00:15Whew, you want to get rid of that.
00:17Whoa.
00:18Our team of top dermatologists...
00:20...have moved into a new purpose-built clinic.
00:23Doors opening.
00:25Our doctor's ready for you now.

01:00This is Skin A&E.
01:05This is Skin A&E.
01:06This is Skin A&E.
01:07The clinic.
01:08Ooh.
01:09We're bursting at the seams.
01:10Ooh.
01:11Felt a...
01:10big relief and pressure dr. Magnus makes an eye-watering cut
01:15I was a little bit nervous they've been being close to the eye and dr. Dev puts his musical skill
01:20skills to the test hmm thank you what that was middle C yeah
01:25I actually hit it fine I was fine
01:30it's morning at the skin A&E clinic and while some
01:35of the team are still waking up what's the most important thing we need to find
01:40the kettle let's find the kettle others are ready and raring to go
01:45and practice
01:50the first to arrive at the clinic today is Martin he's come with his interpreter
01:54Julie
01:55I come from the south of Wales
02:00I'm 58 and I am a handyman
02:05in the local deaf club near where I live
02:10everybody knows I'm the comedian at work yeah I have a good laugh with all
02:15colleagues at work they all know me very well for being the funny man Martin the doctor
02:20is ready for you now Martin's appointment is with dr. Emma
02:25Mullin hello is it not
02:30hello come on in have a seat with dr. Emma hello
02:35good morning hi Martin how are you
02:40a bit nervous bit nervous oh don't worry what can we do for you today
02:44huh
02:45yeah I thought it might be that so how long have you had that for
02:50nine years oh gosh that's quite a while yes has it been growing has it been
02:55getting bigger yes it has it stopped growing for a while got painful became
03:00more loose and then grew again I've been to see the doctor a few
03:05times so I thought maybe it was a cyst and they said it could be something called lipoma
03:10and then eventually within time after a while the diagnosis was lipoma
03:15I love my motorbike and with that comes a helmet of course yeah
03:20and then of course that doesn't fit very well it doesn't feel right yeah it's
03:25kilted yeah it's not comfortable at all it doesn't fit properly it's does anybody ever
03:30comment on it or oh yeah many times yeah lots of people have called me silly names yeah
03:35but I'm not happy with it no no I I completely see why yeah
03:40there's been occasions where I've met friends within the deaf
03:45community and they're very very you know well known for being quite
03:50quite direct and there have been quite forward jokes about me looking like the devil and
03:55you know and then there's also my hearing friends have said oh what's that and it's just it's
03:59it it's
04:00it does not my confidence I really don't want it there anymore so I'm just gonna have a little look and a little
04:05close feel of it is it sore or anything today it's not it's a little bit
04:10tender when it when you move it around it I'll be I'll be really gentle okay let's have
04:15a look all right
04:20so I can feel that but it feels like there's a lump underneath but it's nice
04:25and loose and it's mobile Martin had an ultrasound scan which showed this to be a
04:30lipoma these are sort of benign little fatty tumors under the skin but I could feel a hard bone
04:35lump underneath so my thoughts were as he previously had a knock on the head and get a little lipoma
04:40or little swellings forming over those things to sort of protect them and it's like in that body
04:45body's natural response to trauma so
04:50to remove it what we need to do is put some local anaesthetic in all around it which is like a little injection
04:55and that'll make it go numb and then we'll do a little tiny incision over the surf
05:00surface off it and then try and get the lipoma out through a small opening and
05:05what I'll do is I'm going to run it horizontally so the scar just looks like it joins in one of your
05:10wrinkles not your wrinkles your forehead line
05:12I shouldn't
05:12okay I'm sorry
05:15so you will have a little scar yeah okay I mean
05:20I'm intrigued to see how it will be carried out now and rest back
05:25there we go so I'm going to follow this line here
05:30so I'll probably do it there because that's kind of in the middle
05:33right so
05:35the next thing is to put in the local anaesthetic
05:40I'm concerned about the needle I hope it's not too deep
05:45you're okay with injections
05:47think about it's motorized
05:48yeah think of something nice
05:49that's brilliant
05:50yeah think of something else
05:51ready
05:52yeah
05:52sorry about this
05:55are you being very good
05:58are you being very good
05:59are you being very good
06:00are you being very good
06:01are you being very good
06:02are you being very good
06:03are you being very good
06:05are you being very good
06:07are you being very good
06:09are you being very good
06:11are you being very good
06:12are you being very good
06:13are you being very good
06:15are you being very good
06:16are you being very good
06:17are you being very good
06:19are you being very good
06:20are you being very good
06:21are you being very good
06:23are you being very good
06:24are you being very good
06:00Okay, we need to get the light on nice.
06:05It's very bright, sorry.
06:07Okay, so we're just gonna check to see if...
06:10If it's gun numb, if he feels anything sharp, to let us know.
06:15He should...
06:15You should just feel a light pressure.
06:17Everything feels okay?
06:20Yeah, it sounds as though it's...
06:23Great, so we're gonna start.
06:45It's not gonna be one of those, it just pops.
06:47I'm gonna have to cut it out bit by bit, I think.
06:50A classic lipoma is sort of really soft and squishy and just fat cells and...
06:55So you can go and it all just comes out in one go.
06:58But Martin's was quite hard.
07:00Quite fibrous and firm.
07:02So I just knew it wasn't gonna come out easily.
07:06You okay?
07:07Yeah, yeah.
07:09Yeah, don't like to look.
07:10Yeah.
07:12Brave.
07:13Very brave.
07:14Julie, did...
07:15I did seem to be a bit squeamish with the procedure.
07:18I have to say I had one...
07:20One eye on Martin and one eye on her in case she keeled off that chair.
07:26Coming up...
07:27I've come to have this looked at, this...
07:30lovely little lump on my ear.
07:31Or big lump.
07:32Big lump, yeah.
