02:44And all the glass was broken
02:46And I was very close
02:48All this glass just came in through all my face
02:55I couldn't open my eyes anymore
02:58I need the ambulance quickly
03:00Because I really think this is serious
03:03So it was 2am on the Sunday
03:05It is very difficult to find a specialist for eyes on a hospital
03:10I say to Paulina
03:12We need to pray
03:13And we just stay in that bed in the hospital just praying
03:17And eventually their prayers were answered
03:20Ophthalmology specialist Dr Peter Hayden
03:23Was the surgeon on call
03:25Dr Peter Hayden just appeared
03:27And we call him like an angel
03:30Because he is really taking care of me
03:33And I think that's the point where the miracles start to happen
03:37Today Jonathan is under the care of Dr Hayden again
03:41While his right eye has healed
03:44He still can't see out of his left eye
03:46The reason I was attracted to ophthalmology
03:49Was the eye is the most important organ
03:51In sort of interpreting the outside world
03:53All ophthalmologists feel very privileged
03:56To be treating one of these primary senses
03:59And everyone realises how important vision is
04:02Okay, it's been a long recovery
04:04We've tried a number of things
04:05So I think this is one of the final steps in your recovery
04:08So it'll be good to get it out of the way
04:10It took several months for all the stitches across his left eye to be removed
04:15The other aspect to the injury was that the glass penetrated the lens
04:20And as a result of that injury to the lens
04:23The lens was removed at the time of the initial surgery
04:26So you've been walking around now for 18 months without a lens in your eye
04:30Without the lenses inside you actually can see much
04:33You see things moving or colours
04:36But it's very difficult to define things or to see very clear
04:40Dr Hayden will insert a new acrylic lens into Jonathan's left eye
04:45But there's no guarantee it will fix his sight
04:48I really need to see again
04:51In the RPA emergency department
05:02A distressed little bundle has just arrived by ambulance
05:08Oh I know, I know, I know, I know, it's terrible
05:1622 month old Arlen is struggling to breathe
05:19And his parents Molly and Sahaad are consumed with worry

05:34Tell me what's been going on
05:36He's been like on and off unwell
05:38He took a real turn on Friday
05:40We took him to the doctor
05:41They said it's just a virus
05:43And as long as he can keep fluids up
05:45And then today he just really wasn't taking any fluids
05:48So I took him to the hospital
05:50And here we are
05:51I don't have children myself yet
05:53But I've got three little nieces
05:55So I can totally relate to how parents must feel
05:58When their child is unwell
05:59And a big part of our job is supporting the parents
06:04Through what they're going through
06:05As well as the patient or the child
06:07How many doses has he been unwell for in total?

08:15Looking at the baby's brain looks really normal

09:06I don't have the heart to wash it
09:08Because it has a little bit of her there
09:12She was 28 weeks when we lost her
09:18I labored for two days
09:20Because they had to induce me

09:41A few weeks before she was stillborn
09:43Baby Jackson was diagnosed with foetal anemia
09:47A rare and dangerous condition
09:48Where the red blood cell counts are so low
09:51The organs are not properly nourished
09:54We've gone through so many investigations
09:58To find out
10:00What was causing the anemia
10:02And they couldn't find anything
10:05So the doctors
10:07Did what they can to help our daughters
10:10Survive and go through the pregnancy
10:13Jackson was given blood transfusions
10:15To boost her red cell count
10:16But it wasn't enough
10:19After Jackson's third and last transfusion
10:22Within 24 hours
10:24She passed away
10:26It wasn't caused by the blood transfusion
10:28It was because of her condition
10:30I'm going for a walk

11:21Everything looks really good
11:23Baby weighs nearly 2.6 kilos
11:26It's on 37th centile

