01:29and sister Vicky
01:32Barbara's gone to ward
01:34so get ready to share a shift
01:39I love my job today I think it's amazing with the team at Bansley casualty
01:44the early shift has begun
01:51I need your sister to help
01:53today sister Vicky is running the hub
01:57so what do we know
02:00no
02:00no it's my last long day
02:03is it
02:04no
02:05but there's a problem
02:07I don't think messages are feeding through to EPM 8 now
02:12operational support manager Antonia has become aware of a disruption to the hospital network
02:18the tracking system's down the internet's down
02:22I'm just trying to get older I and T for an update as well
02:25because it's affected quite a lot of things
02:27there's always something
02:30but before the team can work out the scale of their connection problems
02:37the first ambulance arrives
02:39Jeannie has been brought into casualty for the second time this week
02:46paramedic Andy hands over her care to advanced clinical practitioner Chantel
02:52Jeannie had a full attended Bansley District A&E on Sunday night
02:57GP then referred patient to a crisis response team
03:03found to be abnormal at that time
03:05physio rang 999 immediately
03:09and still on scene when we arrived
03:11she does have a pacemaker
03:14there you go
03:15she's got a rhythm strip
03:16like I say it has become quite irregular
03:18okay
03:19and we'll leave Jeannie in your care
03:21thank you
03:22thank you very much
03:23see you Jeannie
03:24okay love
03:25bye
03:25bye
03:26you alright Jeannie?
03:28I'm Chantel
03:28I'm coming to see you
03:29Jeannie's son Stephen and daughter-in-law Carol are with her
03:33right Jeannie
03:38you're 92 year old
03:39you don't look it
03:40thank you
03:41so tell me in your own words
03:43what happened on the 3rd when you fell
03:46I've had a little read
03:47I was just walking through the kitchen
03:49and I just fell
03:50I think I tripped
03:53and how did you go down on the floor forward?
03:56face forward
03:57face straight on the floor
03:58yeah
03:58since you've fallen and hit your face
04:02have you felt sick
04:03like nauseous
04:04vomited
04:05no
04:06not been coughing any blood up?
04:07no
04:08good
04:08and are you taking your painkillers?
04:11yes
04:11are you taking them?
04:12yeah
04:13oh yeah
04:13everyone always says yes and they never do
04:16so I always check
04:17and you're worried about anything
04:20because you know your own body
04:21apart from your face
04:23when I take a deep breath or I'm all
04:30yeah
04:30it's the ribs
04:32because it said the ribs were broken
04:34you've broken your ribs
04:35yes
04:36take a big breath in
04:37and it hurts
04:40yes
04:41where's the pain?
04:43across the chest
04:44across the top
04:45okay
04:47breathing difficulties like Jeannie's
04:52can cause serious illness
04:54when it comes to chest injuries
04:56there's not much we do
04:57unless it's a big trauma
04:58yeah
04:59the problems with a chest injury
05:01is if
05:02frailty's not on your side
05:04when you get older
05:05but then if you're not taking good breaths
05:06and being able to cough and clear
05:08you're just going to be at risk of getting a pneumonia
05:10so we have to try and keep you
05:11moving
05:12breathing
05:14coughing
05:14comfortable
05:15because when you shallow breathe
05:17you get infections down there
05:19okay
05:20if you can't breathe properly
05:22you're at higher risk of getting a chest infection
05:24or a pneumonia
05:24especially in the older population
05:26because they don't move around as much
05:27so that's my priority
05:29with Jeannie
05:30elsewhere in the hospital
05:35an alarm has been triggered
05:37medics rush Graham
05:39a 74-year-old outpatient
05:41from radiology
05:43where he collapsed
05:44during a CT scan
05:45to recess
05:47I had to have a cannula put in
05:57apparently I'm difficult to cannulate
06:01and my head was really
06:04felt like it was getting really hot
06:06but the worst it's been
06:08all of a sudden
06:10it's about to get a circus
06:11there's a people around me
06:14and swapping me on both
06:15the CT scan was to check
06:33how Graham's body has responded
06:35to treatment for cancer
06:37I got a little bit of cancer
06:41on the oncologist
06:44I don't know
06:47the last time of chemo
06:48was about June time
06:52and then
06:54he's going to decide
06:57whether I need to go back on chemo
06:59or just monitor it for a while
07:01trainee advanced clinical practitioner Jess
07:05is looking after Graham
07:08so what I'm going to do
07:10if it's okay with you
07:11is I'm going to just try
07:11and take some blood from that cannula
07:13oh from the cannula
07:15yeah I'm going to see
07:16if I can manage it
07:16I know you've had contrast through it
07:18it's not the greatest
07:18but with how anxious you've been
07:20I'm going to see if we can
