Transcript
00:00 And in our "Say Nee" doc, we will talk about menopause and how it affects women's health and life.
00:08 How can we handle this and how can we understand this condition better?
00:13 And to give us more information about menopause, we will have a guest this morning,
00:18 Dr. Anafe Orbegoso Gloria, one of OB-GYNs.
00:22 Good morning, Doctor. This is Diane Carrere.
00:25 Diane.
00:28 Good morning, Doc.
00:29 Doc, what is the so-called pre-menopause stage and how does it differ from the perimenopause stage?
00:37 Maybe what we should differentiate or define is the perimenopause and the menopause.
00:48 When we say menopause, it means that it has been 12 months since a woman has not regurgitated.
00:59 This is because the ovaries, the female organs, no longer function
01:08 and the female hormone estrogen and progesterone are produced.
01:15 The average age of menopause is from 45 to 55 years old.
01:24 On the average for Filipino women, it's 47 to 48 years old.
01:30 It's a bit young.
01:32 The so-called perimenopause is the time before the period of menopause completely ends.
01:44 So, perimenopause is a bit longer.
01:48 Sometimes, even if you're 39 years old, you can already have perimenopause.
01:54 But sometimes, if you have menopause late, you can still have perimenopause until you're 51 years old.
02:02 So, those are the years before your period completely ends.
02:08 The average age for menopause is 46 years old.
02:12 What changes will happen to your body and even your mood when you're in the perimenopause and menopause stage?
02:24 We can feel a lot of things when we have menopause or perimenopause.
02:34 So, it is always viewed as negative. Some people are afraid that this stage has already reached their life.
02:44 So, what we usually experience is what we call "hot flashes".
02:52 Hot flashes are when your body is hot, like a flame. It can last for a few seconds to a minute.
03:04 Sometimes, it's just rare.
03:08 Almost 70 to 80% of women will experience the hot flash.
03:14 Aside from the hot flash, there are also night sweats.
03:19 It's when you're sweating a lot at night and you suddenly wake up with a lot of sweat.
03:27 It's also true that the period doesn't become regular.
03:33 There's a so-called "skip and delay".
03:38 That's what happens during menstruation.
03:44 We also have what we call "genitourinary symptoms" or "syndrome".
03:50 That's when we experience dryness in our vaginal area.
03:54 Sometimes, we experience frequent infections in our genitals.
04:00 It's also when we experience pleasure in our genitals and it's hard to get orgasm.
04:11 Aside from that, we experience a lot of body aches.
04:16 You can sing, "Hands, waist, head."
04:21 So, the aches in our bodies are constantly changing.
04:25 We also experience mood swings.
04:30 It's possible that the mood swings are due to your weight or fatigue.
04:36 You can't rest because of the hot flashes or night sweats.
04:41 You might experience insomnia.
04:43 So, it becomes a headache.
04:46 Sometimes, you get angry easily.
04:48 You feel like you're being apologized for.
04:50 We can also connect the stage of menopause to the stage of women's life
04:56 where there are a lot of changes.
04:59 We're getting older and we experience diseases.
05:03 Aside from that, our parents are getting older and we're taking care of them.
05:08 We have more responsibilities.
05:11 When we're working, our positions are higher.
05:17 So, there is really more pressure in our lives.
05:20 Sometimes, this is also the stage where we have issues with our love life.
05:27 At this stage, some people have problems in their relationship as a couple.
05:38 There are a lot of changes.
05:41 You mentioned the symptoms of menopause.
05:44 To be clear, if a woman's period is stopped for 3 to 7 months,
05:50 can we say that she's in the menopause stage?
05:54 We can't say that.
05:57 Like I mentioned, it can be in the perimenopause.
06:02 When the period stops for 12 months, that's when we can say that she's in the menopause stage.
06:08 Okay, so 12 months.
06:09 So, it's like a year when the period stops.
06:14 That is the sign that you are in the menopause stage.
