00:00You
00:05Morning, Sudri.
00:06Last week, it was a really bad smell coming from right here.
00:09OK.
00:10OK.
00:11Really bad.
00:12OK.
00:13Really bad.
00:15Cockroaches.
00:16We have mice.
00:17Sewage coming out.
00:18Bathrooms coming up in the kitchen.
00:20These things and it's constant.
00:26It moves around, doesn't it?
00:28It's like water inside.
00:29Yeah, it does.
00:30It feels quite squishy, doesn't it?
00:34If you're going to die, you're going to die.
00:35Get out and enjoy yourself.
00:36Don't call me and wait for death.
00:38Get out and enjoy yourself.
00:40Get out and enjoy it.
00:41That's why I like it.
00:42Very positive.
00:43I like it.
00:45One of the biggest things that predict outcome is often motivation.
00:50But you're the most motivated person I know.
00:52It's really frustrating.
00:53Hey, cheers, it's mad.
00:55Cheers.
01:00Cheers.
01:01Cheers.
01:02Cheers.
01:03Cheers.
01:05Cheers.
01:10Cheers.
01:11Cheers.
01:12Cheers.
01:13Cheers.
01:14Cheers.
01:15Cheers.
01:20Cheers.
01:25Cheers.
01:26Cheers.
01:27Cheers.
01:29Cheers.
01:30Cheers.
01:31Cheers.
01:33Claire, please.
01:34Claire, please.
01:35Hi.
01:36Hi.
01:37Hello.
01:38I don't think we've...
01:39Have I...
01:40seen...
01:41Dr. Prestwich.
01:42You look familiar.
01:43Have you seen my...
01:44Maybe my children.
01:45Perhaps.
01:46Yes.
01:47They're usually the ones...
01:48The main ones to worry about.
01:49Exactly.
01:50How can I help?
01:51How can I help?
01:52Um...
01:53I've had this weird...
01:54light...
01:55Ding pain.
01:56Sort of across here.
01:57Hmm.
01:58I saw my cousin.
01:59She...
02:00Does some therapies.
02:01And she said to me,
02:02Oh yeah, that's that.
02:03And I...
02:04I can't remember what she said.
02:05But went...
02:06Combined with the headache.
02:07And then I had it this morning.
02:09And it sort of...
02:10Comes and then it goes.
02:11If you feel my head...
02:12This side...
02:13Mm-hmm.
02:14Is a bit swollen...
02:15Okay.
02:16So...
02:17The headache started...
02:18This morning.
02:19This morning.
02:20I took some Nurofen.
02:21And I've taken Paracetamide.
02:22And it's...
02:23Still...
02:24I can feel it.
02:25Still there.
02:26If I had...
02:27Just had the headache.
02:28I wouldn't be so worried.
02:29Um...
02:30But...
02:30It was this electrical thing.
02:31I get it about twice a week.
02:33So...
02:34Like a zapping...
02:35Kind of sensation.
02:36Yeah.
02:37Exactly.
02:38Does that feel deep or superficial?
02:39Deep.
02:40Deep?
02:41Yeah.
02:42Okay.
02:40Any sensitivity to light or sound?
02:42I mean I live with three children.
02:44So I think I've...
02:45Got sort of...
02:46Noise PTSD.
02:47But not...
02:48Not particularly.
02:49Okay.
02:50All right.
02:50Let's examine you now.
02:51Do some observations.
02:52Just keep your head still.
02:53And try and follow the...
02:55Shiny bit of my pen please.
02:56Any double vision?
02:57No.
02:58Any double vision?
02:59No.
03:00Any movements look normal to me.
03:02So...
03:03Erm...
03:05Right.
03:06Now you're gonna look at me again.
03:07Cover that eye.
03:08Great.
03:09Looking here.
03:10Tell me when you see me.
03:10My fingers moving.
03:11We're cheating.
03:12Looking at my nose?
03:13Great.
03:14No.
03:15Okay.
03:16Yeah.
03:17Yeah.
03:18Yeah.
03:19Okay.
03:20Sorry.
03:20So just close to your eyes for me.
03:21Can you feel that touch on your forehead?
03:23Mm-hmm.
03:24Does it feel the same on both sides?
03:25Yeah.
03:26And here?
03:27Mm-hmm.
03:28And here?
03:29Yeah.
03:30Okay, you can open them now.
03:30So we've just tested cranial nerves.
03:31The only thing I haven't done is looked in the back of the eye, which I would...
03:35like you to get done with an optometrist for an eye check and an eye health check.
03:39Mm-hmm.
03:40This is...
03:40This is the most thorough exam I've ever had.
03:43Yeah.
03:44Usually it's...
03:45Can you take 2-paracetamol?
03:46Well, that's probably going to be the advice, but let's see.
03:48Yeah.
03:49Yeah.
03:50I'm just going to pull on you.
03:51Does that feel okay?
03:52That feels nice.
03:53Oh, has it improved your hair?
03:55A headache?
03:56I like...
03:57I like that.
03:58Yeah.
03:59Can I just check the other side for comparison?
04:00Yeah.
04:01And that all looks fine too.
04:04I mean...
04:05Okay.
04:06I don't think there's anything...
04:10worrying going on.
04:11So, yeah, sometimes when a headache comes on really, really suddenly...
04:15we think, is there something going on in the brain?
04:17Mm-hmm.
04:18It doesn't sound like that kind of headache and there's no neurology to...
04:20go with it.
04:21So, I think the most likely headache we're experiencing at the moment is a tension headache,
04:24which is where...
04:25the head is essentially being squeezed by the muscles and soft tissue around the head.
04:30because they're being pulled on by muscles in the neck, shoulder and back.
