01:26Progeria is a rapid aging disease.
01:30It only affects about 400 people, 500 people worldwide.
01:38I know they say that I supposedly age eight times faster, so if that's the case,

02:08Tiffany is currently under my care primarily for her aortic valve.
02:13She's unfortunately developed some narrowing of the aortic valve, which is called aortic stenosis.
02:21This is a serious condition, and if it's left untreated, patients can develop a lot of other problems,
02:26like congestive heart failure.
02:34The typical lifespan was only about 15.
02:37We don't have a whole lot of experience treating older patients with progeria.
02:43They may stop growing.
02:45They may have other problems that would affect the elderly, like loss of hair or loss of teeth at a
02:49young age.

06:36Tiffany as a child was not anything like a sickly child.
06:41She did everything that everybody else did.
06:44She was just smaller.
06:49I started noticing things changing about me when I was like 12.
06:55I started noticing that my fingers were different.

07:03Like all my girls were painting their fingernails and doing stuff like that and I couldn't do that because I

07:09I knew I was different when I was little.
07:11I just didn't know why.
07:13And I didn't put a whole lot of thought into it because I was too busy living my life.

08:50I was born with a physical disability called arthrogryposis.

09:04Just want y'all to know that I have been struggling with this for like the past three minutes now,

09:11I ain't had to struggle this hard since the night I lost my virginity.
09:17I've had to learn how to adapt to everyday life and learn how to do everything that everybody else does.

09:32Obviously I can, I can swing them, but I can't make them bend like your average person can.
09:43When my mom was pregnant with me, there wasn't enough room for my joints to fully develop.
09:48So my arms can only bend a certain amount.

11:20Whenever I try to lay down and relax, it's like my brain just won't shut off.
11:25He was initially born with his legs kind of up by his ears.
11:29So we had to really kind of stretch him out and get him flexible.
11:32It was very painful for him to be stretched like that.
11:38The doctors were telling us that he probably wouldn't be able to eat on his own.
11:43He wouldn't be able to go to the restroom.
11:45He would have a feeding tube his whole life.
11:47That he would not be able to walk.
11:51I was heartbroken and my immediate thought process was how was he going to navigate life?

12:04I do worry about you being, you know, off by yourself all the time and working so hard.
12:22I have a doctor's appointment this morning to see Dr. Arshi and my six month echocardiogram.
12:30It's always a little nerve wracking.
12:40I am feeling scared.
12:43I'm not going to lie and say I'm not terrified because I am.

13:13I know that there's always a chance today's the day.

13:25We're going to take a look at the left side of your heart to start.
13:28What is that measuring?
13:30So that picture measures the left side of your heart, the left atrium.
13:34Helps us check your heart to look for any leaking of the heart valves or any kind of abnormal blood
13:38flow.
13:39And there's a picture of all four chambers of your heart.
13:42That's the heart.

13:45Wow, that tiny little thing.

13:49I bet if more people saw their heart beating they would take care of it.
13:53You would think.
13:55Well, most people don't get to see their heart beating like this unless something happens.
14:14Growing up, I had my mom, Linda, my dad, Michael, and my two brothers, Todd and Chad.
14:23That picture is Christmas.
14:26And my dad took that picture on a timer because he loved taking family pictures.
14:33Tiffany's mother and brother both have progeria.
14:36Her brother was affected pretty severely and unfortunately passed away at a young age.
14:43That is my brother Chad's best friend wedding.
14:48And Chad was so happy.
14:51It's Todd and Chad and me.
14:53But that was right before he died.
14:56Shortly before he passed away.
15:01My brother was very funny and always had everyone laughing.
15:06He was, you know, like 18, 19 years old and bones were breaking.
15:11And then by the time he was 30, he was having aortic valve surgery.
15:17My brother was like, I really want to know.
15:20We have to figure out what I have.
15:23We had lots of tests done.
15:26Doctors discovered my mom, Chad, and I all had progeria.
15:32That was 17 years ago when I was 31.
15:46She actually took a lot of blame for us having progeria.
15:52And she would apologize to me all the time.
15:55Like, I'm so sorry that I gave this to you.
15:59And I'm like, oh, who gave it to you?
16:02Like, it's not your fault.
16:04But she had that guilt for like, ever.
16:10You know, I don't know, it was like a month before my mom passed away.
16:15She said to me, she's like, Tiffany.
16:18She looked right at me and she was like, Tiffany, you cannot be tamed.
16:23And I'm like, I know.
16:27Why would you want to tame this?
16:33When I got diagnosed, it wasn't like devastating to me.
16:38It became devastating.
16:39And it really sank in when my brother died.
16:44He got the open heart surgery.
16:47And then from that point, they just discovered heart disease all over his body.
16:51And when I get things like my legs checked,
16:55and if I feel like things are going wrong in my feet and my legs,
16:59it really puts me in kind of a panic, a little quiet panic,
17:04because that's how my brother really lost his life from complications with that.
17:12Chad was only 39 when he died.
17:24I woke up this morning and I kind of got sick to my stomach.
17:27I was nauseous.
17:28It's not uncommon for people to have their valves replaced.
17:31I don't want that to happen.
17:33That is the last thing that I want to do,
17:36because I know that changes everything.
17:39I don't know how much of that my body can take.
17:41Like, I saw my brother and my mom,
17:43and I don't know how they went through that.
17:46So, I don't know.
17:50I don't want to think about it.