07:34It's like...
07:35a bit of a cyst or something there.
07:36And...
07:37Oh, there we've got a bit fat again.
07:38I don't know what's going on here.
07:40There we go.
07:41Bye-bye.
07:43There we go.
07:50It's late morning in the clinic and the day is well underway.
07:55Oh, cup of tea.
07:57Oh, you're amazing.
07:58Oh.
07:59I think we deserve this.
08:02Next on the patient list...
08:04Next is 23-year-old Chloe from Bristol.
08:07I am an apprentice rowing.
08:09I work on the train tracks, making sure that everything runs smoothly.
08:13I play...
08:14Field hockey as a goalkeeper.
08:16At the moment, we're top of the league, so hopefully we'll win.
08:19This year, undefeated.
08:21Okay, Chloe, the doctor will see you now down the hall on your lap.
08:24Okay, thank you.
08:25No problem.
08:26Also undefeated by any skin condition...
08:29is Chloe's doctor today, Adil Shiraz.
08:32Hello.
08:35Hello there.
08:36Hiya.
08:37Come through.
08:37Take a seat.
08:38I'm Dr Shiraz.
08:39Nice to meet you.
08:40Hi, Chloe.
08:41I have Chloe, yes.
08:42Yes.
08:42What could we do for you, Chloe?
08:43So, I've come to you to have this looked at.
08:47This lovely little lump on my ear.
08:48Or big lump.
08:49Big lump, yeah.
08:51How long has it been, Nick?
08:52So, about two, three years.
08:54The GP said they think it's a keloid, but they're like, I can't.
08:57I'd say 100%.
08:58And how did it all start?
09:00So, I had piercings, and then...
09:02I had a knock to the head.
09:03Okay.
09:04Caused a bit of trauma.
09:05Yeah.
09:05Took the piercings out.
09:06Yeah.
09:07Cleaned them.
09:08I thought, okay, they'd be fine.
09:10And then, sort of, the lump started appearing.
09:12And then, over time, it's just got bigger and bigger and bigger with no signs of stopping.
09:17A keloid is a scar that heals abnormally.
09:20It heals too fast, and there's a lot...
09:22A lot of overgrowth of tissue.
09:24Maybe Chloe's had a bit of infection in the ear where she had the...
09:27And that's why she formed it, but we may never know.
09:30How does it affect you day to day?
09:32Making me so anxious.
09:34Um, I feel like I can't go out.
09:37And the...
09:37Hobbies I play, like field hockey and ice hockey, I wear a helmet.
09:42Right.
09:42And it's starting to affect how my helmet fits.
09:45So, I feel like...
09:47Like, it's going to start hindering my sports.
09:49Right.
09:50Which I'm a little bit concerned about.
09:51Right.
09:52Okay.
09:52Do you mind if I have a look at it?
09:53Yeah, yeah, have a look.
09:54Let's get you across here.
09:56Yeah.
09:56Now, I'll just get some...
09:57This lamp has made my confidence basically zero.
10:02My body image is not great because I just see this on my ear all the time.
10:07And I'm just sick of it.
10:09So, now, you've got the key load, of course, on this side.
10:11Yeah.
10:11But you have a little portion.
10:12On this side as well.
10:13Okay.
10:13So, it's obviously coming through.
10:16Okay.
10:16Um, to...
10:17So, we'll come around here.
10:19Let's have a closer look here.
10:22So, there...
10:22This bit is quite...
10:23Sorry if it's painful.
10:25Yep.
10:25So, this bit is quite...
10:26That's okay.
10:27There.
10:27Let's have to go along there.
10:30So, this bit will...
10:32Be a bit tricky this side.
10:34I need to make sure I get all of the keloid.
10:36Otherwise, it will...
10:37But I also need to be careful that I don't remove excessive cartilage, which will...
10:42change the shape of the ear.
10:44For this type of keloid, I think surgery is the best option.
10:47For you, once I take it off, you will have a little bit of a scar there.
10:51I think that's...
10:52Better than having this, I think.
10:54I would agree.
10:55Good.
10:55So, if you are happy...
10:57And I want you to lie yourself down.
10:59Nearly two years ago, I got engaged to my partner and...
11:02I feel like it's starting to sort of hinder the wedding plans because all I'm gonna see
11:07are my...
11:07My wedding photos is my lump on my ear and I don't want that.
11:12Now, this bit will sting.
11:14Are you okay with needles or you've got a...
11:15I should be fine.
11:16Okay.
11:17I'll give...
11:17I'll give you a massage, is that okay?
11:19That'll be nice.
11:21The topping massage.
11:22That's supposed to help.
11:23Is it?
11:24Yeah.
11:25It's supposed to distract you.
11:26Okay.
11:27And I set it coming up.
11:28Now, this will sting.
11:30But it will go numb.
11:32Mm-hmm.
11:33Keep breathing.
11:34Keep breathing.
11:36Well done.
11:37Oh, wow.
11:39Yeah, it's just the other bit now.
11:41That's why it feels a bit...
11:42Painful.
11:43Mm-hmm.
11:43Well done.
11:44You did really well.
11:47That worked, you see.
11:48Yeah.
11:48Yeah.
11:49Yeah.
11:49Yeah.
11:49Yeah.
11:52Does that feel sharp?
11:56No, it feels good.
11:57I mean, I don't feel it.
12:00Yeah, which is good.