04:34Mm-hmm.
04:35So...
04:35that squeezing pressure, as well as being painful, can give some...
04:40other...
04:41symptoms as well.
04:42Yeah, this is day one of this headache.
04:43There might not even be a day two.
04:45But if there is, I'm going to give you a headache diary and we can start logging things.
04:49If anything changes, we can...
04:50you know, we can review it.
04:51Is that okay?
04:52Yeah, that's great.
04:53Absolutely.
04:54And yeah, do come back...
04:55back to us...
04:57...if these headaches don't settle down or get worse.
04:58Yeah.
04:59Thank you so much.
05:00Take care.
05:01Bye.
05:02Bye.
05:03Bye.
05:05Sophia?
05:06Sophia?
05:07Sophia?
05:07Hi.
05:10Come on through.
05:12Come and take a seat.
05:14My name's Emma.
05:15Emily, how are you today?
05:16Very good, thank you.
05:18How are you?
05:19Good, thank you.
05:21How can I help you?
05:22So I've come in, I think, for a swab test.
05:25Yeah.
05:25For an endometriosis diagnosis.
05:28So you've spoken to the doctor about this?
05:29Yes.
05:30Yes, this is kind of the preliminary part of it.
05:33So I've already been in for an old...
05:35Intrasound as well.
05:37Perfect.
05:38Are you currently...
05:40Are you using any form of contraception or hormonal pills to try and regulate everything?
05:43No.
05:44Okay, great.
05:45So what we would normally do for this is two types of swabs.
05:50We'll do a swab to check for unthrush and bacterial vaginosis just to see if there's any effects.
05:55And then also a swab for STIs.
05:58So we look at chlamydia and gonorrhea specifically.
06:00Is that okay?
06:01Brilliant.
06:02So if I could ask you please just to remove everything...
06:05And then head on the pillow and paper towel over your waist.
06:09Are you ready?
06:10Yes.
06:10Perfect.
06:12What have you got planned for the rest of your day today?
06:15I'm actually going into work.
06:17What do you do?
06:18Marketing.
06:19So...
06:19Yeah.
06:20So is that all on...
06:21What is that on like social media or...
06:23So it's like Instagram.
06:25TikTok.
06:25Well, it goes over my head.
06:27I know it's such a great sort of use.
06:30It's got a good mix of like pre-advertising and things like that, but I...
06:33It's got a good mix of different things.
06:35And it's always good to hear when people enjoy their jobs.
06:37Yeah.
06:38Okay.
06:38So if I can get...
06:40I want you just to drop your knees outwards to whatever feels like a comfortable position
06:43for you.
06:43That's brilliant.
06:45Just opening up the speculum.
06:48Doing really well.
06:49Are you okay?
06:50Yeah.
06:51Okay.
06:51First swab.
06:53Done.
06:55And then last swab.
06:58All done.
07:00Well done.
07:00Perfect.
07:02You did so well.
07:02Are you okay?
07:03Yeah.
07:03All good.
07:05All good.
07:06All good.
07:07All good.
07:08All good.
07:09All good.
07:10Well done.
07:13Brilliant.
07:14Well...
07:15Give us a call on Monday.
07:18We will go through your results.
07:20Any issues, just let me know.
07:21My name is Emily.
07:22Perfect.
07:22Thank you so much, Emily.
07:23Good luck with everything.
07:25Have a good day.
07:27You too.
07:30I'm always getting some weird stuff sent to my email.
07:34Every time I...
07:35Over my emails, I've always seen cats and dogs.
07:37I don't know what it is.
07:38Now I've got a horse.
07:40Oh, my God.
07:40Yeah.
07:41What a fuzz.
07:42Must be some group thing.
07:43I'm going to reply to one.
07:45of these.
07:45I'm going to give out some cute cats.
07:48No, they are so cute.
07:50Come and have a seat.
07:51Thanks for coming down.
07:53Thanks for seeing me.
07:53Right, there we go.
07:54Thanks for seeing me.
07:54Now.
07:55How are you?
07:55Um, not great, to be honest.
07:58No.
07:58Um...
08:00Yeah.
08:00I'm going to try not to cry, but I feel like I'm already starting.
08:04This must be done.
08:05Just take your time.
08:07Don't worry.
08:07Just take your time.
08:07Don't worry.
08:10I know you've had a really tough time, haven't you?
08:15I know you've had a really tough time, haven't you?
08:35Let me say what you have a minute that you've had the horrible long COVID, haven't you?
08:40And you had COVID, I think, was it July or something, 2021? It was a while ago, wasn't it?
08:45And it really knocked you for six, didn't it?
08:47And then you were under all these sort of specialists and it's affected all sorts.
08:50Yeah, over the course of three years, built up to working 30 hours.
08:55And then I sort of burnt out, which just wasn't functioning well.
09:00And I decided I needed to go on sick leave.
09:03But it's been really hard and I got COVID again.
09:05I can't believe it's the second week.
09:07You are the most unlucky person.
09:09And I didn't even go anywhere.
09:10Where did you get COVID?
09:11My boyfriend.
09:12So he's going into the office every day.
09:15And poor thing, he came home with it.
09:17I mean, it's easily done, isn't it?
09:19It just shows it doesn't matter how many people...
09:20People you see, you only see one.
09:21Exactly.
09:22But you can't put yourself in a rubber ball and, you know, never...
09:25You never see anybody ever, can you?
09:26Yeah.
09:27That would be ridiculous.
09:28I think that's the fourth time.
09:29And so...
09:30It's been...
09:31Instead of sort of recovering from the burnout...
09:34Yeah.
09:35I've been recovering from COVID because it hits me really hard and I get...
