00:05Intro
00:27Saudara linfoma merupakan salah satu kanker darah yang paling sering ditemukan di dunia
00:32dan mempengaruhi ratusan ribu orang setiap tahunnya
01:32And of course to discuss it, we also have here our guests in Kompas TV studio
01:37Dr. Kevin Tay, a senior medical oncologist on Co-Care Cancer Center Singapore
01:42And Dr. Jonathan Tay, a senior radiation oncologist AARO Singapore
01:47Hello, doctors, how are you?
01:49Thank you for having us
01:51Thank you for coming here from Singapore
01:53Okay, firstly, I would like to begin with Dr. Kevin first
01:56Doctor, to begin with, what exactly is linfoma and why is it considered such a complex group of cancers?
02:02Lymphoma is a cancer involving the lymphatic system
02:06But specifically, it's actually a cancer involving a type of white blood cell called the lymphocytes
02:11There are over 80 different types of lymphoma
02:14And it's a very heterogeneous population
02:16It ranges from very slow growing to very fast growing
02:20That's what makes it so difficult to treat and to diagnose as well
02:24Because there are so many different types
02:26Okay, so we're talking about the subtypes itself
02:29So how important is it accurately subtype identification
02:33In determining the most effective treatment plan?
02:36It is really crucial to get the right diagnosis
02:38Because when you do not have the right diagnosis
02:41Then everything else below, the treatment plan, the management plan
02:45All differs between a slow growing lymphoma and a fast growing lymphoma
02:49So it's very important to have the right diagnosis
02:52Yes, there's also differences between those lymphoma itself
02:55So that's why we have to have the treatment
02:58Absolutely
02:59Because different subtypes really means there are different therapies that are given
03:05Yes, okay, Dr. Jonathan
03:08Diagnostic technology have evolved significantly
03:11We're talking about modern right now
03:13That's right
03:13So how have molecular profiling and also let's talk about genomic testing
03:17Changed the way lymphoma is diagnosed today?
03:20So the way we treat lymphoma today is like moving from a generic off-the-rack suit
03:25To a custom tailored outfit
03:27Historically, doctors looked at lymphoma cells under a microscope
03:30To classify them based on how they looked
03:33Today, molecular profiling and genomic testing allows us to look directly inside the cancer cells
03:38At their unique DNA and genetic blueprints
03:42So this additional information is a game-changer
03:44It helps us identify the lymphoma better than microscopy
03:48It also helps us predict exactly how aggressive a lymphoma can be
03:52So most importantly, it lets us match a patient-specific genetic mutations
03:56With precision drugs designed to attack only the cancer cells
03:59Often allowing us to completely bypass traditional harsh chemotherapies
04:03So in short, we are no longer just treating the name of the disease
04:07But we are treating each genetic signature of each patient's cancer
04:10Okay, that's how detailed it could be
04:13That's right
04:14We're talking about comparing to the conventional imaging
04:17PET CT scanning is also considered a standard tool in lymphoma management
04:22So what advantage does it provide compared to that conventional thing?
04:26So PET CT is one of our best tools against lymphoma
04:29Because it gives us a complete head-to-toe picture of what's happening inside the patient's body
04:34So conventional imaging like CT scans use size to judge if a lymph node is affected by lymphoma
04:40But normal size nodes may still harbour lymphoma cells
04:44So PET scan actually lights up active lymphoma areas
04:48And when combined with CT, shows us the exact lymph nodes or organs
04:51Where the spots are located
04:53And that enhanced clarity over conventional imaging
04:56Helps us design the best treatment plan from the beginning
04:59We also use it midway through treatment to customise care
05:03For example, if the cancer is being stubborn
05:05We can step up treatment
05:07Like adding targeted radiation
05:09Or changing to stronger chemotherapy to knock it out
05:12But if it's clearing up fast, we can safely back off
05:15Dropping extra chemo cycles or radiation to reduce unnecessary side effects
05:20So once the treatment is finalised and finished
05:23A final PET scan tells us if the active cancer is completely gone
05:26Giving us a clear sign that all is clear
05:29So for me as a general public
05:31That don't have medical history obviously
05:34So if a patient has like a simple same lymphoma disease
05:38But they could have like a different personalised treatment
05:42That's right, yeah
05:43It depends on the type of lymphoma
05:46And also how the lymphoma responds to the treatment
05:49And that can be adjusted accordingly along the way
05:51Okay, this is really interesting
05:53Because we're talking about
05:54Usually people would talk about chemotherapy
05:56But again now they have breakthroughs
05:58And I'm going to ask more about that question in the next segment
06:02Because we have to take a break first
06:04And saudara, setelah jadi yang satu ini
06:06Kami masih akan kembali untuk membahas ya
06:07Jangan kemana-mana, tetap di Bincang Kita
06:36Okay, Dr. Kevin back to you
06:38Treatment again
06:39We're talking about move traditionally
06:41Chemotherapy
06:42But now we have a lot of breakthroughs
06:45How does this breakthroughs transform a lot of patients out there?