06:17 Alright, so, with the changes you mentioned,
06:21 how should we handle these changes that you mentioned?
06:27 Well, first, we should think that it's not the end of our trip.
06:36 There are a lot of treatments available.
06:39 We can do something to ourselves to reduce or alleviate these symptoms of menopause.
06:47 The most important and accessible thing is lifestyle modification.
06:53 Doc, we just had a problem with the signal.
06:59 150 minutes in a week of cardiovascular exercises,
07:06 like walking, zumba, gardening, or any form of sports will be good.
07:15 Aside from increasing your physical activity,
07:19 we should also avoid processed foods and meat.
07:27 We should also stop drinking and smoking.
07:36 Aside from that, it's also good to visit OB-GYN
07:43 to check and screen for common cancers in women,
07:50 like breast, cervix, or uterus.
07:55 We can do pap smears and mammograms.
07:59 Aside from that, we also screen for colon cancer and other organ system cancers.
08:08 And then, of course, for the cardiometabolic health.
08:11 So, we can see if you have risk for developing hypertension, diabetes.
08:17 Aside from that, we want to advise you on how to prevent osteoporosis
08:25 so that you can have a good quality of life for your menopause.
08:30 And then, apart from that, we will initiate the treatment
08:35 depending on what you feel.
08:40 So, if it's more of genitourinary, meaning vaginal symptoms,
08:47 then we have a medicine that we can give to relieve that.
08:52 But if it's more of hot flashes or vasomotor symptoms,
08:57 we also have a medicine that we can give to stop it or to take.
09:03 Or if you want prevention of other diseases that you will possibly encounter later on in your life.
09:12 You said that at that age, perimenopause and even menopause,
09:19 you should move around and look for physical activities that you can do regularly.
09:26 Go to your doctor.
09:28 Women really need a healthier lifestyle.
09:31 Doc, I have another question.
09:33 Is it possible for a woman to get pregnant during the perimenopause stage?
09:37 Yes, it's still possible because there are still eggs left.
09:44 So, other women who are concerned about that can still use contraception.
09:51 But it's better to consult a doctor
09:55 because if we get pregnant during the perimenopause,
10:01 the quality of our pregnancy is not good.
10:06 So, if you don't want to get pregnant, it's better to get advice.
10:11 Doc, do you have any advice on supplements or to drink to manage the symptoms that you mentioned earlier?
10:23 The most that we can recommend is for prevention of osteoporosis,
10:28 which is taking your calcium.
10:30 If you feel that you don't have exposure to sunlight,
10:34 then also your supplements of vitamin D.
10:38 At least 1,200 mg of calcium is needed with vitamin D every day.
10:46 And then we should regard food as our medicine.
10:53 So, it should supply us with adequate amount of nutrients to get us through the rest of the menopause.
11:03 If we are specific about the symptoms and we will use hormonal treatment,
11:10 we should really consult our doctor.
11:16 Lastly, Doctor, for women who are afraid of reaching this stage due to various changes,
11:26 what is your message?
11:28 Well, we should not view menopause negatively.
11:34 It is something that we should accept positively and think about it as another opportunity for us to do better things in life.
11:45 Because if we don't have a rule, we won't think about the possibility of getting pregnant.
11:52 And we have more freedom because our kids are already grown up.
11:57 We have reached a status where our life is better.
12:02 So, we are free and we can also do still a lot of things.
12:08 Maintain that figure, maintain that beauty and age gracefully, but by being healthy.
12:16 So, lifestyle changes are really important.
12:20 And let us live life happily and accept the things.
12:28 But also, if we are bothered, treatment is always available and your doctors are here to help you all the time.
12:37 Well, thank you very much for your pieces of advice, Doctor.
12:41 It is important for these people to know, not only women, but also men around women, especially partners, husband and children.
12:51 Thank you so much, Doctor Anafe, for the free telemedicine this morning.
12:56 Thank you, Doc.
12:58 Thank you, Diane.
12:59 Happy Women's Month to all of you.