09:40all of the symptoms.
09:41So each time you get COVID, it sort of brings you back to square one again.
09:44Yeah.
09:45So like I've got stomach problems from COVID, which I've seen a consultant about and that was...
09:50sort of, you know, I was tapering off the medication.
09:52It's getting better.
09:53I'm now back on the full medication.
09:55So poor Emma had COVID quite badly early on in the pandemic and it was...
10:00resulted in leaving her with some huge amount of symptoms of long COVID.
10:03She's fatigued.
10:04She's dizzy.
10:05She feels really unwell and it's required her...
10:06She hasn't been able to work.
10:08She was just getting everything back on track and...
10:10getting back into work and then she got COVID again.
10:12And unfortunately that's been a bit of a setback.
10:15I'm hopeful that it's not going to be as big a setback as perhaps it was first time
10:18and that she's got the support from the long...
10:20COVID clinic who are really helping her.
10:22And hopefully with the support from us and from them, we can...
10:25get Emma back on track and back into work, which she's desperate to do as soon as possible.
10:29Yeah, it's been really...
10:30hard and...
10:32I'm just sort of...
10:34a bit scared.
10:35I'm scared of going back to work actually because it was...
10:37it was really hard.
10:38Yeah.
10:39Yeah.
10:40when I stopped sort of being able to function properly.
10:43And I'm just...
10:45yeah.
10:46And it's also a lot to sort of...
10:48accept that.
10:49Um...
10:50I don't think I'm going to be able to work full time.
10:53I mean, the good thing, Emma, is that you have...
10:55got better.
10:56You know, you were functioning, working 30 hours a week, you know, really picking up.
11:00And then it's gutting that it's happened.
11:02And maybe we can learn something from that.
11:04If you say you've got...
11:05burnout, well, what maybe caused that?
11:07Can we look back and think, well, how can we prevent that coming?
11:09Did we go up to...
11:10Yeah.
11:11Did we...
11:12Were you doing too much outside of work?
11:13Was it...
11:14you know, were you not doing enough?
11:15You know, we can look at that and try and perhaps explore.
11:17And I remember someone once describing COVID, long COVID to me...
11:20that most of us wake up in the morning with our battery full and we're ready to go for
11:23the day and you're waking up and you haven't been...
11:25charged all night.
11:26No.
11:27So you've only got that little bit and if you do too much, you're out.
11:30You're drained and you can't...
11:31Yeah.
11:32So it's just sneaking in while you slowly build it back up.
11:35We did build it back up last time.
11:36So it does work.
11:37Yeah.
11:38The one thing is that you...
11:40If you're not ready, you're not ready.
11:43And if you go back too soon...
11:45it won't work.
11:46Yeah.
11:47If you'd broken your leg and it was a physical injury and you had a...
11:50physical job, you'd be like, well, I can't work.
11:51Yeah.
11:52And because we can't see it and you look lovely and healthy...
11:55you sort of look in the mirror and think, well, I should be okay.
11:58But it's not.
11:59It's a...
12:00problem.
12:01One of the biggest things that predicts outcome is often motivation.
12:04And if someone's not...
12:05motivated to do something, it's really hard to get better.
12:07But you're the most motivated person I know.
12:09So I...
12:10I honestly think you'll...
12:11we'll get there.
12:12It's just a slow...
12:14ploddy road.
12:1580.
12:1680 is, isn't it?
12:17Yeah.
12:18But you've done it before.
12:19I've done it before.
12:20And you've got up to...
12:2030 hours a week and you're brilliant.
12:21And your employer, I can tell you now, they would have booted you out if they didn't
12:23think you were worth having back.
12:24So the fact that...
12:25they're being so supportive means they want you to come back.
12:27It's just we have to do it carefully.
12:29Okay?
12:30Yeah.
12:31All right.
12:32Well done, sweetheart.
12:33Well, listen, good luck with everything.
12:34Don't forget your broly.
12:35I think you might need it.
12:36Thank you so much.
12:37Not at all.
12:38It's nice to see you.
12:39Take care.
12:40All the best.
12:41Bye.
12:40Bye-bye.
12:41Bye-bye.
12:42So my name's Safa.
12:44I am...
12:45I'm doing your blood test today.
12:47And it's just all routine bloods for you today.
12:49So we're doing like...
12:50your full blood count, your diabetes, cholesterol, liver, kidneys, thyroid.
12:53We've got your vitamin D in it.
12:55And your B12 as well.
12:56Amazing.
12:57Is that okay?
12:58Yeah.
12:59And also your ferritin.
13:00That's also added.
13:01Okay.
13:02So which arm is usually best for bloods?
13:05You've got lovely veins.
13:06Whichever one you would prefer me to go into.
13:09I mean, this is...
13:10That's my mousing hand.
13:11So maybe that one.
13:12Yeah, that's fine.
13:13I cycled over.
13:14So everything should...
13:15to be like...
13:16ready to pump out.
13:17That's good.
13:18That's so good.
13:20Does Sam still work here?
13:22Yeah, she does.
13:23She's actually on leave at the moment.
13:25Yeah.
13:26Slight scratch.
13:27I only actually became a HCA.
13:30just one year ago.
13:32Before that I was a phlebotomist as well.
13:34I can do...
13:35Bloods.
13:36Blood pressure.
13:37ECGs.
13:38Vaccinations.
13:39Diabetes.
13:40I think when you work as a receptionist you...
13:45do get that one to one.
13:46But I think it's just that one to one in the room.
13:48Yeah.
13:49I love my room.
13:50as a HCA.
13:51I remember coming here...
13:53when I turned 40 MOT.
13:55Okay.
