00:08Emily Burns, 18, died due to a lack of safety planning after being discharged from Adolescent
00:15Mental Health Services in North East London, an inquest has revealed.
00:20Her parents have spoken out, saying their daughter was let down when she needed help
00:24the most.
00:25The coroner found evidence of care failings but could not conclude that any specific aspect
00:31of her treatment directly contributed to her death.
00:35Emily from Wolven Forest had been receiving care from CAMHS for anxiety, depression and
00:41anorexia.
00:42She started antidepressants in February 2022 and was referred for psychotherapy in April
00:482022, but faced delays and received a short course of therapy that was inadequate for
00:54her needs in January 2023.
00:57When Emily turned 18, her care was transferred to an adult team, but she continued to struggle.
01:04Despite multiple requests from her and her family for medication reviews and therapy,
01:10her care was not properly addressed.
01:12She received an initial assessment on May 5th 2023, but this was not from a fully qualified
01:18therapist, an inquest heard.
01:21Months before she died, Emily hugged her mum before school and said she thought her
01:26medication was not working and needed to be increased.