06:49This part is probably the most exciting part of cancer treatment
06:53Because we do not just have chemotherapy these days
06:56We have a lot more therapeutic options
06:58And one of the major advances really is targeted antibodies
07:02And that has been around for a number of years
07:05But even more recently
07:07We have breakthroughs in new immunotherapy treatments
07:11Such as bispecific T-cell engagers
07:14And as well as even more specific immunotherapy treatments
07:18Such as CAR T-cell therapy
07:21Okay
07:22So using this, I think patients have a lot of options
07:26In terms of treatment modalities
07:30That we can use to fight against lymphoma
07:33Because it has really fundamentally changed
07:35The way we think about treating cancers
07:37Rather than just using chemotherapy as the backbone
07:41We now have a way that we can ramp up our immune system
07:45Weaponize it
07:46And then we can then use it to identify the cancer cells
07:49And kill off the cancer cells using our own immune system
07:52Of course
07:52And also as a patient also respond differently
07:55They respond differently
07:57And then also the side effect profile is a lot better
08:00Compared to just conventional chemotherapy
08:03True
08:04Dr. Jonathan
08:06With also this breakthrough in drug therapy
08:08What is the role of radiation itself?
08:11Is it still necessary to some people?
08:13I mean like the thought of being radiate also frightens them
08:17Yeah
08:17So actually decades ago
08:19Radiation or x-ray therapy
08:20Was actually the main treatment for lymphoma
08:22And although it could, you know
08:24Cure lymphoma
08:25It also came with significant late side effects
08:28Because large areas of the body
08:30Were exposed to high dose of radiation
08:31So patients were cured
08:33But there was concern about, you know
08:35Injury to organs like the heart
08:37And secondary cancers caused by radiation
08:39Many years later after treatment
08:41Now fortunately
08:42The advances to chemo and immunotherapy
08:44Allowed us to reduce the extent
08:46And amount of radiation required
08:48Now in many lymphomas
08:50Modern chemo immunotherapy alone is curative
08:53However we still add radiotherapy after chemotherapy
08:56For example if the disease was big and bulky from the start
08:59On some lymphomas like Hodgkin's lymphoma
09:02Studies have shown that cure rates remain higher
09:04When you combine radiation with chemo
09:06So finally some early stage
09:08Slow green lymphomas can be cured by low dose
09:10Targeted radiation as well
09:12Without chemotherapy
09:13Oh okay
09:14And also they could like complete each other as well
09:16Yes that's right
09:17Sure
09:17Okay so Dr. Kevin
09:19You talk about CAR T-cell therapy work
09:21It is something that
09:23Generate a lot of excitement
09:24Why does it?
09:25Yeah
09:25I think CAR T-cell
09:28It's really
09:28The idea is quite novel
09:30You're using the patient's own T-cell
09:33T-cells are basically our own immune system
09:35That fights against cancer
09:37We have always known that we could
09:39Our immune system can kill cancer cells
09:41But now we have found a way
09:43To really utilize our immune system
09:46To the fullest extent
09:47So what do I mean by that?
