00:00earlier before the rations trial we saw that it is not only one person who has TB in a family
00:07every time a person will either give a history of TB in some other member or new members keep
00:14coming with disease which means that people are having a shared environment of poverty and food
00:19insecurity so in rations trial what we wanted to show was that if we give food basket to the family
00:25members their immunity improves and new TB does not develop in the in the family because there's
00:32a shared environment of increased infection and also food insecurity so that was one thing that
00:38we showed and what I can I can say that the trial was done in a challenging time because when we
00:46started in 2019 in August within few months COVID started so it was a very challenging time where
00:55delivery of food basket became very difficult even our health workers who are doing the screening and
01:01delivering food baskets they started developing sometimes COVID or their family members were sick
01:08because of the lockdown our travel became sometimes very difficult but despite of all these challenges
01:14the team really worked hard to complete the trial during the COVID time
01:19okay so after the publication of the rations trial findings one set of findings were on the household
01:31members how we reduce the new TB in the family members and the other set of publication included the
01:39outcomes in the patients patients with tuberculosis how we reduced how we were able to reduce the
01:45mortality loss to follow-up which is a big problem in tuberculosis people don't take medicines
01:50throughout the treatment and adverse effects so these two publications caught attention and during
02:00um 2024 uh WHO decided to revise its 2013 guidelines so the 2013 guidelines of WHO on TB and nutrition
02:12they mainly focused on nutrition assessment and counseling of patients with tuberculosis and if they
02:19have severe undernutrition food support was suggested however uh new evidence was not available till the
02:27patients trial was completed so when uh rations trial findings were published in 2020-23 in 2024 we were uh we were told to attend
02:37as uh technical resource persons uh guideline development group proceedings where they decided that they
02:45will be uh revising these guidelines so uh over a period of six months there were multiple meetings and presentations
02:53where we also uh presented our findings of qualitative research uh within the rations trial so the qualitative
03:02research is really different from what was published in lancet where we give percentages and proportions and
03:09this much death or this much TB in qualitative we actually go and ask people who were part of the whole trial
03:15um so how did they perceive um so how did they perceive the food basket so it was a very positive uh feeling
03:23because the qualitative uh findings were led by a third party that is uh forum for medical ethics society
03:31so they went and talked to the patients their household members our health workers and uh what was found was that
03:40everybody invariably said that because of the food basket they were able to take these hard medicines
03:47otherwise it was very difficult for them to complete the course with such uh so many medications so
03:55ability to complete medicines because of the food basket the household members felt that they were uh they
04:01could see that their uh ability to work was better the patient started working better the health workers
04:10gave some wonderful stories of uh moving stories of how uh they were perceived by the trial participants
04:18and uh we also discussed with various you know program persons also within charcoal where people felt
04:26that finally in long term in short term definitely if we give food support uh the mortality will reduce
04:33the ability of the patients to complete treatment will will be better but over a period of
04:39time if new tb in families reduce finally our burden of tuberculosis in india will reduce and we'll be
04:47able to achieve the goal of ending tb which is which is the most important at the moment in uh our you know
04:55scheme of things