11:30So it means it's trending in the right direction
11:32All the measurements
11:33So that's really good
11:35While he's doing well
11:37Baby Ronan will require one more blood transfusion
11:40In a week's time
11:42Just before Jamie is booked in
11:44For a caesarean section delivery
11:46I think as a midwife
11:48Working with women who've had a previous loss
11:50It's such a privilege
11:51Obviously for Jamie and Ramon
11:55Having the blood transfusions again in this pregnancy
11:58Was really triggering
12:00Because of what had happened previously
12:02So just being really, really sensitive to their needs
12:05More emotionally from my role
12:07Than physically
12:09Alright, it's going to be a long week
12:11But I'm excited
12:13I'm a lot more scared with this pregnancy
12:15Than I was with Jackson
12:17My innocence towards pregnancy
12:19Has been taken away
12:21In the ED
12:3122 month old Arlen is still struggling to breathe
12:34And doctors don't know why
12:38His oxygen levels are dangerously low
12:41Right now, the medical team are trying to open up his airways
12:45With medication
12:47So at this age we try to avoid a diagnosis of asthma
12:50And things like that
12:51Even though he's come in wheezy
12:53With what we generally describe it as a viral induced wheeze
12:56Rather than a diagnosis of asthma
12:58Usually even if you pick something up from daycare
13:01He usually clears it pretty quickly
13:05About a day or two usually
13:07Yeah, he's a really fast healer
13:09Arlen may normally be a resilient little boy
13:13But today, he's not keen on all the testing that has to be done
13:17No, it's so annoying
13:20The x-ray shows some subtle inflammatory changes in the lungs
13:28That we suspect is coming from a viral infection
13:33With the possibility of a pneumonia needing to be considered as well
13:37Pneumonia in the very young and very old can be lethal
13:41I just want to err on the side of caution and get the paediatrics team to see him
13:44Okay? A couple of reasons
13:46One, that you've come back from overseas
13:48And he's been unwell for four days
13:50And also the fact that he's had a trial of this
13:53Burst therapy and he's still got some work of breathing there
13:57Arlen won't be going home today
14:0834-year-old Jamie is back at RPA with husband Ramon
14:12For their unborn son to receive his last blood transfusion
14:18Baby Ronan has the same fetal anemia disease
14:21That took the life of his sister, Jackseen
14:23She passed away within 24 hours of her last transfusion
14:28It's mostly during the transfusion when I get really anxious
14:32Because it's quite intense for me
14:36So I have to stay still
14:38And a lot of the trauma from losing Jackseen comes back
14:42So then it only lasts for about 30 minutes
14:45But to me it's the longest 30 minutes of my life
14:47To help the couple through another nerve-wracking treatment
14:50Midwife Lou will be in attendance
14:54I feel like every blood transfusion that they've had
14:58Has definitely brought like a layer of stress
15:02I think for Ramon watching Jamie actually go through the procedure physically
15:08It's invasive and it hurts

15:15I just want to get the day done

15:26Dr Mogra will perform an ultrasound
15:28To check the position of the fetus before the transfusion
15:31Any procedure we do in pregnancy has a risk
15:34One of the risks of the transfusion is baby not surviving
15:38And the other risk is requiring free-term delivery
15:42Preparations are made in case there is an emergency
15:47We're doing a fetal blood transfusion
15:49Which is not expected to end in a delivery, a birth
15:52But there's an outside chance of it
15:54That's a baby's heart beating?
16:04Yes
16:06And I just need to see where the cord is
16:10It is for me to understand where I'm going to put my needle in
16:14So just making sure where the placenta is, where the cord is
16:19I'm getting my angles right for the start of the procedure
16:24Jamie will be given a local anaesthetic
16:27Before donor blood, rich in red blood cells
16:30Will be transfused through a fine needle directly into the umbilical cord
16:34We've got the blood
16:36The theatres and the NICU
16:38And the delivery ward have all been notified

16:45I know I was calm earlier
16:47But it's usually this way
16:48Every time I come to this room
16:49That's when I get really anxious
16:51Because the real thing is happening
16:52We don't know what's the result going to be
16:53If it's going to go well or not
16:55At some point they might have to stop if it's not going well
16:58So a lot of things are going in my mind right now

18:18He's high on that extra, extra dose of blood

18:27This may be loud to other people, but to me, it's music to my ears
18:32We're looking forward to seeing our son