09:48We take the patient's blood
09:50And then we draw the T-cells into a pack of blood
09:54And then we
09:55In the lab
09:56We weaponize it
09:57We put specific weapons into the T-cells
10:01In the immune system
10:02And then we put it back into the patient
10:04To better detect the lymphoma
10:07And then to better
10:08To kill the lymphoma cells more effectively
10:11That's what it does
10:12That's what the CAR T-cell does
10:14And more importantly
10:16It's a one-time treatment
10:17Just a one-time treatment
10:19To eradicate all the cancer cells
10:21So I think to the patient
10:24They do not have to come for repeated treatment
10:26They just do it one time
10:28And then you could potentially still be cured
10:29With this novel immunotherapy
10:32In that one time
10:34In that one time
10:35So it has really changed the landscape
10:37Of how we
10:39It's really one of the most important
10:42Treatment that we have available these days
10:44To eradicate lymphoma
10:46Okay
10:47So also talking about
10:49The bispecific T-cell engager as well
10:53Now how does it differ from the CAR T-3 therapy?
10:56So that's a little bit different
10:57It's still a novel immunotherapy
10:59The idea is still the same
11:01So you think about bispecific T-cell engagers
11:03As a
11:04Now we have built a construct
11:07We've constructed an antibody
11:08Where you have the antibody bound to the T-cell
11:12And then you have antibody that's bound to the tumor cell
11:14So you're bringing it in close proximity together
11:17So that our T-cells can then kill off the lymphoma cells
11:23Because you're bringing it close together
11:25Okay
11:26And you do not have to
11:28You can just buy it off the shelf
11:31You do not require the patient's blood
11:34The problem with
11:35One of the downside of doing CAR T-cells
11:37Is that you still have to go through a whole process
11:40You need to take out the blood
11:42And you need to put in the lab
11:43It all takes about 3 to 4 weeks
11:45Okay
11:45As opposed to the bispecific T-cell engager
11:49It's off the rack
11:50You can just use it on the day itself
11:52It's just like any other therapeutic drug options
11:54Okay
11:55Yeah, and you use it on the day
11:56And the downside to that
11:58Is that you have to keep continuing to use it
12:01Week after week after week
12:03For it to maintain its effectiveness
12:06Unlike the CAR T which is a one-time thing
12:08Yes, yes
12:09It is really exciting to hear
12:11That one-time thing could actually cure it
12:13Back to Dr. Jonathan
12:14We're talking about radiotherapy
12:16You still say that it's here
12:18We're saying about it
12:19So you mentioned actually
12:21The long-term side effects about it
12:23Is there any advances to make it safer?
12:26Yeah, so radiation technology has become safer
12:29Smarter and far more targeted than ever before
12:32We now combine radiation with advanced chemotherapy
12:35And accurate PET-CT imaging
12:36We no longer need to radiate wide areas of the body to high doses
12:41So good responses to chemotherapy
12:44Allows us to reduce the radiation dosage required
12:47And PET-CT allows us to better target the areas we need to treat
12:50Yeah, so lymphomas are barely regular shape
12:54You know, they often sit right next to vital organs
12:56And modern X-ray systems
12:59Like IMRT and VMAT can sculpt the radiation beams
13:02To cover the complex shapes
13:04Delivering adequate doses to the lymphoma
13:06While avoiding nearby healthy organs
13:08To make sure that our beams hit the exact spot every single time
13:12You know, modern radiation machines can take a quick CT scan
13:15Of the target area right before treatment
13:17So this allows us to make the sub-millimeter adjustments to position
13:21To ensure a perfect match every time we want to fire the radiation
13:24This is called image-guided radiation therapy
13:26And this allows us to reduce the treatment area required
13:30And then we can spare more normal tissue from radiation
13:33Yeah
13:33So when we give radiotherapy to, for example, the chest area
13:36If there is lymphoma in the chest
13:38One of the top priorities is to spare the heart
13:41And there's this technology called breath-hole technology
13:43During radiotherapy
13:44In which when the patient holds a deep breath during treatment
13:47He actually freezes movement, right?
