00:00hello there welcome to the second week of putting dr g on the spot in the month of august when we
00:06talk about the aging process in men for their sexual and reproductive function and this week
00:13we're specifically talking about the prostate although prostate is not directly related to
00:25sexual or reproductive health it actually is crucial part of the reproductive health as far
00:32as production of seminal fluid or the semen the semen itself accumulates in prostate mixed with
00:39sperms and create ejaculations as men get older by definition the age of 40 then the prostate gets
00:47bigger so some statistics shows that men at the age of 40 will already have 20 percent of bph benign
00:56prostatic hyperplasia at the age of 50 probably about 30 percent 60 probably about 50 percent and 70
01:04about 80 percent and 90 nearly a hundred percent so what exactly happens actually there is one component
01:12of the testosterone which is the active form of testosterone called dihydrotestosterone and this
01:19testosterone is deranged and the prostate gets bigger as a result of that and then men will begin to get
01:26urinary problems what sort of urinary problems will men encounter or first of all they'll start waking
01:33up at night to urinate it's because they don't empty their bladder completely then they will have frequent
01:38urination during daytime and this is partly because the excess amount of urine the residual urine that
01:45is left behind then you'll get frequency urgency and also slowness of urine that will constitute
01:53hesitancy a bit of waiting whenever they urinate and also dribbling incomplete emptying and straining
02:02and all these are not related to cancer it's just related to enlarged prostate
02:08the enlarged prostate itself will continue to grow at some point it will need treatment and that's the
02:14reason why when you see older men standing at the urinal they take longer to urinate and they take much
02:20longer in order to shake it off so when this begin to cause problems like affecting quality of life
02:27lack of sleep and also embarrassment due to incontinence of urine then certain medications come in handy
02:35alpha blockers 5 alpha reductase inhibitors pde5 inhibitors and also all these combinations can help
02:44to reduce that frequent urination at some point an advancing age will cause men to have prostate that
02:51might cause blockage and acute retention of urine at that stage you don't have a choice but have to face
02:57the problems of an operation called the TURP this TURP is a scraping of the prostate opening up the channel
03:06so men can urinate like how it was when they were 25 for the following week we're talking about more
03:13aging process this time we're going to talk about what happens to your testosterone as you get older
