00:01Previously on Doc, I had a car accident.
00:03Lost a lot of my memory.
00:05When you look at him, all you see is the man who you think let your son die.
00:08You have to leave him.
00:09I'm handing over my own rape kit.
00:11No more hiding.
00:12What did you tell him?
00:13That you have a thing for him.
00:14Joan knows about the code.
00:15If you both intend to stay here, it ends immediately.
00:18Amy, she's not wrong.
00:19This can't happen again.
00:20My father was a doctor here.
00:22If I resign, will you not put any of this in my file?
00:25I've been having memory seizures.
00:27I feel like my brain is trying to warn me.
00:33Okay, just tell me because I don't understand.
00:36Why are parents of kindergarteners talking to me about college?
00:40It's like let your kid master the art of wiping himself before fantasizing about him at Stanford.
00:50Hello?
00:51You flew in to help me with my stressful week.
00:53You're not helping.
00:54Excuse me, ladies.
00:56She'll have something with alcohol.
00:58Dealer's choice.
01:01So are you planning to see mom while you're here?
01:03We had a fight.
01:04We're not talking.
01:05What'd she do this time?
01:06When I tell you, you're gonna die.
01:10Jess?
01:13Jess?
01:14Mom?
01:15Are you okay?
01:16Oh, for the last four days, I keep flashing on that damn snow globe.
01:29You don't remember it, right?
01:30I don't.
01:31And it doesn't seem like this is helping you.
01:32My mind is trying to tell me something.
01:36Hi, sweetie.
01:37Can you give that a fist bump?
01:51All right.
01:52Let's get you in there.
01:53No.
01:54Heads up, she had three waffles.
01:56And how exactly did that happen?
01:57Well, like her mother, she's a persuasive negotiator.
01:58Please.
01:59She'd be disbarred in a heartbeat for never holding up her end of the deal.
02:03Oh, before I forget.
02:04Would you mind swapping weekends?
02:05David has a work thing.
02:06He'd like me to go.
02:07Oh, that sounds exciting.
02:08But if you've got plans, I'll figure it out.
02:09No, no.
02:10You don't know?
02:11I'll do it.
02:12No.
02:13No.
02:14No.
02:15Heads up, she had three waffles.
02:16And how exactly did that happen?
02:17Well, like her mother, she's a persuasive negotiator.
02:18Please.
02:19She'd be disbarred in a heartbeat for never holding up her end of the deal.
02:21Oh, before I forget.
02:22Would you mind swapping weekends?
02:23David has a work thing.
02:24He'd like me to go.
02:25Oh, that sounds exciting.
02:26But if you've got plans, I'll figure it out.
02:27No, no.
02:28It's fine.
02:29I'm wide open.
02:31Still not seeing anyone.
02:34We've been divorced long enough to dip a toe in these waters.
