02:47Rogan was diagnosed with his debilitating hip condition at six months old.

02:54He went lame quite a lot and couldn't use his back legs at all.
02:58You could tell that he was in a lot of pain.
03:01He was never able to be a puppy and he's not been able to be a dog.
03:04He's always been restricted.
03:06That's why he's such a big ball of energy.

03:10So after this, we're hoping for him to do what he wants to do for the first time in his life, really.

03:34So my colleague took these radiographs and they show severe hip dysplasia,
03:42meaning that the femoral head is not in the socket at all. It's out here.

03:47And because the head rubs on the top of the rim and then the socket fills in and becomes like a saucer
03:52and these spurs of new bone are due to tugging on the joint capsule and rubbing up there.
03:57So that's quite painful.

04:37We cut the femoral head and neck off and leave a false joint which goes up and down like a piston
04:42and it certainly can give you a much better result than what you have now.

04:46However, it's scientifically proven that no dog bears full weight on the leg.
04:51We want the best possible outcome for him to have the fullest life.

05:07So the gold standard remains hip replacement.

05:11But there are some challenges intrinsic to this breed.

05:15They've got strong muscles.
05:16They're short-legged.

05:18They bounce around and twist and turn.
05:20So there's a lot of force on the implants if you're going to put in a hip replacement.

05:26He has a higher risk than normal of dislocation.
05:31At the moment, the only options for hip replacement for Rogan that we have are cemented.
05:39Cemented down in the femur like so.
05:42And this would be cemented into the socket, into the existing socket like so, and then the head would pop in.
05:47Right?
05:48That's all we got commercially.
05:49Yeah.
05:50Which is fine.
05:51Except this is a very young dog.
05:53It's possible that, given the architecture of this particular dog, that the cement bone interface could break down over time.
06:04This is an uncemented hip replacement that you hammer into the bone and the bone grows into the metal.
06:09Mm.
06:10It's a shell that gets bolted into the pelvis and then you can alter the angle of this plastic liner to give you more active capture of the head.
06:19Yeah.
06:20Hence making it less likely to come out.
06:22In this size of dog currently, we do not have uncemented hip replacements.
06:29It's a shame this kind of thing's not available for the small ones.
06:32Not yet.
06:33Now, so it is theoretically possible for us to make an uncemented implant for this dog, but it will be significantly more expensive because it's custom made for him.

06:47So I'm sorry for introducing variables into the equation here, but it is what it is.