12:02I mean, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don't feel sharp, you know, I don
12:07the air does contain quite a few blood vessels but the keloid itself can also
12:11contain
12:12blood vessels therefore bleeding is unfortunately something we have to
12:17deal with when removing keloids
12:22how are you doing are you okay yeah i'm fine
12:27you
12:32so if you just pull it as much as you can towards you that's it
12:37is that all okay yeah it's fine what does it feel like
12:42it just feels like i can hear like paper cutting
12:47it's like someone scratching paper against my ear
12:52so
12:57you
12:58so that's all gone for you now
13:01that the big one gone is
13:02it yeah big one's gone
13:03lovely
13:05now i'm coming on to the front
13:06yeah
13:07how's that
13:10that's absolutely fine
13:11that's absolutely fine
13:12i'm
13:14i'm
13:17i'm
13:18i'm
13:19i'm
13:21i'm
13:22so
13:23i'm
13:24low
13:25jesus
13:26let's see
13:27have
13:28here
13:31here
13:32you
13:34here
13:38here
13:39here
13:40here
13:40here
13:40here
13:43here
13:45here
13:48there
13:49here
13:50here
13:51here
13:52but we need to make sure there's no real tension.
13:56The more tension there is, the bigger.
13:57Big risk, he'll come back.
13:58Yep.
13:59So we want minimal tension.
14:00They wanted to give the best...
14:02shape possible cosmetically for Chloe
14:04and suturing was the best option in this case.
14:07However, I was quite careful when suturing to minimise any tension
14:10and actually it came together very nicely.
14:12OK, right, shall we give you a mirror?
14:17Oh, my gosh, I have an ear again.
14:20That's amazing.
14:21And your stitches are all sort of...
14:22on the back here, which are sort of hidden because it's behind.
14:25So you will hardly see that scar.
14:27Once it heals...
14:28It feels so much lighter already.
14:30Like, it felt so heavy before.
14:32Because it's bending my ear forward.
14:34Oh, my God, that...
14:37Wow, you are a magic man.
14:40You are magic.
14:42Thank you very much.
14:44I saw my ear that was normal.
14:46I mean...
14:47I didn't really know what to think.
14:50A bit shocked, really.
14:51Do you want to have a look at it?
14:52Yeah.
14:53Here's your gloves in case you want to touch it.
14:57Have a look there.
15:01That feels weird.
15:02I think my partner is going to notice a lot of difference.
15:06She will see that...
15:07I'll be more confident and probably more excited for the wedding day
15:10and sort of get...
15:12get more involved in the planning.
15:14How was that for you?
15:15Oh, such a relief.
15:17But thank you so much.
15:18No, you're welcome.
15:19Thank you very much.
15:20Bye.
15:21Bye-bye.
15:22I'm going to look fine in the photos.
15:24I'm going to look absolutely amazing.
15:27That's one happy patient finally lump-free.
15:32And back in treatment room two, Dr. Emma is hard at work.
15:37relieving Martin from his forehead lipoma.
15:41As soon as I...
15:42snipped through a little bit of it,
15:44I'm seeing muscle.
15:45So it's like a big...
15:47quite diffuse thing.
15:48So it's going to take a little bit of time maybe to remove.
15:52Normally, a lipoma, when you cut into the skin,
15:54you see this sudden very golden yellow swelling.
15:57It's kind of fat.
15:58When I went into Martin,
15:59I just saw a bit of muscle and a bit of sort of...
16:02fibrous tissue.
16:02There was a little bit of fat.
16:04So I just thought,
16:04oh, this one's not quite normal.
16:07It looks quite tough and it's quite fibrous.
16:11Yeah.
16:12I don't know.
16:15Oh, it's...
16:16It's...
16:17It's...
16:17Donna I think I'm better swapping sides with you. Okay, I can go this way. Yeah.
16:22Oh fabulous. Dosey-dose. Just so I can see in this end of it better.
16:27So let's have a look in this way.
16:32You okay?
16:37Everything feel okay? All right? Yeah, yeah.
16:41It's like...
16:42It's an old, like a bit of a cyst or something there.
16:46It's like a tough one.
16:47It's like a little bit of a cyst wall.
16:49Or something.
16:51And they're...
16:52So we've got a bit fat again.
16:53I don't know what's going on here.
16:55Everything, look, that was a bit grey and then...
16:57This looks a bit fatty, like a lipoma.
16:59There were bits that looked like muscle.
17:01There was a bit that looked like a...
17:02cyst and there was bits that were clearly lipoma.
17:04None of it looked really concerning, but I've not...
17:07really seen one like that before.
17:09That's like lipoma tissue, isn't it?
17:11Yeah.
17:12That's kind of...
17:12how you expect it to look.
17:13Yeah.
17:14There we go.
17:15There's an edge too.
17:16I don't know if this bit's coming out.
17:17This is like soft and not fibrous.
17:20Now he's being generous.
17:22I think that's it.
17:26I feel like...
17:27flat.
17:28That feels nice and...
17:29flat.
17:30There's no bump there anymore.
17:32We've taken...
17:32We've taken out whatever it was.
17:33The end is in sight.
17:37It's time to stitch up.
17:38I didn't feel...
17:39I didn't feel...
17:40I didn't feel...
17:41I didn't feel...
17:42anything from...
17:43the start to the end.
17:44I could feel a bit of...
17:45a bit of pressure when pulling the skin...
17:47to do the stitches.
17:48But it felt fantastic.
17:50What kind of bike do you have, Martin?
17:52Yamaha 650.
17:53Yeah.
17:54Oh.
17:55That's a big bike.
17:56It's called a...
17:57wagon store.
17:58Ooh.
17:59That sounds really cool.
18:00Right.
18:01We're all done, Martin.
18:02I wouldn't want him on his motorbike...
18:04for a couple of weeks.
18:05We'll have to wait, yeah.
18:06We'll have to wait, yeah.
18:07Oh.
18:08Oh no.
18:09Sorry, Martin.
18:10Oh no, we've made the lip quiver.
18:12Sorry.
18:13There we go.
18:14It's looking good.
18:15It's looking good.
18:16Right.
18:17Sit up, but don't get down yet.
18:20Yeah.
18:21Just make sure you don't feel dizzy.
18:22And do you want to have a look at it in the mirror?
18:24Yeah.
18:27Wow.
18:31Wow.
18:32Good.
18:36I was very...
18:37very shocked.