02:37i thought i'd give another 10 years come on i still worry about you there is somebody but it's um
02:44some pretty serious obstacles at the moment so intriguing yeah i wish you've always been too
02:49much of a romantic my achilles heel you know what they say if you can't be with the one you love
02:55yeah it's more your move
03:07whoa whoa sorry no problem
03:13morning morning i haven't seen you in a few days did you want to look i know this is it's weird um
03:20so i asked dr ridley to keep us in different cases for a while your chief resident whatever you think
03:27is best
03:31early in the year for snow gloves oh yeah i just um uh never mind no what i've been having these
03:46memory hits sometimes i see a person hear what they're saying in the past but sometimes it's
03:51just these flashing images memory seizures i've read about that and they may not even be real
03:57right the things you think are memories yeah but the last time it was i was able to figure it out
04:03must all be so trippy yeah that's one word for it hey just you know it's 906 and there's someone
04:11watching so amy's back in the saddle jonah's running the show and everything's settled with
04:20you and norah at home well if everything's settled with the board is what we really need to be talking
04:24about we seem to smooth things over with you knows so long as there's no harm it'll be no foul i
04:31appreciate whatever you did you can repay me by wooing the whale i've got on the line an investor high
04:38profile investor interested in donating at least 10 million there are several departments that could
04:44really use an influx of cash i want it for labor and delivery turning our birthing center into
04:49something boutique which would be a real revenue generator for the hospital hi i was told to come
04:55here oh dr matra perfect timing the donor i told you about just happens to be her father
05:02who has specifically requested your presence on today's tour you should be here soon i've already
05:08cleared it with dr ridley so have fun oh and i'd like a check in my hand by the end of the day
05:15i just can't believe it it's never happened to me before in fairness fainting supposed to be a
05:24surprise i just got off with frank he's ditched his big conference and catching the first flight home
05:29got one of the last good ones i think last guy i dated refused to leave his fantasy football draft
05:33when i got a flat tire oh and this is my sister casey i had a long very stressful week so she flew in
05:42from new york for some girl bonding time while my husband's out of town and we all know the best
05:46bonding happens in hospitals right hey i'm dr heller i'll be working with dr clark um do you have a
05:56diagnosis looks like a pretty standard basa bagel event i'm not even gonna ask what that means sudden
06:01nervous system misfire caused a drop in her heart rate and blood pressure that's what made her faint
06:06it's common usually harmless we'll do an ekg just to be safe thank you both so much you're gonna be
06:13fine yeah i know do you think i'm deaf i'm right here and i would argue more invested in my doctor's
06:21prognosis than you are okay i i just wanted to make sure you wanted me an ekg doctors can my sister
06:29give me an ekg she loves getting overly involved in my life jess unfortunately there is some training
06:34involved any cardiac ectopy none normal sinus rhythm since i've been here oh jess what the hell
06:40hold on hold on don't touch her my apartment my rules casey what is happening she's asleep
06:47she's fully asleep right now i am taking back control you are my guest so start acting like it
06:54we'll set up a sleep study asap i'm sorry i should have caught that don't worry it's a very
07:00subtle shift your first year resident i hope doctor really doesn't hear i miss something so big
07:05i swear i'll be on top of it next time anna there's gonna be plenty you don't know there's
07:09still things i don't know when i'm chief resident it's gonna be okay our job here is to teach you not
07:14scare you it came on faster than usual but the moment sal started acting like an insane person don
07:23you're making it sound worse than it was you made missy cry he called our spin instructor a size four
07:28monster but she's really a size two so it was a thing anyway like the doctor told me i poured two
07:33packets of sugar into a bottle of orange juice made him drink it and for like 30 seconds he was normal
07:39but then he started seizing i was terrified so i called 911 two units on board did you eat before
07:46spin class i put a cookies and cream protein bar in his bag but of course he didn't eat it it tastes
07:50more like cookies and drywall i get look i get why my sugar was low but why did i cease it sounds
07:57like you overexerted yourself on an empty stomach and that caused the hypoglycemic crash don oj was
