:18there does it feel sore yeah whenever anybody is in any pain or discomfort in the mouth we have to
38:25be
38:25very careful of the airway because it needs to be kept clear at all times so we have to consider
38:31if
38:31this allergic reaction any damage with nova we could see that she was in pain but we had absolutely no
38:38idea what was causing it let me have a little listen to your chest we thought i'm starting again
38:46so it's like comes in spasms don't it we need to get you sorted out
38:58rest are fine because she's able to sing so she's not lacking in breath
39:03is that the only thing i know is there is a more to it you can sing but can you
39:11sing it a bit quieter

39:18in birmingham paramedic daisy and student paramedic morgan are with a patient who's been arrested
39:25she's allegedly assaulted reception staff and the police
39:37we can't see any abnormalities the heart rate was a little bit high wasn't it yeah it was up a
39:44little
39:44bit we didn't do an ecg to sort of rule out anything to do with that um but she was
39:50quite erratic
39:51so we would expect the heart rate to be a little bit higher than we don't think there was anything
39:55medical going on no not at all
39:57okay
40:16daisy and morgan's observations show that this patient doesn't need to go to ame
40:22she can now be taken to the police station
40:27she is understanding what we're saying as well she's acknowledging things when you said you said
40:31you can sing sing a bit quieter she went she went louder she's stood up when the best she's eye
40:37contact she understands we were there to determine whether the patient needed help medically or if it
40:45was something that the police needed to deal with and ultimately we decided that it was something for the
40:50police she's under arrest all right i don't understand i know i know i know
41:00she's under arrest yes she's done other things it was not a normal case it was no this is not
41:07what
41:07you see every day it was a bit of a mystery in terms of we didn't know exactly what was
41:11going on
41:12we couldn't pinpoint it oh dear i felt so from that yeah i did but then also she knew we
41:22were there
41:23she was acknowledging us she knew we were talking to her what a strange way to be i just don't
41:30get it
41:30i don't get why she started doing that like what is going through her head
41:44right plan of action in stoke paramedics nick and kyra are with nova who's been experiencing severe pain
41:53in her face once we get the hospital we'll have a word and explain we need the specialist people on
42:00the ward that know more about this and they can hopefully start looking at medication we decided to
42:08take nova to hospital due to us not really knowing what was causing this mouth pain and having a knock
42:14-on
42:14effect with the back more tests needs to be performed so we get down to the bottom of it
42:19right let's get rid of these you can have some oromorph now and then once we get the hospital
42:26we'll see what we can try and sort out for you oromorph is liquid morphine it's already prescribed
42:34for nova by the gp to help manage her chronic pain right you're going to get on here ducky we're

44:15gently as we can sweetheart okay at royal stoke university hospital nick wants to get nova seen
44:22by a specialist to determine the cause of the excruciating pain in her face
44:34oh she was a lovely lady i know it's not nice when they're in so much pain and discomfort and
44:42we
44:42can't really do anything to help them yeah it's really frustrating well hopefully she gets sorted
44:47she could be back with while at home with a little dog soon yeah
44:56nova was diagnosed with trigeminal neuralgia
45:00a condition where nerves in the face get compressed
45:03she was monitored in hospital for 10 days and given new medication to better manage the pain
45:11maureen had a scan that revealed she had an inflammation of her bowel
45:16she was given antibiotics to help treat the infection
45:22and after collapsing on a narrowboat jenny had scans in hospital these showed she hadn't had a stroke
45:30doctors diagnosed an inner ear infection that can cause vertigo and nausea
45:41next time his legs have gone completely a man has a suspected stroke you know when you closed your
45:48eyes then did you feel like you were going leaning to one side at all yeah
45:52we've only got a very short window of opportunity that we can help and save and treat that stroke so
45:58time really is not on our side while hiking a man falls injuring his leg when we got a call
46:05saying
46:05mountain rescue were on scene i think our first thought is where on earth are we going yeah and how
46:11are we getting there what we don't want to do is put any more strain on your heart all right
46:16and pain
46:16from coughing masks a heart attack i have to be careful how i explain it to him if we put