13:56Because Sam gave it to me.
13:57She was so funny.
13:59Because I got...
14:00like an A plus on my report.
14:02That's good.
14:03And she was really impressed.
14:05Yeah.
14:06And I was like...
14:07Okay.
14:08It's so funny.
14:10So if everything went fine with it...
14:12then it's usually every five years...
14:13that you're called in for another.
14:15check.
14:16That's all done.
14:17I'll just take the needle out.
14:23So...
14:25with the results...
14:26they should be back...
14:27maybe by Wednesday or Thursday.
14:28Okay.
14:29We say...
14:30if there's a call...
14:31anything urgent...
14:32a doctor will be in contact straight away.
14:35Okay.
14:36All done.
14:37Beautiful.
14:38Thank you so much for your time.
14:39You're welcome.
14:40It's okay.
14:41Have a super day.
14:42You too.
14:44Thanks.
14:46What?
14:47Is that a delivery?
14:48Oh yes.
14:49It's been delivered.
14:50Yeah!
14:51That was...
14:52What is it?
14:53Let me have a look.
14:54It's so cute.
14:55It's so cute.
14:56That.
14:57I like it.
14:58I've got my heels as well.
14:59I've got my long...
15:00I've got my thick black heels.
15:01Oh my god.
15:02Why are you not selling my heels?
15:03I've been delivered.
15:04I'm so happy.
15:05I love you.
15:06I love you.
15:09I love you.
15:10Hello.
15:11Come on through.
15:13Good morning.
15:14Maybe good.
15:15Maybe good.
15:16I like London weather.
15:17Come and have a seat.
15:18How are you?
15:19I'm okay.
15:20I'm more happy to visit museum than GP.
15:22Okay.
15:23Well tell me what's brought you in though to see us today?
15:25Sorry.
15:26I used Google Translate a little bit.
15:27That's okay.
15:28Let's have a look.
15:29Swelling on the neck that appeared a month ago.
15:30Oh.
15:30Okay.
15:31Painful?
15:32It's not painful like cutting.
15:35Okay.
15:36Like you burn your skin.
15:37Yup.
15:38Yup.
15:39And when I put my neck like that...
15:40Then you...
15:40You can feel it.
15:41It looks like I have ping pong ball inside.
15:43And any problem swallowing?
15:45Little bit.
15:46And food?
15:47You can eat food okay?
15:48No.
15:49Food is okay.
15:50Okay.
15:50I don't know a long time ago.
15:51It's six, seven...
15:52Six, seven...
15:53Weeks.
15:54Weeks.
15:55Coming...
15:55Let's have a seat on here.
15:56Let's just have a look at it.
15:57Okay.
15:58We can have a look and compare left and right.
15:59So that side...
16:00It's okay.
16:01It's quite flat.
16:02And that side...
16:03There's quite a bulge, isn't there?
16:04Can I press it very gently?
16:05Yeah, yeah, yeah.
16:05That's okay?
16:06You can press more.
16:07Don't worry.
16:08Okay.
16:09And it moves around, doesn't it?
16:10It's like water inside or something else.
16:12Yeah, it does.
16:13It feels quite squishy, doesn't it?
16:14Mm-hmm.
16:15And if you turn and look your head that way a little bit...
16:17Okay, then it sticks...
16:18It should look like something...
16:20It's pulling.
16:21It's pulling because it's attaching to the muscle perhaps and pulling a little bit.
16:24Let me just have a little...
16:25Listen...
16:26I don't think it's got any blood in it, but let me just check.
16:31No, it's fine.
16:33No.
16:34I can't hear anything.
16:35I can't hear anything in there, which is good.
16:36And...
16:37And it's funny.
16:38And it's been there so...
16:39And it's growing.
16:40Okay.
16:40Come and have a seat.
16:41Come and have a seat.
16:42Let's just have a chat.
16:43Is it limp or not?
16:44It...
16:45So a lymph node...
16:46Because you know why I'm afraid?
16:48My wife's of cancer.
16:50Oh, dear, yes, I'm sorry.
16:51And she have operation the same.
16:52And doctor check under and there as well.
16:55And I'm in panic.
16:56Maybe I have the same problem.
16:58How is your wife?
16:59You mentioned your wife.
17:00The doctor guarantee five years life, no more.
17:05It will be more.
17:06Okay.
17:06Five years ago.
17:07Wow, and she's still okay.
17:09Yeah, yeah, because.
17:10She has a lot of chemical and radiation.
17:13Okay, okay.
17:15She loses her immunity.
17:17Yeah, but it doesn't matter.
17:17This is life.
17:19Well, it's difficult.
17:20But I can understand why then you'd worry if lumps and things come up.
17:23We need to check.
17:24So it doesn't feel like.
17:25It's like a lymph node because normally a lymph node is a bit firmer and is smaller.
17:30This feels more like a sort of what we call a fatty lump, perhaps a lipoma.
17:34Before.
17:35It starts that this one start growing as well.
17:38Maybe it's going there.
17:39You know, you see this one.
17:40Let's have a look.
17:40So this is a small cyst here as well, isn't it?
17:43It starts from there.
17:44After that, it goes there.
17:44Okay.
17:45So this one first and then this one.
17:47That's interesting.
17:48But this one going up when I have...
17:50Blood pressure 200.
17:52Okay.
17:52And after that, it starts going like pain.
17:55I don't know if it's together or separate.
17:58How long do you think the lump at the back of the head...
18:00How many weeks?
18:01Is it months?
18:03No, it's years.
18:04Years?
18:05It is.
18:05Okay.
18:05Because first time, you know, when this problem starts, you start scratch like...
18:10Mosquito bite you.