07:21just get a little bit off that
07:22alright
07:22primarily I want to check
07:24that your calcium's okay
07:25alright
07:26so when you've
07:27got a cancer diagnosis
07:29background of cancer
07:30we like to check your calcium
07:32because if you're feeling a bit off it
07:34and we're not entirely sure why
07:35that can sort of answer
07:36a lot of questions
07:37alright
07:37sometimes you can have
07:40low calcium or high calcium
07:42and in a cancer patient
07:42that needs to be treated
07:43as a medical emergency
07:44so that would mean
07:45that he's brought in
07:46for treatment
07:46and an infusion
07:47to reduce that
07:48or to bring his calcium levels up
07:50so we like to say
07:52that if you've got
07:52bones, moans and groans
07:54so if you've got bone pain
07:55or you've just got general aches
07:56and pains
07:57that can sometimes be
07:58attributed to
08:00calcium levels
08:01that aren't normal
08:02if Graeme's calcium levels
08:05have become too high
08:06he will need immediate treatment
08:08oh
08:09in the hub
08:21sister Vicky is just
08:23half an hour into her shift
08:24I can't
08:26it's just messaging
08:27that's not coming in
08:28the now that's system
08:28I'm not going to be able to
08:29so you're telling me
08:30I have to do this for a patient
08:31which is it
08:32I'm going to report it
08:34so today we haven't got
08:35the tools that we generally have
08:36and our patient tracking system
08:38is down
08:38so we can't see the plotting
08:40of the patient's arrivals
08:41it's going to be one of them
08:43do you?
08:43yes
08:43so currently we're having
08:45a few issues
08:46because I do believe
08:47all the internet's down
08:48throughout the hospital
08:49and it's national
08:51as well as what I've found out
08:52so we're having to do it
08:54manually and verbally
08:55between members of staff
08:56so on top of one problem
08:58and then another issue arises
09:00but if the server problems
09:04were not enough
09:04casualty is now
09:06quickly filling up
09:07with patients
09:08who's your cubicle for?
09:11Barbara?
09:11I've put a pretty nice
09:12very interesting
09:13Barbara's gone to ward?
09:15yes you've gone
09:15right it's just so I can
09:16tick her off this list
09:18so currently we've got
09:2025 patients in the department
09:21so then it gets a little bit
09:24empty when I need to have
09:25staff moving and things
09:26right close your eyes for me
09:32can you feel me touching you?
09:33yeah
09:3492 year old Jeannie
09:35has fallen at home
09:37advanced clinical practitioner
09:39Chantelle is assessing
09:40her injuries
09:41good stuff
09:42and pop your arms out
09:43Jeannie's son Stephen
09:44and daughter-in-law Carol
09:46are by her side
09:47have a look at your lip
09:49oh it's healing
09:50nicely that
09:50yeah
09:51you heal well
09:52okay
09:53can I pop you a glass of
09:55I'm just going to have
09:57just a little look
09:58look straight forward
09:59so it's important to do
10:01another thorough history
10:02on Jeannie
10:02she might have injured
10:03something else that we
10:04didn't initially pick up on
10:05that can be quite common
10:06so I did a really thorough
10:08work up
10:09a good examination of it
10:11just to make sure that
10:12there's nothing else
10:12underlying anything broken
10:14that we've missed
10:15the first time
10:16can you just follow my pen
10:17torch
10:18can make you feel a bit
10:19dizzy sometimes
10:20this
10:20down there
10:22okay
10:23can you move your eyebrows
10:24close your eyes really tight
10:26nice and tight
10:27okay open your eyes
10:28can you puff them cheeks out
10:30and smile for me
10:33can you bite down
10:35and can you bite your teeth
10:36together
10:37good stuff
10:37okay
10:38he's laughing at you
10:41I've described my bedside manner
10:50as quite light-hearted
10:52I'm really sorry Gina
10:53but it's that
10:54you didn't tell me it was a
10:55wedding anniversary
10:55I like to just be myself
10:58patients coming to A&E
11:00can be quite scary
11:01especially if you're older
11:02there's a bit of a notion
11:04where we like to keep
11:05patients in
11:05and that can be quite
11:06worrying for them
11:07some people get scared
11:14thinking they're not
11:14going to end up going home
11:15if they're coming to
11:17hospital
11:17and they're scared
11:18of dying in hospital
11:19so I try and keep it
11:20as light-hearted as
11:21possible
11:21and reassure them
11:22that my job is to
11:24try and get them back
11:24home
11:24can I ever listen
11:26to your lungs
11:26yes
11:27let me feel your
11:29pacemaker
11:30is it there
11:30yes
11:31how long has this
11:32pacemaker been in
11:33for
11:33she had it checked
11:38in May
11:39did she
11:39yeah
11:40it will help me
11:42sorry
11:43is this painful
11:45yes
11:45is it painful
11:49around there
11:49yeah
