Damon confronts Elena, frustrated by her denial and calling her out for lying to him, to Stefan, and ultimately to herself about what is happening between them. He passionately kisses her, but she pulls away, insisting that she cares about him yet delivering the devastating blow that she loves Stefan and that it will “always be Stefan”.

Earlier in the sequence (or interwoven in edits like this), Damon believes he is pouring his heart out to Katherine (whom he thinks is Elena at first or is confronting directly), offering to let go of the past 145 years of pain to start over. However, Katherine ruthlessly rejects him, telling him the ultimate truth: she never loved him, and it was always Stefan.