18:4431-year-old Jonathan was a bystander to an ugly brawl
18:48Ending up with his eyeball slashed by broken glass
18:52Today, he and fiance Paulina are hoping surgery to replace the lens
18:57In his left eye, will finally give him his sight back
19:00I guess I'm very privileged to live in that era where the technology is such
19:05To go one day from having a patient who can't see anything
19:08And then operate on them
19:10And then to see them the next day and take the pat off
19:12You know, it's quite amazing
19:15What a difference it can make to people's lives
19:16What we're going to do here is just get your anaesthetic stage of things set up
19:20And put some drops in your eyes and then give you the anaesthetic needle
19:26Which will numb the eye and make sure that you're all comfortable for the surgery, okay?
19:29Yep
19:31Waiting outside, Paulina is a bundle of nerves
19:33I wasn't very emotional because this is a day that we are waiting for a long time
19:40I think Jonathan deserves this opportunity to recover
19:45We don't have a lot of chances to recover at all, 100%
19:51But at least to give to him the opportunity to stay better, to see again
20:00Okay, nice and still for me, okay?
20:03Jonathan isn't fully sedated
20:05If he moves at all during the delicate operation
20:08It could cause even more damage to his eye
20:11I'll just make a little side point incision
20:13So I can put some gel in the eye to inflate it
20:16The gel stops the fine structures in the eye from collapsing
20:22So now I need a main incision to get the lens into the eye
20:26It's got to be a slightly larger incision than the other incision we made
20:29Choosing the correct powered lens is a difficult estimation
20:34Everything hinges on Jonathan staying still
20:37And Dr Hayden getting the lens in the right position in his eye
20:42The front of the eye is very, very small
20:43So it usually is about 2-3mm of space that we're working in
20:48So not much room to go
20:50The injector's in the eye now
20:52Both legs of the lens are behind the pupil now
20:55Which is where we want them to be
20:57Dr Hayden's obviously very experienced
20:59He did that very, very, made it look very easy
21:01But I can tell from a registrar's point of view
21:04It's definitely not that easy
21:06Especially something like this that we don't see everyday
21:08Now the lens is in the right spot
21:10Dr Hayden has one more job to do
21:13Okay, so now the task is just to remove the gel from the eye
21:18Which we use to inflate the eye initially
21:20So the machine is like a vacuum cleaner
21:24It's like an accelerator on the car
21:26We can hear how much vacuum is being applied just based on the sound
21:29This is always a very satisfying feeling seeing the pupil come down
21:36It just means that the eye is going back to its natural state
21:41Last thing we'll do is put some antibiotics in the eye
21:44Just to prevent any infection in the post-operative period
21:48Throughout the whole operation, Jonathan has not moved a muscle
21:52Well done, okay, I'll just clean your eye, okay?
21:56But it's still unknown if he will regain sight in that eye

22:26I'm nervous but not as nervous as I thought I would be

22:35I'm just really looking forward to meeting this little boy
22:37I feel very connected to this little guy
22:40He's been such a fighter during the pregnancy and surprised us
22:43And it has gone against all odds actually
22:44Oh my God, I'm going to be a crying mess, I can feel it
22:49After the devastating loss of their first baby
22:54An army of doctors and nurses will do all they can
22:58To ensure the survival of the young couple's second
23:01So all the doctors are going to come too
23:04I have to get a whole stack of bloods and so
23:08We'll be doing that
23:09With the neonatal intensive care team on standby
23:14Dr. Keren Horton is preparing for all scenarios in the delivery room
23:20We know that baby Ronan has anemia
23:23He might need a blood transfusion
23:25We've got all of our equipment here so that we can give that to him
23:28Whether he needs it urgently or later today
23:29They can have a complication called jaundice and they can need lots of phototherapy which is a blue light

23:44They can also need something called an exchange transfusion
23:46Which is where we essentially need to exchange their whole blood volume with new red blood cells
23:51And so we actually have all of that equipment ready to go in case that happens today
23:55And hopefully that's the worst bit over
23:57Yeah I hope so too
23:59And then I guess the last thing that we need to consider for Ronan is
24:02We might need some respiratory support for the first couple of days of life
24:05So he's definitely an unusual case

24:35And there's an element of unpredictability in all of it
24:39So we always have to be quite tuned in to what's happening
24:43Most of the time you can leave a birth feeling exhilarated and happy
24:49And occasionally things don't go as planned and it can leave you feeling really sad

26:42Laser treatment to remove the remaining scar tissue
26:46It's like looking through a dirty window
26:48And when we laser that, the light will be allowed through to the back of the eye better
26:53So I expect that that will improve his vision as well
26:56But I just want to wait for the eye to settle after his surgery before we do that laser
27:01I was hoping that he might be able to see, you know, a little further down the chart without using the pinhole
27:07But, you know, realistically, as Jonathan and I are both aware, there are some limitations just given the injury to his eye
27:19Almost 12 months ago to the day, Jamie and Ramon's first baby was stillborn
27:26I'm ready though
27:28We've waited so long for this

29:47He had some oxygen issues
29:53We tried to put the little tubes in his nose with oxygen
29:56Which he didn't love
29:58But about 5am he was too tired to fight it anymore
30:01So we got the tubes in and his oxygen was stable
30:06It seems like he's not 100% obviously
30:10But he's on the way

