As difficult as life in Syria's most embattled city has become, civilians attempting to leave probably should have expected no different. (Explosion)

A fresh wave of attacks as UN and Red Crescent teams tried to help hundreds of elderly men, women and children escape --- in this latest video posted to a social media website whose content Reuters cannot independently verify.

The violence comes a day after mortar fire sent aid workers here scrambling for cover in a basement. UN Coordinator Youval El Hillo and others waited for hours undeterred by this latest setback.

(SOUNDBITE) (Arabic) U.N. HUMANITARIAN COORDINATOR IN SYRIA, YACOUB EL HILLO, SAYING:

"Today is the first day that the U.N. workers made it into here since the last time which was a year ago, and that was the last time. So what was achieved today is a good thing and it is a good beginning and we must continue."