18:10Okay.
18:11After that, you start question, oy, what is it?
18:13Because I don't like...
18:15A hospital, you know, and I come into you because wife tells me, get out.
18:19Yeah, yeah.
18:19Good.
18:20Your wife is quite right to send you to us.
18:21I genuinely don't think this is anything cancer-y or horrible.
18:25But I think we should do a scan, an ultrasound scan, and check both the lump on the back of the head and the...
18:30The big one here and just make sure what's going on, okay?
18:33The important thing is when lumps come...
18:35You come see us.
18:36We'll make sure it's all right.
18:37Okay, thank you.
18:37Okay, don't you worry.
18:38We'll do the scan.
18:39We'll get an...
18:40There's no point we can check, okay?
18:41Thanks.
18:41Take care.
18:42Bye-bye.
18:42Nice to see you.
18:43Was it dinner?
18:44No, no, it's fine.
18:49Oh, well.
18:50Shall I just turn it off and on again?
18:53Yesterday was your printer.
18:54I know.
18:54There's paper.
18:55Yeah, replace drum.
18:56I've done so that.
18:58Everything is brand new in here.
19:00There should not be so my baby needs to just turn it off and...
19:04All right.
19:05I'll turn everything off and on.
19:06How do I do that?
19:07Is this it?
19:10No.
19:12Yeah, let's see.
19:15Oh, you're a lifesaver.
19:17There you are.
19:17Thank you so much.
19:18That's perfect.
19:19I thought all the others were...
19:20By the way.
19:25I thought all the others were...
19:29I mean...
19:30Hello. Good morning. Hi. How are you? All right. Thank you.
19:35Do you have a seat? Yeah. How's it going? What's been happening?
19:40Uh, yeah, I just recently I feel dizzy. Yeah.
19:45And the last time, last Thursday. Yeah, yeah.
19:50So after that, I just lie down in bed. Yeah.
19:53And when I lie down, I feel...
19:55Oh, it's so worse. Yeah.
19:57The room is spinning. Yeah.
19:58Next day, I feel...
20:00It's gone.
20:01Okay.
20:02So I'm lazy and also I'm working.
20:04Yeah, yeah.
20:05What work do you do?
20:06Uh, barista.
20:07Okay. Yeah, yeah.
20:08This is why sometimes I'm worried...
20:10I think I will have problem because it happened maybe about more than one month.
20:15Same thing?
20:16Yeah.
20:17The same feeling.
20:18And any other symptoms?
20:19Otherwise, you're...
20:20Oh, well...
20:21Your toilet is okay.
20:22Yeah.
20:23Your appetite...
20:24As a note...
20:25No problems.
20:26You have no ear issues.
20:27I don't have...
20:28Any sore throat, ears...
20:29No.
20:30All okay.
20:30No.
20:31I'm thinking maybe high blood pressure.
20:35You can check...
20:36Can you take your jacket off?
20:37Perfect.
20:38My...
20:39My...
20:40Your mother has the...
20:41Yep.
20:42These problems?
20:43Mm.
20:44Hm.
20:45She had a lot of pressure and after that she had stewed.
20:50On February, my birthday, my memory is gone for about...
20:55I couldn't remember anything.
21:00This was in February.
21:01February, my birthday.
21:02Went out to celebrate and...
21:04Yeah.
21:05I lost my memory.
21:06We can't say for sure that it isn't...
21:10It's a cardiovascular event, something to do with the brain and the vessels.
21:14So if it is...
21:15Happening acutely, go to hospital.
21:17Get seen that day.
21:20It's wild.
21:21Could you marry me?
21:23Crazy...
21:24Uh...
21:25We're all happy.
21:26Yeah!
21:27Ce- TONY
21:37We're all happy.
21:39We're here.
21:42Some good stuff 먹ers...
21:45or it could be something else vascular which needs investigation.
21:50I'm going to get you to lie on here with your head just...
21:55so your head really high up over the edge yeah and keep your eyes...
22:00open.
22:01Okay.
22:02I did an examination called...
22:05right manoeuvre and treatment which is Anthony's manoeuvre and the hope.
22:10Right manoeuvre is to look for benign paroxysmal positional vertigo.
22:15Fairly quickly we're going to sit on the end of the bed keeping your eyes...
22:20open the whole time.
22:21I keep your eyes open.
22:23The idea of the...
22:25manoeuvres are to realign the crystals in the milieu and therefore...
22:30realign the brain to think that you are now not dizzy.
22:35Any...
22:36any different?
22:37Still feel this side is...
22:40heavier than this side.
22:41Yeah.
22:42I'm going to check your ears out.
22:45I'm feeling sick at all and dizziness right now.
22:50It is mildly inflamed that side.
22:54Yeah.
22:55No.
22:56No.
22:57No.
22:58No.
22:59No.

23:25No.
23:26No.
23:27No.
23:28No.
23:29No.
23:30No.
23:31No.
23:32No.
23:33No.
23:34No.
23:35No.
23:36No.
23:37No.
23:38No.
23:39a vascular event too what's that mean vascular is to do with your
23:44uh vessels inside your head brain you know
23:49arteries veins that sort of thing and if there was a small little clot or
23:54something tiny tiny it can affect your balance i can give
23:59you tablets which can help but they're kind of masking the symptoms if it was me
24:04probably want to have scans and things like that so i'd prefer you yeah
24:09yeah there is something called a dizzy clinic which is very good we'll do blood tests general
24:14ones anyway so they're going to give you a form for the blood test at reception and we should
24:19talk when we've got the bloods so if you go today i'll have them next week i'll give you a call next
24:23week
24:24we can discuss yeah but i'm going to do the referral anyway if you get the
24:29vertigo again badly where you're vomiting you should go to hospital for it
24:34okay okay you know you've had it three episodes so far which i can't really
24:39explain i've tried to treat you today for this vertigo
24:44has it made any difference do you feel any different from when you came in or not things not really
24:49then it needs investigating thank you so much all right you're welcome have a good day really appreciate
24:54no problem take care and um get to the phone

25:04Don't worry.
