00:14 And because of her unwavering resolve,
00:17 despite daunting odds, I was born.
00:20 Premature, 4 pounds 11 ounces, but healthy.
00:25 Ever since hearing this story, I have had a crippling fear of childbirth.
00:30 And so this week, I am flying to Texas to meet with a team of medical professionals
00:35 to learn how to deliver a baby in hopes of overcoming that fear.

00:49 And so my first stop is to meet with a physical therapist to experience the closest thing possible to labor contractions.

04:44 my next stop is to a state-of-the-art healthcare training facility
04:48 to learn what it's like to be the one to actually deliver the baby.
04:52 And that happens here, in a fully functional one-to-one replica of a delivery room.

05:21 She also can still follow and trace you.

05:30 So the vitals we're seeing here are produced by the robot.

05:49 This one also does postpartum hemorrhage, so it will bleed.
05:52 If the uterus doesn't go back to being firm, she'll bleed out until it goes back to being firm.
05:58 Is that what happens?
05:59 That's what happened to me.
06:01 What happened?
06:02 My placenta didn't fully detach, and I was bleeding out.
06:05 So they had to rush me into two different surgeries when I had my son.
06:10 Oh, my gosh.
06:11 I am starting to realize that a complication in childbirth, like my mom's experience, isn't as uncommon as I originally thought.
06:20 Out of the 385,000 births every day, roughly 31,000 of those experience complications.
06:28 And the percentage of known pregnancies ending in a miscarriage is estimated to be up to 20%.
06:36 In fact, so many people are anxious about pregnancy that there's even a word for it, antenatal anxiety.
06:45 But now, being a mom, did it feel worth it?
06:48 Oh, yes, 100%.
06:49 All of that?
06:50 All of it.
06:51 I wouldn't trade him in for the world.
06:52 He drives me crazy, but I would not trade him in for the world.
06:57 For my final challenge, I will be performing two separate deliveries, the first simulating a standard vaginal delivery,
07:04 and the second simulating a C-section, all while being observed through this two-way mirror,
07:09 where Sarah will be controlling the birth from the other side.
07:12 So she reacts to everything that you're doing, whether it's good or bad.
07:16 So if I mess something up, you will activate the appropriate response from Lucina.
07:22 Correct.
07:23 But before I can successfully deliver a baby, I am meeting with Amber, a first-time expecting mother,
07:30 in order to see firsthand what it's like to go through this process.

07:36 I've actually been having contractions starting last night, so--

07:45 There are contractions happening.

07:49 Some of them can be pretty intense.

07:51 If it doesn't get any worse than this, we're aced.

07:59 I can't wait to hold him and just have that bond with him.
08:02 And it's crazy how much you can love somebody you've never met.

08:11 I've never thought too intensely about the love and the bond that you develop with the baby as you are pregnant
08:18 and how that can be a source of courage and bravery and trump all of the other feelings.
08:25 And after meeting Amber, I was also curious to compare her experience to that of a more experienced expecting mother.
08:31 So this is your fourth time having a child?
08:33 Yeah, this is my fourth time.
08:34 This is my second girl, too.
08:36 But you must be a pro at this now.
08:38 Oh, yes.
08:39 Okay, so is this just like another day at the office?

08:46 I think this day is not a secret.
08:48 I think it's how you be more patient and control your mind.

08:56 This one is kind of very special, first girl for him.

09:05 Well, this is the biggest birthday present I've ever had.

09:27 Most doctors experience people at a bad time in their life with a problem that needs to be fixed.
09:33 I, on the other hand, get to deal with young, healthy people at a happy time in their life.
09:37 Getting to see new families or families expand makes my job a pleasure.

09:46 So, this patient's come in and she's complaining of contractions, and we need to determine whether or not she's in labor.

10:12 Well, first let's--

10:16 First thing we got to do, we have to determine if their cervix is dilated.
10:20 So, this tells you what you're looking for when a patient is 10 centimeters dilated.
10:25 She's what we call completely dilated.

10:30 Yeah, that's got to fit a baby's head through there, so, yeah.
10:33 If I'm pregnant and I'm delivering a baby, there's no way around this.

10:41 If you want to get it out vaginally.

11:23 A lot of things can go wrong during birth.
11:25 The most common are lack of progress, infection, the baby's not getting the oxygen that it needs.
11:30 So, during delivery, if the baby's head delivers and the shoulder is impacted underneath the pubic bone,
11:36 that can create a condition called shoulder dystocia.
11:39 And that is an obstetrical emergency, but we're trained to manage it.
11:43 One of the reasons I wanted to do this was because my mom had a life-threatening situation when she was pregnant with me
11:50 where the placenta covered the uterus, and they had to do an emergency C-section.
11:55 So, she may have had what was called a placenta previa?
11:57 Yeah.
11:58 That's what it sounds like.
11:59 But I feel like since I've known that my whole life, it's always, "This is terrifying to me."
12:04 Absolutely terrifying.
12:05 Yeah, yeah, I get it.
12:06 We have a strategy for dealing with it, but yeah, it can be scary.
12:09 Oh, that one's a big one.

13:45 And while Cassie will be role playing the mother's top half, we will be using a robotic lower half to simulate the actual birth, all controlled by Sarah in the next room.

15:55 Well, that's why we're always ready for those emergencies, and we have maneuvers that the nurses are trained to do so that we can make sure the baby's still delivered safely even though the shoulder got stuck.

16:04 But that was just representing a more common delivery scenario.
16:08 For my second delivery, I will be attempting a C-section, inspired by my mother's experience.
16:14 And with C-sections being 80% more likely to have complications, I was expecting the worst.

16:25 You feel for the pubic bone, and then you go a couple finger breaths above the pubic bone.
16:31 This is your scalpel, and you open it like that.
