00:58We've had Buddy almost seven years since Buddy was four months old.

01:10We have three grandchildren, so Buddy has been a part of their life for seven years.

01:23Buddy presented today to have his spleen removed.
01:26He came in a couple days ago for some kind of alarming symptoms not doing super well, the stunted abdomen.
01:33Great boy.
01:35We went on holidays for a week and when we came back, Buddy refused to eat.
01:40So when we got home and he wouldn't eat for six days, we were concerned.
01:44Last Thursday, we noticed that his stomach was bloated.
01:48Monday morning when we got up and it looked like he was pregnant.
01:52So then I phoned into the clinic and they said, bring him in right now.
01:57On x-ray, we can see that there's a really big mass in Buddy's abdomen.
02:02And via ultrasound, we can tell that it's attached to the spleen.
02:06We're going to take one more blood sample before we proceed.
02:10That wasn't that bad.
02:13Whenever you're dealing with a spleen, the highest risk is always hemorrhage.
02:18The spleen is so filled with vessels and vasculature, those masses can bleed at any time.
02:24To the point where that animal becomes anemic or dies because of an abdominal bleed.

02:34We're where we want to be.
02:36Because Buddy is currently stable and he's not anemic or anything like that,
02:40we should operate on him as fast as possible because really he could go downhill quite quickly.

03:45Kate won, well I had her for 11 years but unfortunately she ended up with cancer.

07:13This one's going to be interesting because it's a really big mass and there's a lot of risk associated with it.
07:18Because of the size of this mass and because of the risk of these masses spontaneously bleeding into the abdomen, we need to go in and surgically remove the spleen in its entirety.

07:45This morning when I went up there to greet him, tail wagging.

07:56Estimates of this thing were 20 plus centimeters diameter in at least two directions.
08:03So this is going to be a big hole.

08:06This surgery requires such a large team because it's a large surgery.
08:09Like the mass is literally like this big in that dog.

08:16For Buddy, there's significant risk for him that he might bleed a lot during this procedure.
08:22And cut through that.
08:25It's worrisome.
08:26A very dramatic journey to head on.
08:29And to lose him would be devastating.
08:32I'm being strong for everybody.
08:35He's not ready to leave us yet because we're not done.

08:51Buddy has got a huge mass on the spleen.
09:06I think it is the biggest splenic mass I've seen.
09:10This thing is heavy.

09:12Probably 10 pounds.
09:13It's a pretty intense thing when you have a spleen that's 10 pounds in your body.

09:20So tail of spleen here, tail of spleen there, mass in the middle of the spleen.

09:31Like the amount of healthy spleen in this dog is very minimal.
09:34The majority of the spleen has been taken up by this invasive tumor.
09:38No such thing as routine.
09:40If we don't remove this, ultimately he could die of abdominal bleeding.
09:45I feel like we've done the easy part now.

09:50One by one, I think we just ligate our way across that.

09:53With splenectomies, you can never have too many clamps in your pack.
09:56There's so much blood that goes to that organ coming off of the pancreas, coming off of
10:02the stomach.
10:03So we need to clamp all those vessels off to prevent any sort of bleeding when we lift that
10:08mass out of there.
10:09Do you want to try and attack that side?

10:14You're pulling on something diaphragm-related.

10:18Every time you move, I can see it in the rest.
10:20So it's pulling on things we don't want to pull it on.
10:23If there's any sort of movement that we don't like, if something's going to tear or rip or
10:28not going to go well, there's a huge risk of hemorrhage.

12:42He's always and forever getting attacked by a squirrel.

14:16I mean, quite frankly, my arm's getting tired and all I'm doing is balancing this thing.

14:24When you're doing surgeries with multiple surgeons, it's because it's an intense surgery.

14:34It's a long procedure when you're lifting a large spleen out of an animal.
14:39So it's so important that we communicate back and forth with each other so we know exactly what we're doing.

15:07We're just going to flush and then close.
15:10Surgery went fantastic with Buddy.
15:12We were able to remove that mass.
15:15We can live without our spleen.
15:17We can live very well without our spleen.
15:19However, when you have a mass growing that large, it's one of two things usually.
15:25A hemangioma or hemangiosarcoma.
15:29The latter of which is a really scary type of cancer.

15:39We're going to send the spleen away to a histopathologist and they're going to be able to tell us if that is truly a benign or cancerous type mass.

15:54We just got to cross our fingers and wait to see what happens with that, but I'm hoping for the best.

17:18So now I guess we'll sedate him and we'll try and get it cleaned up.
17:23He's getting pretty sleepy.

17:27It seems to not be getting better.
17:29So we need to delve into this a bit more assertively.
17:38He now needs to actually become a patient.
17:40So that was the original wound.
17:42We're going to sedate him, clean it, flush it.

17:48Because we don't want it to get worse and him to get systemically sick.

18:30And he'll live with the doctor.

18:42I don't want to even think about what it would be like without him.

20:39The joy that Buddy brings to all of our family is just absolutely priceless.
20:44Like, on Wednesday, on his checkup, he had lost 12 pounds.
20:48Yeah, that's being less, I think, 10 pounds alone, so.

20:52I'm hoping for at least another 10 fabulous years with Buddy.
