A popular sandwich chain in the U.K. and elsewhere has reportedly revealed that a second customer has died after having an allergic reaction to a product.

A popular sandwich chain in the U.K. and elsewhere has reportedly revealed that a second customer has died after having an allergic reaction to a product. 
According to the BBC, the person died in late 2017 after eating a Pret a Manger super-veg rainbow flatbread sandwich which is thought to have contained milk-based yogurt despite being marked dairy-free. 
Subsequent testing by the restaurant chain determined the ingredient did, in fact, have small amounts of dairy though the yogurt supplier has denied any negligence in the person's death. 
News of the fatality comes after Pret a Manger agreed to revamp its food labeling practices following the death of a 15-year-old girl in 2016 after unknowingly eating sesame in a baguette.
The New York Times reports that an inquest found "the company had been warned of allergic reactions to its baguettes six times before [the teen's] death on July 17, 2016." 
The coroner in her case indicated that no law had been broken, but the death was a tragic sign that changes needed to be made in the food labeling system.