18:38Very shocked when I saw my forehead.
18:40It was incredibly flat and smooth.
18:42There was a tiny little stitch there, but I am so happy.
18:45I'm so pleased.
18:46I'm so happy.
18:47I'm so pleased.
18:47You okay?
18:48Yeah.
18:49Okay.
18:51Thank you very much.
18:53Right, Martin.
18:54Do you want to see what came out?
18:56You...
18:57Yes, please.
18:58Do you want to have a look at it?
18:59Yeah.
19:00So this is it here, Martin.
19:01So there's lots of little bits of...
19:02like, thickened fat.
19:04So that was the lipoma underneath.
19:06And then...
19:07there was all these bits.
19:08They looked like...
19:09It looked like they'd been a cyst.
19:12Yeah.
19:13Yeah, it looks fairly big.
19:16It looked a little bit like that.
19:17Scottish food, haggis.
19:19Very small fatty bits.
19:22Yeah.
19:23Very strange.
19:24Very fatty.
19:25I was quite shocked.
19:26Lovely to meet you.
19:28Thank you very much.
19:30It's lovely.
19:31It's a pleasure.
19:32Lovely to meet you too, Julie.
19:33I'm so pleased.
19:32Yeah.
19:33So pleased.
19:35Take care.
19:36Bye.
19:37Bye.
19:38Bye.
19:39Bye.
19:40Bye.
19:41Bye.
19:42Bye.
19:43Bye.
19:44Bye.
19:45Bye.
19:47Bye.
19:48Bye.
19:49Bye.
19:50Bye.
19:51Bye.
19:52Bye.
19:53Bye.
19:54Bye.
19:55Bye.
19:56Bye.
19:57Bye.
19:58Bye.
19:59Bye.
20:00Bye.
20:01Bye.
20:02Bye.
20:03Bye.
20:04Bye.
20:05Bye.
20:06Bye.
20:07Bye.
20:08Bye.
20:09Bye.
20:10Bye.
20:11Bye.
20:12Bye.
20:12Bye.
20:13Bye.
20:14Bye.
20:15Bye.
20:16Bye.
20:17Bye.
20:19Lucky for Dr. Natalia.
20:22There will be no delays.
20:22as her next patient has just dropped in, 28-year-old Aaron from Surrey.
20:27I do breakdancing, I spin on my head, you kind of de-stress, you let go of everything.
20:32Just enjoy yourself.
20:36Hello!
20:37Hi.
20:38Hi.
20:39Have a seat.
20:40Can you just be Aaron?
20:41Yes.
20:42I'm Natalia, hello.
20:42Love to meet you.
20:43This is Dutta.
20:44She's assisting me today.
20:45Right, so how can I help you today?
20:46So I've got some...
20:47I've got some bumps on my head.
20:48Okay.
20:49I believe they're cysts.
20:50Okay.
20:51And just really want them out.
20:52Right, okay.
20:53How long...
20:52How long have you had them for?
20:53About five years.
20:54Five years?
20:55And how many do you think you have there?
20:56I think I've got about four.
20:57Two larger ones, two smaller ones.
20:58Two smaller ones.
20:59Why do you want to remove them now?
21:00Well, basically they're getting bigger.
21:01Okay.
21:02And I do breaking.
21:03So I break dancing.
21:04Oh, cool.
21:05So I spin on my head.
21:06You spin on your head.
21:07And it gets quite sore.
21:08Right.
21:09And I don't want it to pop.
21:10No, that'd be bad.
21:11Yes.
21:12The cyst is any fluid-filled lump under the skin.
21:14I guess in theory, you could burst the cyst if you...
21:17put enough pressure on it.
21:18Like if you spin on your head.
21:19But they're quite well kind of closed in.
21:22under the scalp.
21:23So probably wouldn't happen.
21:25Can you do that worm thing where you like...
21:26Worm...
21:27That's like a proper old school move.
21:28Who is that?
21:29It's like...
21:30I'm old.
21:31That's like a throwback.
21:32Oh, it's...
21:32Yeah, yeah, yeah.
21:33So that's my day.
21:34Yeah, yeah.
21:35Okay.
21:36Well, let's have a look at your scalp.
21:37So I think you can stay...
21:37there.
21:38Okay.
21:39No worries.
21:40So I think you need to take your hair down.
21:41Yeah, that's fine.
21:42Right.
21:43Okay.
21:44So I've got one there.
21:47Okay, one there.
21:48Yeah, I've got one there.
21:49A bit smaller.
21:50Mm-hmm.
21:52And then I've got just two on the sides.
21:54A smaller one there.
21:55A bit of three?
21:56Yeah.
21:57That's right.
21:58That's one.
21:59Where's the other one?
22:00And then the other one is there.
22:02And...
22:03Oh, yeah, that's quite simple.
22:05Yeah.
22:06So really...
22:07I think this one affects me the most, spinning.
22:09Yeah.
22:10And then obviously it goes into your head from different angles.
22:12Right.
22:12You can roll onto the front ones and...
22:14Yeah, yeah, yeah.
22:15...come up from the side, if you know what I mean.
22:16I want to...
22:17I want to get this sorted so that I can work on my head spins, work on my windmills.
22:21I've got a few battles coming up.
22:22This year that I would like the lumps to be gone for so I can get back to my training and be...
22:27at the top level.
22:29Okay.
22:30Okay, so I can remove them.
22:32All four of them.
22:33Okay.
22:34That's fine.
22:35I just make an incision across the top of the cyst, scoop the cyst out and...
22:37close it again.
22:38See.
22:39So, do you want to pop your jacket off and we'll get you on the couch?
22:42When Dr Natalia said that she could remove them, I was over the moon.
22:47I had a lot of stress gone and I was just really glad to be rid of them.
22:51Right, so this is...
22:52A little bit you're not going to like.
22:53This is the anesthetic.