13:50So the lymph nodes that we want to treat don't move
13:52And then we can treat a smaller area
13:54And then in addition, expanding the lungs with the deep breath
13:57Also acts like a cushion
13:58And it moves the heart away from the treated area
14:02It's like you're holding your breath to save your life
14:04Or save your heart
14:06So Dr. Juyenton, from your experience and expertise
14:08What actually should the patient or the family do or don't during the treatments?
14:14Or what do they have to prepare?
14:16Yeah, they just have to maybe practice how to hold their breath for about half a minute at a time
14:19There's actually a device called a breath-holding coordinator
14:22That they breathe into that measures the exact amount of air in their lungs
14:25So the patient is guided by the machine
14:27And they can see, you know, like how deep their breath is
14:32And whether or not they're breathing correctly
14:33And holding it correctly
14:34And the safety aspect of it is that if the patient's breath hole is not deep enough
14:38For example, it actually tells the radiation machine to stop treating the patient
14:42So that way it's really, really safe
14:44Alright, okay
14:45I still have a lot more questions
14:47But we still have to take a break
14:48Dan saudara, jangan kemana-mana
14:50Kami masih akan kembali setelah yang satu ini
14:52Tetap di Bincang Kita
14:55Terima kasih
15:12Terima kasih Anda masih menyaksikan Bincang Kita
15:14Dan kami masih melanjutkan obrolan seputar tema pada hari ini
15:18Masih bersama narasumber saya
15:19Ada Dr. Kevin dan juga Dr. Jonathan from Singapore
15:22Dr. Kevin, back to you
15:23So now we're talking about Singapore itself
15:26It's often regarded as a regional leader in lymphoma care
15:30So what factors has actually enabled Singapore to achieve this position?
15:35I think Singapore's strength is not simply about one technology or one treatment
15:40It is the entire ecosystem of cancer care
15:44And that whole continuum of care
15:45Lymphoma treatment these days have become so complex
15:49You really require an entire team of specialists
15:52Such as the medical oncologist
15:54The radiation oncologist
15:56Nuclear medicine specialists
15:58The pathologists
15:59As well as the radiologists
16:01So I think Singapore is positioned such that they have a good strong capabilities in this area
16:06So I think about the whole continuum of cancer care of not just
16:10It goes from the diagnosis
16:12Making an accurate diagnosis
16:14To then giving the best treatment possible
16:18And then furthermore we do have access to clinical research
16:22I think that's also critically important
16:24Because after standard of treatment has been given
16:27Many of times one next for the patient
16:29It is with clinical research that comes the next frontier of treatment
16:34That provides that next generation of treatment to come
16:37I think in Singapore we are thankful and grateful that we have that opportunity
16:44Furthermore I think Singapore regulatory health bodies
16:47The HSA, the Health Singapore Authority of Singapore
16:50Has a lot of tie-ups with the international agencies like the FDA in America
16:56And they sign
16:57Because of the partnerships that we have in collaborations with FDA
17:02And the European medical agencies
17:04We do get access to a lot of these newer treatments in a very quick time
17:09Compared to some of our regional counterparts
17:12Yeah, so we're just not talking about the technology itself
17:16But also there's like a collaboration
17:17Exactly
17:18And partnership
17:19Because like in one patient there's not just going to be one doctor
17:22You're right, exactly
17:23More than one
17:24It's a whole ecosystem
17:26Yeah
17:26That is important to really
17:28I think the whole continuum of cancer care is really important to build up
17:32That sort of infrastructure
17:34Yeah, true, true
17:35Okay, so Dr. Jonathan
17:37There has also been a hype and buzz about proton therapy
17:42Now, okay
17:43As a newer form of radiation therapy
17:45Is it available in Singapore?
17:47And how does it benefit lymphoma disease?