25:05Calm through.
25:06Oops.
25:07How are you doing?
25:09Oh, painful at the moment, Doc.
25:12Take a seat.
25:13Oh.
25:14Right.
25:15What are we doing today?
25:17It seems to have happened this morning.
25:19Yeah.
25:20But at the end of it, it's all.
25:22Okay.
25:23That's it.
25:24There's nothing.
25:24There's soreness on the tip of the penis.
25:26Yeah.
25:27Yeah.
25:28Fair enough.
25:29Just since this morning.
25:30Yeah.
25:31Fine.
25:29Okay.
25:30Take a seat.
25:31Yeah.
25:32Make yourself comfortable.
25:33Have a bit of a chat first.
25:34And how do you feel in yourself?
25:35Any pain?
25:34Any pain in your tummy?
25:35Or around the bladder area?
25:36No pain in the stomach.
25:37No around the...
25:39Around there at all.
25:40Nothing.
25:41Good.
25:42And any skin changes anywhere on the body other than...
25:44When I'm on the tip of the penis?
25:45Just on the end.
25:46Yeah.
25:47It's better when I'm walking and that's the trouble with...
25:49Yes.
25:50I just wonder if you have any...
25:51We'll have a look and see what we can do for it.
25:53Can I get you on the couch?
25:54I'll close the curtain and I'll come in.
25:56Is that alright?
25:57Yeah.
25:58Leave me out of here.
25:59Yeah.
25:59Please.
26:00That would be great.
26:01Great.
26:02Take your time.
26:04If you lie on your back and then just lower your bottom things...
26:07And let me know when you're ready and I'll come through.
26:09Oh.
26:10Are you comfy like that?
26:11Oh.
26:12Are you comfy like that?
26:13Oh.
26:14Are you comfy like that?
26:14Lovely.
26:15Right.
26:16Let's have a little look now then.
26:17So all you need to do is just lower this.
26:18Is that okay if I help?
26:19Yeah.
26:19Oh.
26:20Oh.
26:21Oh.
26:22It's full.
26:23Oh.
26:24Oh dear.
26:25That is a bit red isn't it?
26:26Yeah.
26:27I think it's just a bit of a fungal infection.
26:29Is it?
26:30Yeah.
26:31I'm going to give you a cream for that and that should settle it.
26:33You apply it twice.
26:34You apply it twice a day for two weeks and that should sort it out.
26:37Just on the end.
26:38Exactly.
26:39Where it's sore.
26:40Pull the skin back.
26:41Yep.
26:42Exactly that.
26:43All right.
26:44Yep.
26:44All right.
26:45You done?
26:46Lovely.
26:47Great.
26:48Come through.
26:49So it's a...
26:49It's a skin condition called balanitis.
26:50Yeah.
26:51It's just a very little infection which we can easily...
26:54treat.
26:55Really good to keep hygiene standards high so make sure you're cleaning everything.
26:59changing your underwear all that kind of stuff every day.
27:01Yeah.
27:02And that should really help as well but I'll give you the...
27:04cream.
27:05I'll send it to the pharmacy and you can collect it today.
27:07Okay.
27:08All right.
27:09Any...
27:09questions for me?
27:10No.
27:11No.
27:12No.
27:13As long as I can get that stuff this afternoon.
27:14Yep.
27:14Absolutely.
27:15Start with that on the end.
27:16Yep.
27:17Give it a go.
27:18If it gets worse or it's not helping...
27:19Is it two or three...
27:19How many times a day you do it?
27:20Twice a day for two weeks.
27:21Morning.
27:22Morning and evening.
27:23Evening.
27:24Exactly.
27:24I'll keep it right.
27:25Don't forget your stick.
27:26I'll keep my stick.
27:27Yep.
27:28All right.
27:29Can't go far without...
27:29No.
27:30Really getting out the pub though.
27:32Oh no.
27:33Well how often are you going to...
27:34the pub?
27:35No.
27:36It's very rare now.
27:37So you're drinking less?
27:38Like I used to go about dinner times at night.
27:39No.
27:40I might just go out in the afternoon for a couple of then.
27:42Fine.
27:43It's good to keep active though.
27:44Well done.
27:45I always see you up and about so that's good.
27:46Yeah.
27:47Got to keep on the move.
27:48Don't fancy dying sushi.
27:49Nope.
27:50Got many years ahead of you.
27:51Don't you worry about that.
27:52I've still got people to annoy you.
27:53Yeah.
27:54What do you think I come back in here for every other day Doc?
27:56Wait that's what we're here for.
27:57You're more than welcome.
27:59I'm only joking.
28:00Alright.
28:01Take care.
28:02Take care.
28:03Thank you Don.
28:04You too.
28:05Bye now.
28:04Bye.
28:05Bye.
28:06Bye.
28:07Bye.
28:08Bye.
28:09Bye.
28:10Bye.
28:12Bye.
28:13Bye.
28:14Bye.
28:15Bye.
28:16Bye.
28:18Bye.
28:19Bye.
28:20Bye.
28:21Bye.
28:22Bye.
28:23Bye.
28:24Bye.
28:25Bye.
28:26Bye.
28:27Bye.
28:28Bye.
28:29Bye.
28:30Bye.
