01:19Sergio Murillo Lima de Santana, a 39-year-old tourist from Belém,

01:25had been on holiday with his partner for about six days when they tried a beach zipline that glides over

01:31the dunes.

01:31It was a windy afternoon, and Sergio filmed another tourist, a woman, completing the run with no trouble at all.

01:39Then he took his turn, phone out, recording himself.

01:43He was gliding along the cable, harnessed in, doing nothing wrong when the support structure holding the line gave way.

01:50The pole tipped.

01:51The cable lost its tension, and Sergio plummeted onto the sand dunes below.

01:56He didn't jump or let go.

01:58The whole thing collapsed under him.

02:00Emergency workers rushed him to the hospital, where he was pronounced dead on arrival.

02:04The chilling part is that he captured his own final seconds.

02:09In early July 2021, on a Sunday, 25-year-old Colombian lawyer Yesenia Alejandra Morales-Gomez

02:17went bungee jumping for the first time with her boyfriend at a viaduct near Amagá.

02:22In the municipality of Fredonia, Antiochia, run by an operator called Sky Bungee Jumping Amagá,

02:29they'd put a harness on her.

02:31But the elastic cord, the actual thing that stops you, hadn't been attached yet.

02:36Her boyfriend, standing nearby, already had his full safety system on.

02:42According to the mayor of Fredonia, an instructor shouted the jump signal meant for the boyfriend.

02:48Yesenia, confused and rushed, thought it was for her.

02:51So she stepped off, harness on, no cord connected to anything.

02:56She fell about 150 feet, head first.

02:59Her boyfriend climbed down and tried CPR.

03:02Medical reports say she went into cardiac arrest during the fall, her heart stopping before impact.

03:08The second jump.

03:09The 21st of July, 2015.

03:12The Tablate Bridge, near Longerone, outside Granada, in southern Spain.

03:17Cleo de Abreu was a 23-year-old British woman.

03:20A London fashion student visiting family in Spain, who'd booked a bungee jump with a licensed adventure sports company.

03:26And she'd already done it once.

03:28Her first jump that day went fine.

03:30On the next attempt, the supervisor attached her harness, then turned away to deal with other participants.

03:37The investigation found a miscommunication.

03:40Cleo jumped before she was told to, before the equipment had been made safe.

03:44With the setup incomplete, she didn't get the controlled rebound.

03:49She collided with the wall of a smaller, ancient bridge sitting directly beneath the one she'd jumped from,

03:54and reports say she died instantly.

03:57The Guardia Civil charged two men with manslaughter, the owner of the adventure company and the jump supervisor.

04:04Investigators concluded the supervisor had not carried out his function with due care.

04:09On the 15th of October, 2023, Dan Owen, a 47-year-old American and director of an international school in

04:18Haiphong, Vietnam,

04:19was on vacation in Laos with his 15-year-old son, Cooper.

04:23They went to Green Jungle Park, a zipline attraction near Luang Prabang, and rode the course with a guide.

04:29The hardware worked perfectly.

04:31Apparently, that's not what got them.

04:33As father and son were descending from a tree at the end of the zipline, they were swarmed, a source

04:38close to U.S.

04:39Diplomats described hundreds of what looked like Asian giant hornets, though the exact species was never confirmed.

04:46Both were stung more than 100 times across their bodies, which were covered in red spots.

04:52They were rushed to a local clinic, then to Luang Prabang Provincial Hospital, already in critical condition.

04:58Cooper was unconscious on arrival and died about half an hour later.

05:03Dan arrived conscious and died roughly three hours after that.

05:07The doctor said both suffered severe anaphylactic shock.

05:11The park called it an unforeseeable, extraordinary natural occurrence.

05:16Three on the line.

05:17Around 10.30 a.m. on the 19th of November, 2019,

05:22a 48-year-old South Korean tourist was ziplining in Vang Vang, Laos,

05:27the country's notorious adventure tourism town.

05:31He'd flown in that Sunday with two colleagues from a company in Busan,

05:34and they'd booked the zipline tour through a local agency.

05:38The three of them went out on the line one after another,

05:41the man bringing up the rear behind his two co-workers.

05:44The cable on this commercial zipline was held up, in part, by a tree about 23 feet high.

05:51That tree collapsed.

05:52As it fell, the line slackened suddenly.

05:55The two colleagues ahead got off with scratches and bruises on their legs and backs.

06:00But the 48-year-old fell from the line and was killed.

06:04His name was never released in English-language reporting.

06:07Local officials launched an investigation into possible negligence

06:11and announced safety checks on all eight zipline attractions in the town.

06:15The Korean embassy sent a consul and asked police for a thorough inquiry,

06:19and the man's family flew in the next day to bring his body home.

06:22The Superman Leg

06:27The flight of the Gibbon Canopy Zipline near Mei Kampong in Chiang Mai Province, Thailand.

06:34Spencer Charles Donaldson, a 25-year-old Canadian from Fruitvale, British Columbia,

06:39was riding the course with his girlfriend.

06:42The company said he'd completed orientation and was within the 275-pound weight limit.

06:48Shortly after he was released from the start of the line, the cable failed, and he fell.

06:54His family says it happened on the Superman leg,

06:56and they blame an improperly assembled cable and the absence of a backup safety line,

07:02meaning one failure left nothing holding him.

07:04The company's own preliminary investigation pointed to a metallurgic failure on a swing.

07:10Reports on the fall distance conflict wildly, from 39 feet to about 330 feet into a creek below.

07:17This wasn't the operator's first incident.

07:19In 2016, three Israeli tourists were hurt in a mid-air collision,

07:24and earlier still, two women crashed and were hospitalized.

07:28The operator reportedly accepted full responsibility, not for lifting.

07:32On the 22nd of October, 2019, Dean Anson Sanderson,

07:37a 50-year-old Australian father of three from Adelaide,

07:40was on a bucket-less trip through far north Queensland with his wife Shannon.

07:45They booked jungle surfing canopy tours, a zip line through the Daintree rainforest,

07:50and were sent down one line together, in tandem, between two platforms.

07:55Partway across, the line lost momentum, and then it gave way.

07:59Witnesses said it snapped.

08:01Both of them fell up to 82 feet to the forest floor.

08:05Shannon survived with serious injuries.

08:08Dean did not.

08:09He died of blunt force trauma from the fall.

08:12The coronial inquest uncovered the detail that defines this case.

08:16New wire rope grips had been installed on the course,

08:19and the invoice for them carried a warning that they were not for lifting.

08:23The operations manager admitted he hadn't seen that warning,

08:28and that he was unaware of the legal requirement for annual engineer inspections.

08:45Joaquin Romero was a 34-year-old employee,

08:48working the platform that day, clipping riders onto the line.

08:51The course had a weight limit of 250 pounds,

08:55and Romero reportedly feared what would happen if too much weight hit the system at once.

09:00He was helping a woman get clipped in when she started sliding out onto the line

09:05before she was fully secured.

09:06Romero grabbed her harness to stop her,

09:09and that pulled both of them off the platform and out over the drop,

09:12about 70 feet above the ground.

09:15Because he believed they would both fall due to their combined weight,

09:18Romero let go of the harness and sacrificed himself for the woman's safety.

09:22The woman stayed on the line and was uninjured.

09:25Cal Fire crews found him with major trauma,

09:27raised him with a low-angle rope rescue system,

09:30and airlifted him out.

09:31But he died on November 1.

09:33His cause of death was multiple blunt force injuries.