17:50Yes, so in recent years
17:51Three proton therapy centers have been set up in Singapore
17:55Now traditional radiation uses x-rays
17:57That are like a flashlight beam
17:59So it shines towards the tumor but continues out through the other side
18:02Exposing the nearby healthy tissues to unwanted radiation
18:06Now proton beam therapy in contrast is different
18:09It travels to a specific depth
18:10Hits the target tumor
18:12And then it stops abruptly
18:13There is almost no radiation past the target
18:16And because of this
18:18We avoid exposing healthy organs such as the heart and lungs
18:22To unnecessary what we call overflow radiation
18:25So this significantly lowers the risk of organ complications
18:28A year down the road
18:30So radiation itself carries a tiny, tiny risk
18:32Of causing a new cancer decades later
18:35Now proton therapy lowers this risk
18:37By reducing the overall radiation exposure of the body
18:39This is especially valuable for young adults
18:42And patients with very highly treatable and curable lymphoma
18:45Since these patients are going to live very, very long, full lives
18:47And after beating their lymphoma
18:49So we really want to prevent treatment complications
18:51You know, 20, 30 years down the road
18:53It's just as far as treating the cancer
18:55As to preserving health in decades to come
18:57Of course, it's really exciting to hear proton therapy could do that
19:01I mean, as we know that we also have side effects
19:04So they're radiating in one part
19:05And then the other part got effect of it
19:08So this would be the solution of it, Doc
19:10That's right
19:10That's why it's actually really good for children as well
19:13Because they have long full lives
19:14And they're still developing
19:15And you really want to reduce their radiation exposure
19:17Okay, great to know
19:19Well, Dr. Kevin
19:20Compared to neighboring countries
19:22What advantage do lymphoma patients have
19:25When they're seeking treatments in Singapore?
19:28Right, so I wouldn't frame it as Singapore
19:30Having better doctors than our neighbors
19:32I think you all have very good doctors in the region
19:36You know, good cancer centers
19:37And it is actually
19:39The particular advantage that Singapore has
19:41Is that we have a concentration of good expertise
19:44And technology within a relatively small healthcare ecosystem
19:49And then for a very complex lymphoma case
19:51We can bring together all this specialized expertise
19:54Like specialized pathologists
19:58The radiation oncologists
19:59The medical oncologists
20:00In a very quick fashion
20:02And sometimes making that diagnosis is very important
20:05And furthermore, we can provide the sort of newer treatment
20:10And specialized, personalized care
20:13One of the important things for an international patient
20:17Who is traveling all the way to Singapore for treatment
20:20And really several factors to consider
20:23Among this is the ability to really obtain a diagnosis
20:27Not just accurate, but also in a very quick time
20:30Because coming all the way here
20:32I mean, I think that's a very important consideration
20:36Ultimately, the most important thing for any lymphoma patient
20:39Wherever they are treated
20:40Is to really have an accurate diagnosis
20:42And an experienced multidisciplinary team
20:45Yes, of course
20:46I would highlight experience as well
20:48Because we have to be
20:49You guys have to be trusted by the patient itself
20:53So what is actually like
20:54If we're choosing Singapore
20:56What is specifically the treatment that only specialized there
20:59That I couldn't find in other places?
21:01I alluded earlier that sometimes we do get faster
21:04Access to some of these novel treatments
21:06And a lot of these novel treatments
21:08Has really, you know, break that new frontier
21:11Increase our survival outcomes
21:14And it's actually the access to all these newer treatments
21:17That give us the potential edge
21:19So thankfully, again in Singapore
21:21We are able to get these novel treatments
21:24At a quicker time
21:25Compared to some of the other regional counterparts
21:29And that has really made the difference for us
21:31Not just for international
21:34International patients
21:35But even for our own local patients
21:37And we are
21:38I mean, with the use of this
21:41Utilization of these newer treatments
21:43I have personally witnessed
21:45Improvement in my patient's life
21:47And they are living longer and longer
21:49And cancer survivorship is a real thing
21:51It's a true thing
21:52Yeah, of course
21:53It's really something that exciting
21:57Because like you could feel like
21:59Giving the patient hopes itself
22:00So Dr. Jonathan
22:02You, your experience in the field
22:04Like how interaction with the patient itself
22:08And you could bring more hope
22:10With all those treatments
22:11What actually, like
22:12Do you have like an interesting story
22:14With that patient?