11:50I find it difficult
11:51seeing anybody in pain
11:53but seeing older
11:54patients in pain
11:55can be quite upsetting
11:56it's hard
11:57to try and find out
11:58if they're in pain
11:59because a lot of
11:59older patients
12:00are quite stoical
12:01especially from Barnsley
12:02as well
12:03they don't want to
12:04admit any weakness
12:05sorry
12:06oh
12:07is that where
12:08it's really painful
12:08yes
12:09I'm real
12:13oh
12:16it could be that
12:18you've fractured a rib
12:18oh
12:19okay
12:22right let's have a
12:24listen down there
12:25big breath in
12:27I know
12:29can you sit forward
12:31for me
12:32are you alright
12:38yeah
12:38you sure
12:39yeah
12:39have a good look
12:44at your back
12:45my job is to try
12:47and figure out
12:47where the pain is
12:48to try and
12:49encourage them
12:50to be honest
12:50about the pain
12:51because it helps
12:51me diagnose
12:52pain
12:53and if it were
12:54my own grandparents
12:54that would really
12:56upset me
12:56you're quite
13:04crackly down
13:05there at the
13:05bottom
13:05so I might
13:06send you for an
13:07x-ray
13:07just to make
13:07sure there's
13:08no fluid
13:08or anything
13:09okay
13:09yes
13:10you have got
13:11a rib
13:11that's extremely
13:12tender
13:12so it might
13:13be that you've
13:13cracked that rib
13:14yeah
13:14there is also
13:17a concern
13:17that Jeannie
13:18may have
13:19damaged her
13:19pacemaker
13:20chest x-ray
13:22I'll have a
13:23chat with the
13:23pacemaker team
13:24and then
13:25we'll go from
13:26there
13:26is that okay
13:26yes
13:27Jeannie heads
13:31for an x-ray
13:32to see if
13:33either her
13:33pacemaker
13:34or her ribs
13:35have been
13:36broken
13:36it's been
13:43an interesting
13:44IT day
13:45today so far
13:46in the hub
13:47sister Vicky
13:48and the team
13:49have called
13:49an emergency
13:50meeting
13:50to address
13:51the global
13:52internet outage
13:53if you require
13:55any porters
13:55for any
13:56movements
13:56for x-ray
13:57CTs
13:58just come to
13:58coordinator
13:59we've been
14:00writing them
14:00down
14:00and allocating
14:01porters
14:02when we see
14:02them
14:03we've only
14:04got two
14:04female
14:05AMU beds
14:06male's full
14:07medicine's full
14:09with five
14:10definites
14:10and 13
14:11potentials
14:11surgery
14:12only had
14:12one bed
14:13ortho were
14:14full
14:14gynae tool
14:14beds
14:15anybody
14:15thought
14:16it was a
14:16wonder
14:16in resus
14:20advanced
14:21clinical
14:22practitioner
14:22Jess
14:23is trying
14:24to find
14:24out
14:24what
14:24caused
14:25cancer
14:25patient
14:26Graham
14:26to
14:27collapse
14:27once we've
14:30done this
14:30we're going
14:30to give you
14:31some fluids
14:31just because
14:32your blood
14:32pressure is
14:32a little
14:32bit low
14:33alright
14:34the blood
14:35pressure has
14:36always been
14:36a bit low
14:37it's a little
14:38bit too
14:38low
14:38this one
14:39a bit
14:40surprising
14:40considering
14:41my weight
14:42well that's
14:43what I'm
14:43thinking
14:43you're quite
14:44a large
14:44chap
14:44so a blood
14:46pressure like
14:47that is
14:47fairly low
14:49advanced
14:53clinical
14:53practitioner
14:54Jess
14:54takes some
14:55bloods
14:56there'll be a
14:56little bit of
14:57me
14:57shimmy in this
14:58about
14:58alright
14:58so just
14:59bear with me
15:00I know
15:00you're not a
15:00fan of it
15:01Graham has
15:03a fear
15:03of medical
15:04procedures
15:05oh bless you
15:07we were getting
15:07so worked
15:07up weren't
15:08you
15:08I don't think
15:11that helped
15:11today has it
15:12you getting
15:12so stressed
15:13about this
15:14and you're
15:16obviously
15:16feeling generous
15:17today
15:17you're giving me
15:19plenty of the
15:19good stuff
15:20you know
15:23sweetheart
15:23all done
15:24less stress
15:24I hope
15:25I'm going to
15:26pop this side
15:26up and then
15:27I don't lose
15:27you off this
15:27side at bed
15:28I'll be back
15:30with you shortly
15:30okay
15:31there you go
15:32I'm basically
15:35just giving him
15:36a bit of an
15:36MOT
15:37see if we can
15:38figure out
15:38if it is
15:39just a basic
15:39what we call
15:40phase vagal
15:41so just a
15:41passing out
15:42episode
15:43that is
15:43experienced
15:43today
15:44he said
15:44he was
15:44quite anxious
15:45before having
15:46it done
15:47because of
15:47he knows
15:48it's quite
15:49tricky to
15:49get blood
15:49from
15:50so I think
15:50that might
15:50have played
15:51a part
15:51so we just
15:53want to make
15:54sure that
15:54all the
15:54scaries
15:54are ruled
15:55out
15:55which is
15:55essentially
15:56our job
15:56in
15:56Aene
15:56Graham
15:58must now
15:58wait for
15:59the results
15:59of his blood
16:00tests
16:00I think
16:02there's a
16:02vampire farm
16:03somewhere around
16:04there
16:04just
16:04down to my
16:05blood
16:06there