22:54So it's going to feel sharp and a sting and you're going to feel like it's getting really...
22:57swollen and sharp.
22:58Do you feel anything sharp here?
22:59Sharp.
23:00Sharp.
23:01Sharp.
23:02Sharp.
23:03Sharp.
23:04Sharp.
23:06Sharp.
23:02Sharp.
23:03Sharp.
23:04Sharp.
23:05Sharp.
23:06Sharp.
23:07Sharp.
23:08Sharp.
23:09Sharp.
23:10Sharp.
23:11Sharp.
23:12Sharp.
23:12Sharp.
23:13Sharp.
23:14Sharp.
23:15Sharp.
23:17Sharp.
23:18Sharp.
23:20Sharp.
23:21Sharp.
23:22Sharp.
23:23Sharp.
23:25Sharp.
23:26Sharp.
23:27Sharp.
23:28Sharp.
23:37No.
23:40Let's come out.
23:42It's squeezing itself out.
23:47Okay, that's fine.
23:49It's looking out well.
23:52Let's go.
23:53Let's go.
23:55Let's go.
23:57Let's go.
23:58Let's go.
23:59Let's go.
24:00Let's go.
24:01Let's go.
24:02Let's go.
24:03I mean, it's really smooth.
24:04Yeah.
24:05So the first one's out.
24:06Fantastic.
24:07Okay.
24:08So do you stitch them up straight away?
24:09Yep.
24:10Straight away, yeah.
24:11Right, okay.
24:12Otherwise you'll bleed all over the place.
24:13Let's go.
24:14Let's go.
24:15Let's go.
24:16Just it's very vascular.
24:17Why?
24:18Because the hair has so much blood supply.
24:19Right.
24:20Because there's so many follicles and stuff.
24:21Got ya, got ya.
24:22When she was working on my head, I could relate the noise similar to walking on ice.
24:26The crunch.
24:27Just breaking underneath the foot.
24:29It was very similar to that.
24:32The crunch.
24:33Wow.
24:34Here's my head key.
24:35Oh no, it was wyglądaő.
24:37So next好了.
24:38First up, everybody wasdr competitors.
24:39Oi.
24:41All right.
24:43Bye.
24:44One stop.
24:45Sie're going.
24:46You just know what else is.
24:48Come here.
24:49It's too warm.
24:50One Stop.
24:52Yet be at work.
24:53It's like I have promised you.
24:55Who loves travelling with a table.
24:56This imaginary one stops.
25:01That's alright, no worries, do what you have to do.
25:11That one's out too.
25:12Alright, last one, it's the big one.
25:13Fantastic.
25:21Key tab.
25:26Oh.
25:31It's all right.
25:32When she got to the back of my scalp,
25:34I could feel a lot of pressing.
25:36A few crunches and a little bit of cutting.
25:40Oh yeah, it is.
25:41OK.
25:43Oh.
25:46How are you doing there?
25:47Mm-hmm.
25:50OK, dab.
25:51I felt a big relief in pressure when it removed
25:54compared to the ones on the side of my...
25:56Just so relieved to get it out.
26:01Oh, we got some hair.
26:05That's part of the...
26:06The capsule right there, yeah.
26:08Have a dig around there and just make sure that it's all gone.
26:12Yeah, OK.
26:13Let's teach you that.
26:15You can have a bit of...
26:16Mm-hmm.
26:17And it may feel like someone's hit you on the head.
26:19I had to put a lot of pressure through...
26:21the areas to get the cysts out because they were quite kind of stuck down.
26:25So I did warn...
26:26Aaron, that he may have a headache afterwards.
26:28All right, let's have a look.
26:29OK.
26:30So I'm going to give you a mirror.
26:31Can you hold that and...
26:33Can you see...?
26:36Yeah.
26:37Fantastic.
26:38But be nice and smooth.
26:39Thanks for doing that.
26:40I really appreciate it.
26:41No problem.
26:42No problem.
26:43So come and have a look.
26:44OK.
26:45So...
26:46They're not beautiful.
26:47Wow.
26:48This is the bigger one in the back.
26:50Big boy, yeah.
26:51But it's kind of broken into pieces.
26:52OK.
26:53And this is the one in the front.
26:54Mm-hmm.
26:55But that's the cyst.
26:56OK.
26:57There you go.
26:58Working my way down.
26:59Some had red bits more...
27:01pink.
27:02One had hair in.
27:03That was quite interesting.
27:04It looked like a little Mohican.
27:06They actually looked a little bit like pomegranates.
27:08Lovely.
27:09All right.
27:10Nice to meet you.
27:11You're welcome.
27:12Cheers.
27:11Have a safe journey long.
27:12Appreciate that.
27:14See you later.
27:15It's the end of the road for my sis.
27:16We've lived...
27:16life for the past five years together.
27:18And now it's time to part our ways.
27:20I would definitely...
27:21will not miss them.
27:22I'm just glad to be rude.
27:23Are you...
27:24tempted to try Outbreak?
27:26dancing?
27:27I don't think so.
27:28I don't know if spitting on my head is such a great idea.
27:29Maybe do the worm.
27:30.
27:31I don't know if it's done.
27:32I don't know if it's done.
27:33I don't know if it's done.
27:34I don't know if it's done.
27:35Break dancing might...
27:36not be at the top of Natalia's to-do list.
27:38But after a busy day, just how do our...
27:41dermatology team let loose?
27:43What's the first thing you'd like to do after a hard day in the clinic?
27:46Shower.
27:47I love cooking because...
27:51it just makes you forget about everything.
27:53I literally collapse in a heap.
27:55A little stupefied.
27:56if it's late enough with a glass of wine.
27:59I actually watch a lot of television.
28:01like a fairly large amount of reality TV.
28:06so...
28:07Give my kids a cuddle because they just make it...
28:11all melt away.
28:12Our derms and nurses are going to have to wait a...
28:16little longer before they can relax...