22:16Oh yeah, I mean
22:17We've seen patients with
22:17You know, very large
22:19Lymphomators, tumors
22:20And it looks like
22:22The situation is not hopeful at all
22:23But then with the correct diagnosis
22:26Correct molecular and genomic
22:27And then the correct drug is used
22:29And then you see the disease
22:30Just melt away
22:31And it's very gratifying
22:32And after that
22:33We follow that up with
22:33Some radiation
22:34To make sure that
22:35It doesn't come back anymore
22:36And these patients
22:37Then they go on to live
22:38Very long and full lives
22:39Wow, that's really exciting to know
22:41Especially when they're
22:42Younger patient, right?
22:43Yes
22:44So there are more hope
22:46For younger patients as well
22:47So if I could ask a final question
22:50Like what do you want to share
22:52For the patients
22:53As also family out there currently
22:55Especially if they're facing
22:57Lymphoma disease
22:58Yep, so lymphoma care
22:59Has seen extraordinary breakthroughs
23:01In recent years
23:02So treatments are smarter
23:03And yet gentler on healthy organs
23:05And more effective than ever before
23:07As Dr. Kevin has said
23:08So in general
23:09Cure rates are high
23:09Compared to other cancers
23:11So my
23:13You know
23:14What I would advise
23:15Is to find a good team
23:16Of lymphoma specialists
23:17Who can work together
23:18And they will guide the patient
23:20Towards a full recovery
23:21Of course
23:22And Dr. Kevin
23:23I know when people heard
23:24Lymphoma disease
23:25And then you got lymphoma disease
23:27It would be heartbreaking
23:28But again
23:29As doctor whose expertise in this
23:31Is there like a final message
23:32That you would also like to share
23:34For patients and families
23:35To not be discouraged
23:37No, I think you said it best
23:39When you said
23:39Actually our patients
23:40They all need hope
23:41And I do see that in lymphoma
23:43Because one thing
23:44That I want to tell
23:46People who are watching
23:47And patients with lymphoma
23:49That even stage 4
23:51Metastatic lymphoma
23:53There is a chance of cure
23:55Wow
23:56This is
23:56And this is
23:57This is one of those things
23:58About lymphoma
23:59That's different
24:00From the other cancers
24:01Even when you have
24:02Metastatic disease
24:03There's that potential
24:04Chance of cure
24:06And with all these
24:07New treatments
24:10And as I look beyond
24:12What is currently available
24:14What is in the research pipelines
24:16I am very excited
24:17And optimistic
24:18What the future will bring
24:20All these new treatments
24:22Not just immunotherapy
24:24But also what's coming
24:26Some of these novel
24:27Genetic therapies
24:28And targeted treatments
24:30I'm very excited
24:31About what the future will bring
24:33And more hope
24:35For our patients
24:36All right
24:37Oh my goodness
24:38I am so excited also
24:39To hear this
24:40Because this is also good news
24:41That people could hear
24:42That there's chances
24:43And there's hope
24:44Even in stadium for itself
24:46There still could be possible
24:48To cure
24:49Wow
24:50That's amazing
24:50Thank you so much
24:51Dr. Kevin and Dr. Jonathan
24:53For your time
24:53And for coming here
24:55From Singapore
24:55Thank you very much
24:56For your expertise as well
24:57Thank you
24:58Dan saudara perbincangan hari ini
25:00Menunjukkan bahwa penanganan lymphoma
25:02Telah memasuki era baru
25:03Mulai dari diagnosis yang semakin akurat
25:05Precision medicine
25:06Hingga terapi inovatif
25:08Seperti inovid therapy
25:09Dan CAR T cell therapy
25:11Yang membuka harapan baru bagi pasien
25:13Dan yang terpenting
25:14Deteksi dini
25:15Diagnosis yang tepat
25:16Dan pengobatan yang dipersonalisasi
25:18Tetap menjadi kunci