18:06dislocation
18:06ankle
18:07it's a bit
18:08complicated
18:08because it's
18:09second
18:09metatarsal
18:10she's
18:11fractured
18:11dislocation
18:11so
18:12don't fall
18:12with a
18:12horse
18:13no
18:13no
18:14car
18:14accident
18:15no
18:15I've seen
18:19older people
18:20who fall
18:20on one side
18:21and maybe
18:21break a hip
18:22and maybe
18:23break a
18:23shoulder
18:24or a
18:25forearm
18:25but I've not
18:26seen a young
18:27person do
18:28two places
18:29on the same
18:29leg
18:30it's really
18:35hard though
18:35slipping
18:36is something
18:36you just
18:37can't protect
18:37yourself
18:38for
18:38it's not
18:38the forces
18:39are

18:51sleep
18:52there'll be
18:53someone
18:53putting your
18:54ankle back
18:54in place
18:55and then
18:55there'll be
18:56someone
18:56doing the
18:56potting
18:57so I need
18:57two more
18:58friends
18:59so that's
19:05the bone

19:06sitting on
19:06this one
19:08your shin
19:08bone's
19:08pushed forward
19:09so we need
19:09to push
19:09that back
19:10a bit
19:10because that's
19:11that's your
19:12ankle joint
19:12so it needs
19:13to be equal
19:13space all
19:14around
19:14so we're
19:15doing a
19:15squeeze
19:16and a

19:31and your
19:31foot foot
19:31no
19:32Louisa's leg
19:34needs correcting
19:35quickly
19:35to restore
19:36her blood flow
19:37and reduce
19:37the risk
19:38of permanent
19:39injury
19:39the internet
19:45may be down
19:46but the ambulances
19:47don't stop arriving