28:18because there's still a few patients left to see.
28:21My name is Lee.
28:22I'm 39 years old.
28:24And I'm an electrical maintenance engineer.
28:26from Burnley.
28:27Hello.
28:28Come for my appointment.
28:29Can I have a name please?
28:30It's Lee.
28:31Lee.
28:31Have a seat.
28:32I'll let you know when you're ready.
28:34No problem.
28:35Three years ago I got involved with Ultra Wax.
28:36black collar boxing to raise money for charities.
28:38Stepping in the ring is very daunting.
28:41It takes a lot of careers to get in there.
28:43But win or lose, it's the most incredible feeling.
28:46Lee's booked in to see Dr Magnus Lynch.
28:51Hello.
28:52How are you, Lee?
28:53Hello.
28:54How are you, Lee?
28:55It is, yeah.
28:56I'm Magnus.
28:56It's nice to meet you.
28:58Have a seat for me.
28:59Cheers.
29:00And just tell me how I can help...
29:01Tell me what you're concerned by.
29:02So the reason I've come in today is obviously for the assist that I've got on my eye there.
29:06Two and a half years ago.
29:07I had like a little white tag on there.
29:09Yeah.
29:10And I put...
29:11I cut that off.
29:12Okay.
29:13And then from that I got a little lump.
29:14Yeah.
29:15And the little lump's grown into...
29:16What do you see today?
29:17Okay.
29:18And what kind of symptoms does it cause you?
29:19How does it affect your life?
29:21I was part of a charity boxing.
29:23Yeah.
29:24I used to get in the ring.
29:25I used to do a bit of sparring.
29:26And everything like that.
29:27Yeah.
29:28I can't do that now because I feel like it's a target for someone.
29:30Right.
29:31And I know that...
29:32So you're sort of keen to get back to doing that?
29:33I'd love to get back to doing that, yeah.
29:35Yeah.
29:36It gets me down because I'm dying to get back to training because since I've stopped...
29:41I've put on a lot of weight.
29:42What kind of work do you do?
29:44So I'm an electrical manager.
29:46I'm an engineer.
29:47So I work in an engineering firm in aerospace.
29:49Does it have some impact on...
29:51I wear safety glasses around the factory.
29:53Yeah.
29:54And once I put them on I can feel it will...
29:56I'm going up against safety and that's what I'm going to pull it forward.
29:58So it's uncomfortable as well as you don't like the look of it.
30:00Very well.
30:01I'm comfortable, yeah.
30:02Whenever I'm talking to anyone I feel like they're talking to that, not talking to me,
30:05you know, people.
30:06People will be like, what's that there?
30:07You know, they'll...
30:08So people have made comments in the past?
30:10Oh yeah.
30:11Okay.
30:12Can I just have a quick look at that?
30:13Of course you can, yes.
30:14Yeah.
30:16Okay.
30:17Okay.
30:18Okay.
30:19Okay.
30:21That's fine.
30:22So the good news is that I can treat it for you.
30:26Oh, wonderful.
30:27Yeah.
30:28Yeah.
30:29So I'm just going to draw and show you what...
30:31what we might do.
30:32Yeah.
30:33Yeah.
30:34Perfect.
30:35So let me just grab a pen.
30:36Okay.
30:37Okay.
30:38Okay.
30:39Okay.
30:40Okay.
30:41So...
30:41what I would propose to do is to make a cut along that purple line that I've drawn
30:45there.
30:46Yeah.
30:46But I'll also be removing that piece of skin because that's a big bit of skin that we
30:50don't want there anymore.
30:51Okay.
30:52So when I stitch it, because I've removed some of the skin, then it will flatten out.
30:55That makes sense?
30:56Yeah.
30:56Yeah.
30:57Can't wait.
30:58Good.
30:59Okay.
31:00If you just take your top path off.
31:01Do that.
31:02To receive treatment today, it'd be amazing.
31:04The only thing I'm nervous about is...
31:06the needle to begin with.
31:08I have passed out before with needles.
31:10So I'm just...
31:11I'm just going to numb the area.
31:12I'll be as gentle as I can.
31:14Okay.
31:15When you're performing in...
31:16injection very close to the eye.
31:18It's essential that the tip of the needle is facing away from the...
31:21eyeball because occasionally a patient will move unexpectedly and you don't...
31:26want them to move in such a way that the needle causes damage to the eye itself.
31:31So that's a good reaction coming.
31:36Right.
31:37That's a difficult bit done.
31:38You're doing really, really well.
31:39Take a minute.
31:40Take a minute.
31:41A tiny bit more.
31:43Right.
31:44That's all the hard part finished now.
31:45You can relax.
31:46Okay.
31:47Nothing painful or sharp from now on.
31:50There we are.
31:51Let's just pop that over to cover your eyes there.
31:53All right.
31:54We're starting the procedure now.
31:57Are you okay there?
31:58Yeah.
32:01And that's all.
32:03You don't know.
32:04It's chicos that you can do either.
32:05All right.
32:06All right.
32:08OK.
32:26All right.
32:29All right.
32:30I was a little bit nervous at first, obviously everything would be close to the eye.
32:35But I didn't really feel anything that concerned me.
32:40I was a little bit nervous.
32:45That's interesting actually, so it doesn't look exactly like a cyst.
32:50I just wonder if it might be a harmless growth of the glands.
32:55It wasn't typical for a cyst. I thought it might be the hydrosystoma.
32:59The growth of the glands which are in the eyelid look a little bit more translucent.
33:04I don't have contents like a normal cyst.
33:06Hydrosystomas are quite rare. I've probably removed two.
33:09In the past, I didn't have any concerns about performing surgery in this area.
33:13However, it is an area where you need to be more careful.
33:16It's all getting very smoothly, not too much longer now.
33:20Are you okay under there?
33:21Let's see.
33:22No.
33:23Yeah.
33:24Okay.