08:03the right call but you might have overshot the mark with the sugar so we've given you a small dose of
08:08insulin and that should bring your sugar down and you should be stable in a minute thank you doctor
08:14so you were diagnosed with diabetes eight months ago can i ask you if you've made any changes to your diet
08:20not just his diet we overhauled his entire lifestyle both of ours last six months we've been
08:26eating clean and working out every day we each lost about 10 pounds and when we plateaued our
08:30doctor recommended ozempic are you a diabetic too no but it's a godsend for her not so much for me
08:38we'll find the right dose sweetie i don't i don't i don't feel right sal what's happening sal what's
08:45happening excuse me heart rate's low weak pulse blood sugar's at 45 but you only gave him two units
08:54okay but that's not much drop at that low get the d50 sal oh my god sal sal hold on
09:01i guess she's not coming the time is still yours katie anything you want to share not really
09:18you know you can say anything there's nothing to say dad not without her here katie please
09:23when you keep things inside it's so bad for you fine she came home last night while he was helping
09:31me with homework and walked right by us like we weren't even there i know that must have been very
09:36painful then he tells me she'll come around after two years he's still making excuses for her
09:45michael i'm sorry i didn't mean to minimize you just keep acting like what's happening isn't real
09:52how is that gonna help
10:22the insulin dose we gave him shouldn't have caused his sugar to crash like that which
10:26tells me the orange juice and sugar wasn't the cause of his earlier spike glucose is 122
10:31back in the safe zone what is going on with him he has something called brittle diabetes which means
10:36that his blood sugar swings wildly it can crash or spike without warning for him to survive we need to
10:42keep it in a very tight range for him to survive he is walking a tightrope the only way to stop it is to
10:48figure out what's messing with his metabolism let's get a ct scan stat i'll call radiology
10:55i've never done anything in my sleep but snore frank would have told me this can come on suddenly
11:01oh god what if it's a brain tumor okay i know you can't help it but let's not jump to worst case
11:06scenarios casey's right there's plenty of other explanations and most are far less serious than
11:10a tumor but the sleep study will give us a clear idea what's going on okay when you wake up we'll have
11:15some answers okay so where do we go while we wait um well i've got other patients to take care of but
11:27the uh the waiting room that's that's probably your best bet that makes sense what with me not being a
11:34doctor and all but i'll let you know as soon as i know anything okay sorry you got dragged into this
11:44it's fine i'm happy to help i looked him up quickly he's quite a philanthropist yeah he's a really good
11:50guy well you've been here almost four years any idea why he's doing this now no we don't really talk
11:57about his agendas and here he is my sonya
12:03dad this is dr michael hamda our chief medical officer max was singing your praises pleasure to
12:09meet you my pleasure's mine and thank you for your interest in obstetrics max suggested it was
12:14the department where i could truly make an impact this way then
12:17i heard your patient seized i reversed it with a d50 bolus hyperglycemia right on the heels of
12:28hypoglycemia i'm surprised brittle diabetes wasn't your first though it was on the list but he eats
12:33well he exercises he's even on his mpeg so something's not adding up i'm scanning for a
12:38pancreatic mass possibly an exocrine tumor well that'd be a hoot i haven't seen one of those in 30
12:44years glp1s can mask a lot of underlying issues yes and this new fad of using them for weight loss
12:51is a ticking time bomb still doctors hand them out like fun-sized butterfingers at halloween they can be
12:56very effective when used properly they're a shortcut patients end up like a scarecrow and then balloon
13:02again the second they go off the meds i wish the danes would manufacture a pill that made people more
13:06disciplined instead that is a bit reductive they reset maladaptive appetite triggers so that willpower
13:13has a chance against biology they're a crutch even with a higher power alcoholics have to
13:18choose to stop drinking the switch gets flipped only when they flip it free will is an illusion we're
13:23all just molecular checkers floating in the abyss acting like gods but utterly powerless over our
13:29destinies darren's our resident philosopher a bit of a nihilist apparently
13:34pancreas is clear no tumor okay it's 10 years ago 3 a.m your time i have just called from nepal because