09:23and get him a birthday cake yeah and um birthday steak as well as a tradition in our house good boy so
09:30gentle it's late morning and sarah has brought in brodie desperate to see if noel can get her
10:00visla out of pain sit sit sit down good boy he's had surgery performed by another vet but it's gone
10:11badly wrong in 2022 more than one-third of my surgical cases have been revisions of surgeries that have
10:19gone wrong every surgeon can have complications i certainly have sometimes it's just bad luck but
10:28other times better implants or better technique could have been used from the outset i'm really
10:36hoping that we're going to be able to start a a new journey in in putting right what's obviously not
10:44been done correctly our poor dog has has gone from an animal that's he's never complained of any any pain
10:51to somebody who really is struggling i think the noise he's making at the moment is because he's hungry
11:02sarah and her family live in surrey they've had nine-year-old hungarian visla brodie since he was a
11:09puppy and he has a younger visla brother fido they are dogs that are made to go fast
11:15and on a normal day we will take them for three walks and at the end of that they'd come back and
11:25they'd want to go out for a run
11:30it was out on a walk in february 2021 that the family's beloved brodie suddenly went lame
11:36our local vets diagnosed that he'd got slight elbow dysplasia she put him on anti-inflammatories
11:44and all the swelling went down and he started going back to a normal gate however the vet referred
11:49brodie to an orthopedic surgeon who told sarah that brodie's condition was so severe he needed a
11:55partial elbow replacement he felt that there was virtually no cartilage in his elbow and he needed to
12:02have an implant we were really really concerned because um by this stage brodie was back on four
12:10legs he was running around normally and he seemed to be back to his normal self and we were being told
12:16that his leg could go at any point and that the dog of this stature couldn't couldn't survive on three
12:22legs and that they'd have euthanates and it was such an emotional fraught situation that that we agreed
12:30to go ahead with it come on then boys let's go brodie has not been able to put weight on his front leg
12:36without pain since the surgery it's very traumatic to see your beloved pet rattling almost with with
12:44the amount of painkillers that he has to be on i want my dog back as he was and you will do anything
12:52for the animal that you love come on brodie now brodie please oh poor old bro come on brodie
13:01come on please come in all right there you go there's pages and pages of history but can you tell
13:07me how many months now has he held that leg up you can sit down oh mate i'm sorry you you are next to
13:13the wheel since the surgery he's never put it down poor fellow so he hasn't put it down then for
13:19nearly nine months yeah okay i've already had several surgeries from this surgeon that i've
13:28been asked to have a look at which is profoundly sad but we're gonna have to face it yes noel has
13:36been given the radiographs that were taken of both elbows before brodie's operation and there is evidence
13:43of whiteness here and there is evidence of some osteoarthritis here okay mm-hmm and there's also
13:49a concretion of bone here on the left and on the right associated with mineralization around the
13:58joint having said that would i have felt that this patient definitively warranted this procedure no i
14:07wouldn't at that point that's my opinion we were blackmailed emotionally into doing this lauren told me
14:14you felt you had a gun to your head that was exactly how i felt you go in front mama and talk to
14:20the head end so i would wonder if the x-ray picture for this leg doesn't look that different to the
14:26other leg how come the other leg had a big surgery and that one's doing okay okay we're going to need
14:33a ct scan yeah and the girls will take him through yep thanks ladies we're not going to get your dog back
14:43to what he was before it's not going to happen i'm sorry i wish it could happen but it's not
14:50it's okay i know you've got a poorly arm haven't you it's okay oh poor man come on beautiful
15:02although not as a result of brodie surgery the original vet has since been struck off by the
15:07royal college of veterinary surgeons in january 2022 for performing inappropriate hip surgeries without
15:14adequate investigation or informed consent it's really hard to hear that we're not the only one
15:22that he's having to look at i think gives me reassurance in one way but also complete desolation
15:29in another that somebody has has been playing god with other people's beloved pets thank you very much
15:36very much sarah all right well there's not going to be any real surprises here these images very
15:47clearly show me that the elbow itself is destroyed been destroyed by the operation yes the joint hasn't
15:58just collapsed here the humeral radial joint has also collapsed so there is no good cartilage in
16:05any of that region there i just don't feel that that people this poor should be allowed to practice
16:12in the first place you can put in any implant after one day course with one year of experience
16:19the damage is we see every single week disasters so in my opinion the only way to keep the leg is to
16:28fuse the elbow solid okay it's a plate system that completely fuses the elbow solid so they use
16:35the leg like that but it gets them out of pain we've got to do something sooner rather than later or
16:40it's all it's going to get worse hello baby

16:52well the consult really disclosed answers that were worse than we expected
16:59come on i really wish that he'd come to to see noel in the first instance where we could have discussed
17:07all sorts of preventative um options before even surgery would have been on the cards good boy

17:40my quay there's a new girl in town
17:48two-year-old boston terrier rogan is back for his total hip replacement to try to get him out of pain
17:57as he's so young there's a risk that a cemented system may loosen over time
18:02so he's had custom implants made especially for him
18:08all right we've got to be on our ball game now
18:12first the top of the femur bone is removed
18:16the diseased cup is drilled out and a titanium mesh shell with a cup liner is screwed in place
18:22the stem is inserted then a 3d printed jig is used to drill a hole into the femur to secure it with a pin
18:33finally the head is attached to the neck of the stem and re-articulated within the cup
18:41after removing the femoral head noel prepares the old socket ready for the new cup
18:46right this is the cup that's got an outer shell made of titanium with trabecular metal into which
18:52the bone will hopefully grow this is the first cup of this specific type that has ever been made
18:59designed to exactly fit rogan unlike existing hip implants for small dogs it won't be cemented into
19:06place we're going to anchor the cup down with these little ears
19:13which take locking screws noel then drills out the femoral canal
19:25so we drill the hole here into which we're placing pegs
19:28which will hopefully hold the stem in place until it's solid
19:35finally the head is positioned into the titanium cup
19:39just put the new head on we now put the head into the cup
19:50hold it there
19:53just tugging on the femur to see if i can dislocate it which i cannot
20:01rogan has a new hip joint but it will be several weeks before it's clear whether this
20:05groundbreaking surgery has been successful all right rogan well done let's go to x-ray now please