18:05So, I took a look at your echocardiogram.
18:10I think we need to start at least getting some plans in place.
18:24I'm waiting for Dr. Archie to go over my test results with me.
18:32So, I took a look at your echocardiogram.
18:36If you remember, we've been looking at your heart
18:38because of the aortic valve that's had some narrowing to it.
18:42You had pretty consistently had moderate aortic valve narrowing.
18:46And at this point, it looks like it's progressed.
18:49It's gotten a little bit worse than last time from six months ago.
18:52It started to approach the severe range.
18:55So, now it looks like your aortic valve is severely narrowed.
18:59But I think the time has come that we need to start putting some plans into place
19:03for when the time comes for it to be replaced.
19:06It's not going to be as straightforward in your situation.
19:10So, we have to kind of figure out what's the best approach
19:13and how we can most safely replace your aortic valve.
19:16Is it considerably worse?
19:19It is worse.
19:20It's gone from moderate to severe.
19:23It's definitely made a jump.
19:24You had been very consistent with the same numbers for, you know,
19:29probably about two years.
19:30And it's definitely taken a turn.
19:33I would say it's gotten worse.
19:37Yeah.
19:38That sucks.
19:38Yeah.
19:40Your measurement was about 30 to 35 for a very long time, for several years.
19:46A severely stenotic valve, that pressure measurement is over 40.
19:50Yours is about 45 now.
19:51So, you've taken quite a big, quite a big jump.
19:55In six months?
19:56In six months.
19:57Yeah.
19:58I didn't want to believe him at first.
20:01But I knew that it was real.
20:03So, I had to like, this is real Tiffany.
20:07I was disappointed.
20:15Do you think it's going to be able to be done without having to do open heart surgery?
20:20That is our hope.
20:21We try to replace most aortic valves through catheters in the groin.
20:25With your size, we're probably going to be looking at pediatric valves.
20:28It's a little different than what we usually do,
20:31so we may need to get the expertise of one of our pediatric cardiology colleagues.
20:36What's going to be a little bit more challenging in Tiffany's situation is that,
20:40as a result of progeria, she has a small stature,
20:43and we would need to make sure we have an appropriately sized valve that we can place in her.
20:49And if I don't catch it in time, could I have a heart attack?
20:52So, the most concerning trouble with a badly narrowed aortic valve is congestive heart failure.
20:59That could happen if the pressures in the heart just get too high.
21:02So, that's why we really want to get ahead of this.
21:07I think we just put a big one on here for you to think about.

21:26This is one of those moments that you will rarely see me having an emotional breakdown.
21:47I miss my mom, and I'm just so tired of being alone all the time.
22:05Mom died in September 2024, less than two years ago.

22:20I feel like I am going to live forever, because I can't wrap my head around not being here.
22:29I think my mother actually was the oldest living person with any form of progeria that they're aware of.
22:37If you would have met her, you would see where Tiffany gets her fire for life.

23:26When you're laying there awake at night and you really can't sleep, what's kind of going through your mind?
23:31What are you thinking about?
23:31Just usually thinking about, you know, the old Tanya moving in, and it's just a big step.
23:38So it's, it's just a lot to try to process.
23:41It's just a lot to, to keep up with.
23:45Ryan told me about Tanya probably about six months ago.
23:51And I did worry about that in the beginning.
23:54I don't want him to be hurt because he's been hurt by other people.

25:57somebody's intentions are with you, whether they're just trying to use you or they feel sorry for you.

26:15I don't feel good with chaos for so long. Like, sometimes I'm like, OK, we've got to get these kids
26:19outside because I can't take the screaming, you know, just the playing and stuff like that.

30:26And to me, that just felt like it was too far away.
30:29And I don't know if that was just Tiffany wanting it to be that long or the doctors felt like,

30:35We can wait until March that it wasn't an emergency.
30:48I have been sick all weekend, and my brother and my friend, Michelle, told me to go to the urgent
30:57care yesterday.
30:58And I am so glad that I did.
31:02Um, my heart rate has been through the roof all weekend.

35:55First I was somewhat in shock and now that that has worn off, the reality of it is I have
36:02to have it done or I'm going to die too early.
36:06And I'm not ready for that.

36:21I mean, you know, that just wasn't supposed to happen, you know.
36:26It was horrible.
36:46There were some complications during the surgery and it took over four hours.
36:54So then we went in to see her and I got to admit, I was a little concerned about, like,
37:05I could just, I mean, she just went through the ringer, so.
37:10She did not look well and I was a little bit concerned.
37:13I could see the fear on Todd's face.
37:18It was just very scary.
37:20And I left, I left there thinking I might, that might have been the last time that I'm going to
37:28see my friend.
