Five years ago, Ethan Cole publicly called off his engagement and sent Claire Foster to prison. On the day of Claire's release, Ethan demanded that Claire donate her kidney to save Fiona Miller, who had been in a car accident, disregarding her risk of congenital heart disease. Under Ethan's coercion, Claire reluctantly agreed, but on the day of the surgery, her heart suddenly stopped, resuscitation efforts failed, and she was presumed dead.
