00:00The transport ministry on Friday released the final report on the private jet crash
00:05at Elmina Township in Selangor in August last year.
00:08The Beechcraft Model 390 crashed on Persia Ran Elmina on August 17, killing 10 people
00:15including six passengers and two crew on board the aircraft.
00:19The report by the Air Accident Investigation Bureau concluded that non-compliance and human
00:24error by the flight crew was the primary cause of the accident.
00:28It revealed that the incident was caused by the accidental extension of the lift-dump
00:32spoilers by the flight crew during a pre-landing checklist routine.
00:37The report said other contributing factors included deviations from standard operating
00:42procedures, inadequate crew training, regulatory grey areas and deficiencies in communication
00:48and decision-making.
00:50Lift-dumpers or spoilers are devices used to reduce an aeroplane's lift, thereby increasing
00:55drag and are commonly located on the wings.
00:58When an aircraft needs to slow down, pilots deploy the lift-dumpers which extend over
01:03the wings to decrease lift and increase drag simultaneously.
01:07However, during landing, lift-dumpers in most newer aircraft activate automatically to keep
01:12speed more manageable.
01:14The report also said the two crew members' seat arrangement breached the flight manual
01:18and protocol, with them being in the wrong seats before the crash.
01:22The Air Accident Investigation Bureau said it is crucial to emphasize that the findings
01:27are not intended for the purposes of apportioning blame or liability, but rather to prevent
01:32future accidents and improve aviation safety.
01:36The aircraft which departed from the Langkawi International Airport en route to Subang Airport
01:42crashed just two minutes before landing.
01:44An e-hailing driver and a delivery rider were hit by the plane on the ground and perished
01:49during the incident.
