00:00 Yes, I think when I appeared before, in fact I know when I appeared before,
00:04 I said that the union had failed to join the dots.
00:07 There were a number of individual incidents and they remained as individual incidents
00:12 and we didn't bring them together.
00:14 I think some of our policies and processes were not fit for purpose
00:19 and I think the fact that we have appointed a director of people has helped in that regard.
00:25 I think when you've had, and what happened 12 months ago was more than a warning,
00:31 when you have a television programme which lays bare the ills of an organisation all in 30 minutes,
00:39 you would be rather foolish not to take heed and to sense check that the changes which had been made
00:46 in the previous 12 months, some changes had already been made,
00:49 just to sense check that the things that went wrong before could not happen again.
00:55 And there are many things that we have done, a whistleblowing helpline,
01:00 giving our staff more confidence in being able to speak up,
01:05 and us demonstrating that we're prepared to listen.
01:08 There are a whole raft of things, but I think to bring it down to one thing,
01:14 it was not joining the dots when there were a series of incidents over a period of time,
01:18 because they didn't all happen in three or four weeks,
01:21 over a period of time and not bringing those things together and saying,
01:24 we've got a problem here, how can we address it?