13:44i can't figure this out there's no judgment no boss just a sick patient and two old friends what do you
13:50tell me to do a small bowel aspirate and a methane breath test sebo not bad dr larson not bad at all
14:08positive methane breath test confirmed by a small bowel aspirate the sebo infection all right
14:13hmm i don't know how you saw that coming it was a team effort also check out his triglycerides they're
14:19through the roof are you sure these are right mm-hmm okay tests confirm sal has a sebo infection
14:30which explains the brittle diabetes the blood sugar swings it's rare but uh it can be treated with
14:36antibiotics oh but why did it happen underlying causes are usually dietary what do you mean dietary
14:45excess processed carbs fatty foods that doesn't make sense he's been eating so well i'm afraid that's
14:53not what the blood work shows
14:57sal what did you do i've been cheating with fast food donuts pretty much anything i pass on the way home
15:07but you couldn't have eaten much the ozempic i stopped taking it four months ago it made me nauseous
15:13and miserable and then when i stopped taking it i was always hungry so i started sneaking going to
15:18drive-throughs pay cash leave the wrappers in a neighbor's trash bin you've been doing all that behind
15:26my back
15:29i need some air
15:35i know i shouldn't eat like that but when i'm tired or i'm stressed i lose control
15:40food's all i think about it's all i care about there's something wrong with me there's nothing wrong
15:48with you sal whether it's food or work or alcohol everyone has something that slips out of control but
15:55that doesn't mean you can't take back control how i'm obsessed you need to have the right support
16:02dawn's been supporting me dawn is your wife she can't also be your coach your dietitian your trainer
16:08let me bring in a professional yes please
16:13i need this to stop
16:15can you paint me a picture of what 10 million dollars could achieve well that could fund private
16:26recovery suites cutting-edge neonatal tech the kind of upgrades that turn hospitals into headlines
16:31shauna show me where you work you mean internal medicine i'd like to see it if you really want bang
16:41for your buck there's a much more expansive role that money could play there i'd be curious to know
16:45about it but in any case i want to see where my daughter spends all her time
16:51follow me
16:54no don't you dare frank my mother may be a narcissist but yours is a stalker
17:01oh this is getting good why did casey buy me jeans just to make me feel that i gave you those
17:07jeans because when i wore them you said you loved them i couldn't tell they were high-waisted so
17:11casey knows i can't do high-waisted with my short torso liz well at least we know you sleep talk the
17:21truth there's also some good news on the medical front it's not a tumor so what is it looks like
17:27temporal lobe epilepsy it's a part of your brain it processes emotion memory language anything
17:33stressful going on lately yeah she just finished two weeks of brutal parent teacher meetings it's
17:37that bad have you met the parents of a kindergartner in private school no matter what their kid does
17:43they interpret it as the misunderstood behavior of a genius yeah i think i might be one of those
17:48parents my daughter she's finishing up kindergarten now oh well since you're a doctor i buy that your
17:54kid could be a genius right so jess we're gonna get you started on a anti-epileptic
18:02we'll monitor adjust a dose from there thank you
18:08he wasn't wearing a ring i just assumed he was single hello i have epilepsy jess i know the words
18:15sound scary but this form is very treatable and with the right meds there's a good chance you won't even
18:19notice it just so you know he's divorced
18:29and here we have the internal medicine office sort of an all-purpose space and here we have
18:37dr larson we met at the fundraiser three years ago right after sonia started as an intern
18:42i i'm so sorry i i don't i'm sure all businessmen in suits blur together but you were very kind about
18:50sonia well she's an excellent doctor she would have been excellent at anything she chose to do but i'm
18:56very proud she chose this selfless profession mr matrix considering a sizable donation for obstetrics
19:01oh great though we could really use support in this department exactly what i was telling him
19:06please don't let me keep you it's nice to see you again you too well this was very edifying thank
19:14you for such a comprehensive tour dr hamda our pleasure uh please let me know if you have any
19:19other questions if you wouldn't mind could i steal sonia away for a quick lunch oh i really can't i have