20:28unfortunately
20:29she's hit the table
20:30haven't you June
20:31must have
20:32you must have
20:32because you've broken
20:33the leg on the table
20:34haven't you
20:34eh
20:35probably my leg
20:37not your leg
20:38the table leg
20:39June's injury is unfortunate
20:42she's got a very large
20:43skin tear
20:44across her
20:45top of her chest
20:47here
20:47possibly a cut
20:49underneath her chin
20:50one to her elbow
20:52an extensive bruising
20:53to her face
20:55I just fell down
20:56I'm
20:58I'm not busy
21:00I'm not
21:00I'm not
21:00I'm not
21:01I'm not
21:01I'm not
21:01I'm not
21:01I'm not
21:01I'm not
21:02I'm not
21:03I'm not
21:03I'm not
21:03I'm not
21:03I'm not
21:04I'm not
21:05June goes for a CT scan
21:06to check what internal
21:08damage has been
21:09caused by a fall
21:10the scan will find
21:12any bone fractures
21:13internal bleeding
21:15or organ damage
21:17the team in casualty
21:31are halfway through
21:33a difficult shift
21:34so all systems
21:35have gone down
21:36and I mean
21:38every single one
21:39of them
21:39a global internet
21:41outage
21:41means that
21:42sister Vicky
21:43has no digital record
21:44of where any
21:45of her patients are
21:47so the system's still down
21:49the porting system's still down
21:50the tracking system's still down
21:52so we should still do everything on paper
21:54I can't see any systems getting fixed
21:58because obviously it started last night
22:01and there's still nothing being solved
22:03so you're going to go for your dinner
22:05aren't you?
22:05No
22:05Yes please
22:06If the internet problems were not enough
22:09casualty has a higher number of patients than usual
22:12so the team are having to pull together
22:15Jack of all trades
22:16master of none
22:18In recess
22:22advanced clinical practitioner Jess
22:24has an update for Graham
22:25after his collapse
22:27So I mentioned earlier
22:28didn't I
22:28that it looks like
22:29the nodule that they've noted
22:31on your left side of your lung
22:32has got bigger
22:33Oh right
22:35I didn't know about that
22:37Yeah so that's
22:38they've reported your scan today
22:39so they've been pretty speeding
22:41I suspect your
22:42little funny-do around there
22:44has probably sped things up a little bit
22:46getting a report done
22:47It may be that this
22:50is what's causing the change of breathing
22:51Alright
22:52So like I said
22:53just waiting for them bloods to come back
22:55make sure there's no added infection
22:56on top of everything else
22:57that you've got going on
22:58Graham was diagnosed with cancer
23:00four years ago
23:01I started off in my bowel
23:05that was all successfully removed in 21
23:09and they monitor you for five years
23:12and they find it in my lungs
23:15I'm just seeing more of the hospital
23:20than I would like
23:21but it's just what comes with old age I suppose
23:24Considering what's gone off today
23:29I want to play it safe
23:30and bring you in for tonight at least
23:32for the medical team to review you
23:34I'd love to say that
23:35I want to be really brave and get you home
23:37but I think because you've got
23:38so many things going off
23:39I think it's the ample opportunity
23:41to just touch base with you
23:44and they'll be able to touch base
23:45with the chest team as well
23:46about your results of your scan
23:47so it may be a quick turnaround
23:51and they may say
23:51I don't know why your knees kept you in
23:53but I'd rather do that
23:54and be safe
23:55than send you home
23:56It's all right to be in tonight
23:56I've got such a brilliant health service here
24:02and they don't let you go
24:05if they've got a few queries about you
24:07The doctors found that a blood clot
24:12was reducing Graham's oxygen levels
24:14His battle with cancer is still ongoing
24:17In cubicle 3
24:21June is back from radiology
24:23where a scan revealed
24:24no internal injuries from her form
24:26Sister Benita will now treat her skin wounds
24:30June has a skin tear
24:53across the top of her chest
24:54She's sustained multiple wounds
25:04and her skin's so fragile
25:06I wouldn't say it's life-threatening
25:08or anything like that
25:09but it'll just prolong her stay
25:11and she's so thin
25:13and not got much meat on her bone
25:16she's going to need managing
25:17in terms of making sure
25:19that she gets back on her feet properly