28:31Bye.
28:32Bye.
28:33Bye.
28:29one would you like why don't you sit on that one because then if we
28:34need to have a look at anything easier isn't it maybe that's more sensible so tell me all right
28:39are we in today what's happened so three weeks ago or four weeks ago we were here as well yeah
28:44Gromit's place like last February so Gromit's last February 2020 okay about 18
28:4924 the media's had Gromit's put in which we often use when a patient has glue ear which
28:54when fluid builds up in the middle part of the ear that can stop sound transmitting clearly through
28:58so the hearing
28:59isn't so good and for a child that's really important at these really formative learning years
29:04so what the ENT doctors do is put a gromit which is a tiny piece of plastic just to make a sort of
29:09almost like a vent in the eardrum to allow some fluid to come out to allow the pressures to equalize
29:14to sort out this middle ear problem the problem with these types of things and having any intervention
29:19gromits foreign bodies as we call them in the ear is that sometimes it's slightly prone to infection
29:24so four weeks ago her ears started bleeding oh from the inside okay
29:29got here yeah and the guy says oh it's probably falling out yeah that's why it's bleeding so okay
29:34fine and the bleeding stopped after like a week okay it was fine but then since last
29:39week it was a really bad smell coming from my ear okay like really okay really bad
29:44okay and the smell hasn't gone away it's still exactly the same okay this
29:49morning it was like there's one going but i cleaned out lightly okay but sort of crusting
29:54and like dry was it dry coming out overnight very dry and but i
29:59got a phone come from school it started bleeding again any temperature i mean is she unwell i mean
30:02you obviously sent her to school
30:04so she's not well and happy and there's nothing wrong as far as hearing you think we're listening
30:09doing things i mean it's okay it says what here and there but i'm not sure if that's yeah i don't know
30:14okay but overall so come over here because you're so tall look how tall you are i know i think that's over the
30:19summer holiday you're fine now and what year are you in at school you're in year one you've gone up
30:24have you started reading yet i bet you have you're a clever close she can read already
30:29i knew you would you see clever close now what we need to do is have a look inside of your ear tickles a little
30:34bit so you're a good girl staying still okay and do we have any follow-up with the ent for the grommets
30:39so they just said they'll fall out we've been discharged now do you want to see the grommets look at that
30:44little white tube it looks like a hammer yeah it looks like a little bead that's in there but it's good
30:49never been here so that's coming out and it will fall out on its own yeah we don't need to do anything
30:52yeah exactly let's have a look
30:54so i can see there's been a bit of blood on your ear look there's a bit of blood on the around the
30:57outside of your ear just there
30:59yeah that's okay it doesn't make me worry but i'm just gonna have a look inside okay
31:02and you try and stay still
31:04and we'll have a look really gently there's quite a lot of blood in there to see past okay
31:09and the grommet let me see if i can see it i can't see it
31:14yeah i think it's come out on this side but it's definitely still in on the other side yeah
31:19the canal is a little bit wet and sticky and that's what you're seeing coming out yeah
31:24and sometimes what can happen i can't smell it now but sometimes it can we can get
31:29a bit of an infection in the ear canal yeah so rather than inside the ear it looks like it
31:34it's the canal that's a bit wet do you ever put your finger in your ear and wiggle your ear we do some
31:39sometimes do we okay that's okay but sometimes when you do that it causes a little bit of damage
31:42and that could have caused a little bit
31:44of bleeding and a little bit of thing but there's no grommet in there so it's not your thoughts it
31:47doesn't matter but i
31:49wonder whether rather than just throwing i don't want to throw antibiotics at her and things like that can i take a
31:54swab of it which is just like a cotton wool but let's find out what it really is yeah like what because if there's a
31:59smell a smell normally means infection yeah now it could be fungal it could be bacterial it could be viral so there's lots
32:04different things that can cause funny smells let's find out what it is yes then when we find out what it is
32:09the lab will then tell us it's xyz and this is how we treat it yeah and then we actually
32:14didn't want to buy anything over the counter because i didn't know the grommets are still in there or not so the grommets aren't i can't see the grommets
32:19the ear drum looks good yeah which is why she's not got a temperature she's well she's happy i think this is
32:24an outer ear problem okay so i think so all i need to do i'm going to show you what i'm going to do and i'll talk
32:29you through it and it doesn't hurt it's quite fun but you must never do this at home this is
32:34only something for the doctor to do okay you stand up for me really tall
32:39right well i want you to stand let me see very good now if you stand like that and you'll just feel
32:44funny like i'm scratching the inside of your ear aren't i isn't that funny can you feel it tickles a bit
32:49doesn't it okay so we'll get a good sample then we can work out actually what the
32:54bug is well done what we're going to do is let's if she's not unwell yeah unless you don't
32:59don't wait let's find out what it is this will come back quite quickly and then we deal with it right
33:04okay right we've got that so i've got mommy's number so i know how to call you so but obviously if
33:07anything changes blood's coming out yeah
33:09anything different bring her straight back in but no fingers in the ears
33:12commonest cause i appreciate it thank you so
33:14thank you so much thank you so much take care bye bye
33:19thank you

33:51good now I'm here
33:53yeah
33:54yeah
33:56how things
33:58how things
33:59I've had ongoing problems with my flat
34:01right
34:02sewage
34:03coming out
34:04bathrooms
34:05coming up in the kitchen
34:06what through the pipes
34:07yeah
34:08the door
34:09the cupboard the cooker
34:10yeah yeah
34:11now I'm suffering with these little like fruit flies and things
34:14yeah
34:15so my daughter love her to death
34:16mm
34:17she's told me what to do like vinegar
34:19yeah
34:20in the sink
34:21hot water
34:22nesting in the drainage
34:23done
34:24and all that
34:25and then my hands
34:26yeah
34:27I mean I love my own home
34:28super dry
34:29yeah
34:30I have got a barrage of all different kinds of cleans
34:34Yeah
34:34I put the rubber gloves on for a little while,
34:37because of the Raynaud's.
