'A press conference has been held as a second independent investigation into the treatment of Yusuf Mahmud Nazir, who died at five-years-old after being sent to hospital with tonsilitis, has found failings with his treatment.'

00:00Hi, I'm Sonia Ahmed and I'm Yusuf Swann.
00:03Our healthy child, full of life, was diagnosed with tonsillitis,
00:08a harmless condition that spiralled out of control because it was left untreated.
00:14Despite begging for help at every step, including Rob Room, the GP, Sheffield Children's Hospital,
00:21we were faced with repeated rejection and delays from healthcare professionals.
00:24The failings we faced during this critical time led to the unimaginable and the traumatic loss of our child.
00:36They say a mother's instinct is a powerful thing.
00:38It can neither be ignored nor explained, yet my concerns were ignored each and every time.
00:44My son was left to die right beside me.
00:47He was crying in pain, yet received no pain relief.
00:50A 10-year-old candida technique was used, causing it to burst, meaning along with no pain relief, my son received no medication.
01:02Junior staff were making complex decisions, while not a single consultant or a senior member of staff was in charge of his care.
01:11The report concludes that 13 missed opportunities to escalate Yusuf's care, all while I was trusting the NHS to protect him.
01:22They failed him, catastrophically.
01:25Why was my son treated like a number on a list, and not a living, breathing child desperate for help?
01:33Each day feels like an uphill struggle as we relive the trauma of losing him, while simultaneously confronting the frustration of unanswered questions.
01:42We will never forget the traumatic experience of losing our child in such a sudden and preventable way.
01:48The pain will haunt us forever, knowing we couldn't protect him from the failures that led to his death.
01:56Yusuf's name must mean something.
01:58It must stand for accountability, for change, for urgent needs to listen.
02:04Really listen.
02:05Listen to parents when they say something's wrong.
02:08As a family, we need closure, and this can only happen when we have answers.
02:12He was very special.
02:14He was a very special child.
02:16He was loved by everybody, and he was a very active child, and he had a long, long life ahead of him.
02:23He enjoyed his family time.
02:28He enjoyed his independent time, and he was full of energy, full of life.
02:32I think the plan is to write to the coroner, following the report, and the failures that have been identified,
02:39and ask them to consider opening and ingress in these circumstances.

02:45And you've been still a conservative champion today, I've seen.
02:48You've spoken to Wes Streeting in the past.
02:49Are you confident that this is going to move forward on a national level, and changes are going to be implemented?
02:55Yeah, we have been in close correspondence with Wes.
03:00Wes has been very supportive to the family.
03:02He's supported us since Yusuf died in his previous position as the Shadow Health Minister,
03:08and has continued his support, strong as a Health Secretary of State.
03:14He has been very supportive, and we can see that he will help us get our inquest,
03:20and we have faith in him to make changes.