31:09So it all fits with what we were talking about with viral pneumonitis

32:19Doctors need to rush him to the neonatal intensive care unit
32:24To address any complications from his fetal anemia
32:27The blood test that we're going to be doing initially for Ronan is we'll do something called a full blood count
32:31So we're looking at how many white cells he has which are your infection fighting cells
32:36We're looking at his red blood cells so they're the ones that transfer oxygen around your body
32:40And we'll be also looking at his platelet count and they're the cells that help you to clot
32:47We're going to pop a cannula so a little drip probably in his hand
32:50And we'll start him on some light therapy while we wait for all of our blood results to come back
32:54And then we'll make a decision from there so does he need to stay with us in the nursery
32:57Or is he able to go back to mum?
33:00What the blue light is is that's treating for jaundice which is when you have breakdown of red blood cells and your red blood cells leak out
33:07What we think might have happened in his case is that he might be breaking down red blood cells
33:12And so then that releases bilirubin which is a chemical into his bloodstream
33:15And it's really important that we keep those levels low otherwise it can cross over the blood brain barrier and cause damage to your brain
33:23There's a lot going on but keeping an ever watchful eye is new dad Ramon

34:02The first test results come back quicker than expected
34:07He's jaundice level so his bilirubin is within the normal range so we've also stopped the light therapy
34:12And then what's been really fabulous is that his haemoglobin or the amount of red blood cells that he has are completely normal
34:18So 151 which is very normal for a baby who's just been born today
34:21Ramon shares the good news with Jamie who's not allowed out of bed yet after the caesarean
34:27He's like making faces
34:31Even though it's really positive that we have these results right now we still are very early in his journey
34:37Because we still don't have an answer about the cause of his fetal anemia
34:41We're still in a space where we don't know the answer and so we don't know where he's going to from here
34:45And so it'll be interesting to see what his results do over the coming days
34:49But it will also be interesting to see if we can find an answer for him and his family
34:54Because at the moment as the cause remains unknown he's really still not out of the woods just yet
34:59It's been an agonising two years of surgery and treatments for 31 year old Jonathan
35:14After glass from a smashed window severely damaged his eye
35:17So how's the eye been going since I've seen you two weeks ago?
35:22I don't feel pain
35:24Yes
35:25That's a good thing
35:26After surgery to replace the lens in his left eye
35:29Jonathan's vision is better but still very blurry
35:32You can see that kind of white area there is the corneal scar
35:36So the aim of today would be to do a laser procedure to make a window and allow light to go through the pupil to the back of the eye
35:51Once the laser is done it's like a one-off procedure
35:54Generally the scar tissue doesn't grow back and the window just remains open
35:58So this is the little lens that we're going to use on the surface of the eye just to focus the laser

36:36Basically Jonathan was looking through a dirty windscreen and we were cleaning that windscreen by making a new window
36:42Jonathan will also be fitted with a contact lens to help get things into focus

40:57It's a very different boy
41:00It took really about two weeks for him to be back to himself
41:02It was almost like he had to learn to walk again
41:05Because, you know, he'd just been
41:08He had lost so much weight
41:10And all of his muscles had kind of gone
41:13Happily, he made a full recovery
41:16We went to a friend's wedding
41:18And he was the centre of the dance floor for that
41:20After two years of treatment, Jonathan's hoping today will be his last visit to ophthalmologist Dr Hayden
41:36And everything's been going okay with your eye
41:38I know that you have seen the optometrist and she fitted you with a contact lens
41:44Do you think we could get you to pop your contact lens in your eye and we can check the vision once you've got that in?

43:06A miracle baby who's confounded everyone
43:10Passing every blood test with flying colours
43:13Since he was born three weeks ago
43:15When Jamie gave birth, we were initially told that we were going to be staying in the hospital for four weeks
43:20But it only took us two days in the neonatal unit and then like five days overall
43:25So I was in a bit of a panic and, you know, because we didn't have anything
43:30Uh oh
43:32Despite extensive testing, exactly what caused the fetal anemia in both Ronan and Jaxine is still a mystery
43:40We're keen to know just for future pregnancies as well
43:43And just foreclosure as well from our daughter
43:47Yeah
43:48Ronan will continue to be closely monitored
43:53This is a huge blessing and a lot of people worked really hard for him to exist in this world
43:58Everyone from the fetal medicine department down to me giving birth
44:02They have been very wonderful