25:21Right, the plan for this
25:23is to give it a bit of pain
25:26and the reason it hurts
25:30is because underneath you skipped
25:32it's exposed
25:33and that's why it's making it sore
25:36so if I can clean it
25:38as quick as I can
25:39try and see if you swing back
25:41then it hurts that real
25:42not first
25:43Can I just have a little look
25:46and see if you've got so many legs
25:48She's full of fluid
25:51downwards
25:52and she looks like she's got some
25:54little areas
25:56that are being dressed
26:03in the community
26:04but because of circulation
26:06so poor
26:07it just starts to
26:08cause a lot of other problems
26:11Sister Benita
26:13brings June some painkillers
26:15Are you okay
26:16swallowing these
26:17big them like that?
26:19You tend to find that
26:23fairly stoic
26:24Barnsley folk
26:25they don't come forward
26:27quick enough
26:27in terms of
26:28painkillers
26:29and it'll be so painful
26:32when they're exposed
26:34to wear it
26:35so
26:35Have you lived in Barnsley
26:38all your life?
26:40More or less
26:41More or less
26:42What did you do
26:43work-wise?
26:45Did you work?
26:47And what did it sell?
26:50Pork pies
26:51Pork pies
26:52People in Barnsley
26:54like pork pies
26:55though, don't they?
26:57It's actually really nice
26:59to have those conversations
27:00about her life
27:01and what she's done
27:03and I always find
27:04in older people
27:05they've been through so much
27:07and you learn
27:08such a lot from them
27:09Did you have children?
27:11Yeah
27:12How many children
27:14did you have?
27:15Two
27:15Two boys
27:17Two boys
27:18My two eldest
27:20they're 16 and 30
27:24well he's just turned 17 actually
27:25it was his birthday
27:26over the weekend
27:27and they're at a point now
27:30where they're actually
27:31beginning to get on
27:32with each other
27:33Sister Benita
27:37starts to treat
27:39June's wounds
27:39Right, are you ready?
27:44Here, hold me
27:45I'm sorry, I know
27:54Right
28:01I'm going to leave that
28:02for 20 seconds
28:03I'm just going to go
28:04and get some tweezers
28:05In the hub
28:09there's no sign
28:11of things slowing down
28:12What are them two
28:13waiting for?
28:14Them two aren't waiting
28:15for that patient
28:15to bring you a bit to see
28:16Bit to see, right
28:18So
28:18I'm just ringing
28:22for a little bit
28:22of advice
28:23about a patient's pacemaker
28:25I'm plodding on through here
28:26I'm not entirely sure
28:27what's happening
28:28with the system yet
28:30I have not given up
28:33my holidays
28:34On you off
28:36soon?
28:37Next
28:38on Saturday
28:39Saturday
28:39Have you lost any
28:41of your weight?
28:42Have I yet?
28:44I've even started
28:45calorie counting now
28:46on an NHS advert
28:47it just
28:47It says it can't do much
28:50for me
28:50because my BMI
28:51is perfect
28:52it's perfect
28:53Me it's done
28:54say that it's perfect
28:55Jeannie is having an x-ray
29:01to see what internal injuries
29:03she sustained
29:04in her form
29:05I'm going to have a walk
29:20across to cardiology
29:21and let them have a look
29:24at this ECG
29:25and see if they're happy
29:27with it
29:27I don't have my grandparents
29:29anymore
29:29but I've seen my grandparents
29:30in pain
29:31so I empathise a lot
29:32with older patients
29:33that I see in the hospital
29:34I do think
29:35they're very proud of me
29:36and
29:37oh gosh
29:40I know my grand
29:41I can't
29:41I can't
29:41after this
29:44I'm going to get the nurses
29:45to do her blood pressure
29:46while you're laying down
29:47and standing up
29:48and I'll check your x-ray
29:50and then if everything's okay
29:51and you pass the tests
29:53you can go home
29:54I know she's really proud of me
29:57and I know my family
29:58are proud of me
29:59and I do know
30:05I'm good at my job
30:05so
30:06anyway you've got me crying
30:08ready for the finale
30:12so this is your x-ray
30:16have you seen your pacemaking
30:17like this before
30:18no
30:19no
30:19so it's absolutely fine
30:21and you'll be good to know
30:23that I can't see any
30:23with fractures either
30:24I think you've just injured
30:26and got a bit of bruising
30:27around your muscles
30:28of your chest wall
30:29and it's a bit bruised
30:31yeah
30:32okay
30:32your blood pressure's okay as well
30:34so I'm happy for you
30:35to go after your painkillers
30:36right
30:37who's taking you home
30:38me