20:38it's early afternoon and a tabby cat has been rushed to fitzpatrick referrals for assessment

21:11oh bless you it's been a tough morning tough morning for everybody he's had his pain relief and things
21:18two-year-old vinnie has been hit by a van and seriously injured
21:22she's sat in reception now with him thank you okay bye
21:29zoe is still traumatized by discovering vinnie just after the accident
21:36i just dropped the kids off to school and then i had a phone call from my neighbor
21:40and she said uh she explained that the cat had just been hit literally just been hit
21:45so i went running outside and he was just by the side of the road and uh he was struggling to move
21:52and then i just drove him straight to the vet
21:58after taking x-ray pictures of vinnie's broken back legs the vet referred him for specialist treatment
22:04hi there hi i'm so sorry you're going through this trauma oh my god you're gorgeous aren't you oh
22:14sweetie all right so lauren told me that we got hit by a post truck yeah i went under the wheels i saw
22:20it on my cctv this morning yeah oh my lord do you mind if we go through and assess the injuries

22:44hello vinnie he's very sweet all right big guy i know i know i know
23:02vinnie lives with zoe her husband simon and their two teenage children in surrey the kids love vinnie so
23:10he'd love to lay on their beds and they're doing their homework and he's laid across their homework
23:15books the family got the handsome tabby as well as his sister honey as kittens they were the only
23:23two kittens of that litter we walked in and i put my handbag on the table and and vinnie just
23:31climbed in my handbag and was purring and i just said to simon oh i've got to have these cats
23:37vinnie jones is my husband's favorite sports personality i suppose he's a big wimbledon
23:45supporter always wanted to call our children vinnie and i was never quite sure so when we got the cat i
23:52said you could you're allowed to call the cat vinnie he's got a full thickness de-gloving
24:01all the skin's gone his tibia is sticking through his skin here
24:09all right rapid heart rate but otherwise okay lungs okay
24:14if we can confirm that he does not have internal injuries then we're in a race against time with
24:19infection in the limb if we leave it longer than the golden window which is 12 to 24 hours
24:24before the bacteria become an infection then we've got a problem it's okay it's okay
24:31although both of vinnie's back legs are injured his left leg is in a worse condition
24:37the foot is floating in the breeze out here and the ankle is stuck through no skin here it's literally
24:44not attached to anything apart from the tendons the blood supply and the nerves
24:51he could feel his foot as far as we can tell okay that means that the main nerve that goes down the
24:58front and the back is probably still there okay that's hope because if they were dead so that would
25:06be that would be amputation what i would try and do down here is what's called an ankle fusion
25:13a pussycat or a dog can easily manage with that and still keep a leg it's a very recognized procedure
25:20okay yeah i'm sorry
25:25you know we're lucky we've got here so far you are okay do you want me to proceed yes okay all right
25:33do you think you would let us try and take some blood from you my friend
25:36i think the kids are going to be devastated i just feel so sorry for him i'm really sorry for him
26:01yeah yeah i'm going to move all of the leg and then click this one first vinnie is being prepared
26:15for surgery after being run over by a van scans have confirmed he has no internal injuries but his
26:22right leg is fractured and his left leg needs immediate attention to try to save as much soft
26:28tissue as possible so there's some tar left from the road on here so we're just cutting that away
26:36we just cut off everything instead and stitch up everything that's still alive
26:40and then if we have enough things still left alive the leg will be fine
26:44if we don't the leg will die but i wouldn't be doing this if i didn't think that we had enough
26:49with the wound cleaned noel begins the process of reattaching vinnie's left foot
26:54just flipping the tail is out of the way so we can get to the bone behind that which is called the
27:00calcaneus there is extensive damage around the ankle so noel will fuse it solid by drilling out the
27:08cartilage and securing it with a metal plate we've got to be very careful not to damage this blood vessel
27:16which is in here if we damage that the foot dies
27:24so just screwed the two main ankle bones together now i'm going to try and align everything
27:29so now we've reattached the foot it's all solid again sadly this skin may well die you can see
27:45there's a blood vessel ruptured there but once you get it wrapped back around the lining of the muscles
27:51it's in with a chance if you can do this surgery very quickly
27:58noel uses a different technique to stabilize the fracture in the right tibia
28:02using two standard plates when you take two of these plates so that they're at right angles to
28:08each other it's incredibly strong because you can't bend the plate on its side
28:12i have seen hundreds of legs chopped off that could have been saved if there is a blood and
28:23nerve supply most traumas in my opinion are fixable noel has completed the operation within 15 hours of
28:32vinnie's accident he now has a fighting chance of avoiding infection
28:42all right let's see if i can do this stupid thing
28:46i can only feel one hole
28:51there's two holes oh yeah yeah yeah and then just push