34:38Yeah.
34:39And I had to chip this down myself, it grows a bit.
34:42Raynaud's disease.
34:44It is a vascular problem and can predispose
34:49you to having problems in your extremities.
34:51Your hands may feel cold, they may feel...
34:54They're painful, they change colour.
34:55Easily an infection can get hold because of the lack of...
34:59circulation.
35:00I've been to Nigeria.
35:01Yeah.
35:02You've been to Nigeria?
35:03Yeah.
35:04Okay.
35:04I've come home with nothing.
35:05Yeah.
35:06Yeah.
35:07This is London, England.
35:08Yeah, I know.
35:09In my home.
35:10Probably because of what's gone on and the smell.
35:13I feel horrible.
35:14I feel horrible with it myself.
35:15In an ounce.
35:16Yeah.
35:17I don't even cook in my house.
35:18Yeah.
35:19My poor is both.
35:19They bring me this for the oven on.
35:22Yeah.
35:23This for the microwave.
35:24And then, I mean, with a fan.
35:26Yeah.
35:27Because of all these things.
35:28And it's constant.
35:29Yeah, of course.
35:29Yeah.
35:30Yeah.
35:34Young people who are realising me.
35:38Yeah.
35:39Yeah.
35:40Yeah.
35:41Yeah.
35:42Yeah.
35:43Yeah.
35:44Yeah.
35:45Sure.
35:46Yeah.
35:47Yeah.
35:48Yeah.
35:49Yeah.
35:50Yeah.
35:39Thank you very much.
35:44.
35:54We have cockroaches, we have
35:59mice. You're cleaning and it's alright, take your time, I'm just going.
36:04I'm going over to the shop. And in the minds, I'm OCD now.
36:09Stress is in. We need to treat it.
36:14Does it wake you up? It's annoying.
36:19Now, I've had to trim it down because it's scratch my arm.
36:22And the histamines, you're trying anything for it?
36:24I do take the hair fever one because I feel like I've come out in like...
36:27Yeah. Which one?
36:29Yeah. Purity. Purity.
36:31Purity, yeah. Okay. Yeah.
36:33But this...
36:34This is a contact dermatitis. Contact dermatitis.
36:37And it's due to many different things.
36:39And all the things you've been talking about can cause this.
36:43The stress...
36:44Mm-hmm.
36:45The fact that the moisture, rubber gloves, you said...
36:49Travel, dehydration can cause it. So if you're on long flights, this can...
36:54All affect it. And the more moisture you put on, the better.
36:58Mm-hmm.
36:59And it's...
36:59It's due to all those different things.
37:01Sharon's clearly got a number of issues and a lot of...
37:04There's a lot going on with her housing and that really needs to be sorted out.
37:08Skin issues...
37:09Can be a sign of stress, understandably. And that doesn't help.
37:14But she's itching. So histamine's being produced.
37:19She needs to have antihistamines. Some of the spots looked infected. So she really...
37:24Needs to use the moisturizers and steroid to promote that healing.
37:29And I know you've got loads of potions and stuff.
37:32Wherever you wash your hands the most, you keep...
37:34This cream that I'm gonna give you. And you use it as a soap.
37:38Mm-hmm.
37:39Yeah?
37:39Then you dry your hands and then you put more as a moisturizer. Same cream.
37:43Okay.
37:44That's...
37:44That's a soap and a moisturizer.
37:45Mm-hmm.
37:46And then I'm gonna give you another cream which is a steroid type cream.
37:49Don't be scared about the word steroid.
37:51No.
37:52It's...
37:53You're gonna use that...
37:54Separately.
37:55Twice a day.
37:56Use that for 10 days.
37:57Uh...
37:58Twice a day.
37:59It'll all be written on it.
38:00After you've put the steroid cream on, when it's dry...
38:04You put more of the moisture cream on.
38:06Mm-hmm.
38:07It's gonna make a big difference.
38:08But if it hasn't made a difference...
38:09How so?
38:09In a good 10 days.
38:10Come and see me again.
38:11Yeah.
38:12So have you got an appointment for them to come?
38:14Or not?
38:15Council?
38:16No?
38:17Uh...
38:18I...
38:19Kind of...
38:19Give up.
38:20Been so long.
38:21As they said, when are they coming?
38:22No.
38:23Unless somebody can...
38:24It's all...
38:25It's a...
38:26Three weeks in between each...
38:27Do you need any help?
38:28Do you need any help?
38:29With contacting the council?
38:30No.
38:31They know me, okay.
38:32Okay.
38:33Fine.
38:34I was gonna say.
38:34We have link workers who can liaise between you and the council.
38:37No, I do it myself.
38:38Okay.
38:39Bye.
38:40All right.
38:41Well, I thank you for seeing me.
38:42You're welcome.
38:43Good luck with the...
38:44Yeah.
38:45Well, thank you.
38:46You're welcome.
38:47Take care.
38:49How are you feeling about your asthma?
38:50It's been very bad lately.
38:51Very bad.
38:52Very bad.
38:53Okay.
38:54I've been using my inhaler most days.
38:56Ooh, it's not bad.
38:59When to add the...
39:02When to add the...
39:04just to blow in the oh yeah the breathing test yeah when i blew into it i went a bit giddy
39:09and my body started shaking you should never think you're quite all right with asthma because you're
