00:57grazie e buona domenica
00:58grazie a tutti
01:30so lung cancer is a one of the most well-known principal solid tumors for human and we know
01:39that we have generally 2.5 new diagnoses ever here worldwide lung cancer today is not a unique
01:47disease but we know that according on the pathology feature also biology understanding we can
01:55split measure lung cancer in different kind of subgroups the major is adenocarcinoma lung cancer
02:03that more or less account for 40 percent of all the new cases
02:18in the discussion with the patient is important to identify the core stage but the diagnosi of
02:24adenocarcinoma today among all the diagnoses of lung cancer is mostly the the best diagnoses because
02:31we have the opportunity through the molecular profile the immune mediating profile to have a very
02:38big scenario treatment strategy to deliver in the clinical setting and for adenocarcinoma our
02:44treatment strategy and the progress in this treatment strategy are today a reality to have a long-term
02:51effective benefit
03:12you can discover lung cancer generally having a city scan identify a small age along the lung or after
03:20the development of some new uh symptoms like a cough dyspnea of chest pain of course this is something of
03:28initial workflow that to move the doctors who ask to request new testing in particular city scan that is the
03:37best diagnostic tools and methods to identify new lesions in the lungs
04:01the stages of lung cancer are very very different we move from stage one to stage four and the difference
04:08is uh important stage one in stage two are disease that are generally localized in the lungs with a
04:16absent or small involvement than the lymph nodes for stage one institute generally we have a high
04:23potential of a cure through surgery plus or not the addiction of a therapeutic strategy for stage three
04:32we have a disease that generally spread to the lymph node and is confined in the lungs and here we
04:40have
04:40the opportunity to consider the combination strategy including surgery chemotherapy immunotherapy
04:47also radiotherapy stage four is the stage that confirms that we the tumor spread outside of the lungs we
04:57have in the situation to have a metastasis sometimes in other organs like bone like a brain a lot of
05:03adrenal
05:04gland in for stage four we have the opportunity to use different therapeutic strategy including chemo include
05:12the target age and immunotherapy sometimes also treatment for oligo disease but in this stage stage
05:19stage four the opportunity for cure very very limited and we dedicate our treatment to improve and to
05:25prolong the survival of our patients early diagnosis of course is the key for a cure a high rate of
05:42cure
05:42in particular in particular in particular in particular for stage one where the nodal is isolated in the lungs without
05:47any node involvement or for stage two where we can identify some lymph node involvements the opportunity to have a
05:55high cure rate is very very high
05:57achieved with the only surgery or the surgery followed by medical treatment
06:18the surgery
06:19cari fratelli e sorelle buona domenica
06:27generally the use of the drugs as a chemo immunotherapy on target agent for patients with early stage
06:36are considered only to maximize the results of the surgery and to reduce the
06:42rate of the relapse and long-term relapse. For patients with advanced stage
06:47chemo, immuno or also other drugs like target agents are used to prolong the
06:55survival to maintain the disease under control.
07:30So, more or less 15 years ago, the opportunity to have a cure,
07:34recovery and long-term benefit of patients with lung cancer was very, very limited, less than 10%.
07:40Today, we increased dramatically the opportunity to have a cure for patients with a radically
07:46resected disease, stage one, stage two, and we have a high opportunity to have a long-term
07:52benefit also in patients with the stage four lung cancer using the treatment strategy like
07:59chemotherapy, immunotherapy or target agent with a significant improvement in the two,
08:04three, four years survival rate, something that was imaginable in the 10 or 15 years ago.
08:12The first thing I would like to tell you is thank you. Thank you for the support of your prayers.
08:19Thank you to God for your generosity. We all renew our trust in the kindness and the mercy of God.
08:35We know that for adenocarcinoma histology, the molecular profiling that is able to identify different
08:42kind of gene alteration is able to drive us in the selection of the best treatment strategy. This
08:49was the major progress that, of course, moved us as oncologists, as a doctor, to identify the best
08:56treatment strategy. Today, we have more or less 10 different target agents that we can use according
09:03to different kind of gene alteration. We have many opportunity to use immunotherapy alone or in
09:09combination with chemotherapy. But importantly, all of this kind of opportunity and the treatment
09:16strategy should be delivered according to the pathology, according to the biology, to the right place
09:22and to the right patients.