29:02there we go it's early morning sarah's arrived with visla brody he's having surgery to correct a
29:12failed partial elbow replacement which has left him in terrible pain at the moment i'm feeling
29:18nervous because we've waited so long to try and get things fixed now really looking forward to
29:28noel being able to work his magic and uh start life again all right i'll see you soon okay there's been
29:36a lot of animals put through enormous pain and operated by somebody that wasn't as competent and and
29:45shouldn't have been doing the surgery that they did we wanted to have two two opinions
29:51and no other surgeon would even consider uh looking at somebody else's poor work and that was very
30:00traumatic the truth is that any implant can be put in by any vet without an ethics review of manufacture
30:09or technique i think that any operation performed by any vet should be equally held to the same high
30:17standards noel takes away diseased bone from around the elbow joint to gain access to the disc implant
30:34but he's hampered by the effect of the previous surgery
30:38so it's highly likely this bone is going to disintegrate in here as i touch it
30:41because it's infected and it's eaten away by debris from the failed implant
30:51the original implant has caused damage to the bones and resulted in likely infection
31:00this is the implant coming out the problem was that a bit of bone on the outside here wasn't placed
31:06properly and it fell off and the disc wasn't placed properly and it fell down
31:14noel and his team have designed a new implant system which will fuse together the damaged bones
31:20around the collapsed joint so this plate's been designed so that it has a ski tip here
31:27which is going to go up the back of the humerus the shape of the new plates will help to distribute the
31:32load lessening the chance of a future fracture the first plate is screwed to the humerus
31:45noel uses bone graft he harvested earlier from brodie's humerus bones to help fuse the joint
31:52he attaches a second plate to the first and screws it into the radius and ulnar bones
31:57i'm just taking the graft and i'm packing it into the gaps

32:19all righty that's the best i can do for you brodie sorry you've been through so much buddy
32:27i was just going to give him his methadone 0.1 still it's 5 pm vinnie's condition has worsened
32:39and he's under close observation in the prep area all right big guy
32:48hey mate hi buddy good boy four days ago noel performed emergency surgery to repair his broken
32:56back legs after being run over by a van all right big guy i'm just taking this off buddy
33:04he was doing great and he had very good signs of recovery but then he started to go downhill
33:15very soon it became apparent that he was showing signs compatible with sepsis which is when bacteria
33:22from the road traffic accident had somehow made its way into the bloodstream and circulated in the bloodstream
33:37then the immune system mounts a huge response to that and and it can be fatal so we've been battling
33:44for the last several hours now to uh save his life well short of taking this leg off there's nothing i
33:51can do about that i don't think he's strong enough for an anesthetic i don't think he is either
33:55finney is too weak to have an amputation to try and halt the spread of the infection
34:01we can't anesthetize him to take his leg off so we're going to have to see how that manifests
34:05noel's team will continue to give antibiotics and other drugs intravenously in the hope of fighting
34:13the sepsis that's threatening vinnie's life if we can manage him overnight and i'm getting stronger
34:20he might tolerate an anesthetic maybe tomorrow so i guess we'll just have to see
34:39fix patch referrals orthopedics and neurology how can i help

