An investigation in South Australia has uncovered serious breaches of the rules governing how patients are restrained. It has found one boy was restrained face down and secluded for more than five hours at a mental health ward at Adelaide’s women's and children's hospital. A report has made several recommendations including retraining staff on policies and record keeping and developing child-specific rules.