19:25so many patients and but i understood that dr ridley had excused your absence no actually she just
19:31texted me i she really needs me back ah i understand of course your mother would love to see you this
19:40week for dinner if you're able i'll let you know i'm gonna get back to work please right this way
19:55where's son i went downstairs to get a coffee thought you took him for tests no
19:59is there a vending machine on this floor
20:09what the hell is wrong with you we're in a hospital all because you won't stop eating
20:14garbage that's gonna kill you and what about me what am i supposed to do if something happens to you
20:19i messed up i'm not gonna stand here and watch you kill yourself i'm sorry i'm done with you
20:29dawn dawn honey is she okay gone gone i need a crash guard now got it what's happening to her what's
20:40happening
20:44when parents are unified it provides stability for the kids but when they have different ideas it can get
20:49tough sometimes one parent feels like they have to defend the other to maintain that stability
20:57what i heard you say last week katie is that you want your dad to start telling more of his truth
21:05i don't think you can do that
21:08michael
21:09i really don't want to start bad mouthing your mother katie katie
21:23i understand how you feel no you're just doing the same thing over and over again
21:29michael she needs you to say you don't like how amy's acting that you know amy's hurting her and it's not
21:35right
21:39it's not right
21:44it's not right
21:44i'm not here to pressure you no no i want to know what's going on so i can help you
21:58you remember ravi dar
22:06well he's the son of my father's business partner
22:10and when he attacked me my mother was the only one i told
22:15she made me promise never to tell anyone else and i kept that promise for five years
22:27but when ravi was arrested it all came out
22:33let's just say my father's reaction hasn't been what i would have liked
22:39he blames you he just can't face it neither of them can
22:46i certainly don't want to talk about it
22:49so you don't see them
22:54i've been working very hard in therapy to sort out what i'm gonna say how i'm gonna say it and when
23:01i'll be ready to and it's not gonna be at lunch in the hospital cafeteria or dinner this week
23:10i'm so sorry you were put in this position
23:12you know i put myself through med school so i wouldn't be beholden to them
23:17so this just feels beyond gross him coming in here with his money trying to lift his leg on my
23:24territory is it possible he's just desperate to find a way to make it up to you
23:29you're telling me to forgive him absolutely not it's not my place to tell you anything
23:34you are my boss not right now i'm not
23:39it just it feels like this might be an opportunity for you
23:44to let him have it give him a chance to step up
23:50i know how it feels to have your family torn apart because it's too hard to deal with grief
23:55the only way through it is through it
24:03what happened her heart decompensated due to malnutrition
24:07she lost 26 of her body weight in two months thanks to glp ones the abuse of glp ones we need
24:15to introduce nutrition slowly to avoid refeeding syndrome and you have to get her to stop the meds
24:21whatever she was hoping for when she started this this can't be the ending that she wanted
24:28jess is sleeping i ran a loading dose of kepra checked the levels it's in a therapeutic range so
24:35i feel good about it good job dr clark did you know that casey's a makeup artist of the stars
24:40she's known everyone from lady gaga to zendaya how cool is that that's that's good to know
24:45i need you to help me convince jess to quit teaching because if stress lowers the seizure
24:53threshold i think it's just it's too risky of a job and i don't want her seizing every time
24:57his parent teacher week so here's the thing with temporal lobe epilepsy it's not just about triggers
25:02it's the cumulative load it's it's it's the tonnage okay and you know having her extremely charismatic
25:09impossible to ignore sister show up when she's already had a very stressful week i'm sorry are you
25:14saying i gave my sister epilepsy no no of course not epilepsy that's something she has but i am
25:18saying that standing next to you someone who fills every room she walks into that could be taking a
25:24toll on jess that she just she hasn't fully processed yet no jess and i aren't like that you obviously
25:30know nothing about our relationship i don't jess i always knew you were the favorite fun successful
25:37sister i made my peace with that let's get you back to your room you don't get to steal my doctor
25:49um okay maybe you're right
25:55is she okay she's fine she's stable but she needs help she's been starving herself it's my fault