32:25ok
32:26if my heart is not beating

32:31as soon as you're chatting to us
32:32I think it will be
32:34yeah
32:35Mr. Dramatic Heart
32:36yeah
32:36that's my heartbeat
32:38so this one is
32:40your heart rate
32:41and this one's your oxygen levels
32:42oh
32:43but it's not
32:44it's not quite picking you up properly yet
32:46so you see this all
32:47it's all wiggly
32:48so we need this finger to stay
32:51yeah
32:51does that mean
32:52almost dead
32:54no
32:55not quite
32:55sometimes if you get a bit excited
32:57or a bit scared
32:58your heart rate jumps up and down
32:59so keep that finger nice and still for us
33:01and then we'll get some real numbers
33:02yeah

33:33now we're going to go wait for a doctor
33:35go back in that waiting room
33:36there the teeth
33:40I think it might have to have a cap
33:43by the dentist
33:44they might have to cap
33:45cap and both
33:46unless they can glue
33:48in recess
33:55factory worker Louise
33:57has badly broken her ankle
33:59I'm just pinching your potting trolley clay
34:01I like it
34:02Dr Soudan is joined by a team
34:05who will work together
34:07to move Louise's leg back into position
34:09got everything ready
34:12we're good to go
34:13first
34:14Louise is sedated
34:16with an anaesthetic
34:17just see how she responds to you
34:18playing around with her ankle
34:19you're holding the whole leg or no
34:22yeah I've got it
34:23Louise has broken
34:24and dislocated her ankle
34:26so this
34:27is not a simple procedure
34:29for some joint dislocations
34:31you also need the muscles to relax
34:34to be able to effectively get
34:36the joint back in
34:38the drugs we need to give to relax
34:42can also affect your ability to breathe safely
34:46so we do a procedure called sedation
34:49whereby we try and get
34:50enough drugs into the person
34:52that they are still breathing for themselves
34:55their heart rate
34:56their blood pressure is stable
34:57but yet in their head
34:59they haven't got a clue
35:00what they were doing to them
35:01and they shouldn't be feeling any pain
35:03you're holding
35:04you're holding?
35:04you're holding?
35:04mm-hmm
35:05I don't know if this is the...
35:10I don't know if this is the...
35:11I don't know if this is the...
35:13Louise's ankle is manipulated back into position
35:16and placed in a cast
35:18okay?
35:24thank you for helping
35:25you have a rate pass
35:26thank you so much for coming and giving us a hand
35:37Louise now needs another x-ray
35:43to see if her ankle is back in the correct position
35:45and if not
35:46she will need emergency surgery
35:49today has been a busy shift in casualty
36:06I bet you have to pay extra for one of them
36:1080 page passports
36:12do you?
36:13the hospital's computer network has gone down
36:17so sister Vicky has spent her shift
36:19resorting to old school communication methods
36:22so has anybody took anybody around from CDU?
36:27operational support manager Antonia
36:30has been working to get Barnsley back online
36:32it's probably a global issue
36:35that's happened
36:36so that's all been reset now
36:38so we're slowly getting them back up
36:40and the system should hopefully be back up and running in a bit half an hour
36:44so we can start using it again
36:45to be honest I'd rather be in here than sorting the IT
36:49modern day
36:52June
36:53bet you didn't have this in your shop did you?
36:56IT issues
36:57you know that diet Darren?

37:02why don't you're counting?
37:03you're counting?
37:04you're counting a lot
37:05you're counting higher
37:06how high can you go?
37:08how high can you go?
37:09Louise is having an x-ray
37:18to see if her broken ankle
37:19is back in position
37:21yeah
37:26lots better
37:28a lot better
37:29no no but you've got that back on there
37:32the fellowship is gone
37:33yeah
37:34Dr Soudan and the team
37:35conclude that the manipulation
37:37has been a success
37:39Louisa's leg will remain in this position
37:44until it can be operated on
37:46they need to do a CT before they operate
37:51just to get a better picture
37:53so
37:53are you okay going home?
37:56yeah
37:56okay
37:56if we need to wait till morning
37:58to kick you out in daylight
37:59that's an option
38:00that's all I'm thinking
38:01but if you think you can safely
38:03get from the taxi to the house
38:05yeah
38:05yeah
38:06I'd rather be in my own bed
38:07if nothing was going to happen over the weekend
38:09yeah
38:09pain-wise
38:10what's it like?
38:11it's throbbing
38:12okay
38:13like
38:15I want to move it like that
38:16okay
38:18it's throbbing
38:19annoying
38:19throbbing
38:20rather than painful throbbing
38:21it's stuck
38:22yeah
38:23but it is
38:24we've put a pot on
38:26I look forward to
38:29coming to work
38:30and wondering
38:31who I'm going to meet
38:32and what kind of problem
38:34they're going to have
38:35and even if I'm tired and grumpy
38:37I try to remember
38:38what a privilege it is
38:39for people to
38:40kind of let me in
38:42on what's going on for them
38:43how have you been a nurse?
38:45me?
38:46never
38:46I'm a doctor
38:51how have you been a doctor?
38:53about 15 years
38:54you like it?
38:56love it
38:56it's the best job in the world
38:57it's the best job in the world
38:58and I hope at the end of
39:01like
39:01them seeing me
39:03that we've got them
39:04in a better position
39:05than when they came in
39:06do you need some
39:07painkillers from me?
39:08yes please
39:09yeah
39:09okay

