00:00Cartilage is the holy grail in orthopaedics. The reason for that is it's an amazing substance.
00:18It's a very complex structure within the joint. And the reason why it's so interesting and complex
00:24is for a number of reasons. Now, from gravity, from weight, biomechanical forces, the movement
00:32that has to take place through everyday life, all our joints, all our diarthrodial joints
00:37have cartilage, highline cartilage inside them. And the purpose of cartilage is to allow frictionless
00:45movement, which is pain-free, and to protect the underlying bone and give you pain-free
00:50movement every day in all your joints for all of your life. Now, the amazing thing about
00:55cartilage is that it doesn't have a blood supply. It doesn't have a lymphatic drainage or supply.
01:02It covers the end of the bone. You're born with it. And that's all the cartilage that you
01:06have for the rest of your life. So this substance, which is around five to eight millimeters thick
01:11in the knee, has to take all that force for such a long period of time and still survive.
01:17And so when you think about it in those terms, it is quite an amazing substance.
01:22Now, people sometimes think about cartilage in the context of osteoarthritis. And that's perfectly
01:28fine, but not everybody gets osteoarthritis. So osteoarthritis is not a part of aging. There
01:34are lots of people that have aged, but they have healthy cartilage and joints. So osteoarthritis
01:39is a pathological process that affects cartilage and degenerates the cartilage.
01:47Also, the left is a path of age, and loved ones where you walk around them. And it
01:51is a path ofulfalty. There's two parts of the story. I love how this is. So