34:54megan has returned to pick up rogan two days ago he had surgery to replace his right hip joint
35:00with a bespoke implant we're excited to have him home but we're a bit apprehensive
35:07because i know his recovery it's going to be a lot so we need to be extra careful with him he's
35:13only allowed in a crate so we've actually positioned three crates around the house in the main room so
35:19he doesn't have to do any kind of walking at all
35:24so the cup is specifically designed for rogan look at that and it's specifically designed so
35:30that the head is 12 millimeters which is similar to that as opposed to the commercially available
35:36one which was too small yes so he's the first dog in the world to have this particular cup
35:42only the best for my boy and as you can see because we brought that head down this muscle's
35:48now tight and that means that the leg is going to be held rigid for a couple of weeks don't worry about
35:54it so he does this now well at least it's you putting a little bit of pressure on it yeah yeah
35:59he's doing fine yeah do nothing for two weeks just let everything chill now who've we got here

36:09we shall miss him he's such a lovely boy i'm ecstatic to have him back the house has been
36:20ridiculously quiet without him his uh boston brother has been moping around the garden and
36:25the house not knowing what to do with himself so everyone will be happy it's back
36:40yeah his blood pressure is around 108 it hasn't gone below 90 the whole time
36:47vinnie the cat has been fighting for his life after being hit by a van
36:51this is a disaster this leg needs to come off now he's suffering from sepsis but the team have
36:58managed to stabilize him his best chance now is for noel to amputate the leg are we stable enough to
37:05take this leg off as gangrenous all right all the muscles died so he must have had a much more
37:13significant crush injury we can't have predicted that that's just the luck of the draw if noel removes

40:12when it comes to meal times the service at the practice is second to none feeling time is probably
40:23my favorite part of the job they're just there licking all the food off your hands and it's a bit
40:28messy but it's nice i've seen some patients come in with a dietary requirement less longer than i have
40:34myself mum and dad like to bring in very specific snacks and treats you want a carrot mate everything
40:42that you could possibly imagine from steak to um boiled egg is that nice
40:50oh you don't have to eat it all at once but visla brodie is not tempted by the practice's five-star menu
40:56he's recovering from revision surgery to fuse his elbow after a previous operation failed
41:02but as noel suspected the joint was already infected after the original surgery
41:08hello hi sir noel here um brodie's fine you'll be very much aware that i thought brodie had an
41:15infection from the outset so that's why i started intravenous antibiotics and ostensibly it hasn't worked
41:22so we now start on a different iv antibiotic yep so uh we can only hope that that the situation turns
41:31a corner in the next uh few days what i think we should do is get you in to feed him because his appetite
41:37has been zilched today yes so i need you to bring whatever uh he normally eats please okay

43:04it's now three months since boston terrier rogan's right total hip replacement and the family are not
43:10only celebrating his recovery but a new addition to the fold quite a snuffly one
43:22initially he started barking and growling he's never been around a baby before both of the dogs are
43:28good with kids but never been around a newborn before so it was interesting wasn't it yeah
43:34it's been difficult to try and juggle rogan being in a crate with us needing to look after the baby
43:43we're keeping an eye on him to make sure he's not doing too much
43:46and giving him as much freedom as we can to try and keep him happy as well as us happy
43:53look at rogan rolling around it's nice to see him going around again isn't it yeah
43:59the operation has been successful his hip is fully healed i think it's with him he bounces back
44:05really really fast he bounced back too fast i think because we had to up his sedation medication
44:10oh yeah that was a bit of a worry wasn't it boys
44:20enough we've still got a quite a long road ahead of us he's still got a diseased hip on the other
44:27side that still needs surgery i'm not looking forward to it no not at all but i'm hoping he
44:33might take to it better because he has got a good hip now yeah i think now rogan's getting more freedom
44:40and he's able to move around a lot more i think he's a lot happier yeah definitely no regrets no
44:45regrets at all just a lot more sleepless nights before we had the baby than what we hoped
44:57at home in surrey hello visla brody's infection has cleared since his operation brady's been much
45:05more joyful it's as though he's come out of a depression after a year of pain he's not only
45:12able to walk on his right front leg brady he's running come on this way
45:23is this fun is this fun he's really enjoying walking and running and playing with his brother
45:30they are inseparable so being able to take them for a walk together it's it's very exciting
45:36feels like we're getting back to normality gently good boy there we go i'm incredibly grateful to noel
45:46and that the team at fitzpatrick referrals he's running and he's with us and that's happened because
45:53of us fighting and noel fighting whoo whoo whoo whoo i know come on and that means everything