33:25Let's see.
33:26Let's see.
33:27Okay.
33:28Let's see.
33:29Yeah.
33:30Yep.
33:36I like it.
33:41So that's all gone very smoothly.
33:43Just popping some stitches in.
33:44It will feel a bit weird.
33:45You feel a bit of pulling.
33:46Maybe a bit of tickling from the stitches.
33:48Don't worry about that.
33:49No, it's fine.
33:51I can't wait going forward.
33:52Now as soon as this heals, I can get back in the gym and hopefully I can...
33:56...start losing some weight.
33:57Would you like to have a look?
33:58Yes, please.
33:59Can't wait.
33:59Yeah, okay.
34:00See you.
34:01There we go.
34:02There's the mirror.
34:03Have a little look.
34:06Oh yeah, that looks a lot better.
34:08Yeah.
34:08It's all gone.
34:09So a little tiny bit of freezing.
34:10Nice and flat.
34:11Yeah, perfect.
34:13Yeah, good.
34:15We're going to put a dressing on there now.
34:16Yeah.
34:17So you'll be able to go back to your boxing now.
34:19Bit of time.
34:20When it's for you.
34:21I just want to say a massive thank you to Dr. Magnus.
34:24I can't wait to go forward.
34:25He's been...
34:26...a massive difference in my life.
34:27Thank you very much.
34:28No problem.
34:28Cheers.
34:29See you.
34:29Bye-bye then.
34:30Bye-bye.
34:30Bye-bye.
34:30Bye-bye.
34:31Bye-bye.
34:31I won't be feeling like everyone's looking at it.
34:33So yeah, it's amazing.
34:34I can't wait.
34:36Coming up...
34:37It's like mashed potato.
34:38My sausage and mash for my dinner.
34:41But I've got not the right meal to have ready, is it?
34:46Bye-bye.
34:51It's been another jam.
34:56It's been a packed day at Skin A&E, but before the clinic doors close there's one
35:01last patient left to be seen.
35:03I'm Caroline, I'm 64 years old.
35:06And I've come up from Worcester today.
35:09Have a seat for me Caroline.
35:11I work in school and I deal with children.
35:16With any speech and language difficulties and I love everything.
35:21That's the moment of it.
35:22I also sing so that keeps me very busy.
35:26Caroline's appointment is with Dr Dev Shah.
35:31Hello.
35:32Come on in.
35:35Welcome.
35:36Hi.
35:37I'm Deb.
35:38Nice to meet you.
35:39How are you doing?
35:40This is Cherry.
35:41Good to see you.
35:42Take a seat my love.
35:44What can we do to help?
35:46Well, it's this lump that I have on my wrist here.
35:49Yes.
35:50I don't know whether...
35:51anything can be done with it.
35:52Right.
35:53I've had it for over 40 years.
35:55Okay.
35:56I had a little lump and I wanted it done before I got married in 1981.
36:01And I went to the doctor in 1980 and said, can you take this off please?
36:05I'm getting married.
36:06I'm getting married next year.
36:07Yeah.
36:08And they said, oh, it's too small.
36:11Oh, we'll be far bigger.
36:12And it's cosmetic anyway.
36:13Right.
36:14And now...
36:16Now I can feel a second one.
36:17Coming up.
36:18How is it affecting you?
36:20I do...
36:21I do work with children.
36:22Okay.
36:23A lot and with their parents and I can see them looking.
36:26And so I put my hand behind my back or sit on it or it's not...
36:31not nice at all.
36:32Many children actually have said, oh, what's that lump on?
36:36It's on your wrist.
36:37It does shake your confidence somewhat.
36:40I...
36:41I do sing as well.
36:42So when I'm out performing then I'm very...
36:43How do you?
36:44Okay.
36:45I'm very...
36:46very conscious of it, you know.
36:48What...
36:49What do you sing?
36:50Oh, I sing opera, operetta.
36:51musicals.
36:52Do you?
36:53Yes.
36:54Do choral work.
36:55Yeah.
36:56I love it.
36:57I do...
36:56I do feel really awkward.
36:58You think, perhaps I should hold my...
37:01hand differently when I'm performing.
37:04Hence why I always...
37:06wear a long sleeve.
37:08Nine times out of ten.
37:10So...
37:11I'm going to come a bit closer and have a little look.
37:14So...
37:15Ooh, and is it sore?
37:16No.
37:17Okay.
37:18I think this is probably...
37:21something called a lipoma.
37:22Oh, right.
37:23Which is a fatty lump.
37:24Yes.
37:25Okay.
37:26But...
37:27I think we can only do it for you if you sing to us.
37:30No.
37:31It's ridiculous.
37:32When he said, we can definitely take this out, I was...
37:36ecstatic.
37:37So your resultant scar is that one in the centre?
37:41Yeah.
37:42Yeah.
37:43That'll be smashing.
37:44It's a hundred times better than what it is.
37:45Yeah.
37:46And hopefully...
37:46we'll just shell out.
37:47Yeah.
37:48And that should be it.
37:49Brilliant.
37:50So local anaesthetic going around.
37:51Yes.
37:51It'll all be numb.
37:52Yes.
37:53You just need to keep your hands still.
37:54Yep.
37:55You shouldn't feel anything.
37:56We'll...
37:56we'll get you onto the couch.
37:57Well, super.
37:58All right.
37:59Come and lie yourself down, my love.
38:01Lipo.
38:01Numbers and cysts can form all over the body, but generally...
38:06cysts cause more problem when they're about a centimetre or two because they become hard.
38:11And they're annoying.
38:12Lipomas can be, you know, much bigger and cause no problem.
38:16So when you see a lump on someone's skin that's quite big, it's unusual.
38:21that it would be a cyst.
38:22It's more likely to be a lipoma.
38:24Shut up, Scott.
38:26Coming in.
38:27That's okay.
38:28Okay.
38:29Okay.