26:03i lied i binged i pushed her to the brink she never would have started the shots if i wasn't such a fat
26:09loser hey this is not on you we all make enough bad choices by ourselves we don't need to take
26:16responsibility for other people's too
26:23why don't you tell me about your relationship with food
26:26i've been on some kind of diet since i was seven years old my mom and dad meant well but both fat
26:35kids who didn't want me to suffer like they did they were always watching everything i ate saying
26:43you don't want that in front of my friends
26:48girls i grew up feeling like my parents loved me more when i was in shape
26:54that must have been really painful i guess when dawn took over my diet it all came back
27:01in the beginning she loved me how i was we'd make big meals god's nice dinners
27:07she doesn't want to do that anymore i love her but it feels like she can't love me unless i change
27:18like she did and i want to i want to do that for her but you can't get healthy for dawn
27:25you have to do it for yourself what if i can't what is happening to you is not your fault but getting
27:35better that is your choice
27:44over the past few weeks katie's been working with her dad on being more honest with her feelings
27:50so we felt it was the right time to get you two together and see what's going on katie
28:00they're gonna get a divorce what makes you say that i saw on her phone
28:05i was looking for apartments you took my phone amy it's not the issue here i'm sorry
28:12i'm sorry you're sorry to her is it true that you're looking at apartments yes does michael know
28:22this not unless katie told him i didn't what made you want to look through your mother's phone
28:30were you looking for something in particular i wanted to read their text anything you want to share or
28:36ask about are you sure
28:51i don't understand how you can just give up on him
29:06hey first day back yeah just uh checking in on you oh good i know you were having a strong
29:17reaction the things you were remembering yeah well you know i spent the last four days scrolling through
29:22a million snow globes online not that i know what i'm looking for just that there's snow and a globe
29:29yeah those would be the two main ingredients it must be like living in an episode of black mirror
29:36oh sorry it's um kind of like the twilight zone oh yeah i'm ready for rod surling to just
29:41come around the corner and explain it all to me hey looks like don's waking up okay
29:52what happened
29:53you passed out from malnutrition i'm fine i just didn't eat enough today stupid i know
30:07i swear it won't happen again this isn't about today your labs indicate that this has been building
30:12for some time probably since you started taking the medication i like how i look how i feel
30:18do you like how you feel right now when sal got the diagnosis we started the diet and the workouts
30:27to help him i never thought about how much i weighed or how i looked but i knew he wouldn't stick to
30:35it unless we did it together first i hated every setup every carrot stick but then i started losing weight

39:39Hey.
39:41Um, can you take that?
39:43How'd it go?
39:44Really good, actually.
39:46It meant a lot to Jess that I brought it up.
39:48Obviously, I took all the credit for the idea.
39:49Of course you did.
39:51Her husband just got here, so once they started getting all lovey-dovey, I thought I'd give him some space.
39:55Probably wise.
39:57Well, it looks like you're wrapping up here, so any chance you might want to grab a drink with someone?
40:03I don't know.
40:04Charismatic, who fills every room she walks into.
40:09I just got out of something kind of serious.
40:19My flight to New York leaves at 9 a.m.
40:23I'm not looking for anything serious.
40:25Dr. Clark, sorry to make you come back to HR, but somehow we're missing three forms.
40:39No problem.
40:40Any chance you're the daughter of Brian Clark?
40:44Yes.
40:45I see the resemblance.
40:47He's a great doctor.
40:49Kind, funny.
40:51Never met a dad joke he didn't like.
40:52He really got a raw deal here.
40:58What do you mean?
41:00Nothing.
41:01Sorry.
41:02Long day.
41:04You're all set.
41:05Did something happen before he left here?
41:10Not that I know of.
41:11You just said he got a raw deal.
41:13I shouldn't have.
41:14I don't...
41:15Please.
41:16Tell me what you know.
41:17Why don't you just ask your father?
41:19I would.
41:20But he killed himself three months after he left this hospital.
41:26If you don't tell me, I will go to Dr. Hamda and I will report that you violated confidentiality.
41:33Hannah, please don't do this.
41:36You opened the door.
41:36Now, I need to know what's on the other side.
41:42All I know is your father filed an HR complaint against Dr. Larson.
41:48And a week later, he was gone.
41:49I need to know what's on the other side.