39:18yes
39:19young cause they're dead
39:21I'll not be going out
39:25for Thai food again
39:27Louise came back
39:29for surgery after two weeks
39:31where she was fitted
39:32with two plates
39:33and eleven screws
39:34she returned to work
39:36six months later
39:37in cubicle three
39:45sister Benita
39:46has almost finished
39:47cleaning June's wounds
39:48you've got a bit of it left
39:51but I think you've lost

40:27no
40:27do your legs just give way?
40:30I'm just thinking
40:33when you're being a bit
40:34full of fluid
40:35I wonder if they
40:36just feel like
40:37you're a bit full
40:40then when I get back up
40:41that's all you'll see
40:44the medical team
40:48will have a look at her
40:49and establish what it is
40:51that's making her fall
40:52because although she's
40:53sustained all of those injuries
40:54she's done that
40:56as a result of falling
40:57and that seems to be
40:58the recurring problem with her
40:59will you be glad to get rid of me?
41:02no
41:03no I'm not
41:06right
41:08is that okay?

41:22very very good
41:23June moved out of casualty
41:26to the acute medical unit
41:27she remained in hospital
41:29for two weeks
41:30before returning home
41:31to be looked after
41:32by her family
41:33in pediatrics
41:37eight-year-old Henry
41:39has fallen off
41:39a climbing frame
41:40and shattered his front teeth
41:42come and have a seat
41:44emergency nurse practitioner
41:45Tony
41:46now needs to check
41:47if Henry
41:48has broken anything else
41:50right let's have a look then
41:52let me
41:53use this bright torch
41:54it's going to work today
41:55look up to the ceiling
41:57open your mouth right wide
41:58yeah it says that one
42:02next to it
42:03sorry dude
42:04all right
42:05a little bit wobbly
42:07into it
42:08sorry I'm just going to have
42:08to lift your lip up
42:09a little bit
42:09so I can see
42:10he's gone with a right
42:12bang bless him
42:13let's look at this side
42:15as well
42:15our main concern is
42:17is there anything else
42:18going on in the mouth
42:19or the jaw
42:20and open your mouth
42:20really really wide
42:21wide it's gone
42:22perfect
42:23and you can see
42:24he broke his mouth
42:24really really well
42:25and his jaw was okay
42:26so it's a little bit bruised
42:28but it all feels nice
42:29and hard under there
42:30so I don't think
42:31there's anything broken
42:31it is a tough one
42:33and especially at his age
42:34he's going to be
42:35conscious about his teeth
42:36he's going to be
42:37all worried about them
42:39that other tooth
42:40at top's a little bit wobbly
42:41but it's still
42:41quite well fastened in
42:43so it's not
42:43I don't think he's going to
42:44come out
42:44any time soon
42:45you do need to see
42:47a dentist
42:47it's obviously about
42:48them first too
42:48there's not really
42:49anything we can do
42:50with them
42:50so
42:51I'm going to let you
42:52go home
42:53okay

42:57conversation the week
42:58before saying like
42:59you've got to look
42:59after your teeth
43:00and then he goes
43:02and does that
43:03today
43:03he does the falling
43:04and silly things
43:05right we'll let you
43:06get it going then mate
43:06right thank you
43:08very much
43:08hopefully the dentist
43:09will see him quite
43:10quickly
43:10he'll get that
43:11them tooth sorted
43:12put a cap off
43:14some fixing on
43:15temporarily probably
43:16until he's a little
43:16bit older
43:17and then
43:17we'll have some
43:18more work done on it
43:20and be back smiling
43:21before in no time
43:22oh you love the dentist
43:23don't you

43:35new front teeth
43:36built by his dentist
43:37and he's now back
43:38to running around
43:39on the school
43:40playground
43:40the day shift
43:49is almost at an end
43:51and operational support
43:52manager Antonia
43:53has finally been able
43:55to connect to the
43:56hospital network
43:57it looks
43:58as if it's slowly
44:00feeding back through
44:01now
44:01it's almost time
44:03for sister Vicky
44:04to clock off
44:05with her holiday
44:06just around the corner
44:08so where's this one

44:19all over water
44:20bungalows and stuff
44:21and
44:21it's very posh

44:25we managed to get
44:27all the patients
44:27back on the system
44:28the system up and