38:30Okay.
38:32Needles don't bother me.
38:33It didn't hurt.
38:34Either that or I'm a tuffle.
38:36Bird.
38:37Are we ready?
38:41Yes?
38:42.
38:43.
38:46.
38:56It's a cyst.
39:01It's a cyst.
39:02Not a lipoma.
39:03Oh.
39:04So we're just going gently.
39:05So we're just...
39:06We're just trying to work out how we get this out.
39:11It's very thin wall, so it's quite fragile.
39:15When I made the second incision on Caroline's arm, I was shocked that I had ruptured the
39:19cyst.
39:20Because I still thought it was a lipoma.
39:22And it was really annoying because I don't like to...
39:25Doing that to cysts.
39:26Oh, yeah.
39:27Oh, yeah.
39:31It's just...
39:32Hang on.
39:33Okay.
39:34I'm sorry.
39:35I'm sorry.
39:36I'm sorry.
39:37I'm sorry.
39:38I'm sorry.
39:39I'm sorry.
39:40I'm sorry.
39:41I'm sorry.
39:42You can...
39:43Hold that like that.
39:44Yeah.
39:45I've got it.
39:46Right.
39:47Right.
39:51Get out.
39:52Get out.
39:53Get out.
39:57Snip, snip, snip.
39:58I just want to get underneath because I'm a bit worried about all the...
40:01There we go.
40:02There we go.
40:03There we go.
40:06Can you?
40:11Do give us a music lesson before you go, just a quick one.
40:16Just a little one.
40:18Just try and get me to hit middle C.
40:21Is that it?
40:25What's that?
40:26Is middle C?
40:27Yeah.
40:28All right.
40:29Go ahead, Chez.
40:30You go.
40:31That was very deep.
40:33Lift your cheekbones.
40:34Mmm.
40:35Mmm.
40:36There you go.
40:37Mmm.
40:38Mmm.
40:39There you go.
40:40Mmm.
40:41There you go.
40:42Middle C.
40:43Mmm.
40:44Mmm.
40:45Mmm.
40:46Mmm.
40:47Mmm.
40:48No idea.
40:49No idea.
40:51So unmusical.
40:52There we go.
40:53There we go.
40:56Mmm.
40:57Mmm.
40:58Mmm.
40:59Mmm.
41:00Mmm.
41:01Mmm.
41:01Mmm.
41:03Mmm.
41:04Mmm.
41:05Mmm.
41:06Mmm.
41:06Mmm.
41:07Mmm.
41:08Mmm.
41:09Mmm.
41:11Mmm.
41:12Mmm.
41:13There we go.
41:14Lovely.
41:15It's all out.
41:16Is it really?
41:17Is that lid?
41:16Yeah.
41:17Yeah.
41:18Yeah.
41:19Everything's out.
41:20It's all come out.
41:21It's all.
41:21It won't come out.
41:22It won't come back.
41:23I enjoyed having her singing lesson mid-op.
41:25It was a rarity for me.
41:26Mmm.
41:27Too high.
41:28That was too high, wasn't it?
41:30I was...
41:31Mmm.
41:32Mmm.
41:33Mmm.
41:34Mmm.
41:35Mmm.
41:36Mmm.
41:36Mmm.
41:37Mmm.
41:38Mmm.
41:39There you go.
41:40What, that was middle C?
41:41Yeah.
41:42Told you I've got it.
41:43You are.
41:44You're so musical.
41:45I actually hit it fine.
41:46I was fine.
41:47I was so musical.
41:49They did a good job and...
41:51Well, in the end, I would give him a ten.
41:56Go and have a look.
41:57Oh, wow.
41:58So, all that ridging will go.
42:00So, don't...
42:01Yeah.
42:01Don't worry about that.
42:02And the swelling, too.
42:03Thank you very much.
42:04No problem.
42:05Oh, that's beautiful.
42:06A little bit of needlework.
42:07Oh, thanks.
42:08The lump is gone and...
42:11I'm not just excited.
42:12I'm elated.
42:13Do you want to have a look at what we took out?
42:15Oh, yes.
42:16Please, please.
42:17Come over to the trolley.
42:18And if you want to pop some gloves on...
42:19There we go.
42:20Yes.
42:22And I will take a photo of you.
42:24You ready?
42:25One, two, three...
42:26That's...
42:27Wow.
42:28Oh, that's amazing.
42:29Oh, I've got to put it...
42:30Oh, I've got to put it...
42:31Can you go?
42:32Oh, look.
42:33It's starting to split.
42:34Yeah, no.
42:35Just squeeze it a bit.
42:36You'll see what came...
42:36Oh, amazing.
42:39A lot of people would probably go...
42:41Ooh.
42:42But I thought it was fantastic.
42:44It's like mashed potato.
42:46Oh.
42:47Tomorrow when I have my sausage and mash for my dinner.
42:51That I've got...
42:52Not the right meal to have ready, is it?
42:57Lovely to see you.
42:58Have a safe trip back.
42:59Take it easy.
43:00I will.
43:01Thank you very...
43:01Pleasure.
43:02Absolute pleasure.
43:03Look after yourself.
43:05Now I've had it done...
43:06I can sing.
43:07I can use my hands properly...
43:09And say, hello.
43:11Here I am.
43:12Without the lump.
43:14Ha, ha, ha, ha, ha, ha.
43:16Mmm.
43:17Mmm.
43:18Mmm.
43:19Mmm.
43:20Mmm.
43:21Mmm.
43:21Mmm.
43:22Mmm.
43:23Mmm.
43:24Mmm.
43:26Mmm.
43:27Mmm.
43:29Mmm.
43:30Mmm.
43:31Mmm.
43:31Mmm.
43:32Mmm.
43:33Mmm.
43:34I should be able to wear a...
43:36short sleeve dress and no gloves.
43:38So thank you very, very much indeed.
43:41...
43:42...
43:44...
