00:00Hello there, welcome to Movember November and the second installation of this month.
00:05We're going to talk about prevention of male cancer and we're going to talk about prostate cancer.
00:17Prostate cancer is very, very well known and in fact the number is increasing.
00:23Some people argue that it's because men live longer and some people argue that it is probably
00:28because of genetic manifestation and it's true, both of them is true and therefore we as we get
00:36older and also especially someone who's got family history of prostate cancer, you need to be a little
00:42bit more cautious and think about prevention and I want to share with you three perhaps quite
00:47important prevention that you might want to know. First of all, is there any drugs that can actually
00:53help you to prevent prostate cancer. In fact, this is a very famous trial called prostate cancer
00:59prevention trial using a group of drug called 5-alpha reductase inhibitors, specifically
01:06phenasteride. This medication helped to shrink the prostate and they initially thought that perhaps
01:12shrinking the prostate, we might see significant reduction in prostate cancer. So men who took this
01:19over seven-year period, they see a 20-23% reduction of prostate cancer in the low grade. So that
01:27creates
01:28a lot of excitement about chemo prevention. However, in the same study, it also showed that high-grade
01:34prostate cancer is increased a little bit and therefore the study itself is stopped and therefore the drug
01:41itself is not indicated. Many people argue that it's because prostate cancer is shrunken, perhaps the pickup rate
01:47is higher. Therefore, you actually see the high-risk prostate cancer and then that might be the case. However,
01:55what is determined is that the manipulation of the testosterone itself by this type of medication
02:03might have a significant improvement in the prostate cancer prevention. The second group of study
02:10study actually is a big trial that is looking at 30,000 men over a period of about 30 years
02:18in the
02:18frequency of ejaculation. It is noted that if one ejaculate more than 21 times might reduce the risk of
02:26prostate cancer by about 20 to 23%. Quite significant you say. Although that study was really really quite
02:34exciting and generate a lot of enthusiasm but some people argue that men who ejaculate more perhaps are
02:41actually more healthy and therefore they are the actual risk of prevention may not be so significant.
02:50One thing that is good that this study proved that frequent ejaculation doesn't contribute to a higher
02:56risk of prostate cancer. For one formal prostate cancer, we are going to talk about another prevention of
03:02male cancer. What more important to talk about prevention of penile cancer. These, we're going to talk about
03:10HPV and circumcision. Stay tuned for next week's Putting Dr. G on the Spot.
